Abstract

The purposes of this study were to evaluate the influences of oleic acid, propylene glycol andiontophoretic on the transport of propranolol HCl and to know optimum formula. Eightconditions were prepared according to 23 factorial design emplyoing oleic acid, propyleneglycol, and iontophoretic. Enhancement effect on transport of propranolol HCl wasinvestigated across hairless rat skin. The study was carried out by using vertical diffusioncell. Transport of propranolol HCl across membrane was determinedspectrophotometrically. Data were analysed based on compartment modeling to determinetransport kinetics and then the optimum formula was proposed. The transport kinetics ofpropranolol HCl followed first order. Oleic acid and propylene glycol increased the potentialof the drug to difuse (AD) but had no effect on drug release rate from donor compartement tomembrane (Ka) and from membrane to acceptor compartment (KR). Iontophoretic increasedKa and decreased KR. Based on values of Ka, AD, KR, optimum formula containing oleicacid, propylene glycol and iontophoretic which have the percentage and current density of4,8%, 20% and 0,11 mA/cm2, respectively.ABSTRAKPenelitian ini bertujuan mengetahui pengaruh asam oleat, propilen glikol dan iontoforesisterhadap parameter transpor transdermal propranolol HCl dan mengetahui komposisiformula optimum. Delapan formula disusun berdasarkan desain faktorial 23 denganmenggunakan pemacu transpor asam oleat, propilen glikol dan iontoforesis. Pengujianpenetrasi dilakukan secara in vitro menggunakan sel difusi. Kulit tikus sebagai membranmengalami praperlakuan dengan asam oleat dan propilen glikol, diikuti dengan iontoforesis.Jumlah propranolol HCl yang tertranspor ditetapkan dengan metode spektrofotometri.Analisis data menggunakan modeling berbasis kompartemen dilanjutkan dengan pemilihanformula optimal. Transpor propranolol HCl dijelaskan melalui model tiga kompartemen.Asam oleat dan propilen glikol meningkatkan potensi obat tertranspor (AD), namun tidakmempengaruhi kecepatan absorbsi dari kompartemen donor ke kulit (Ka) maupunkecepatan pelepasan dari kulit ke kompartemen reseptor (KR). Arus iontoforesismeningkatkan harga Ka dan menurunkan KR. Berdasarkan harga parameter Ka, AD dan KR,komposisi formula optimum adalah asam oleat 4,8%, propilen glikol 20% dan iontoforesis0,11% mA/cm2.

Abstract

Ethanol extract Kencur (Kaempferia galanga L.) has been proven to have antiinflammatory effect as well as to decrease the side effect of NSAIDs drug use, to develop the formulation of ethanol extract of kencur, then made in the dosage form of topical patch. Topical patch preparations are expected to avoid effects on the gastrointestinal tract, first- pass metabolism, and facilitate the use of the patient thereby increasing patient compliance. In the patch formulation, the most important things to note is the penetration enhancer/ enhancer, and enhancer used in this study is sodium lauryl sulphate. This study aims to determine the effectiveness of sodium lauryl sulphate in the preparation of a topical patch of kencur extract as anti inflammatory on the number of neutrophils in the blood. The research method uses canrrageenan as an inflammation inductor and neutrophil count observation was conducted using blood smears, then was observed on the first, third and seventh. Group of treatment which was given sodium diclofenac gel as positive control, group of treatment which was without enhancer as the 1st group, groups of treatment which were givenenhancer 1%, 3%, and 5% as the 2nd, 3rd and 4th group. The result was analyzed by one way ANOVA, followed by Post Hoc Duncan. The experimental results prove that there was a significant difference between the number of neutrophils and macrophages in the negative control group who were not given enhancer and the treatment group were given enhancer sodium lauryl sulphate values ( p < 0.05 ). As well as the group treated with the addition of enhancer sodium lauryl sulfate showed a greater amount of decline compared to the treatment group that contains only ethanol extract kencur.

Abstract

Propranolol has an intensive first pass metabolism, resulted in a low oral bioavailability. One alternative to circumvent such problem is the delivery by transdermal route. The objective of this study was to evaluate the effect of oleic acid 10 % (in propylene glycol 20 %) as enhancer, with and without iontophoresis, on transdermal transport of propranolol. Propranolol delivery was examined based on the in vitro transport studies across the rat skin (after hairremoval) in a vertical diffusion cells system. Skin was pretreated with the mixture of oleic acid 10 % (in propylene glycol 20 %) for 3 hours. Iontophoresis was performed at a current density of 0.25 mA/cm2 for 3 hours. Donorcompartment was filled with propranolol solution (5 mg/mL in citric buffer pH 5), while the acceptor phase was filled with phosphate buffer saline at pH 7.4. The results indicate that the enhancement methods increase the transdermal penetration of propranolol (p<0.05). The flux without anyenhancement methods was 13.16 ± 0.79 mg/cm2/hour. The flux with either oleic acid - propylene glycol pretreatment, iontophoresis or combination of both were 28.75 ± 3.04 mg/cm2/hour, 40.47 ± 5.78 mg/cm2/hour, and 85.42 ± 16.94 mg/cm2/hour respectively. Based on mathematics calculation, if an iontophoretic patch of 12 cm2 is used after skin pretreatment with oleic acid - propylene glycol mixture, the steady state plasma concentration of propranolol could reach 24.65 mg/mL. Therefore, therapeutic level might beachieved. This indicated a promising future of transdermal delivery of propranolol.Key words : propranolol, transdermal, enhancer

Abstract

Snakehead fish (Channa striata) have high albumin content, a protein needed for cell development and the formation of new tissue. The aim of this study was to determine the influence of snakehead fish extract emulgel given topically on incision wounds in white rats. The parameters of wound healing consist of wound length, a number of neutrophils, macrophages, fibroblasts and density of collagen. The white rats divide into three groups of (n = 6), one group was given the emulgel base as the negative control, one group of povidone-iodine as the positive control, and one group of snakehead fish extract 10% emulgel. White rats were sacrificed on the third and seventh days for microscopic observations. The results showed that snakehead fish extract emulgel can accelerate incision wound healing: decrease wound length, increase the number of neutrophil and macrophages cells, increase the average number of fibroblasts and increase collagen density on white rats.

Abstract

Transdermal transport was an alternate to propranolol HCl delivery to overcome the low bioavailability by oral route. Kinetics of transdermal transport can be predicted by software WinSAAM. The aim of this research was to know kinetics of propranolol HCl transdermal transport with the present of enhancer oleic acid, propylene glycol and iontophoretics. Propranolol HCl transdermal transport was examined through the hairless rat as membrane on the vertical diffusion cell in the in vitro permeation. Enhancement methode used was oleic acid in propylene glycol and iontophoretics at varying concentrations. The donor phase contained 5 mg/mL propranolol HCl in citrate buffer, and the acceptor phase contained phosphate buffer saline at pH 7.4. Results of propranolol HCl transdermal transport analyzed by WinSAAM software. Parameters of transdermal transport were the rate of mass transfer from donor compartement to skin (Ka), available dose to transport (AD), and the rate of mass transfer from skin to acceptor compartement (KR).The results indicated that propranolol HCl transdermal transport with the present of enhancer can be explained by three compartement model and first order kinetics. Theoretically, value of AD influenced by oleic acid, interaction of oleic acid-iontophoretics and interaction of propylene glycol-iontophoretics. Value of Ka inliuenced by iontophoretics and interaction of oleic acid-propylene glycol-iontophoretics. Value of KR induenced by iontophoretics.

Abstract

Egg whites traditionally used for the treatment of burns. In this research, egg white was formulated as gel dosage form for easy to use and covered unpleasant odor. The aim of the study was to determined effectivity of egg white in gel dosage form against the healing of superficial partial thickness burns (second degree) on white rats (Rattus novergicus) Wistar strain with parameters were macrophages, fibroblasts, collagen deposit density and wound healing. Research subject were male white rats aged 3 months. Second degree burns on the back of rat with a diameter of 2 cm were made by using a hot coin with temperature 80 ° C for 20 seconds. 18 white rats divided into 3 groups of treatment that is negative control using 0.9% NaCl, positive control using Bioplacenton®, and 40% egg white gel as treatment. Based on the experimental results, white egg gel given topically decreased the number of macrophages and increase the number of fibroblasts, increase the density of collagen deposits and speed up wound healing on second degree burns. White egg gel had significantly difference with negative control of NaCl 0.9% and no significantly difference with Bioplasenton as positive control.

Abstract

Hipertensi atau penyakit darah tinggi merupakan peningkatan tekanan darah sistolik lebih dari 140 mmHg dan tekanan darah diastolik lebih dari 90 mmHg pada dua kali pengukuran dengan selang waktu lima menit dalam keadaan cukup istirahat atau tenang. Penyakit hipertensi juga disebut sebagai “the silent diseases” karena karakter dari penyakit hipertensi tidak menampakkan tanda dan gejala yang jelas. Gaya hidup sehat menggambarkan pola perilaku sehari-hari yang mengarah pada upaya memelihara kondisi fisik, mental dan sosial berada dalam keadaan positif. Modifikasi gaya hidup dapat menurunkan tekanan darah serta meningkatkan efektivitas terapi farmakologik. Tujuan penelitian ini adalah pengetahui pengaruh gaya hidup terhadap efektivitas terapi antihipertensi pada pasien di Puskesmas “X” Wilayah Surabaya Selatan. Penelitian ini merupakan penelitian non-eksperimental menggunakan rancangan penelitian cross sectional dan dilakukan pengambilan data secara retrospektif. Gaya hidup diukur dengan menggunakan kuesioner dan tekanan darah diambil dari rekam medis. Sampel penelitian adalah pasien dengan diagnosis penyakit hipertensi yang datang berobat ke Puskesmas “X” Wilayah Surabaya Selatan pada periode bulan Februari-Maret 2020 sebanyak 67 pasien. Hasil penelitian menunjukkan bahwa tidak ada hubungan atau korelasi antara gaya hidup saat menggunakan obat antihipertensi dengan efektivitas terapi
