Abstract

Background: Fasting during Ramadan could have negative effects on individuals with Type 2 Diabetes Mellitus (T2DM), as it would lead to hypoglycemia or hyperglycemia. Non-compliance with the recommended diet during fasting is one of the factors that can trigger these conditions. Knowledge about the appropriate diet plays a crucial role in successfully adhering to the diet.  Objective: The aim of this study is to assess the effect of providing T2DM education during Ramadan fasting on knowledge and fasting blood glucose levels.  Method: This study is an experimental research conducted using a randomized controlled trial design with a pretest-posttest control group design. The participants of this study were T2DM patients from Tlogosari Kulon Community Health Center and Kedungmundu Community Health Center who were enrolled in the Prolanis program and did not have any complications related to kidney failure or stroke. A total of 46 participants were divided into a treatment group and a control group. The treatment group received nutritional education on managing their diet while fasting during Ramadan. This education was provided one week prior to the start of Ramadan fasting. Data collection included assessing knowledge, food intake, nutritional status, and fasting blood glucose levels before and after Ramadan fasting. The collected data will be analyzed using univariate analysis to describe the research data and to test the impact of nutrition education on knowledge and fasting blood glucose levels levels. The independent t-test with a significance level of α=0.05 will be used for this analysis.Result: There was a significant difference before and after being given nutrition education in intervention group on increasing knowledge levels in the intervention group (p=0.029) also lowering the blood glucose levels                 (p = 0.000). Providing education to the intervention group increased the knowledge score from 68.56 +12.81 to 79.26+9.47. There was an increase in knowledge leves by 10.70+3.34 in the intervention group. Nutritioneducation also reduced fasting blood glucose levels in intervention gropus from 177.78 + 79.61 to 141.35 + 43.78. There was a lowering in the blood glucose levels by -36.43 + 71.89 in the intervention group.Conclusion: There is a relationship between fasting ramadhan diabetes education with knowledge and blood glucose levels.

Abstract

Konsumsi 5 porsi sayur dan buah menjadi salah satu rekomendasi penerapan gizi seimbang sebagai upaya pencegahan penyakit tidak menular dan risiko obesitas. Sayangnya, hanya 2,5% remaja Indonesia konsumsi 5 porsi sayur dan buah setiap hari. Hanya 30% remaja Kelurahan Pedurungan Tengah yang mengonsumsi sayur dan buah setiap hari dan cenderung memilih makanan cepat saji. Upaya pencegahan obesitas melalui edukasi gizi pada remaja menjadi salah satu alternatif metode pencegahan risiko penyakit tidak menular sejak dini. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk meningkatkan pengetahuan dan kebiasaan konsumsi sayur dan buah sebagai pencegahan obesitas remaja melalui kegiatan edukasi gizi dan pendampingan penerapan konsumsi sayur dan buah. Kegiatan pengabdian kepada masyarakat ini menggunakan pendekatan edukasi, demonstrasi dan praktik isi piring. Peserta kegiatan sejumlah 24 orang remaja yang didampingi oleh kader kesehatan di wilayah Kelurahan Pedurungan Tengah Kota Semarang. Hasil evaluasi pemahaman menunjukkan hasil yang sangat memuaskan sebesar 92% memiliki pengetahuan baik tentang konsumsi sayur dan buah. Hasil monitoring dan evaluasi menunjukkan bahwa seluruh peserta dapat mendemonstrasikan model isi piring makan harian. Edukasi ini dapat meningkatkan pengetahuan dan penerapan porsi konsumsi sayur dan buah. Program edukasi gizi ini sangat penting untuk mendukung kebiasaan pola makan seimbang sebagai upaya pencegahan obesitas.

Abstract

Introduction: People with type 2 diabetes mellitus (T2DM) who choose to fast during Ramadan are at increased risk of metabolic complications, particularly hypoglycemia and hyperglycemia. Ensuring comfort and a sense of safety is essential to minimize these risks. Pre-Ramadan education focusing on dietary management and regular health monitoring may support safer fasting practices. This activity aimed to assess the impact of pre-Ramadan education on glycemic control and fasting safety among individuals with T2DM.Methods: This community service activity was conducted among individuals with T2DM participating in the Prolanis program at Tlogosari Wetan Primary Health Center, Semarang City. Educational sessions on dietary management during Ramadan fasting were delivered using PowerPoint presentations, and booklets were distributed to 46 participants. Health monitoring was subsequently conducted through home visits during Ramadan to assess complaints and the risk of glycemic disturbances, including blood glucose measurements, in 27 participants.Results: Pre-Ramadan education was implemented during Prolanis activities in February and March 2024, followed by health monitoring throughout Ramadan. Reported symptoms suggestive of hypoglycemia included sweating, chills, tremors, headaches, and hunger, while symptoms related to hyperglycemia included polyuria, dehydration, nausea, vomiting, and fatigue. Blood glucose monitoring performed two hours before breaking the fast showed values ranging from 73 mg/dL to 235 mg/dL. Approximately 25% of participants achieved controlled blood glucose levels during Ramadan fasting. Comparison of pre- and post-Ramadan fasting blood glucose measurements indicated a tendency toward improved glycemic control during Ramadan.Conclusion: Pre-Ramadan education programs can help glycaemic control and monitor the risk of complications in people with T2DM who fast during Ramadan.
