Abstract

Puger is the largest fish producing sub-district in Jember district, which also has a prevalence of high nutritional problems. During this time there are “mitra” who are a group of fish processors and marketers (POKLAHSAR) namely "Ekonomi Puger Bahari (EPB)" and "Duta Tongkol" who try to explore the potential of fish in Puger District. Tuna fish abon produced by “mitra” has a soft texture (srundeng like) so that it is less preferred by consumers therefore this paper argues that there's a need to add banana blossoms as a compound, does not have P-IRT (product permit), and the marketing of the product is not optimal. The author believes that if there is a need for development in terms of products implemented in several respects, among others: 1) Training on modified abon production; 2) Management of P-IRT; 3) Training in business financing and marketing management; 4) Production and marketing assistance. This activity can help partner problems. Expected targets and outcomes, namely: 1) Abon modified tuna and banana heart products that have P-IRT, high protein content is expected to be a solution to KEP nutritional problems; 2) The method of business financing management and appropriate marketing management has been achieved. In addition to directly, partners have marketed their products through social networks and resellers. Partners experience additional turnover and number of buyers; 3) Mastery of skills in the form of skills makes modification abon. This activity has an impact on up-dating science and technology in partners. Keywords: abon modified; tuna fish; banana blossom; optimization.

Abstract

SIMRS application is an important component in the application of medical record processing. In Balung Hospital, Jember, the application of SIMRS is still not running optimally, such as entering patient medical record data on SIMRS and the attitude of SIMRS users or admins who forget to enter patient data into SIMRS. This shows the attitude of users who lack awareness in using SIMRS. The purpose of this study was to analyze the acceptance of SIMRS, especially among SIMRS users in Balung Hospital using the Technology Acceptance Model (TAM) approach. The method used is analytic with cross sectional approach with interview and observation techniques. Analysis of research data using univariate analysis and bivariate analysis with simple linear regression. The results of this study show, there was an effect of Perceived Ease of Use on Attitude Toward Using SIMRS, there was an effect of Perceived Ease of Use on Perceived Usefulness, there was no effect of Perceived Usefulness on Attitude Toward Using, there was an effect of Perceived Usefulness on Behavioral Intention to Use SIMRS, there was an effect of Attitude Toward Using SIMRS on Behavioral Intention to Use SIMRS, and there was an effect of Behavioral Intention to Use SIMRS on Actual Usage of SIMRS. The suggestion of this research is that there is a need for regular monitoring and evaluation of the medical record unit and SIMRS in the use of SIMRS based on the performance of the user

Abstract

Data tim pengelola Sistem Informasi Kesehatan (SIK) Lumajang Dinas Kesehatan Kabupaten Lumajang menyebutkan bahwa permasalahan dalam pemanfaatan SIK yaitu puskesmas atau pengguna yang belum siap dan menerima aplikasi SIK Lumajang. Hanya 11 dari 25 puskesmas yang telah memanfaatkan aplikasi SIK Lumajang sebagai aplikasi sistem informasi manajemen di puskesmas. Penelitian ini bertujuan untuk mengidentifikasi kesiapan pengguna dan pengaruhnya terhadap penerimaan aplikasi SIK Lumajang sebagai aplikasi sistem informasi manajemen puskesmas di Kabupaten Lumajang dengan konsep teori TRI dan TAM. Penelitian ini adalah penelitian kuantitatif dengan rancangan cross sectional, dilakukan di 11 Puskesmas yang telah memanfaatkan aplikasi SIK Lumajang. Pengambilan data menggunakan metode wawancara dengan kuesioner dan dokumentasi SIK Lumajang bulan September 2016 sampai dengan Oktober 2017. Sampel sebanyak 54 responden diambil dengan teknik multistage random sampling. Data yang dikumpulkan berupa tingkat kesiapan pengguna sistem informasi manajemen di puskesmas sebagai variabel bebas, serta data persepsi kegunaan, persepsi kemudahan, sikap terhadap penggunaan teknologi, niat perilaku menggunakan teknologi, perilaku atau penggunaan teknologi sesungguhnya sebagai variabel terikat. Uji bivariat dianalisis menggunakan uji regresi linier sederhana. Hasil penelitian menunjukkan bahwa pengaruh antar variabel penelitian seluruhnya berpengaruh positif dan signifikan (p<0.05) sesuai dengan teori yang ada. Variabel dengan pengaruh terbesar terdapat pada niat perilaku menggunakan terhadap penggunaan teknologi sesungguhnya. Kata kunci: kesiapan pengguna; puskesmas; SIK lumajang

Abstract

The highest prevalence of malnutrition cases is in Gorontalo District which is 3.2% in 2018 and the highest public health center in the Tibawa area is 4.2% in cases of malnutrition and underfives carrying a red line are 4.7% and 2 toddlers do not gaining weight for 3 times in a row The results of growth and development monitoring were carried out at the posyandu until September 2018. This study aims to analyze the effect of nutrition assistance with media booklets to improve the knowledge and practices of hamlet heads in determining the nutritional status of children under five. The research method was quasi-experimental with pre -post, sample selection with a total sample of 77 respondents,37 interventions and 40 controls.The results showed that there were no significant differences in knowledge of the treatment and control groups before the pre intervention (p=0.857). There were significant differences in knowledge before and after the intervention group pos1 (p=0,000) and post2 (p=0,000) whereas in the post1 control group (p=0,000), there were significant differences at post2 (p=0.395) there were no significant differences. There was a difference after the intervention between the treatment and control groups post1 (p=0,000) and Post2 (p=0,000). The practical variables did not differ significantly between the treatment and control groups before the intervention (p=0.921).In the treatment group between pre and post1 there was a significant difference (p=0.046).Between pre and post2 (p=0.157),there was no significant difference. In the control group there was no significant difference between pre and post1 (p= 0.157) and post2 (p=0.564).After the intervention there were no significant differences in the treatment and control groups at post1 (p=0.557) as well as post2 (p=0.583). The conclusion was booklet mentoring significantly increased the knowledge and practice in of mothers in determining the nutritional status of toddlers.There is no effect of practice after post2.

Abstract

Hypertension is a serious medical condition that significantly increases the risk of heart illness, kidney disease, etc. The prevalence of non-communicable disease, including hypertension, has increased year by year and became the spotlight for policy makers. Hypertension requires long and continuous treatment which causes high cost of illness. The high economic burden of hypertension is also causes y several factors such as gender, length of stay, severity, comorbid and payment method. This study used descriptive method that aimed to perceive the cost of illness of hypertension inpatients in RSD Balung Kabupaten Jember. The result showed that the average direct cost was IDR 2,171,919, the average of indirect cost was IDR 478,118, and total average cost of illness for patient with hypertension in this study was IDR 2,650,037. Total average cost of illness for hypertensive patients could be said to be quite large considering that the patient’s income were lower than City’s Minimum Salary in Kabupaten Jember, IDR 2,355,662.90.

Abstract

Background: In 2014, National Health Insurance Program (BPJS Kesehatan) implemented Back-Referral Program (BRP) to facilitate access to health care for patients with chronic diseases in stable conditions. However, the program did not run well at District General Hospital of Balung which had the lowest back-referral program (6 participants) in 2015-2016.Aim: The study aims to examine the BRP at Balung District General Hospital of Jember in 2017.Methods: This study is designed as qualitative case study. The informants consisted of head of treasury verification, secretary of JKN service controlling, officers of local government’s free care scheme, coordinator of internal and neurological disease, specialists in internal and neurological diseases, officers of Social Security Agency for Health, officers of patient eligibility verification, and participants of Back-Referral Program. The study was conducted from October to December 2017.Results: Results show that the attendance of BPJS Kesehatan officers has no contribution to the services of BPJS Kesehatan in the hospital. Participants also find it difficult to follow the steps of BRP due to unavailability of drugs at primary healthcare centers. Moreover, neurologists have not written any referral recommendation since 2016, so there was no patient admittance.Conclusions: The BRP at Balung District General Hospital did not achieve its target (<5 cases/week). In the contrary, the average number of patients referred to the hospital was 86 patients per week. Balung District General Hospital should implement Standard Operating Procedure for letters signed by responsible physician for patients and evaluate the BRP. Keywords: National Health Insurance, Back-referral program, Hospital, Social security.

Abstract

Provider Initiated Testing and Counseling (PITC) merupakan salah satu upaya dalam menurunkan kasus penularan Human Immunodeficiency Virus (HIV) dari ibu ke janin atas inisiatif pemberi layanan kesehatan. Bidan berperan langsung dalam memberikan layanan konseling sesuai dengan prosedur PITC, namun hasil studi pendahuluan di Puskesmas Kendalsari menunjukkan bahwa sebanyak 67% responden belum mendapat informasi dasar HIV dan layanan PITC dari bidan, sedangkan informasi dasar ini harus ibu dapatkan untuk meningkatkan pemahaman ibu hamil tentang HIV sebelum dilakukannya tes HIV. Penelitian ini bertujuan untuk mengetahui hubungan antara peran bidan sebagai motivator, edukator, dan fasilitator dengan pengetahuan ibu hamil dalam layanan PITC ibu hamil. Jenis penelitian ini adalah analitik dengan pendekatan cross sectional. Sampel penelitian ini yaitu 64 ibu hamil yang telah dites HIV di Puskesmas Kendalsari Kota Malang. Teknik pengumpulan data menggunakan metode wawancara dan dokumentasi. Instrumen penelitian ini adalah kuesioner. Teknik analisis data yang digunakan adalah analisis univariat dan bivariat. Hasil penelitian ini menunjukkan bahwa tidak terdapat hubungan antara peran bidan sebagai motivator dan fasilitator terhadap pengetahuan ibu hamil, dan terdapat hubungan peran bidan sebagai edukator terhadap pengetahuan ibu hamil.Provider-Initiated Testing and Counseling (PITC) is an effort to reduce cases of transmission of the Human Immunodeficiency Virus (HIV) from mother to fetus at the initiative of health service providers. Midwives play a direct role in providing counseling services according to the PITC procedure, but the results of a preliminary study at the Kendalsari Health Center showed that as many as 67% of respondents did not receive basic information on HIV and PITC services from midwives, while this basic information must be obtained by mothers to increase pregnant women's understanding of HIV before taking an HIV test. The purpose of this study is to determine the relationship between the role of midwives as motivators, educators, and facilitators and pregnant women's knowledge of PITC services. This type of research is analytic with a cross-sectional approach. The sample for this study was 64 pregnant women who had been tested for HIV at the Kendalsari Health Center in Malang City. Data collection techniques using interviews and documentation The research instrument is a questionnaire. The data analysis technique used is univariate and bivariate analysis. The results of this study indicate that there is no relationship between the role of the midwife as a motivator and facilitator and the knowledge of pregnant women, but there is a relationship between the role of the midwife as an educator and the knowledge of pregnant women.

Abstract

Pandemic covid19 should be handled together between government and community. The government programs will not be optimal without supporting the community. The problems were the gap in information and education about covid19, especially how to prevent the covid19, vaccine, health protocols, and how to make and use eco hand sanitizer. The aim of the community service was to give the education and covid19 cadres of the neighborhood association. The method of the community service was education intervention, counseling, and training about covid19, vaccine, health protocols (mask using, physical distancing, washing the hands, social distancing, reducing mobility), and making and using the eco hand sanitizer. Pre-test and post-test were conducted to evaluate these community service programs. The result showed that there was an increase in knowledge about the covid19, vaccine 25,6%; implemented the health protocols (19,1%). The media that using for education, counseling, and training were acceptable (65,2%). The community, especially cadres were active in joint the whole activity such as the installation of educational banners in various locations and corners of the village, as well as in the distribution of masks and the manufacture of hand sanitizer. The obstacle in this activity is adjusting the training time for cadres and the community because they still have to work.

Abstract

Masalah gizi stunting di Indonesia tergolong berat dengan prevalensi sebesar 30,8% sedangkan di Jawa Timur sebesar 32,81%. Diversifikasi pangan lokal yang melimpah dan bergizi tinggi seperti ikan tuna, keluwih, dan daun kelor, dapat menjadi solusi penanganan stunting. Kegiatan pengabdian masyarakat berwawasan lingkungan berbasis agro, eco-marine, dan zero waste penting dilakukan dengan mengolah daging ikan tongkol tuna menjadi produk abon dan kepalanya menjadi kerupuk ikan. Puger merupakan kecamatan penghasil ikan terbesar di Kabupaten Jember, namun prevalensi stunting juga tinggi. Terdapat kelompok pengolah dan pemasar (POKLAHSAR) ikan “Duta Tongkol” di Desa Kasiyan Kecamatan Puger yang memproduksi abon ikan tongkol, namun harganya relatif mahal dan hanya bisa dijangkau masyarakat menengah keatas. Oleh karena itu, diperlukan penambahan keluwih serta daun kelor untuk menekan biaya produksi dan meningkatkan nilai gizi. Metode yang digunakan pada pelaksanaan kegiatan ini seperti pendidikan kepada sasaran, transfer ipteks, pelatihan, dan advokasi. Presentase keluwih yang digunakan sebesar 20% mengacu pada penelitian sebelumnya. Hasil pre dan post-test sebesar 0,005 menunjukkan terdapat pengaruh kegiatan terhadap peningkatan pengetahuan sasaran. Setelah pelatihan, pemasaran produk tidak hanya dilakukan secara offline, namun juga online. Produk abon modifikasi yang telah mendapat P-IRT dengan kadar protein tinggi diharapkan dapat menanggulangi masalah stunting. Metode manajemen pembiayaan usaha dan pemasaran produk yang tepat telah tercapai, sehingga memberikan keterampilan baru dalam pembuatan produk abon modifikasi dan dampak up-dating Iptek kepada mitra.

Abstract

Maternal and Child Health (MCH) is unresolved health problems in Indonesia. Mother and child are family members who need attention and priority in delivery of health. In 2012, there were six provinces that were focus of government in resolve problems of maternal mortality, one of them was East Java. Jember District is one of district in East Java that is still having problems with MCH. The addition of the budget in increasing maternal and child health has not been running optimally in Jember. Therefore, it is necessary to research with Health Account approach related to MCH in Jember District. This type of research was descriptive research. The unit of analysis in this research was the institution that carried out direct or indirect activities on the MCH program in 2018 with an appropriate budget focus at the regional level. The results of research showed that expenditure patterns on every program were widely used for investment expenditure and the budget was prioritized for investment expenditure in indirect activities. The advice given is that Jember District Government develops policies directed at healthy paradigm development and builds an integrated financial management information system, and must also create a legal protection to institutionalize implementation of Health Accounts at distrcit level.
