Abstract

Secara global, populasi individu lanjut usia kian meningkat seiring waktu. Salah satu masalah yang sering dihadapi ketika beranjak lanjut usia adalah hipoalbuminemia. Prevalensi hipoalbuminemia lebih tinggi tidak hanya pada pasien rawat inap dan pasien sakit kritis, namun juga individu lanjut usia. Ulkus dekubitus merupakan tanda hipoalbuminemia dan merupakan masalah kesehatan yang signifikan di seluruh dunia, dan umum terjadi pada lansia. Pengelolaannya memakan biaya miliaran dolar per tahun, sehingga membebani perekonomian kesehatan. Albumin dan asupan nutrisi memainkan peran penting dalam penyembuhan luka dan merupakan faktor penting yang tidak dapat diabaikan dalam penyembuhan luka pada lansia. Upaya peningkatan kesadaran masyarakat terhadap albumin dalam PKM ini dilakukan melalui penyuluhan dan skrining atau deteksi dini penyakit pada kelompok lanjut usia. Pada kegiatan pengabdian ini digunakan tahapan kegiatan PDCA agar acara dapat berlangsung dengan baik dan efisien. Kegiatan ini mencakup 50 responden lanjut usia dengan rerata usia 75,92 (±11,14) tahun. Didapatkan 14% dari responden memiliki kadar albumin darah yang rendah (<3,5 g/dL). Dengan terlaksananya program ini diharapkan terdapat peningkatan kesadaran masyarakat terhadap pentingnya albumin dalam penyembuhan luka pada lansia, sehingga kedepannya terdapat peningkatan kualitas hidup komunitas lansia dan mengurangi beban ekonomi akibat biaya perawatan akibat masalah perawatan luka.

Abstract

Hemoglobin and hematocrit are important parameters in assessing anemia and other blood diseases. Hemoglobin transports oxygen from the lungs to the rest of the body, while hematocrit reflects the density of red blood cells in the blood. This community service activity was carried out at Kalam Kudus II High School, Duri Kosambi Village, Jakarta, involving 68 men and women of productive age. Screening activities are carried out by measuring hemoglobin and hematocrit levels using the For-A brand Point Of Care Testing (POCT) tool. The majority of respondents had normal hemoglobin levels (68.9%), while a small percentage experienced mild anemia (21.6%) and moderate anemia (9.5%). The average hemoglobin level was 13.28 g/dL and hematocrit 38.66%. The 18-35 year age group has the highest hemoglobin and hematocrit levels, while the 51-64 year age group has the lowest. Lifestyle factors, nutrition, genetics, and chronic medical conditions influence hemoglobin and hematocrit levels. Regular monitoring is important in detecting and managing anemia and improving public health.

Abstract

Child malnutrition, both over and undernutrition, is a serious problem affecting millions of children worldwide, with significant short- and long-term impacts on children's health and development. This research study the correlation between dietary status and emotional and behavioural problem among primary school children in Ciherang Village. This study is a cross-sectional design in which children who satisfy the specified criteria will undergo weight and height measurements. Their body mass index (BMI) will be calculated and compared to the BMI-for-age chart (BMI/Age). Additionally, the children will complete the Pediatric Symptom Checklist 17 (PSC-17) questionnaire. A total of 246 children from 5 schools in the Ciherang area met the inclusion criteria. The analysis revealed a significant correlation between the average difference in body mass index and the total PSC 17 score (p-value=0.026). This study identified a comparison between children's dietary health and emotional and behavioural disorders. This serves as a reminder of the significance of monitoring children's development and implementing a well-rounded diet to promote children's mental well-being.

Abstract

Diabetes mellitus (DM) occurs when blood sugar (glucose) levels are very high for a long period of time. If the glucose level is less than 126 mg/dL during fasting or less than 200 mg/dL two hours after eating, we diagnose diabetes. In low- and middle-income countries, DM affects as many as 422 million people and is one of the leading causes of death worldwide. Uncontrolled diabetes can cause severe complications such as cardiovascular disease, kidney damage, nerve damage, and vision problems. To prevent this complication, early detection through blood glucose checks is critical. This activity uses the PDCA (Plan-Do-Check-Act) method for diabetes screening in adults at PT. Narindo, North Jakarta. The results of blood sugar checks showed that 5 people (3.97%) had blood sugar levels above normal. Increasing public awareness and knowledge about the risks of diabetes and healthy living is very important to reduce the incidence of diabetes so that it can improve overall health outcomes.

Abstract

Urinary incontinence (UI) is a significant health issue among the elderly, affecting their quality of life. While its etiology is multifaceted, metabolic and nutritional markers such as diabetes panel parameters and albumin levels may play a role in its severity. However, the relationship between these biomarkers and UI remains underexplored. This study aims to investigate the association of diabetes panel components and albumin levels with the severity of UI in elderly individuals to identify potential predictors. A cross-sectional analytic study was conducted involving 93 elderly participants from Bina Bhakti Nursing Home. UI was assessed using the International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICIQ-UI SF). Metabolic markers were analysed using validated methods, including fasting glucose, HbA1c, fasting insulin, HOMA-IR, and albumin. Data were statistically analyzed using Spearman’s Rho and multiple regression analyses to determine correlations and predictive relationships. Significant correlations were observed between HbA1c (r = -0.284, p = 0.006) and albumin (r = -0.259, p = 0.012) with UI severity. Multiple regression analysis confirmed that HbA1c and albumin are significant predictors of UI, with lower levels associated with increased UI severity. Diabetes panel parameters, particularly HbA1c and albumin levels, are valuable predictors of UI severity in elderly individuals. These findings emphasize the importance of metabolic and nutritional health monitoring in managing and mitigating UI symptoms. Future longitudinal studies and intervention trials are recommended to validate these findings and explore therapeutic implications.

Abstract

High total cholesterol is a significant risk factor for coronary heart disease, especially in the productive age population. Early detection of cholesterol levels plays an important role in reducing the risk of health complications that can occur. This community service activity was carried out using the Plan-Do-Check-Act (PDCA) method which focused on health education and total cholesterol screening in 40 productive age individuals, aiming to increase awareness of the importance of cholesterol control. The education provided included information on risk factors for increased cholesterol, such as a diet high in saturated fat and lack of physical activity, as well as preventive measures that can be applied. Screening was carried out to detect individuals with high cholesterol levels so that appropriate preventive measures could be taken. The average total cholesterol level of participants was 224.55 mg/dL, with an average age distribution of 43.97 years. Counseling can increase participants' understanding of the risks of high cholesterol and the importance of a healthy lifestyle to maintain cholesterol levels. Education and screening of total cholesterol are expected to increase participants' awareness of the risks of high cholesterol and the importance of prevention through lifestyle changes. It is hoped that this activity can reduce the risk of cholesterol-related complications in the productive age group.

Abstract

ABSTRACT Low albumin and vitamin D levels in the elderly are often due to factors such as malnutrition, inflammation, and illness. A deficiency in albumin can affect the availability of vitamin D because it binds to albumin. Maintaining normal levels of albumin and vitamin D in the elderly has positive impacts on nutrition, healing, infection prevention, and metabolic function. This cross-sectional study aimed to investigate the relationship between vitamin D levels and albumin levels in the elderly. It was conducted at Santa Anna Elderly Care Home in November 2023. The variables in this study were vitamin D levels and albumin levels. Statistical analysis was performed using the Mann-Whitney test. Out of 47 respondents, the average age was 77.55 years. Mann-Whitney test results indicated no significant relationship between 25-hydroxyvitamin D levels and albumin levels in blood serum (p-value: 0.770). The correlation between these two variables showed a negative correlation, meaning that lower levels of 25-hydroxyvitamin D were associated with higher albumin levels. This is interesting because it contradicts the research hypothesis, suggesting that albumin levels may be influenced by factors outside the variables being studied. The correlation result of 0.044 falls into the category of very weak and cannot be considered a determining variable between two variables. Further analysis using the Mann-Whitney test revealed that there was no significant relationship or difference in mean values between 25-hydroxyvitamin D levels in the hypoalbuminemia and normal groups (p-value: 0.919), and there was no significant relationship or difference in mean values between albumin levels in the vitamin D deficiency and normal groups. Keywords: Albumin, Elderly, Vitamin D  ABSTRAK Albumin dan vitamin D yang rendah pada lanjut usia dapat terjadi karena faktor malnutrisi, inflamasi, dan penyakit. Kekurangan albumin dapat memengaruhi ketersediaan vitamin D, hal ini disebabkan vitamin D terikat pada albumin. Memiliki kadar albumin dan kadar vitamin D yang normal pada lanjut usia berdampak positif pada status nutrisi, penyembuhan, pencegahan infeksi, dan fungsi metabolik. Penelitian potong lintang ini bertujuan mengetahui hubungan kadar vitamin D dengan kadar albumin pada lanjut usia yang dilakukan di Panti Lansia Santa Anna pada November 2023. Variabel dalam penelitian ini adalah kadar vitamin D dan kadar albumin. Analisis statistik menggunakan uji Mann Whitney. Dari 47 responden, rata-rata usia adalah 77,55 tahun. Hasil uji Mann Whitney menyatakan tidak terdapat hubungan yang bermakna antara kadar 25-hidroksi vitamin D dengan kadar albumin dalam serum darah (p-value: 0,770). Hasil korelasi antara kedua variabel tersebut menunjukan korelasi negatif yang berarti semakin rendah kadar 25-hidroksi vitamin D maka akan semakin tinggi kadar albumin. Hal ini cukup menarik dikarenakan berlawanan dengan hipotesis penelitian yang berarti pula sebenarnya kadar albumin disebabkan oleh banyak faktor di luar variabel yang diteliti. Hasil korelasi 0,044 masuk dalam kategori sangat lemah dan tidak dapat diperhitungkan sebagai variabel penentu antara kedua variabel. Hasil analisis lanjutan dengan uji Mann Whitney didapatkan hasil bahwa tidak terdapat hubungan atau perbedaan rerata yang bermakna antara kadar 25-hidroksi vitamin D pada kelompok hipoalbumin maupun kelompok yang normal (p-value: 0,919) serta tidak terdapat hubungan atau perbedaan rerata yang bermakna antara kadar albumin pada kelompok defisiensi vitamin D dan kelompok normal. Kata Kunci: Albumin, Lanjut Usia, Vitamin D

Abstract

Warfarin merupakan antikoagulan yang banyak digunakan pada pasien fibrilasi atrium (AF) untuk mencegah kejadian tromboemboli, termasuk stroke iskemik. Namun, terapi ini berpotensi menyebabkan penurunan fungsi ginjal yang dikenal sebagai warfarin-related nephropathy (WRN). Beberapa penelitian retrospektif sebelumnya menunjukkan hubungan antara penggunaan warfarin dengan penurunan laju filtrasi glomerulus (eGFR), namun bukti yang tersedia masih terbatas, terutama di Indonesia. Tujuan dari penelitian ini untuk mengevaluasi hubungan antara penggunaan warfarin dan perubahan fungsi ginjal pada pasien dengan stroke iskemik dan AF. Metode Penelitian ini menggunakan desain potong lintang retrospektif dengan menelusuri rekam medis pasien stroke iskemik dengan AF di RSUD Cengkareng selama Januari–Desember 2024. Pengumpulan data menggunakan teknik toal sampling, dari 32 data awal, 29 pasien memenuhi kriteria inklusi. Fungsi ginjal dinilai berdasarkan selisih eGFR awal dan akhir dengan jarak minimal 3 bulan. Uji normalitas dilakukan dengan Shapiro-Wilk dan uji bivariat menggunakan Mann-Whitney (α = 0,05). Hasil Sebanyak 13 pasien (44,8%) menggunakan warfarin dan 16 pasien (55,2%) tidak. Kelompok warfarin mengalami penurunan median eGFR sebesar –9,20 mL/min/1,73 m², sedangkan kelompok non-warfarin menunjukkan peningkatan median sebesar +9,90 mL/min/1,73 m². Perbedaan median eGFR antara kedua kelompok signifikan (p = 0,020). Kesimpulan Pemberian warfarin berhubungan signifikan dengan penurunan fungsi ginjal pada pasien stroke iskemik dan AF. Temuan ini menguatkan perlunya pemantauan fungsi ginjal secara berkala serta mempertimbangkan alternatif terapi seperti NOAC pada pasien dengan risiko tinggi.
