Abstract

Diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia resulting from insulin resistance or deficiency. One of its severe complications is diabetic foot ulcers, which are prone to infection, especially by Pseudomonas aeruginosa—a Gram-negative opportunistic bacterium capable of forming biofilms and highly resistant to common antibiotics. This resistance complicates wound healing and increases the risk of amputation, emphasizing the need for alternative treatments. Abelmoschus manihot L. (Gedi leaves) contains bioactive compounds such as flavonoids, tannins, and phenolics, which are believed to possess antibacterial properties. This study aims to evaluate the potential of ethanolic Gedi leaf extract as an active ingredient in antibacterial ointment formulations. Ethanolic extracts were prepared and formulated into ointments at concentrations of 2%, 4%, 6%, and 8%. The formulations were tested for physical properties (pH, spreadability, organoleptic characteristics) and antibacterial activity against P. aeruginosa using the disk diffusion method. Results showed that the ointments had a uniform semi-solid texture, a characteristic herbal aroma, green color, and pH values ranging from 6.3 to 6.7—suitable for topical application. Spreadability ranged between 4.1 cm and 5.3 cm. The inhibition zones for the 2%, 4%, 6%, and 8% formulations were 6.67 mm, 8.00 mm, and 10.67 mm, respectively. No inhibition was observed with the negative control (DMSO), while the positive control (Bioplacenton) showed a 15.00 mm zone. In conclusion, Gedi leaf extract ointment demonstrated effective antibacterial activity against P. aeruginosa, with consistent physical properties across all concentrations. These findings suggest its potential as a natural topical therapy for treating bacterial infections in diabetic foot ulcers.

Abstract

Diabetes mellitus (DM) is a chronic condition characterized by increased blood glucose levels that require long-term treatment to maintain stable blood sugar levels. Garlic and turmeric rhizomes are known to contain secondary metabolite compounds that have the potential to lower blood glucose levels. This study aims to see how the combination of garlic and turmeric rhizome extracts affects the histopathological picture of the pancreas of male white rats (DM). This study uses an experimental design method with a designposttest only control group involving 30 white male rats divided into five groups: Negative control (K-), positive control (K+), and three treatment groups with extract doses of 100mg/kgBW (F1), 200mg/kgBW (F2), and 300mg/kgBW (F3). The results showed the most significant decrease in blood glucose levels in the 300mg/kgBW group (F3), namely (105mg/kgBW). The histopathological picture of the pancreas showed severe damage in group F1 (75% had necrosis), and moderate damage (50% with necrosis) in F2 and F3. The combination of garlic extract and turmeric rhizome has the potential to reduce blood glucose levels and improve the histopathological picture of the pancreas in DM rats.

Abstract

Heart disease is the leading cause of death in Indonesia. The use of complex drugs in the treatment of heart disease increases the risk of Drug Related Problems (DRPs), especially due to the many comorbidities and drug interactions. The purpose of this study was to determine the incidence of Drug Related Problems (DRPs) in heart disease patients at the internal medicine clinic of Royal Prima Marelan Hospital. This study is a retrospective descriptive study of 50 outpatients diagnosed with heart disease in the period January-June 2024. Data obtained from medical records and patient prescriptions were analyzed to identify the types of DRPs based on the PCNE classification. Drug interactions were the most prevalent type of DRP (95,7%), with severity levels classified as moderate (80,5%), minor (12,6%), and major (4,4%). The most frequent interactions occurred between clopidogrel and omeprazole (major). Most patients were male (60%) and aged between 40-55 years (54%).

Abstract

Abstract

Acute Respiratory Tract Infection is one of the infectious diseases frequently encountered in healthcare services. This stusy aims to assess antibiotic use in ARI patiets at Royal Prima Medan Hospital from January 2023 to August 2024 based on the appropriateness of drug selectin, and appropriateness of dosage. This study used a quantitave descriptive design with  a retrospective observational approach through data tracing of outpatient medical records. The sampel size was 135 patienst selescted using a percentage. The results showed that ARI patiens were predominantly male (57,8%) and aged 1-10 years(63,7%). The most prescribed antibiotic was cefixime (48,1%), followed by amoxicillin (17,0%), and cefadroxil (16,3%). Evaluation of antibiotic use showed that all patients met the criteria drugs, and appropriate dosage with a percentage of 100% each. This study indicates that antibiotic use in ARI patients at Royal Prima Medan Hospital is in accordance with the principles of rationel drug use.

Abstract

Kesum leaves (Polygonum minus Huds.) are herbal plants with potential as natural sources of antioxidants. This study aimed to evaluate the antioxidant activity of the ethanol extract of kesum leaves using the DPPH method and to identify the types of secondary metabolites present. Extraction was performed by maceration using 70% ethanol. Phytochemical screening was conducted qualitatively, while antioxidant activity was determined based on IC₅₀ values measured using a UV–Vis spectrophotometer at a wavelength of 517 nm, with vitamin C used as a reference standard. The results showed that the ethanol extract of kesum leaves contained alkaloids, flavonoids, tannins, triterpenoids, and steroids. The antioxidant activity test revealed that the ethanol extract had an IC₅₀ value of 7.9241 µg/mL, which was classified as having very strong antioxidant activity. In comparison, vitamin C exhibited a lower IC₅₀ value of 3.2886 µg/mL, indicating stronger antioxidant activity. These findings indicate that although the antioxidant activity of the ethanol extract of kesum leaves is lower than that of vitamin C, the extract still falls into the category of very strong antioxidants and has potential as a natural antioxidant source.
