Abstract

ABSTRACT Acute respiratory failure (ARF) is a disorder of the pump function of the respiratory muscles or dysfunction of the lungs. Patients with respiratory failure use mechanical ventilation for breathing apparatus. However, mechanical ventilation often interferes with airway clearance and coughing. So intervention is needed to remove mucus from the airways. This study aimed to identify interventions that can be performed to reduce and clear mucus in patients with respiratory failure who are attached to a mechanical ventilator. This study used the narrative review method with the keywords “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. The databases used include Scopus, PubMed, EBSCO, Oxford. The results of 5 articles were obtained which consisted of various effective interventions for removing secretions such as ventilator hyperinflation mode, head down tilt position, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), and chest physiotherapy. This intervention is effective in expelling mucus, increases expiratory flow bias which has the potential to increase secretions removal maneuvers, facilitates mucus cleaning during suctioning, and increases aeration in the dorsal lung without affecting blood gas analysis results. These various interventions can be a choice of solutions to help problems that arise due to the use of mechanical ventilation. Keywords: Acute Respiratory Failure, Mucus Suction, Ventilator  ABSTRAK Gagal napas akut (GGA) merupakan gangguan fungsi pompa otot pernapasan atau disfungsi pada paru-paru. Penderita gagal napas menggunakan ventilasi mekanik untuk alat bantu napas. Namun, ventilasi mekanis sering mengalami gangguan pembersihan lendir saluran napas dan batuk. Sehingga diperlukan intervensi untuk mengeluarkan lendir dari saluran napas. Penelitian ini bertujuan untuk mengidentifikasi intervensi-intervensi yang dapat dilakukan dalam menurunkan dan membersihkan lendir pada pasien dengan gagal napas yang terpasang ventilator mekanik. Penelitian ini menggunakan metode narrative review dengan kata kunci “patient with ventilator OR patient with mechanical ventilation OR mechanically ventilated patient OR patient undergoing mechanical ventilation” AND “intervention OR strategies OR best practice OR treatment OR therapy OR program OR management” AND “mucus clearance OR airway clearance OR sputum clearance”. Database yang digunakan antara lain scopus, pubmed, EBSCO, oxford. Didapatkan hasil 5 artikel yang terdiri  dari berbagai intervensi yang efektif untuk mengeluarkan sekret seperti mode hiperinflasi ventilator, posisi head down tilt, manual chest compression (MCC), mechanical insufflation-exsufflation (MI-E), dan fisioterapi dada. Intervensi ini efektif dalam pengeluaran mukus, meningkatkan aliran ekspirasi bias yang berpotensi meningkatkan manuver pengeluaran sekret, mempermudah pembersihan mukus ketika suction, dan meningkatkan aerasi di bagian dorsal paru tanpa mempengaruhi hasil analisa gas darah. Berbagai intervensi ini dapat menjadi pilihan solusi untuk membantu permasalahan yang ada akibat penggunaan ventilasi mekanik. Kata Kunci: Gagal Napas Akut, Penghisapan Lendir, Ventilator

Abstract

Aims: Student engagement (SE) is a significant factor for success in online learning. The low of SE will impact students' academic achievement. Purpose: This study aimed to describe SE in online learning at the Faculty of Nursing, Universitas Padjadjaran.  Methods: This study was quantitative descriptive research. The population was undergraduate nursing students (N=862). The sample criteria were that respondents were active students of the Faculty of Nursing, Universitas Padjadjaran for the 2020/2021 period from the Jatinangor and Pangandaran campuses. The sampling technique was total sampling with a response rate of 64,3% (n=554). Data were collected using the Online Community and Student Engagement Scale. Data were analyzed using descriptive statistics and presented in frequency distribution tables.  Results: The results showed that more than half of students (52.7%) had a low category of SE, and nearly half (47.3%) have shown a high category of SE in online learning. Students had low levels of Community Building with the Instructor, Community Building with Classmates, and Engagement with Learning (57.4%; 58.3%; and 51.8%, respectively). The SE level that the respondent currently has has the potential to be increased along with the ongoing process during the lecture period. Conclusion: More than half of students had a low category of SE in online learning this could have a negative impact on student achievement and educational completion rates. Therefore, intense interactions through discussions between students and lecturers, constructive feedback, and motivational support are highly recommended to be provided by the faculty and peers for improving the SE.

Abstract

Several studies identified the effect of family visits on reducing patient and family anxiety levels. However, until now there has been no review that synthesizes the results of these studies on this population. This review aimed to identify the effect of family visit management on the anxiety level of patients and families in the ICU. This Study used a scoping review method using CINAHL, PubMed, Science Direct and search engines Google Scholar. The keywords used in English were "family visitation OR visits" AND "anxiety disorders OR anxiety OR generalized anxiety disorder" AND "ICU OR intensive care unit OR critical care." The criteria for selecting the articles included full-text articles in English and published in 2013-2023. The study selection process used the PRISMA-ScR flow chart. We found 11 articles discussing the management of family visits to the anxiety of ICU patients. Based on the results of the analyzed studies, it shows that there are several types of scheduled visit management (in and out hours), flexible family visitations, limited visits, regular visits, and online methods. In general, participants in the studies analyzed showed reduced levels of anxiety, and most studies indicated that flexible visits were positively related to satisfaction with the services provided in the ICU. Furthermore, it is advised that the visiting policies in ICU be modified and that families be given the option to participate in patient care through scheduled attendance and involvement. Abstract

Aims: The development of technology to facilitate the management of schizophrenia is still very limited so that many people don’t know the benefits and efficacy of using technology as an intervention in schizophrenia patients. Objective:This literature review aimed to describe and determine the efficacy and benefits of using the technology used in schizophrenia patients. Method: The method used was a narrative review by combining articles from the PubMed, EbscoHost, Taylor and Francis, and Google Shcolar databases. The keywords used were “adults” OR “middle aged” AND “telenursing” OR “telemedicine” OR “telehealth” AND “schizophrenia patient”. Result: From these 6 articles, it was founde that there were several perceived benefits of using technology as an additional intervention, such as medication adherence, involvement in recovery and symptom control, and early detection of cognitive function in schizophrenic patients. Conclusion: Interventions using technology such as telehealth, telenursing, and telemedicine can be supportive interventions for the care of schizophrenic patients, especially to assist recovery or rehabilitation. Nursing Implications: The existence of this study can provide an overview to nurses regarding what technology-based support interventions can be given to schizophrenic patients along with their benefits.

Abstract

ABSTRACT Acute Respiratory Distress Syndrome (ARDS) is one of the causes of most patients being admitted to the Intensive Care Unit (ICU) and has the potential to cause death due to inadequate gas exchange due to fluid in the alveoli. One of the interventions given is prone positioning to increase lung expansion. This study examines the benefits and impact of prone positioning intervention in ARDS patients. This literature review uses the Preferred Reporting Items for Systematic Reviews (PRISMA) guidelines with the keywords "ARDS, effectiveness, and prone positioning" using the Science Direct, PubMed, and Google Scholar databases. The inclusion criteria were full text in English or Indonesian with no restriction a publication year. 4,164 articles were collected according to keywords, and five articles that had gone through elimination and met the inclusion criteria were selected. Prone positioning in ARDS patients can reduce mortality and increase PaO2/FiO2. However, there are drawbacks, such as causing pressure ulcers in certain areas and does not reduce the potential complications of VAP. Prone positioning benefits patients by reducing mortality and increasing PaO2/FIO2. In practice, prone positioning must be supported by the presence of trained medical personnel, and possible complications must be reduced. Keywords: ARDS, Effectiveness, Prone Positioning  ABSTRAK ARDS atau Acute Respiratory Disstress Syndrom merupakan salah satu penyebab kebanyakan pasien masuk Intensive Care Unit (ICU) dan berpotensi menyebabkan kematian sebab pertukaran gas inadekuat akibat cairan di alveolus, salah satu intervensi yang diberikan adalah prone positioning untuk meningkatkan ekspansi paru. Penelitian ini bertujuan untuk melihat manfaat dan dampak dari intervensi prone positioning pada pasien ARDS. Literature review ini menggunakan panduaan Preferred Reporting Items for Systematic Reviews (PRISMA) dengan kata kunci “ARDS, efektivitas, dan prone positioning”dengan menggunakan database Science Direct, PubMed, dan GoogleSchollar. Kriteria inkluasi yaitu fulltext berbahasa Inggris atau Indonesia dan tidak ada retrisksi tahun publikasi. Terkumpul 4.164 artikel sesuai dengan kata kunci dan terpilih 5 artikel yang telah melalui eliminasi dan sesuai dengan kriteria inklusi. Prone posisitioning pada pasien ARDS dapat mengurangi angka kematian dan meningkatkan PaO2/FiO2, meskipun terdapat kekurangan seperti mengakibatkan pressure ulcer di bagian tertentu dan tidak mengurangi potensikomplikasi VAP. Prone positioning bermanfaat untukpasien sebab mengurangi angka kematian dan meningkatkan PaO2/FIO2. Dalam praktiknya, prone positioning harus ditunjang dengan kehadiran tenaga medis terlatih, dan mengurangi komplikasi yang mungkin terjadi. Kata Kunci: ARDS, Efektivitas, Prone Positioning

Abstract

ABSTRACT Cardiac Arrest or cardiac arrest is a factor in the cause of death from heart disease which is the number one cause of death in the world. The incidence of cardiac arrest can be reduced by providing pre-hospital assistance by bystanders, namely Cardio Pulmonary Resuscitation (CPR). Knowledge is very closely related to education where it is expected that someone with higher education will be directly proportional to the wider knowledge. This study aimed to describe the level of knowledge of Adult Cardiac Arrest Resuscitation in Cardiac Arrest victims by Padjadjaran University students. This study used a descriptive quantitative method. The population in this study were active students of Padjadjaran University, Jatinangor campus (N=22.389). The sampling technique used proportional sampling with a total sample of 393 student using the Taro Yamanee calculation. The instrument used was an instrument from previous study with a validity test (a ≤ 0.05), a Cronbach’s alpha reliability test of 0.73%. The data were analyzed using the univariate analysis method. The findings showed that the level of knowledge about CPR by Padjadjaran University students is in a good category (66.4%). The source of information that respondents receive about CPR comes from social media (27%). From the findings of this study, it is expected that health education can be implemented by spreading information evenly on social media. Moreover, health education can also be considered for inclusion in the learning curriculum for all faculties. Keywords: Cardiac Arrest, Cardiopulmonary Resuscitation, Knowledge  ABSTRAK Henti jantung menjadi faktor penyebab kematian dari penyakit jantung yang menjadi penyebab kematian nomor satu di dunia. Kejadian henti jantung dapat diturunkan dengan memberikan pertolongan pra rumah sakit oleh bystander yaitu Resusitasi Jantung Paru (RJP). Pengetahuan sangat erat kaitannya dengan pendidikan di mana diharapkan seseorang dengan pendidikan tinggi, maka akan berbanding lurus dengan pengetahuan yang semakin luas. Penelitian ini bertujuan untuk mendeskripsikan tingkat pengetahuan mahasiswa Universitas Padjadjaran tentang Resusitasi Henti Jantung Dewasa pada korban Henti Jantung. Penelitian ini menggunakan metode deskriptif kuantitatif. Populasi dalam penelitian ini adalah mahasiswa aktif Universitas Padjadjaran Kampus Jatinangor (N=22.389). Teknik pengambilan sampel menggunakan proporsional sampling dengan jumlah sampel sebanyak 393 orang menggunakan perhitungan Taro Yamanee. Instrumen yang digunakan adalah instrumen dari penelitian sebelumnya dengan uji validitas (a ≤ 0,05), uji reliabilitas Cronbach’s alpha sebesar 0,73%. Data dianalisis dengan menggunakan secara analisis univariat. Hasil penelitian menunjukkan bahwa tingkat pengetahuan tentang CPR mahasiswa Universitas Padjadjaran berada pada kategori baik (66,4%). Sumber informasi yang diterima responden tentang CPR berasal dari media sosial (27%). Dari temuan penelitian ini, diharapkan pendidikan kesehatan dapat dilaksanakan dengan menyebarkan informasi secara merata di media sosial. Selain itu, pendidikan kesehatan juga dapat dipertimbangkan untuk dimasukkan dalam kurikulum pembelajaran untuk semua fakultas. Kata Kunci: Henti jantung, Pengetahuan, Resusitasi Jantung Paru

Abstract

Background: Hope is an essential psychological resource that enables children with cancer to cope with the stress and uncertainty of long-term treatment. However, research focusing on the level of hope among children with Acute Lymphoblastic Leukemia (ALL) in Indonesia or Southeast Asia remains limited, as most studies have centered on adults or on depression and anxiety rather than hope. Understanding hope among pediatric cancer patients is crucial for developing psychosocial interventions that enhance emotional resilience and treatment adherence. Purpose: to describe the levels of hope among children under 18 years old with ALL who were undergoing chemotherapy. Method: This was a quantitative descriptive study involving 66 respondents selected through convenience sampling. Data were collected using the Children’s Hope Scale (CHS), consisting of six items that assess agency and pathway thinking. Ethical approval and parental consent were obtained prior to data collection. Descriptive statistics were applied to analyze respondents’ demographic characteristics and hope scores. Result: The findings showed that most respondents were aged 8–12 years (56.1%), male (59.1%), and diagnosed with high-risk ALL (89.4%). More than half (54.5%) demonstrated a high level of hope, while 45.5% had a low level of hope. Younger children exhibited higher hope levels compared to adolescents. The mean hope score was 22.9 ± 3.95, with a median of 23 and a score range of 16–32, indicating a moderate to high overall level of hope. Conclusion: Most children with ALL in this study maintained a moderate to high level of hope during chemotherapy. These results emphasize the importance of assessing hope as part of psychosocial care. Strengthening hope through supportive communication and family-centered interventions can enhance children’s PWB and promote better adherence throughout the treatment process.

Abstract

ABSTRACT Type II Diabetes Mellitus (DM) is a chronic metabolic disorder that affects not only physical health but also patients’ psychological well-being. Depression is one of the most prevalent psychological complications associated with DM, which can worsen glycemic control, reduce treatment adherence, and impair self-care behaviors. Despite its clinical importance, depression among diabetic patients often remains underrecognized, particularly in regional healthcare settings. This study aimed to describe the levels of depression among patients with Type II Diabetes Mellitus at the Internal Medicine Polyclinic of Majalengka Regional Hospital and to analyze its distribution based on age, gender, and duration of illness. A quantitative descriptive design was employed involving 90 respondents selected through convenience sampling who met the inclusion criteria: adults (≥18 years old) diagnosed with Type II DM and willing to participate. Data were collected using the Beck Depression Inventory-II (BDI-II), a validated international instrument with high internal consistency (Cronbach’s Alpha 0.8). Data were analyzed univariately using frequency and percentage distributions to illustrate the levels of depression according to demographic characteristics. Most respondents experienced moderate depression (40%), followed by mild (24.4%) and severe depression (18.9%), while only 16.7% were categorized as normal. Higher levels of depression were observed among female respondents, older adults (60 years), and those who had lived with diabetes for ≤5 years. Depression represents a significant psychological comorbidity among patients with Type II DM, underscoring the need for integrated and routine mental health screening in diabetic care. Implementing holistic interventions based on a bio-psycho-social-spiritual framework is essential to improve emotional well-being, self-management, and overall quality of life among diabetic patients at both primary and hospital-level healthcare settings. Keywords: Type II Diabetes Mellitus, Depression, Beck Depression Inventory-II, Mental Health, Chronic Disease Nursing.  ABSTRAK  Diabetes Mellitus (DM) tipe II merupakan penyakit metabolik kronis yang tidak hanya berdampak pada kondisi fisik, tetapi juga aspek psikologis pasien. Depresi menjadi salah satu komplikasi psikologis yang paling sering terjadi dan dapat memperburuk kontrol glikemik, menurunkan kepatuhan terhadap pengobatan, serta menghambat proses penyembuhan. Meskipun demikian, gejala depresi pada pasien DM sering kali tidak teridentifikasi secara dini, khususnya di fasilitas kesehatan daerah. Penelitian ini bertujuan untuk menggambarkan tingkat depresi pada pasien Diabetes Mellitus tipe II di Poliklinik Penyakit Dalam RSUD Majalengka, serta menganalisis distribusinya berdasarkan usia, jenis kelamin, dan lama menderita DM. Penelitian ini menggunakan desain deskriptif kuantitatif dengan teknik convenience sampling terhadap 90 responden yang memenuhi kriteria inklusi, yaitu pasien DM tipe II berusia ≥18 tahun dan bersedia berpartisipasi. Pengumpulan data dilakukan menggunakan instrumen Beck Depression Inventory-II (BDI-II) yang memiliki reliabilitas tinggi. Analisis data dilakukan secara univariat dengan distribusi frekuensi dan persentase. Sebagian besar responden mengalami depresi sedang (40%), diikuti oleh depresi ringan (24,4%) dan depresi berat (18,9%), sementara hanya 16,7% yang berada dalam kategori normal. Tingkat depresi lebih tinggi ditemukan pada perempuan, kelompok usia lanjut (60 tahun), dan pasien dengan durasi penyakit ≤5 tahun. Depresi merupakan komorbiditas yang umum terjadi pada pasien DM tipe II dan perlu mendapatkan perhatian klinis yang setara dengan aspek fisik penyakit. Diperlukan skrining psikologis rutin dan intervensi holistik berbasis bio-psiko-sosial-spiritual untuk meningkatkan kesejahteraan emosional dan kualitas hidup pasien diabetes di tingkat pelayanan primer maupun rumah sakit. Kata Kunci: Diabetes Mellitus Tipe II, Depresi, Beck Depression Inventory-II, Kesehatan Mental, Keperawatan Kronis.
