Abstract

Attention deficit hyperactivity disorder (ADHD) is the most common childhood neurobiologic disorder with main  symptom of inattention, hyperactivity, impulsivity and minimal occurrence in two different place settings, for example at home and at school. ADHD disorders will last sustainably from adolescence to adulthood. Mothers of ADHD children tend to be anxious, tired and desperate because they are often blamed by the environment for failing to educate children due to opposing childrens behavior, disobeying orders that result in ADHD children being rejected and experiencing low learning achievement. Mothers are also often must lose his job due to having to take care of his own ADHD child.Methods: Observational analytic study with cross sectional design of 30 mothers of ADHD children aged 6 to 12 years of Bina Karya primary school Surabaya by random sampling method. Assessment of ADHD by mothers using SPAHI (Skala Penilaiana Anak Hiperaktif Indonesia)  and maternal anxiety  assessment using Hamilton Anxiety Rating Scale questionnaire Results: there was significant correlation between maternal anxiety level  and severity of ADHD with score p = 0.003 and r = 0.530 Conclusion: The degree of maternal anxiety is related to the severity of ADHD.

Abstract

Epidural hematoma akut merupakan suatu keadaan yang urgent dan serius. Tata laksana standar berupa diagnosa dini dan pembedahan untuk evakuasi secara cepat. Disisi lain, kasus ini memiliki risiko deteriorisasi yang cepat dan kematian. Hanya sedikit pasien epidural hematoma bersifat asimptomatis yang dapat diterapi konservatif dengan close observation. Kami melaporkan satu kasus epidural hematoma frontalis sinistra traumatik. Kasus ini merupakan kasus jarang yang secara cepat resolusi spontan dari epidural hematom dalam rentang 3 hari. Beberapa variasi mekanisme yang mungkin terjadi sebagai mekanisme resolusi tersebut yang kita dibahas dalam literatur pada terapi epidural hematoma secara konservatif. Indikasi dilakukan operasi boorhole drainase pada pasien ini adalah nyeri kepala yang menetap. Kata Kunci: Resolusi dini, Epidural Hematoma, Boorhole Drainase

Abstract

Brain tumor is a condition affecting many people worldwide.Patients often had similar functional neurological symptoms even if the type of tumors diagnosed were entirely different at a later stage.Some of the neurological symptoms were tingling in the feet, changes in eyesight,tremors in the extremities, headaches or migraines and more. Patients who suffer from brain tumor go through a screening process to identify the cause of the problem. Factors such as age and gender was observed in this study which affected the data distribution of brain tumors. This study aimed to find the data distribution of the brain tumor cases in RSUD Dr Soetomo. The results obtained showed that the highest number of brain tumor found were meningiomas followed by unspecified brain neoplasms as the second highest and adenomas being the third highest tumor type found in RSUD Dr Soetomo. The total number of female patients with brain tumor were higher than that of males. The median age of the patients was found to be 45. The highest number of tumor cases were seen in the adult age group followed by teens, children and elderly. The benign tumor type is found to be higher in number as compared to malignant. The number of supratentorial tumors are also found to be higher than infratentorial overall. Keywords: age; gender; brain tumor

Abstract

Background: Psychiatric assistance is required in the integrated handling of physical health disorders in the field of other disciplines known as CLP. A collaborative approach to the field of psychiatry with other scientific disciplines in accordance with WHO health criteria that includes physical and mental health for improving health and quality of life. Case presentation: A boy, 8 years, was consulted from the Ophthalmology Department with Right eye cataract juvenile, and left eye ptisis bulbi to give pre-operative psychological assistance for eye assessment and diagnostics. Patient also complained the blurred vision since last year and cannot see anything since 10 months ago. The current condition represents the patient cannot speak, often screams, likes to injure his head and scratch his face. Patients treated in the pediatric’s ward with global developmental delay. Patients were also consulted to the child psychiatry section with childhood autism, audiology, nutrition and metabolic diseases of children, pediatric neurology, and finally collaboration with the Surabaya City Health Office for a follow-up management plan. Conclusions: Child psychiatrist as a liaison, interacting with multidisciplinary medical or rehabilitation teams, families, schools, community and local city government are beneficial in the management of child disorders and improving family wellbeing. Abstract

Pendahuluan: Meningioma merupakan tumor otak primer yang berasal dari jaringan arakhnoid. Angka kejadian meningioma di Indonesia menunjukkan peningkatan dari tahun ke tahun. Selain mutasi gen supresor tumor, meningioma dapat dipengaruhi oleh faktor eksternal diantaranya usia dan jenis kelamin. Tujuan: Mengetahui hubungan antara usia dan jenis kelamin dengan angka kejadian meningioma pada pasien di RSUD Dr. Soetomo, Surabaya, Indonesia tahun 2018 berdasarkan derajat keganasannya. Metode: Penelitian ini merupakan studi observasional analitik cross-sectional retrospektif dari data sekunder lembar hasil pemeriksaan patologi anatomi pasien yang dirawat inap melalui SMF. Ilmu Bedah Saraf, RSUD Dr. Soetomo tahun 2018; data yang tidak lengkap akan dieksklusi. Analisis data dilakukan menggunakan uji Chi squre (SPSS 25) dengan tingkat signifikansi p <0,05. Hasil: Didapatkan 45 pasien meningioma yang sesuai dengan kriteria inklusi, dengan mayoritas perempuan (71%) pada kelompok usia 45 – 49 tahun (22,2%), WHO grade I (82%) dengan tipe histopatologi transisional (49%). Terdapat hubungan yang signifikan antara angka kejadian meningioma dengan kelompok usia tertentu (r = 0,718, p = 0,025), dan dengan jenis kelamin (r = 0,441, p = 0,002). Rasio perempuan dibanding laki- laki sebesar 1:2,36. Kesimpulan: Usia dan jenis kelamin berkorelasi erat dengan angka kejadianmeningioma.

Abstract

Highlight: Many studies conclude that the prognosis of gliomas can be determined using the DWI sequence and ADC value on MRI The correlation between DWI and ADC values with glioma grading is related to tumor cellularity, nuclear-cytoplasmic ratio, and extracellular space, which affect the movement of water molecules.   ABSTRACT Introduction: Glioma grading is crucial to know its prognosis. Magnetic resonance imaging (MRI) is used as a preoperative examination that contains diffusion-weighted imaging (DWI) sequences confirmed by an apparent diffusion coefficient (ADC) value that helps assess tissue based on water diffusion. Objective: To prove the relationship between DWI and ADC values with cerebral glioma grading in patients at Dr. Soetomo General Academic Hospital in 2016—2020. Methods: This retrospective study collected medical records and MRI files in DICOM (Digital Imaging and Communications in Medicine) format. Gender, age, tumor histopathology, and glioma grading were collected. DWI and ADC values were obtained using the RadiAnt DICOM Viewer application. The data were analyzed using descriptive and analytical statistics. The chi-square test was used to analyze the relationship of DWI with glioma grading, and the spearman rank test was used to analyze the relationship of ADC value with glioma grading. Results: The majority of 35 patients were male (54.3%), aged 31–40 years old (22.9%), and the most common histopathology was glioblastoma (37.1%), WHO grade IV. On DWI, most low-grade glioma (LGG) patients showed unrestricted diffusion, and most high-grade glioma (HGG) patients showed restricted diffusion. The ADC value of HGG was lower than the ADC value of LGG. Statistical tests showed a relationship between DWI and glioma grading (p<0.05) and a relationship between the ADC value and glioma grading (p<0.05). Conclusion: There was a relationship between DWI and ADC with glioma grading in Dr. Soetomo General Academic Hospital patients for the period 2016—2020

Abstract

Brain tumors were estimated to occur at 6.4 per 100,000 people each year. The incidence rate of brain tumors in children is reported to be 5.7% out of every 100,000 children, while in adults, the reported rate is significantly higher at 29.9% out of every 100,000 people. Banyuwangi Regency has an area of 5,782.50 km2 with a population in 2018 of 1,609,677 people with a density of around 278 people/km2. With such a large population, early detection of brain tumors is important for the public to understand. Early detection activity programs from community health centre need to be held with the hope that health workers will be able to provide education and information regarding early symptoms and signs of brain tumors as well as risk factors for brain tumors for prevention. Increasing public knowledge about alerting the early symptoms of brain tumors is one strategy that can be used to reduce death and disability rates due to brain tumors. The aim of this community service program is to increase the knowledge of the lay public and health workers regarding early detection and early treatment of brain tumors in Banyuwangi Regency. Pre- and post-test questionnaires were given before and after the material to evaluate the increase in knowledge about brain tumors for lay people and health workers in Banyuwangi Regency. The results obtained a significant increase (p < 0.01) between the pre-test and post-test scores so that this community service increases the knowledge of the community in Banyuwangi Regency.

Abstract

Objective: To determine the effect of hormonal contraceptive exposure on the development of meningioma. Materials and Methods: This case–control study, conducted in 2016, included all patients diagnosed histopathologically with meningioma between 2012 and 2013 and treated at Dr. Soetomo General Hospital, Surabaya, Indonesia. Medical record data from these patients were collected and compared with a control group consisting of non-meningioma patients who underwent contrast-enhanced head CT scans and direct interviews. A total of 101 cases and 101 controls were analyzed. Data were evaluated using univariate logistic regression analysis. Results: Patients with a history of hormonal contraceptive use had a 12.31-fold higher risk of developing meningioma (p = 0.000). In this study, women using monthly injectable contraceptives or oral contraceptive pills demonstrated a lower risk of meningioma compared to those using three-month injectable contraceptives. Participants who had used hormonal contraception for more than 10 years had an 18.216-fold increased risk of developing meningioma (p = 0.000). Histopathological analysis revealed no significant association between hormonal contraceptive history and meningioma subtype distribution; however, descriptive data indicated that the transitional type was the most frequent histopathological subtype among the case group. Conclusion: There is a significant association between hormonal contraceptive use and the occurrence of meningioma, particularly with the use of three-month injectable hormonal contraception and long-term use exceeding 10 years. No significant association was observed between meningioma histopathological grade and a history of hormonal contraceptive exposure.

Abstract

Increased intracranial pressure is a further effect of brain injury due to structural damage and osmotic and water imbalances (Edema). Mannitol works in the proximal tubules and mannitol in the absorption of tubular cells by the mechanism of pinocytosis. The fluid transfer will draw fluid into the intracellular, so that the cell will be switched and broke. This phenomenon is referred to as the phenomenon of "Nephrosis Osmotic", in which mannitol administration may occur as a result of accumulation of drugs in the kidneys due to prolonged exposure to mannitol in the kidney and given dosage. The effects of osmotic diuresis occurring plus the dose and duration of mannitol administration are reported to cause renal function disorders (Scr and BUN). The aim of this study was to analyze changes in serum creatinine and BUN in patients with brain injury from before receiving mannitol therapy and during mannitol treatment. From the results of the study the number of patients who met the inclusion criteria, 32 patients. Serum creatinine, the initial average of 0.85 ± 0.17 mg / dl and the last day of the mean SCr 0.74 ± 0.30 mg / dl. While the mean BUN (Blood Urea Nitrogen) was 11.27 ± 2.75 mg / dl and the mean last day was 17.08 mg / dl ± 8.59 mg / dl. From Serum Creatinine and BUN data it can be concluded that there is no significant change.
