Abstract

Highlights: 1. Atrial fibrillation in the presence of an accessory pathway may present with confounding electrocardiographic signs. 2. The clinical recognition of WPW may be hindered by the presence of pre-excited AF. - Case Summary: A 46-year-old man without known past medical history was referred to the tertiary hospital emergency department after being found collapsed at home. There are no any complaints of any headache, nausea, chest pain, or paralysis of extremities before losing consciousness. His chest examination to bilateral auscultation was clear. Chest X-ray, routine blood work, and transthoracic echocardiography did not reveal any abnormalities. The initial heart rate before referral was 250-300/min and the ECG shows irregular wide QRS complex tachycardia. The ECG after cardioversions shows sinus rhythm 86 bpm with WPW type A pattern. The patient got intravenous amiodarone and intravenous lidocaine during the transfer. And the ECG on arrival at the emergency department, the heart rate was 50-150 bpm irregularly and the ECG shows atrial fibrillation with a narrow QRS complex. Discussion: Rapid anterograde accessory pathway conduction during atrial fibrillation (AF) can result in sudden cardiac death. During pre-excited AF, delta waves as the key feature of Wolff-Parkinson-White (WPW) syndrome might be obscured. We should keep in mind the diagnosis of pre-excited AF in patients presenting with irregular and wide complex tachycardia.

Abstract

Heart disease is a heart condition that sometimes causes a person to die suddenly. One indication is a rhythm disorder known as arrhythmia. Multi-class Arrhythmia Detection has followed: QRS complex detection procedure and arrhythmia classification based on the QRS complex morphology. We proposed an edge device that detects QRS complexes based on variance analysis (QVAT) and the arrhythmia classification based on the QRS complex spectrogram. The classifier uses two-dimensional convolutional neural network (2D CNN) deep learning. We use a single board computer and neural network compute stick to implement the edge device. The outcomes are a prototype device cardiologists use as a supporting tool for analysing ECG signals, and patients can also use it for self-tests to figure out their heart health. To evaluate the performance of our edge device, we tested using the MIT-BIH database because other methods also use the data. The QVAT sensitivity and predictive positive are 99.81% and 99.90%, respectively. Our classifier's accuracy, sensitivity, predictive positive, specificity, and F1-score are 99.82%, 99.55%, 99.55%, 99.89%, and 99.55%, respectively. The experiment result of arrhythmia classification shows that our method outperforms the others. Still, for r-peak detection, the QVAT implemented in an edge device is comparable to the other methods. In future work, we can improve the performance of r-peak detection using the double-check algorithm in QVAT and cross-check the QRS complex detection by adding 1 class to the classifier, namely the non-QRS class.

Abstract

Background: Uremic patients are in a pro-oxidant state and show an increased level of asymmetric dimethylarginine (ADMA), which is due to increased PRMT1 activity and reduced dimethylarginine dimethylaminohydrolase (DDAH) as degradation enzymes. Reactive oxidant species may play an important role in increasing the action of PRMT1 and in inhibiting the action of DDAH. Albuminuria and ADMA are closely correlated with progression of cardiovascular disease in chronic kidney disease (CKD) patients as well as indicators for decreasing renal function. Although ACEIs and/or ARBs reduced albuminuria in CKD patients, the results are still conflicting. Several factors in these patients may play important roles in the mechanism of albuminuria such as oxidative stress. The antioxidant N-acetylcysteine may prove to have beneficial therapeutic effect, because it can reduce oxidative stress as shown by evidence in humans, and subsequently increase ADMA. The objective of the present study is to explore the contribution of the antioxidant N-acetylcysteine (NAC) to the decrease of ADMA and albuminuria in non-diabetic CKD patients. Material and Methods: Patients with non-DM CKD stage 1–4 with albuminuria were randomized to receive ACEI and/or ARB alone (control group) or with antioxidant NAC 600 mg orally twice a day (treatment group). Observations were performed for 3 months to measure ADMA and albuminuria before and after-treatment. 80 patients in total 40 in the control group and 40 in the treatment group were used. Results: After oral treatment with NAC, the plasma level of ADMA in the treatment group increased from 0.604 µmol/l to 0.689 µmol/l, whereas ADMA level in the control group exhibited a higher increase from 0.561 µmol/l to 0.743 µmol/l. The increases in these groups were significantly different (p < 0.02). Moreover, the level of albuminuria was reduced from 148.12 µg/mg • cr to 132.7 µg/mg • cr in the treatment group, and from 75.25 µg/mg • cr to 71.85 µg/mg • cr in the control group. The difference was significant (p < 0.001). Conclusion: The anti-oxidant N-acetylcysteine can be used as adjuvant therapy to inhibit the progression of CKD in patients by decreasing the ADMA level and albuminuria.

Abstract

This study aims to determine the effect of Life Reinsurance claims on the company's profit and loss statements after the Covid-19 pandemic in 2020 and to find out the company's strategy in dealing with problems that occur in the income statement of PT. Reinsurance Indonesia Utama (Persero) during the 2020 Covid-19 pandemic. This study uses a quantitative descriptive approach, namely collecting the necessary data from the company PT. Reinsurance Indonesia Utama (Persero) in the accounting division area related to the company's 2020 profit (loss) report in the Covid-19 pandemic conditions. The results of this study prove that there is an increase in the claim expense to the company, the increase in claims is a result of the poor performance of several products, the majority of which are contributed by the Credit Life Insurance (AJK) business in 2020 due to the impact of the pandemic and there are Covid-19 claims such as cover vaccines, self-isolation, and Covid-19 treatment. As a result of the increase in claims expense, it will directly affect the value of the company's Underwriting - Net (HUB) results, high claims payments will affect the company's solvency level as a result of which the company's profit achievement in 2020 has decreased when compared to the company's profit income in 2019.

Abstract

Game merupakan hiburan yang digemari semua kalangan baik dari anak-anak hingga orang dewasa, industri game memiliki ratingnya tersendiri dengan dibatasi oleh usia para pemainnya mulai dari usia 3 tahun hingga ke semua usia. Game ini memiliki jalan cerita dan visualisasi yang akan membuat pemain penasaran dengan akhir ceritanya. Game “The Meet” merupakan permainan game horor dimana pemain diharuskan menyelesaikan objektif untuk segera tidur kedalam rumah barunya. Dalam game ini menggunakan softwareUnreal Engine 4, dengan menerapkan kecerdasan buatan FSM (Finite State Machine). FSM digunakan pada karakter NPC (Non playable Character) yaitu karakter digerakan oleh kecerdasan buatan yang digunakan untuk mendukung game tersebut. Seperti karakter Kuntilanak dan Pocong yang digunakan tiga hal yaitu : Roam, Chase Player dan Patrol. FSM (Finite State Machine) pada pintu dan kunci terdapat dua hal : tertutup dan terbuka, FSM (Finite State Machine) pada nyawa karakter terdapat dua hal : berkurang dan bertambah. FSM (Finite State Machine) pada saklar terdapat dua hal : menyala dan mati. Pada pengujian metode FSM (Finite State Machine)yang diterapkan pada musuh sebagaian besar responden menyatakan bahwa fungsi dari masing-masing state telah berjalan dengan baik, artinya presentase penilaian sekisar 60 user didapatkan hasil yang menunjukan bahwa 74% menyatakan Baik, 22.66% menyatakan Cukup dan 3.33% menyatakan Kurang.

Abstract

The discourse on Islamic educational philosophy has its roots in the integration of Greek reasoning with Islamic thought, giving rise to prominent Muslim philosophers. This study examines the philosophical perspectives of Imam Abu Bakar Al-Razi and Imam Ibn Bajjah on education, focusing on their contributions and relevance in modern and non-modern contexts. Both figures, known for their intellectual legacies in philosophy and science, have provided unique insights into the interplay between reason, religion, and human development. This qualitative study employs a library research approach, utilizing data from books, journals, and scholarly articles relevant to the educational philosophies of Al-Razi and Ibn Bajjah. Data were analyzed descriptively to identify their educational principles and their broader implications for contemporary educational practices. The findings reveal that Al-Razi emphasizes the role of reason and naturalism in education, advocating for a balance between intellect and spiritual growth. His "Five Eternal Principles" (al-Mabadi' al-Qadimah al-Khamsah) underline the integration of divine, material, and rational dimensions in human development. Meanwhile, Ibn Bajjah offers profound insights into individual intellectual growth through his concepts of 'uzlah (solitude) and the development of reason ('aql) as a means to achieve human perfection. His metaphysical and ethical frameworks highlight the necessity of aligning education with the innate potential of the soul, fostering intellectual independence and moral rectitude. The study concludes that the philosophies of Al-Razi and Ibn Bajjah remain pertinent in addressing contemporary educational challenges. Al-Razi’s emphasis on reason and naturalism aligns with modern scientific inquiry, while Ibn Bajjah’s advocacy for intellectual autonomy resonates with the need for critical thinking in education. The implications of their thought encourage a holistic educational approach that bridges traditional wisdom and modern methodologies

Abstract

Salah satu perusahaan manufaktur yang bergerak di bidang otomotif khususnya perakitan roda empat berlokasi di Sunter , Jakarta Utara. Perusahaan otomotif ini sedang melakukan pengendalian kualitas dikarenakan adanya customer claim. Tingkat kesadaran perusahaan beserta karyawan dalam menjaga kualitas dan mengurangi pemborosan biaya operasional merupakan kunci sukses perusahaan untuk menjadi perusahaan berstandar internasional. Dengan banyaknya varian produk yang harus dibuat menjadi tantangan pada saat proses inspection untuk mencegah mengalirnya produk NG. Tujuan dari penelitian ini adalah untuk mengurangi pemborosan biaya operasional yang berasal dari customer claim, meningkatkan kualitas dan memenuhi kepuasan pelanggan. Metode yang digunakan dalam penelitian ini adalah metode QCC, karena metode ini yang paling efektif dalam membantu membuat dokumentasi customer claim yang konsisten, jelas dan terstruktur. Penelitian ini menemukan adanya permasalahan dalam proses inspection di tahun 2020 sebanyak 1 kali dan 2021 sebanyak 1 kali customer claim wire unclamp yang harus ditangani oleh perusahaan. Hasil penelitian ini mendapatkan adanya pengurangan customer claim wire unclamp di tahun 2022 menjadi tidak ada kasus. Sehingga perusahaan dapat menekan pemborosan biaya operasional.

Abstract

Background: Coronavirus disease 2019 (COVID-19) poses a significant global challenge due to its highly transmissible nature and many positive cases and deaths. Myriad factors are known to affect the outcome of COVID-19 patients. Therefore, identifying predictive factors is essential for developing strategies to reduce mortality. Methods: An analytical observational retrospective study was conducted at Universitas Airlangga Hospital Surabaya from January to June 2021. A total of consecutive sampling was performed on all patients in the intensive care unit. Thirty-one variables were included and analysed using a two-step statistical analysis: univariate and multivariate logistic regression. Results: The mortality rate among 116 patients was 61.2%. The univariate analysis showed that dyspnoea, hypoxia, hyperglycaemia, and fever at admission were positively correlated with mortality. The multivariate analysis showed that fever (adjusted odds ratio [OR]=5) and hyperglycaemia (adjusted OR=8) at admission were independent predictors of mortality. Conclusion: Patients with hyperglycaemia or fever at admission face an increased likelihood of dying from COVID-19, with risks elevated by eight times and five times, respectively.

The purpose of this study was to determine the quality of multiple-choice items on the end-of-semester summative questions on PAI subjects at SDN Rowokangkung 01 Lumajang, the analysis was carried out qualitatively in terms of material aspects, construction aspects and language aspects. This type of research is qualitative research with descriptive-analytical method, while the research data is obtained through documentation method. From this study it can be concluded that in terms of the three aspects, the questions have met the criteria of good and feasible use.
