Abstract

Abstract

Several antioxidant defense mechanisms protect biological systems from the negative impact of ROS. Natural antioxidants are mainly obtained from plants, such as vegetables, and one of them is the binahong plant which is rich in antioxidants. This plant is often used traditionally as a blood-thinning drug to prevent stroke. This study aimed to evaluate the effect of binahong (anderera cardifolia) leaf extract on cardiovascular health and the histological structure of the heart of wistar male white rats fed a high-fat diet. This study is an experiment with a completely randomized design (CRD) consisting of 4 groups: normal control, treatment 1 dose 200 mg/kg BW, treatment 2 dose 300 mg/kg BW, and treatment 3 dose 400 mg/kg BW. Histopathological data such as aortic wall thickness and number of foam cells were analyzed using SPSS 23 with one-way ANOVA test and Post Hoc LSD at 0.05 significance level. The results showed that administering a dose of 400 mg/kg BW effectively reduced the thickness of the aortic wall and the number of aortic foam cells in rats.

Abstract

Productivity is a critical measure of individual performance, particularly in the healthcare sector, where human resources play a pivotal role in achieving organisational and national development goals. This study examines the factors influencing work productivity among employees at the Aceh Provincial Health Office, focusing on education, motivation, discipline, work ethic, and work attitude. Using a descriptive quantitative approach with a cross-sectional design, data were collected from 100 employees through observation, interviews, and questionnaires. Univariate and bivariate analyses, including chi-square tests, were employed to assess the relationships between variables. The results indicate that 68% of employees demonstrated good work productivity. Motivation, discipline, work ethic, and work attitude showed statistically significant positive correlations with productivity (p = 0.000). Specifically, 95.2% of highly motivated employees exhibited good productivity, compared to only 25.2% of those with poor motivation. Similarly, 98.8% of disciplined employees and 81.5% of those with a strong work ethic demonstrated good productivity. A positive work attitude also correlated strongly with productivity (78.0% vs. 61.0% for negative attitudes). However, education level did not significantly influence productivity (p = 0.391), with 80% of highly educated and 66.7% of less educated employees showing good productivity. These findings highlight the importance of fostering motivation, discipline, work ethic, and positive work attitudes to enhance productivity in healthcare settings. While education level was not a significant factor, addressing the other variables can create a conducive work environment and improve service quality. This study underscores the need for organisations to prioritise these factors through incentives, recognition, and professional development opportunities to achieve optimal productivity and organisational success.

Abstract

Sepsis is a serious complication that remains a leading cause of death worldwide, especially among young children and the elderly. Delayed diagnosis remains a major challenge, highlighting the need to develop accurate diagnostic modalities, including inflammatory markers derived from routine blood counts. This study aimed to describe the profile of inflammatory markers in sepsis patients at Rumah Sakit Umum Royal Prima Medan in 2024. This observational study employed a cross-sectional design, analyzing 81 medical records of sepsis patients from Royal Prima General Hospital in 2024, selected using simple random sampling. The variables measured included socio-demographic characteristics (age and sex), leukocyte profile (total leukocyte count, eosinophils, basophils, neutrophils, lymphocytes, and monocytes), and inflammatory indices (NLR, PLR, PNR, LMR, and NLPR). Most sepsis patients were male (50.6%) in the early elderly age group (33.3%), with median leukocyte counts of 17,350 cells/µL, neutrophils at 14,660 cells/µL, and lymphocytes at 1,279 cells/µL. The inflammatory marker indices showed trends of NLR 13.21, PLR 290.74, LMR 1.30, PNR 21.50, and NLPR 0.004. These results support the potential of routine blood inflammatory markers as indicators for sepsis progression.

Abstract

Head injuries are one of the leading causes of morbidity and mortality in the world, with Subarachnoid Hemorrhage (SAH) being one of the most frequent complications that can increase the risk of death. The World Federation of Neurosurgical Societies (WFNS) Subarachnoid Hemorrhage (SAH) Grading Scale is used to assess the severity of SAH patients and predict their prognosis. This study aims to determine the relationship between WFNS SAH Grading Scale and mortality in head injury patients with SAH at Royal Prima Hospital Medan. The research method used an analytical observational approach with a cross-sectional design. The research sample amounted to 47 patients who met the inclusion criteria, using medical record data for the period January 2023 to August 2025. Data analysis was carried out using the Chi-Square test. The results showed that the majority of patients were in Grade I (38.3%) and Grade IV (27.7%), with a mortality rate of 23.4%. Statistical tests showed a value of p = 0.001 which means that there is a significant relationship between the WFNS SAH Grading Scale and the mortality of SAH patients. The lower the patient awareness level (GCS), the higher the risk of death. This study emphasizes the importance of using the WFNS SAH Grading Scale in assessing prognosis and determining the priority management of SAH patients in hospitals.

Abstract

Intracerebral hemorrhage (ICH) is a form of hemorrhagic stroke with a high mortality rate. Assessment of severity using the ICH score is crucial for predicting prognosis and determining patient treatment priorities in hospitals. This study aims to determine the distribution of ICH patients based on the Glasgow Coma Scale (GCS) score, ICH volume, the presence of intraventricular hemorrhage (IVH), age, and location of the hemorrhage, and to analyze the relationship between ICH scores and patient mortality at Royal Prima Medan Hospital. This study used an observational-analytical design with a cross-sectional approach. The study population was all patients diagnosed with ICH during the study period, while a sample of 96 patients was obtained using the Lameshow formula. Data were collected through medical records and analyzed univariately and bivariately using the Chi-Square test. The results showed that most patients had a mild GCS score (53.1%), mild ICH volume (33.3%), and no intraventricular hemorrhage (52.1%). The largest age group was >60 years (56.3%), with the most common bleeding location being the right parietal (13.5%). The mortality rate reached 50%. Statistical tests showed no significant association between ICH score and mortality (p = 0.101), although clinically there was a trend towards an increased risk of death as the ICH score increased. Most patients presented with mild clinical conditions, but mortality remained high in more severe ICH scores. This study recommends improved early detection, more comprehensive radiological recording, and routine application of the ICH score as a prognostic tool in the management of patients with intracerebral hemorrhage.
