Abstract

LL-37 is an antimicrobial protein expressed by the CAMP gene in humans. This protein has various antibacterial, antiviral and anticancer effects. Expressing LL-37 as a heterologous protein in E. coli cells has its challenges. LL-37 is a peptide that is so small that it must be engineered with a fusion protein to increase its size solubility and prevent proteolysis of the target protein in cells. Factor Xa is the protease chosen to cleave LL-37 from its fusion protein in this research due to leucine binding factor Xa quite strongly. The aim of this study is, therefore, to express LL-37 as a fusion protein with Histaq_SUMO at the N-terminus linked to LL-37 at the C-terminus through Factor Xa cleavage site. Then, the fusion protein was cleaved by Factor Xa to obtain pure LL-37. In this study, LL-37 was produced by recombinant DNA technology, starting with the construction, expression, purification and cleavage of the fusion protein. The constructed genes consisted of 6xHis, SUMO, the factor X cleavage site, and LL-37, a total of 450 bp inserted in the pD451-SR vector plasmid. The results of this study yielded a SUMO_LL-37 protein with a molecular weight of approximately 17.34 kDa, which could be purified by Ni-NTA under native purification conditions. Based on ImageJ SUMO_LL-37 quantification, it was 1.65 µg/µL. LL-37 can be cleaved by factor Xa from SUMO with an enzyme-to-substrate ratio of 1:12.5 at 37°C with a 24-hour incubation time and results as much as 0.144 µg/µL. This article has a related Erratum.

Abstract

One of the essential and inseparable health services in hospitals is pharmaceutical services. Pharmaceutical services in hospitals are carried out by the Hospital Pharmacy Installation (Instalasi Farmasi Rumah Sakit / IFRS). The implementation of pharmaceutical services in hospitals must be supported by adequate, skilled, and competent human resources so that pharmaceutical service activities can run well and are high quality for patients. The lack of human resources for pharmaceutical services will result in excessive workload and reduced quality of pharmaceutical services. One method widely used to determine the number of staff needs is the Workload Indicators of Staffing Needs (WISN). Analysis of the need for pharmaceutical technical staff at Hospital X in Bandung City was carried out using the WISN method. The data used for the analysis were obtained from interviews, observations, and data collection on pharmaceutical services from the pharmacy installation of the Hospital X. The results of the WISN analysis showed that the need for pharmaceutical technical personnel for central pharmacy installations providing outpatient and inpatient services is 87 people, the total need for the emergency room pharmacy satellite is 6 people, the operating room pharmacy satellite is 6 people, and the pharmacy warehouse is 3 people. From the results of WISN ratio, the number of pharmaceutical technical staff available for the central pharmacy installation, emergency room pharmacy satellite, and operating room pharmacy satellite are still inadequate for the existing workload (WISN ratio < 1). In contrast, for the pharmaceutical warehouse, it is adequate (WISN ratio = 1). Therefore, Hospital X needs to consider adding existing pharmaceutical technical staff to support more optimal pharmaceutical services at the hospital.

Abstract

Antibiotik profilaksis adalah antibiotik yang diberikan pada pasien sebelum pembedahan untuk mencegah infeksi pascaoperasi. Penelitian ini bertujuan mengevaluasi penggunaan antibiotik profilaksis pada pasien bedah obstetri-ginekologi di salah satu rumah sakit Kota Bandung. Penelitian deskriptif noneksperimental ini menggunakan data retrospektif dari 200 pasien periode September 2023. Data yang telah terkumpul diolah dengan menghitung persentase pola penggunaan antibiotik dan parameterparameter yang meliputi jenis antibiotik yang digunakan, bentuk sediaan, dosis, rute pemberian dan waktu pemberian antibiotik. Hasil penelitian menunjukkan kesesuaian penggunaan jenis antibiotik profilaksis yaitu cefazolin sebesar 8,5%, kesesuaian bentuk sediaan dalam vial sebesar 100%, kesesuaian dosis yang digunakan sebanyak 2 gram sebesar 7%, kesesuaian rute pemberian antibiotik profilaksis yaitu melalui intravena kesesuaian sebesar 100%, kesesuaian waktu pemberian antibiotik 30–60 menit sebelum insisi kulit sebesar 68%. Implikasi klinisnya menunjukkan bahwa rendahnya kesesuaian dapat meningkatkan risiko infeksi pascaoperasi dan resistensi antibiotik, yang berdampak pada perburukan hasil klinis pasien dan biaya perawatan. Oleh karena itu, diperlukan intervensi berbasis edukasi dan pelatihan untuk meningkatkan kepatuhan tenaga medis terhadap pedoman penggunaan antibiotik profilaksis, sehingga dapat mendukung pencegahan komplikasi dan peningkatan kualitas pelayanan. Disimpulkan bahwa evaluasi penggunaan antibiotik di rumah sakit ini masih belum sesuai pedoman Permenkes RI No. 28 Tahun 2021.
