Abstract

Abstrak Inisiasi menyusu dini atau sering disingkat dengan IMD merupakan suatu kesempatan yang diberikan kepada bayi segera setelah lahir dengan cara meletakkan bayi di perut ibu, kemudian dibiarkannya bayi untuk menemukan puting susu ibu dan menyusu hingga puas. Proses ini dilakukan paling kurang 60 menit (1 jam) pertama setelah bayi lahir. Dalam upaya menurunkan angka kematian bayi dan mensosialisasikan pentingnya manfaat dari inisiasi menyusu dini, perlu diupayakan program yang dapat meningkatkan IMD. Agar program tersebut tepat sasaran dan sesuai dengan target yang ingin dicapai maka harus diketahui terlebih dahulu faktor yang berhubungan dengan pelaksanaan IMD. Tujuan yang akan dicapai dari penelitian ini adalah untuk menganalisis faktor-faktor yang berhubungan dengan pelaksanaan inisiasi menyusu dini oleh bidan di rumah sakit Prof Dr Aloei Saboe Kota Gorontalo. Hasil penelitian menunjukkan bahwa ada hubungan antara usia, lama kerja, pengetahuan, sikap dan pelatihan dengan pelaksanaan inisiasi menyusu dini di Rumah Sakit Prof Dr Aloei Saboe Kota Gorontalo.   Kata kunci:  Usia, Lama Kerja, Pengetahuan, Sikap, Pelatihan. Abstract Early breastfeeding initiation or often abbreviated with IMD is an opportunity given to infants shortly after birth by putting the baby in the mother's abdomen, then let the baby to find the mother's nipple and suckle until satisfied. This process is carried out at least 60 minutes (1 hour) first after the baby is born. In an effort to reduce infant mortality and promote the importance of the benefits of early breastfeeding initiation, it is necessary to improve the IMD program. In order for the program on target and in accordance with the targets to be achieved it must be known in advance of factors related to the implementation of the IMD. Objectives to be achieved from this research is to analyze factors associated with the implementation of early breastfeeding initiation by a midwife at the hospital Prof. Dr. Aloei Saboe Gorontalo. The results showed that there is a relationship between age, duration of work, knowledge, attitude and training with the implementation of early breastfeeding initiation in the Hospital Prof. Dr. Aloei Saboe Gorontalo. Abstract

Introduction Maternal Mortality Rate (MMR) and Infant Mortality Rate (AKB) are still national health problems. Based on data from the Dumbo Raya Health Center in 2022–2024, there was an increase in AKI cases from 0 to 2 cases, and AKB from 5 to 11 cases, caused by IUFD, congenital disorders, diarrhea, and leaky heart. Comprehensive obstetric care aims to provide continuous services from pregnancy to birth control. Method This research method uses a descriptive method through a case study approach. The subject used in this study was Mrs. F.D, aged 24 years, who was accompanied from 30 weeks of gestation. Results During pregnancy, mothers complain of back pain and frequent urination, which is successfully overcome through education, light massage, warm compresses, and changes in fluid consumption patterns. Labor takes place through sectio caesarea due to macrosomy indications. During the postpartum period, SC wound monitoring, uterine involution, lochea, breastfeeding education, personal hygiene, and family planning counseling are carried out. The mother chooses to use MAL contraceptives and plans to use an IUD. Neonatal visits are carried out three times, the baby is in normal condition. Conclusion The results of care from pregnancy to postpartum show that services take place optimally, characterized by the absence of complications. This comprehensive midwifery care is expected to increase maternal awareness in implementing good health practices and support maternal and infant health as a whole and sustainable.

Abstract

Increased progesterone levels during pregnancy can lead to haemorrhoids, which require proper management to minimize risks to both the mother and fetus. Since haemorrhoidectomy is not a suitable option for pregnant women, alternative treatments with minimal side effects are needed. This study aimed to evaluate the effectiveness of Phaleria macrocarpa suppositories in reducing the degree of haemorrhoids in pregnant women. This study employed a true experimental design with a cross-sectional approach. The study population consisted of pregnant women selected through purposive sampling. Data were collected using observation sheets and analyzed using the Wilcoxon test. The results study show that among pregnant women who used standard suppositories, 23 participants (71.8%) showed no change in the degree of haemorrhoids, while 9 participants (28.8%) experienced a reduction in severity. In contrast, all 32 participants (100%) who used Phaleria macrocarpa suppositories experienced a decrease in haemorrhoid severity. Statistical analysis showed a significant difference (p < 0.05) between the two groups, with a p-value of 0.0013, indicating that Phaleria macrocarpa suppositories were significantly more effective in reducing haemorrhoids compared to standard suppositories. The conclusion is Phaleria macrocarpa suppositories effectively reduce the severity of haemorrhoids in pregnant women and may serve as a safer alternative to conventional treatments. Further research with larger sample sizes and long-term follow-ups is recommended to validate these findings and explore the potential mechanisms of Phaleria macrocarpa in haemorrhoid treatment.
