Abstract

Introduction: Traumatized immature permanent teeth can cause pulp necrosis, leading to imperfect root formation, and thus resulting an open apex. The apical opening of the immature teeth is mostly large and thus may lead to difficulties in performing a root filling. In such cases, apexification is mandatory before doing root canal treatment. The aim of this case report was to inform that apexification and direct veneer was suitable to treat Ellis class IV fracture with necrotic pulpCase Report: 21-year-old female came to the Dental Conservation Clinic RSGM Prof. Soedomo Faculty of Dentistry UGM with chief complaint of right central incisive maxilla fracture and tooth discoloration. Radiographic examination showed an open apex of tooth 11 with a diffuse apical radiolucent area. Root canal treatment and apexification were carried out using circumferential technique and Mineral Trioxide Aggregate (MTA). Vertical warm obturation was performed, followed by intra coronal bleaching in the next visit. Final restoration was carried out using direct composite veneer. Post treatment radiographic showed a 1 mm diameter of radiolucent area at the apex in a good healing process. Conclusion: Apexification using MTA can be used to treat an open apex tooth. 1Student of Specialist Programme in Conservative Dentistry, Dentistry Faculty, Gadjah Mada University 2Department of Conservative Dentistry, Dentistry Faculty, Gadjah Mada University

Abstract

Broken files in the root canal system which are not removed may interfere with thorough cleaning of the root canals. As a result, they cannot be hermetically obturated. This imperfect cleaning can leave necrotic tissue which can potentially cause failure in root canal treatment. The purpose of this paper is to describe the management of retrieval of broken files and re-treating root canals in previously treated mandibular left second molars. The retrieval of the broken file used conventional techniques combined with a Satelec ultrasonic scaler to remove the files from the root canal. It was operated under an endodontic microscope, and after retreatment of the root canal, the tooth was restored with final restoration of endocrown. Retrieval of fractured files using conventional techniques combined with Satelec ultrasonic scaler and the root canal re-treatment followed by final endocrown restoration could restore all four tooth functions properly.

Abstract

Sodium hypochlorite is used as irrigation solution during root canal preparation because of its antibacterial effect. However, it degrades dentinal collagen and generates oxygen free layer on dentin surfaces that might prevent penetration of sealer. Sodium thiosulfate is an antioxidant and has been reported to neutralize that side effect. The objective of this study was to determine the effect of concentration and application time of sodium thiosulfate irrigation on push-out bond strength of root canal obturation material. This study used extracted human mandibular premolars single root post extraction. The root canals were prepared using crown down technique up to file F3. The samples were divided into four groups. Groups IA and IB were irrigated with sodium thiosulfate 5% for 3 and 5 minutes, while Groups IIA and IIB were irrigated with sodium thiosulfate 10% for 3 and 5 minutes. The control group was irrigated with saline for 3 and 5 minutes. The root canals were obturated with epoxy resin sealer and gutta percha, and were incubated for 7 days. Samples were horizontally sectioned into 2 mm thickness in apical third and were tested with push-out technique using universal testing machine. The adherence failure of samples was observed using stereo microscope. Data were analyzed with two-way ANOVA. There was a significant effect between concentration of sodium thiosulfate on push-out bond strength (p < 0.05), but application time showed the same effect (p > 0.05). The application of antioxidant resulted in an increase in the push-out bond strength of root canal obturation material, with 10% sodium thiosulfate demonstrating the highest push-out bond strength.

Abstract

Abstract

Background: Free radicals produced by hydrogen peroxide after teeth bleaching which diffuse through the enamel and dentinal tubules can trigger inflammation of the pulp.  To overcome this negative side effect, exogenous antioxidants have been proposed.  This research was conducted to determine the effect of natural antioxidants, namely 5% and 10% green tea, which was applied on rat molars teeth that were bleached using 40% hydrogen peroxide. Methods: Fifteen molar teeth of male Wistar rats were bleached using 40% hydrogen peroxide on their occlusal surface. Furthermore, the control group (Group I) rinsed using warm distilled water, while in Group II and Group III, the teeth were rinsed using 5% and 10% green tea extract gel. Wistar rats were sacrificed on the fifth day after treatment, followed by hematoxylin-eosin staining. Histological examinations were observed under a light microscope using 400x magnification and the number of the neutrophil were counted. Result: One-way ANOVA test showed that there was a significant effect of green tea extract gel concentration on the number of neutrophils (p 0.05).  Post hoc test results showed that there were significant differences in all pairs of groups compared (p 0.05). Conclusion:  Group III which was treated using10% green tea extract gel has the lowest number of neutrophil compared to Group II which was treated using 5%  green tea extract hel and the control group.

Abstract

ABSTRACTIntroduction: Root canal treatment (RCT) of mandibular third molars is often considered challenging due to anatomical variability, limited accessibility, and difficulty achieving proper isolation. As a result, extraction is frequently chosen. This case report outlines a successful single-visit RCT on a right mandibular third molar with a curved canal and Class II disto-occlusal caries. Case Report: A 30-year-old female presented with a large cavity on her right lower molar and requested a restorative treatment. Three weeks earlier, devitalization and temporary restoration had been performed by another dentist. Clinical examination revealed no periapical lesion, and percussion testing was negative. Radiographic analysis showed a slight curvature in the distal canal. A pre-curved K-File and the One Curve System were used for instrumentation. The canal was obturated in a single visit, followed by restoration using direct composite and short fiber-reinforced composite. Matrix and PTFE tape were utilized for proper adaptation in the distal area. Conclusion: With accurate diagnosis, understanding of anatomical complexity, and appropriate technique, single-visit RCT on mandibular third molars is achievable. This case supports the viability of conservative treatment for necrotic third molars with asymptomatic apical periodontitis, avoiding unnecessary extraction. Keywords single visit, root canal treatment, third lower molar, curved canal, composite restoration.

Abstract

In cases of pulp necrosis, infection products can spread into the periapical tissues, triggering an inflammatory response that stimulates the proliferation of the epithelial cell rests of Malassez. As the epithelial mass enlarges, the central cells—farther from the nutritional supply, undergo necrosis, leading to fluid accumulation. The cyst continues to expand due to cyst wall proliferation and increased hydrostatic pressure in the lumen. This pressure causes the accumulated fluid to spread and compress the epithelial lining of the fibrous capsule. This report presents the successful management of a radicular cyst in the maxillary anterior region through apicoectomy. The patient, who sustained a traumatic injury and fractured the tooth at the age of 10, later experienced pain and gingival swelling. Clinical examination revealed a Class IV Ellis fracture and discoloration of tooth 11. Radiographic evaluation showed a well-defined, rounded radiolucency approximately 20 mm in diameter in the periapical region of teeth 11 and 12, extending toward tooth 13. The treatment plan included disinfection and healing enhancement (DHE), root canal therapy, apicoectomy, and intracoronal bleaching. Apicoectomy proved to be a reliable treatment with a favorable prognosis for anterior teeth.

Abstract

Background: Sodium hypochlorite produces free oxygen and free radicals that cause damage to dentin collagen. Combination with Sodium thiosulfate with the aim of improving root canal hygiene and restoring the oxidizing effect on the dentin surface. The apical third of the root canal is the most difficult part, due to its narrower anatomy, curvature and frequent branching of the root canal. Method: The study used 30 specimens which were divided into 3 groups, group I was irrigated with 2.5% NaOCl, 5% sodium thiosulfate and saline, group II was irrigated with 2.5% NaOCl, 10% sodium thiosulfate and saline and group III was irrigated with 2.5% NaOCl and saline. The tooth length 14 mm measured from apical to crown. The crown down technique was prepared using the M3 Pro gold file. Observation of the level of cleanliness of the root canal was carried out using a Scanning Electron Microscope (SEM) with 5000x magnification at apical third . The results of micrographic photographs are then given a score of 1-4.Result: Irrigated with 5% and 10% sodium thiosulfate showed no significant difference. Conclusion: The differences is not significant, 2,5% irrigation with sodium thiosulfate resulted in a higher cleanliness apical third of the root canalthan 2,5% NaOCl irigation with saline
