Abstract

HIV/AIDS stage 4 is the stage where the HIV/AIDS patients have low immunity protection against infections, which can led to coinfections and complications. This case report presented an evaluation of the diagnosis and treatment of an HIV/AIDS stage 4 patient with cerebral toxoplasmosis coinfection and SIADH complications. A man (47 years, 35 kg), married with two children, complaining of weakness, nausea, vomiting, weight loss, and low appetite. Sodium level were measured, showing a 117 mg/dL level that continued to decrease throughout the patient's treatment. Five days later, the patient lost his consciousness with a GCS score of 224, indicating severe brain injury, and was diagnosed with cerebral toxoplasmosis based on the result of head CT-SCAN with contrast. On day 6, the patient was tested positive for HIV and diagnosed with stage 4 with an absolute CD4 count of 4 cells/μl. Therapy was provided by giving pyrimethamine-clindamycin therapy for cerebral toxoplasmosis, followed by Tenofovir, Lamivudine, and Evafirenz as antiretroviral therapy. Treatment for hyponatremia was done by administration of 3% NaCl and tolvaptan. The patient started experiencing an improvement in consciousness after the 10th day of medication, and sodium levels fluctuated throughout the treatments. Abstract

Sorghum is a drought-tolerant crop that can contribute to Indonesia’s food security and income generation. However, this potential often fails to improve farmers’ income and welfare unless accompanied by economic feasibility, particularly in marketing practices. This study examined market efficiency and partnership models in sorghum development in Daerah Istimewa Yogyakarta and Central Java Provinces. The research analyzed the marketing cost, marketing margin, farmers’ share, and partnership cultivation models of the sorghum. The data were collected through surveys, interviews, and observations to 120 sorghum farmers and 60 traders. The study utilized descriptive statistics, ANOVA to compare farmers’ share in the different marketing channels, and the Analytic Hierarchy Process (AHP) to select the partnership models. The results unveiled Channel II which consisting of farmers, village collector traders, traders outside the city or regency, and national companies, produced the highest marketing margin. Meanwhile, Channel III, comprising farmers, village collector traders, and the local feed industry, generated the highest farmers’ share. The study revealed that sole proprietorship partnerships were the farmers' dominant choice in the sorghum business. Nevertheless, the partnership model with feed industry had also formed in the research area.

Abstract

Cigarettes are one of the causes of health problems in the world. Smoking has been known to have a direct impact on reducing lung function. Smoking increases the risk of vitamin D deficiency. Vitamin D is a vital nutrient as a lung immunomodulator. Deficient levels will cause problems with lung health, especially in active smokers. This cross-sectional research using multivariate path analysis and the SEM-PLS method has three objectives. The direct influence of three independent variables, including obesity, smoking, and demographics. Regarding vitamin D status and lung function, analyzing the direct effect of vitamin D on lung function, and thirdly, analyzing the indirect one of the three independent variables on lung function through vitamin D levels. This research took time from October 2023 to January 2024 and involved 47 active smoker respondents whose vitamin D level status and lung function were measured. There was a significant direct effect of obesity level on vitamin D levels in the active smoker population (p<0.05;f-square=2.889). While demographic factors (p>0.05;f-square=0.030) and smoking frequency (p>0.05;f-square=0.003) did not have a direct significant effect. Demographic factors, obesity, and smoking frequency don’t have significant direct effect on lung function in active smokers (p>0.05).
