Abstract

Latar Belakang: Demam Berdarah Dengue (DBD) merupakan masalah kesehatan yang disebabkan oleh virus dengue, yang ditularkan melalui gigitan nyamuk Aedes, khususnya Aedes aegypti. Trombositopenia adalah salah satu tanda yang diamati pada DBD. Trombositopenia terjadi ketika jumlah trombosit <100.000 sel/mm³ sehingga fungsi trombosit dalam hemostasis terganggu sehingga mengurangi integritas pembuluh darah, menyebabkan kerusakan pada pembuluh darah dan pada akhirnya mengarah pada manifestasi perdarahan. Penyebaran DBD yang cepat dan berpotensi fatal tetap menjadi masalah  kesehatan masyarakat yang signifikan dengan dasar lingkungan. Data dari Dinas Kesehatan Kota Padang menunjukkan adanya peningkatan kasus DBD, dengan 441 kasus yang dilaporkan pada tahun 2022 dibandingkan dengan 366 kasus pada tahun 2021. Kelompok usia 15 hingga 44 tahun adalah yang paling terdampak oleh penyakit ini. Objektif: Penelitian ini bertujuan untuk mengetahui hubungan antara jumlah trombosit dan manifestasi perdarahan pada pasien DBD di Rumah Sakit dr. Reksodiwiryo Padang pada tahun 2022. Metode: Penelitian ini menggunakan desain analitik korelasional dengan pendekatan potong lintang (cross-sectional). Pengambilan sampel dilakukan dengan metode consecutive sampling. Data didapatkan dari rekam medis di Rumah Sakit dr. Reksodiwiryo Padang, dari Januari 2022 hingga Desember 2022, dengan jumlah sampel 100 pasien berusia di atas 18 tahun. Analisis statistik dilakukan menggunakan uji Chi-square. Hasil Hasil penelitian menunjukkan bahwa sebagian besar kasus DBD mengalami trombositopenia berat (49%). Proporsi pasien DBD yang juga mengalami manifestasi perdarahan lebih tinggi (51%). Analisis bivariat menggunakan uji Chi-square menghasilkan nilai p sebesar 0,001 (p < 0,05). Kesimpulan: Dapat disimpulkan bahwa terdapat hubungan yang signifikan secara statistik antara jumlah trombosit dan manifestasi perdarahan pada pasien DBD di Rumah Sakit dr. Reksodiwiryo Padang pada tahun 2022

Abstract

Background: Colorectal cancer is the third malignancy and second leading cause of death in the world. CEA levels before surgery can be used as a cancer marker to help staging, planning, monitoring therapy and disease recurrence. One factor that influences CEA levels is the degree of differentiation of cancer cells. This study aims to determine differences in CEA levels based on the degree of histopathological differentiation in colorectal cancer patients at Dr. M. Djamil General Hospital Padang. Methods: This study was a cross-sectional analytical observational study using medical record data from colorectal cancer patients from January 2021 – August 2023. The sample was 65 patients who met the inclusion and exclusion criteria. CEA examination uses the ELFA method. The Mann Whitney test was used to see differences in CEA levels based on the degree of differentiation, statistically significant if p < 0.05. Results: A total of 65 patients with an average age of 56.89 years, 58.5% men, 41.5% women. A total of 10 patients were stage I, 29 stage II, and 26 stage III. Based on histopathology, 50 patients were classified as low grade and 15 as high grade. Low-grade CEA levels were 67.72 (3.14-200) ng/mL, while high grade were 3.2 (0.57-11.42) ng/mL. There was a significant difference in CEA levels based on histopathological differentiation (p < 0.001). Conclusion: CEA levels are higher in low-grade than high-grade colorectal cancer.

Abstract

Tujuan : Menganalisis hubungan paparan dosis radiasi sinar X dengan parameter hematologi pekerja radiasi. Metode : Penelitian potong lintang pada 57 subjek pekerja radiasi di Instalasi Radiologi RSUP Dr. M. Djamil Padang sejak Maret 2022 hingga Maret 2023. Data dosis radiasi sinar X didapatkan dari hasil pengukuran TLD Badge di Instalasi Radiologi. Data parameter hematologi didapatkan dari MCU dan LIS. Data dianalisis menggunakan uji korelasi pearson (p<0,05). Hasil : Subjek penelitian terbanyak adalah perempuan (56,14%) dengan rerata usia 34,67 (9,95) tahun dan rerata masa kerja >5 tahun (52,63%). Rerata dosis radiasi 2,20 mSv, rerata kadar hemoglobin 13,82 g/dL, rerata nilai hematokrit 41,75%, rerata jumlah eritrosit 4,85 106/μL, rerata jumlah leukosit 7,04 103/μL, dan rerata jumlah trombosit 291,02 103/μL. Korelasi dosis radiasi sinar X dengan hemoglobin, hematokrit, eritrosit, leukosit, dan trombosit berturut-turut adalah (r=-0,109; p=0,422), (r=-0,121; p=0,260), (r=-0,118; p=0,383), (r=-0,141; p=0,295), dan (r=0,153; p=0,257). Kesimpulan : Terdapat korelasi negatif sangat lemah dan tidak bermakna secara statistik antara dosis radiasi sinar X dengan parameter hematologi. Saran dilakukan penelitian kohort dengan analisis berdasarkan kelompok usia.

Abstract

This paper presents the concept of rasa in Javanese culture embodied in Javanese dance. The ethnographic approach in the Ethnochoreology discipline used as a step in data collection was in-depth interviews with actors (dancers, choreographers) in a society with a Javanese cultural scope. The observation step in the data collection effort was carried out using the involved observation model or participant observer, completed by data collection through literature and relevant documents. The theoretical perspective and concept used as the basis for thinking in discussing the case study of the Ela-ela dance in this paper is the concept of rasa, as put forward by Paul Stange. Namely, that rasa is a "tool" or "means" of perception, or a "function" of "whisper of heart or intuition", and rasa in Javanese dance as expressed by Katarina Indah Sulastuti, that rasa in Javanese dance is an element that is integrated with the senses in the body in a comprehensive manner which involves: the sensibility of heart vibrations, sharpness of intuition, imagination, bodily intelligence, and understanding of Javanese culture. Javanese dance, in the example of the case study, the Bedhaya Ela-ela dance is a representation of the rasa in Javanese culture, namely that the Bedhaya Ela-ela dance is a form of dance that substantially depicts the life of the Javanese people and the culture of rasa which involves issues of ethics, aesthetics, and religion in Javanese culture.

Abstract

Background: Vitamin D deficiency during pregnancy is associated with poor pregnancy outcomes. Research shows a complex interaction between iron and vitamin D. Pregnant women are susceptible to iron deficiency due to increased iron requirements during pregnancy. Ferritin reflects body iron stores and may decrease before serum iron. This study aims to analyze the relationship between ferritin levels and vitamin D status in pregnant women. Methods: This retrospective comparative cross-sectional analytical observational study involved pregnant women in the 1st, 2nd, and 3rd trimesters who underwent antenatal care at Hermina Hospital Padang from February to August 2023. Vitamin D (25(OH)D3) and ferritin were measured using enzyme-linked fluorescence assay (ELFA). Univariate and bivariate analysis used the Chi-square and odds ratio (OR) tests, with a significance of p<0.05. Results: Of 163 pregnant women (mean age 30.4 years), median ferritin levels were 25.85 ng/mL and 25(OH)D3 15.5 ng/mL. Low ferritin was found in 63.80% of subjects and sufficient vitamin D in 31.90%, insufficiency in 33.74%, and deficiency in 34.36%. There was no correlation between low ferritin and vitamin D insufficiency (OR=2.04; 95% CI 0.94-4.42; p=0.700). However, there was a significant correlation between low ferritin and vitamin D deficiency (OR=6.59; 95% CI 2.68-16.18; p=0.000). Conclusion: Pregnant women with low ferritin have a 6.59 times higher risk of experiencing vitamin D deficiency.

Abstract

Background: The co-occurrence of tuberculosis (TB) and acquired immunodeficiency syndrome (AIDS) presents a formidable clinical challenge due to the synergistic impact on the immune system. This case report describes a patient with TB/AIDS co-infection exhibiting profound immunosuppression characterized by a severely diminished CD4 count and an unusual reversal of the CD4:CD8 ratio. Case presentation: A 39-year-old male presented with symptoms indicative of both TB and advanced HIV infection, including shortness of breath, weight loss, and oral thrush. Physical examination revealed bilateral lung crackles. Laboratory investigations confirmed pulmonary TB and revealed a critically low CD4 count (6 cells/µL), and a CD8 count of 71 cells/µL, resulting in a reversed CD4:CD8 ratio of 0.08. The patient's HIV viral load was markedly elevated at 598,403 copies/mL. This case underscores the complex interplay between TB and HIV, highlighting the profound impact of co-infection on immune system function. The patient's severely depleted CD4 count and the atypical CD4:CD8 ratio reflect the advanced stage of HIV infection and the superimposed TB. The findings emphasize the need for close monitoring and aggressive management of co-infected patients to mitigate the risk of opportunistic infections and disease progression. Conclusion: This case report documents a rare and severe presentation of TB/AIDS co-infection with profound immunosuppression and a reversed CD4:CD8 ratio. It serves as a reminder of the significant morbidity and mortality associated with advanced HIV and TB co-infection, particularly in cases of delayed diagnosis or suboptimal treatment adherence.

Abstract

Background: Hepatocellular carcinoma (HCC) is a primary liver cancer that often arises in the setting of chronic liver disease, most commonly cirrhosis. Alpha-fetoprotein (AFP) is a tumor marker frequently elevated in HCC, but its diagnostic performance varies. We present a case of HCC with concomitant cirrhosis and a significant elevation of AFP. Case presentation: A 30-year-old male presented with hematemesis, abdominal distension, and jaundice. He had a history of heavy alcohol consumption. Physical examination revealed anemia, icterus, hepatomegaly, and palmar erythema. Laboratory investigations showed elevated AFP (>400 IU/mL), reactive hepatitis B surface antigen (HBsAg), anemia, coagulopathy, and liver dysfunction. Imaging studies confirmed HCC and cirrhosis. This case highlights the diagnostic value of AFP in HCC, particularly when combined with clinical and imaging findings. The patient's history of alcohol abuse and HBsAg positivity are well-established risk factors for both cirrhosis and HCC. The marked elevation of AFP, along with the characteristic imaging features, strongly supported the diagnosis of HCC. Conclusion: AFP remains a valuable tool in the diagnosis and monitoring of HCC, especially in patients with cirrhosis. However, it is essential to interpret AFP levels in conjunction with other clinical and laboratory data to ensure accurate diagnosis and timely management.
