Abstract

In children, acute diarrhea and functional constipation (FC) are still more prevalent in Indonesia, especially in isolated islands, and need comprehensive management. This study aimed to determine the effect of community development from the Department of Child Health Faculty of Medicine, Universitas Airlangga, Surabaya, on Mandangin Island's public knowledge about acute diarrhea and functional constipation in children. This cross-sectional study was conducted on 29 subjects from Mandangin Island, Sampang, Indonesia on 17th February 2022. Subjects were given education and dissemination as community development by pediatricians via hybrid and could ask questions and discuss at the end of the presentation. The subject's knowledge was measured using a pre-and post-test questionnaire about children's acute diarrhea and functional constipation. Each questionnaire (diarrhea and functional constipation) consisted of 5 questions, with a minimum score of 0 and the maximum score of 100. The subject's knowledge was compared before and after the webinar using Paired T-Test and p<0,05 was measured as significant. We assessed the subject's age, gender, occupation, and last education. There was a significant increase in public knowledge about diarrhea (26,9±18,7; 55,1±24,3; p=0.000) and functional constipation pre-and post-webinar (24,8±18,2; 48,3±24,2; p=0.000). These findings support the importance of community development with education about acute diarrhea and functional constipation in children.

Abstract

Background: Ramadan fasting is one religious challenge for children with type 1 diabetes mellitus (T1DM) which required specific management and strict monitoring. Fructosamine, which reflects glycemic control within 2-3 weeks, is an ideal parameter of glycemic control in Ramadan fasting. This study aimed to analyzed the effect of Ramadan fasting on fructosamine level of children with T1DM.Subjects and Method: This prospective cohort study included children aged under 18 years old diagnosed wit T1DM for >6 months at Dr. Soetomo Hospital, Surabaya, Indonesia in Ramadan 1438 H (May-June 2017). The independent variable from this study was Ramadhan fasting and the dependent variables were fructosamine and HbA1c. The sampling technique was total sampling. Forty-two children were included and devided into 27 children in the fasting and 15 children in the non-fasting group. Fructosamine and HbA1c levels were measured before, mid- and after Ramadan and the comparison between groups were analyzed using the independent T-test.Results: The mean age of all the subjects was 8.95 (SD=3.2) years old, 47.6% were males. The mean (SD) HbA1c level of all the subjects was 10.42 (1.16) %. There was a mild reduction of mean fructosamine level after Ramadan in the fasting group. There was no significat differece I fructsamie level before (Mean= 556.8; SD= 180.39; p= 0.620), mid (Mean= 568.07; SD= 175.37; p= 0.520), and after Ramadhan between groups (Mean= 539.41; SD= 173.04; p= 0.410) respectively. The average fructosamine levels after Ramadan was positively and strongly related to HbA1c (r=0.69; p<0.001). There was no case of severe hypoglycemia and diabetic ketoacidosis in both groups. Conclusion: There is a reduction of mean fructosamine level after Ramadan fasting in children with T1DM although it isn’t statistically significant. Keywords: type 1 diabetes, children, fasting, Ramadan, fructosamineCorrespondence: Nur Rochmah. Abstract

An umbilical granuloma is a small, granulation tissue-filled swelling at the base of the umbilicus, commonly occurring in newborns. Various treatment options are available, including the topical application of common salt; however, its availability varies across healthcare facilities, and the most effective method remains uncertain. This serial case report describes infants presenting with umbilical masses accompanied by discharge. Physical examination revealed a flesh-like, protruding mass from the umbilicus, associated with yellowish and reddish discharge and surrounding normal skin. The granuloma was resolved entirely through the topical application of common salt. Our findings suggest that common salt is an effective, simple, and inexpensive treatment for umbilical granuloma. With regular monitoring, this approach could serve as a viable alternative to more advanced therapies.

Abstract

Effective management of Type 1 Diabetes (T1D), especially in children, requires continuous monitoring and care. Digital health applications have become vital in supporting routine T1D management, including insulin delivery, glucose monitoring, nutrition, and physical activity tracking. This study investigates factors influencing user acceptance of a digital diary app designed for children with T1D and their families. Using an extended Technology Acceptance Model incorporating Trust, Perceived Risk, Perceived Enjoyment, and Social Influence, a survey was conducted with 114 participants, including parents, physicians, and dietitians. Data were analyzed using Partial Least Squares Structural Equation Modeling. Findings indicate that perceived usefulness, trust, and social influence significantly affect users' attitudes and intentions to use the app, through the accepted hypothesis that considered path coefficients and p-values. Conversely, hypothesis that shows relation between perceived ease of use, enjoyment, and risk toward intention were rejected, showing unsignificant relations toward user intention to use. Furthermore, this study recommends prioritizing robust security features, fostering user trust, and engaging social networks to enhance digital health adoption in pediatric care. Future research should further explore the roles of perceived risk and enjoyment in sustaining long-term engagement
