Abstract

Kualitas perawat ditentukan oleh kompetensi hard dan soft skill. Caring sebagai bagian dari soft skill adalah esensi mendasar pada profesi perawat. Penilaian pasien mengenai soft skill caring perawat adalah indikator dari kualitas pelayanan keperawatan. Tujuan penelitian untuk mengembangkan model pelatihan soft skill caring dan mengidentifikasi model tersebut terhadap kualitas keperawatan dan kepuasan pasien. Desain pra-eksperimen dengan pretest-posttest tanpa kontrol dengan melibatkan 53 perawat dan 53 pasien pada dua rumah sakit swasta di Kota Bandung. Instrumen penelitian diadaptasi dari Caring Nurse Patient Interactions Scale (CNPI) dan kepuasan pasien diadaptasi dari patient satisfaction with nursing care. Intervensi terdiri dari (1) pemberian materi selama 3 hari, (2) post pelatihan 2, 4 dan 6 minggu. Analisis data dilakukan untuk melihat perubahan penilaian soft skill caring perawat serta kepuasan pasien sebelum dan sesudah intervensi-pelatihan menggunakan uji paired t-test. Analisis General Linier Model Repeated Measure (GLM-RM) dipergunakan untuk analisis follow-up 4 dan 6 minggu. Hasil penelitian menunjukkan model pelatihan soft skill caring terbukti efektif meningkatkan penilaian perawat dan kepuasan pasien, serta dapat dimanfaatkan bagi perawat di rumah sakit.

Abstract

Hasil penelitian menunjukkan model pelatihan soft skills caring terbukti efektif meningkatkan penilaian perawat dan kepuasan pasien, serta dapat dimanfaatkan bagi perawat di rumah sakit. Kata kunci: soft skills caring, perawat, kualitas pelayanan keperawatan Abstract The quality of nursing care was determined by they hard and soft skills competence. Soft skill of caring was the basic competence of nurses, it was affecting patient satisfaction. This study aimed to: a) develop the model of training soft skills caring, b)identify the training model for increasing the quality of nursing care and patient satisfaction. The study was pretest-posttest pre-experimental design without control. The sampples including 53 nurses and 53 patients at two private hospitals in Bandung, West Java Province, Indonesia. The instrument was adapted from Caring Nurse Patient Interactions Scale (CNPI) while patient satisfaction assesment was adapted from patient satisfaction with nursing care. The interventions was soft skills training for 3 days, while the soft skill were assesed at in 2, 4 and 6 weeks post intervention. The pre and 2 weeks post intervention were analysed using paired t-test, while the 4 and 6 weeks follow-up soft skill post itervention was analyzed by GLM-RM. The results show that soft skill training are effective for improving nurse’s competence and increasing patient satisfaction. Futher training can be implemented to increase quality of nursing care and patients satisfaction. Keywords: soft skills, training, quality of nursing, patient satisfaction

Abstract

The prevalence of coronary artery disease (CAD) is increasing and becomes a major health problem nowadays. CAD has an impact on various aspects of the sufferer's life, both physical, psychosocial, and spiritual which affect the patient's Quality of Life (QOL). The main effort that can be done to maintain or improve the quality of life is with the patient's medication adherence. The existence of social restrictions during the COVID-19 pandemic has become a current issue regarding access to treatment of patients with CAD in hospitals so it has not been depicted regarding medication adherence and quality of life of patients with CAD during the pandemic, especially in Indonesia. This study aims to analyze the association between medication adherence and the quality of life of patients with CAD during the COVID-19 pandemic. The study method used is analytical observational with a cross-sectional approach. Primary data were collected from patients with CAD who were outpatient at the Cardiology Clinic of Private Hospital Type A in Bandung, Indonesia with accidental sampling technique. The study was conducted in January 2022 with the minimum sample being 70 respondents (Slovin formula). Data collection uses The Modified-Morisky Adherence Scale-8 (MMAS-8) questionnaire to assess medication adherence and the Seattle Angina Questionnaire-7 (SAQ-7) Questionnaires to assess the quality of life of patients with CAD. The statistical test is Chi-Square. The result showed that 34 (48.6%) respondents had low medication adherence and 43 (61.4%) respondents had good QOL. There was an association between medication adherence and QOL (p-value 0.009). Adhering to treatment can improve the quality of life of patients with CAD.

Abstract

Pendahuluan: PJK berdampak terhadap berbagai aspek kehidupan penderitanya baik fisik, psikososial maupun spiritual yang berpengaruh terhadap kualitas hidup pasien. Intervensi lebih lanjut untuk dapat meningkatkan kepatuhan diperlukan dan salah satu intervensi yang dapat digunakan yaitu dengan memanfaatkan teknologi (smartphone-based application). Tujuan: menganalisis pengaruh smartphone-based application terhadap kepatuhan pengobatan pasien PJK. Metode: Desain studi yaitu quasy experiment with control group dengan jumlah sampel 102 responden (n1=51 kelompok perlakuan & n2=51 kelompok kontrol). Skor kepatuhan pengobatan dinilai menggunakan kuesioner Modified Morisky Adherence Scale-8 (MMAS-8). Hasil: gambaran skor kepatuhan sebelum dan sesudah pemberian intervensi pada kelompok perlakuan berturut-turut dengan median 5.00 dan 6.75 sedangkan gambaran skor kepatuhan pada kelompok kontrol sebelum dan sesudah intervensi dengan median 5.00 dan 5.75. Analisis bivariat menunjukkan terdapat perbedaan signifikan skor kepatuhan pengobatan pada kelompok perlakuan dan kelompok kontrol dengan nilai p 0.035. Simpulan: Terdapat perbedaan signifikan skor kepatuhan pengobatan pada kelompok perlakuan dan kelompok kontrol, aplikasi berbasis smartphone berpengaruh terhadap peningkatan kepatuhan pengobatan pasien PJK. Intervensi berbasis teknologi dapat dimanfaatkan professional kesehatan dalam meningkatkan kualitas layanan terkait kepatuhan pengobatan dengan mengirimkan pengingat rutin kepada pasien yang bertujuan untuk mengurangi kelupaan, alasan ketidakpatuhan yang tidak disengaja

Abstract

Pendahuluan: Evidence-Based Practice (EBP) memberikan dampak positif terhadap kualitas pelayanan kesehatan khususnya di area Keperawatan Medikal Bedah (KMB). Pemahaman tentang konsep EBP dibutuhkan mulai dari jenjang akademik agar mahasiswa dapat dibekali ilmu yang memadai saat praktik dan menjadi perawat yang kompeten di unit/fasilitas pelayanan kesehatan. Intervensi klub jurnal online secara interaktif (Online Journal Club - Based Student-led Intervention) bisa menjadi salah satu strategi metode pembelajaran berbasis grup untuk meningkatkan pengetahuan tentang EBP khususnya bagi mahasiswa Keperawatan di Indonesia. Tujuan: menganalisis pengaruh Online Journal Club terhadap peningkatan pengetahuan mahasiswa keperawatan tentang EBP dalam mata ajar KMB. Metode: Desain studi Quasi Experiment dengan Pretest-Posttest Control Group Design. Total sampel sebanyak 219 responden dengan jumlah masing-masing kelompok intervensi 114 responden dan kontrol 105 responden. Penelitian ini dilaksanakan selama 6 minggu. Uji statistik yang digunakan dalam studi ini unpaired t-test. Hasil: Gambaran skor pengetahuan sebelum dan sesudah pemberian intervensi pada kelompok perlakuan berturut-turut dengan rerata 49.52 dan 65.56 sedangkan gambaran skor pengetahuan pada kelompok kontrol sebelum dan sesudah intervensi nilai rerata yang diperoleh berturut-turut yaitu 48.77 dan 47.63. Analisis bivariat menunjukkan terdapat perbedaan signifikan skor pengetahuan tentang EBP pada kelompok perlakuan dan kelompok kontrol dengan nilai p 0.001. Kesimpulan: terdapat pengaruh Online Journal Club terhadap peningkatan pengetahuan mahasiswa Sarjana Keperawatan tentang EBP dalam mata ajar KMB, diharapkan pendekatan klub jurnal secara interaktif ini dapat menjadi alternatif tambahan metode pembelajaran di akademik. Kata Kunci : Mahasiswa Sarjana Keperawatan, Online Journal Club, Evidence-Based Practice Background: Evidence-Based Practice (EBP) has a positive impact on the quality of health care, especially in the area of Medical-Surgical Nursing. An understanding of the EBP concept is needed starting from the academic level so that students can be prepared with adequate knowledge when practicing and become competent nurses in health facilities. Interactive online journal club intervention can be a group-based learning method strategy to increase knowledge about EBP in the area of Medical Surgical Nursing, especially for undergraduate nursing students in Indonesia. Objective: The aim of the study was to analyze The Effects of Online Journal Club on Nursing Students' Knowledge of Evidence-Based Practice (EBP). Method: Quasi Experiment study design with Pretest-Posttest Control Group Design. The total sample was 219 respondents with group distribution for 114 respondents in intervention group and 105 respondents in the control group. This study was conducted for 6 weeks. The statistical test used in this study was unpaired t-test. Results: The description of the knowledge scores before and after the intervention in the treatment group was respectively with a mean of 49.52 and 65.56, while the description of the knowledge scores in the control group before and after the intervention obtained the mean values respectively, 48.77 and 47.63. Bivariate analysis showed that there was a significant difference in knowledge scores about EBP in the treatment group and the control group with a p value of 0.001. Conclusion: There was an effect of online journal club on increasing the knowledge of Undergraduate Nursing students about EBP in the Medical-Surgical Area. Abstract

ABSTRACT Background. Cesarean section is an artificial birth, in which the fetus is born through an incision in the abdominal wall and uterine wall and the weight of the fetus is above 500 grams. Impacts that occur after caesarean section are pain and sleep disorders. Aim. To know the relation of pain intensity with sleep quality of mothers post cesarean section. Research methods. Using cross sectional approach. The sample of the study were 42 post-cesarean mothers taken by accidental sampling technique. The data was collected at RS TK. II Dustira Cimahi using Maternal Pain Questionnaire (MPQ) questionnaire for pain intensity measurement and The Sleep Sleep Quality Index (PSQI) for sleep quality measurement, in May 2017. Data analysis using Chi-Square test . Results. The highest intensity of pain occurred in the client after cesarean section with severe pain intensity with 22 respondents (52,4%) and uncomfortable there were 20 respondents (47,6%) and more than most client experience poor sleep quality that is 28 respondents ( 66.7%) but there are still clients who experienced good sleep quality 14 respondents (33.3%). The statistical test results obtained p significance number p = 0,000 thus p <α (0.000 <0.05), then H0 is rejected. Conclusion. There was a significant relationship between the intensity of pain and sleep quality in post-caesarean section patients. Keywords: Cesarean Section, Pain, Sleep Quality

Abstract

Beban kerja merupakan sesuatu yang muncul dari interaksi antara tuntutan tugas, lingkungan kerja, keterampilan, perilaku dan persepsi dari pekerja. Beban kerja merupakan salah satu faktor yang dapat mengakibatkan kelelahan. Kelelahan kerja perawat merupakan gejala yang berhubungan dengan penurunan efesiensi kerja, keterampilan, serta kebosanan pada perawat. Tujuan Penelitian: menganalisis hubungan beban kerja dengan tingkat kelelahan kerja pada perawat pelaksana di Instalasi Rawat Inap Lantai 1 RSUD Sekarwangi. Metode: Penelitian ini merupakan deskriptif analitik dengan pendekatan cross sectional dengan responden sebanyak 58 orang. Cara pengambilan sampel menggunakan total sampling. Instrumen yang dipergunakan adalah kuesioner beban kerja adaptasi dari Departemen Kesehatan Republik Indonesia dengan 15 pertanyaan dan kuesioner kelelahan kerja dari Tarwaka dengan 30 pertanyaan.  Kedua instrumen telah valid dan reliabel.  Analisis data menggunakan uji kategorik tidak berpasangan alternatif chi-square Kolmogorov-Smirnov. Hasil: lebih dari setengah responden mengalami beban kerja berat sebesar 56,9%. Sebagian besar responden mengalami kelelahan tinggi yaitu 67,2%. Tidak terdapat hubungan antara beban kerja dengan kelelahan kerja nilai (p=0,338). Diskusi: Beban kerja perawat yang tinggi akan menyebabkan kelelahan kerja perawat. Kelelahan kerja akan berdampak pada pelayanan kepada pasien tidak optimal sehingga berisiko terhadap keselamatan pasien. Meskipun hasil analisis hubungan antar variabel secara statistik tidak bermakna, namun pada kajian secara praktik menunjukkan bahwa beban kerja berlebihan dapat memengaruhi kelelahan kerja. Adanya perbedaan temuan penelitian ini dengan penelitian sebelumnya kemungkinan dapat disebabkan oleh adanya perbedaan lokasi pengambilan data, instrumen penelitian yang digunakan, dan keterbatasan jumlah sampel.  Kesimpulan: Banyak faktor yang memengaruhi kelelahan kerja.  Hasil penelitian dapat menjadi tolak ukur serta pertimbangan untuk dikembangkannya materi pembelajaran tentang beban kerja terhadap kelelahan kerja beserta faktor-faktor lainnya yang akan memengaruhi beban kerja perawat.Kata kunci: Beban kerja, kelelahan kerja, perawatCorrelation Between Workload and Work Fatigue in Executing Nurses in Inpatient InstalationABSTRACTWorkload is something that emerges from an interaction between task demands, work environment, skills, behavior and perceptions of workers. It is one of the factors that can cause fatigue. Nurses’ work fatigue is a symptom correlated with a decrease in work efficiency, skills, and boredom in nurses. Objective: to analyze the correlation between workload and level of work fatigue in executing nurses at the 1st Floor Inpatient Installation of Sekarwangi Regional Hospital. Methods: This research is descriptive analytic with a cross sectional approach with 58 respondents. Samples were taken   using total sampling. The instruments used were an adapted workload questionnaire from the Ministry of Health of the Republic of Indonesia with 15 questions and a work fatigue questionnaire from Tarwaka with 30 questions. Both instruments were valid and reliable. Data were analyzed using an alternative unpaired categorical chi-square test of Kolmogorov-Smirnov. Results: More than half of the respondents (56.9%) experienced a heavy workload. Most of the respondents (67.2%) experienced high fatigue. There was no correlation between workload and work fatigue (p=0.338). Discussion: Nurses’ high workload would cause nurses’ work fatigue. Work fatigue would have an impact on unsatisfactory service to patients so that it risked patient safety. Although the results of the analysis of the correlation between variables was not statistically significant, the practical study indicated that excessive workload could affect work fatigue. The differences in the findings of this research with previous studies might be due to differences in the location of data collection, the research instruments used, and the limited number of samples. Conclusion: Many factors affect work fatigue. The results of this research can be used as a benchmark and consideration for the development of learning materials about the impact of workload on work fatigue and other factors that will affect the nurses’ workload.Keywords: workload, work fatigue, nurses

Abstract

ABSTRAKLatar Belakang: Bedah Sesar adalah suatu persalinan buatan, dimana janin dilahirkan melalui suatu insisi pada dinding perut dan dinding rahim serta berat janin di atas 500 gram. Dampak yang terjadi setelah bedah sesar adalah nyeri dan gangguan tidur. Tujuan: Untuk mengetahui hubungan intensitas nyeri dengan kualitas tidur pada ibu pasca bedah sesar. Metode: Menggunakan pendekatan cross sectional. Sampel penelitian berjumlah 42 ibu pasca bedah sesar yang diambil dengan teknik accidental sampling. Pengambilan data dilakukan di RS TK II Dustira Cimahi, menggunakan lembar kuesioner Maternal Pain Questionnaire (MPQ) untuk pengukuran intensitas nyeri dan The Pittsburgh Sleep Quality Index (PSQI) untuk pengukuran kualitas tidur. Data dikumpulkan pada bulan Mei 2017. Analisis data menggunakan uji Chi-Square. Hasil penelitian: Intensitas nyeri yang tertinggi terjadi pada klien pasca bedah sesar yaitu intensitas nyeri hebat dengan 22 responden (52,4%) dan tidak nyaman terdapat 20 responden (47,6%). Lebih dari setengah responden mengalami kualitas tidur yang buruk yaitu 28 responden (66,7%). Ada hubungan antara intensitas nyeri dengan kualitas tidur pada pasien pasca bedah sesar. Diskusi: Faktor yang memengaruhi kualitas tidur pasien adalah sakit yang disebabkan oleh nyeri. Nyeri pasien setelah seksio sesaria karena terputusnya kontinuitas jaringan (trauma pembedahan) sehingga terjadi gangguan kualitas tidur. Semakin berat nyeri, maka semakin terganggu kualitas tidur pasien. Kesimpulan: Terdapat hubungan yang bermakna antara intensitas nyeri dengan kualitas tidur pada pasien pasca bedah sesar. Pelayanan kesehatan diharapkan melibatkan peran aktif keluarga untuk mengatasi penanganan nyeri pasien pasca bedah sesar, sehingga pasien memiliki kualitas tidur baik.Kata Kunci: Bedah sesar, nyeri, kualitas tidurRelationship Between Pain Intensity With Sleep Quality of Patient Post Caesarean SectionABSTRACTBackground: Cesarean section is an artificial birth, in which the fetus is born through an incision in the abdominal wall and uterine wall and the weight of the fetus is above 500 grams. Impacts that occur after caesarean section are pain and sleep disorders. Aim: To know the relation of pain intensity with sleep quality of mothers post cesarean section. Method: Using cross sectional approach. The sample of the study were 42 post-cesarean mothers taken by accidental sampling technique. The data was collected at RS TK. II Dustira Cimahi using Maternal Pain Questionnaire (MPQ) questionnaire for pain intensity measurement and The Sleep Sleep Quality Index (PSQI) for sleep quality measurement, in May 2017. The MPQ and PSQI questionnaires in this study were not tested for validity and reliability because this questionnaire was standard. Data analysis using Chi-Square test. Results: The highest intensity of pain occurred in the client after cesarean section with severe pain intensity with 22 respondents (52,4%) and uncomfortable there were 20 respondents (47,6%) and more than most client experience poor sleep quality that is 28 respondents ( 66.7%) but there are still clients who experienced good sleep quality 14 respondents (33.3%). The result of the study is that there is a correlation between pain intensity and sleep quality in post cesarean patients. Discussion: The factor that influences the patient’s sleep quality is pain caused by pain. Post sectio caesaria pain due to tissue continuity (trauma from surgery) is interrupted resulting in sleep quality disruption. The more severe the pain, the more disturbed the patient’s sleep quality. Conclusion: There was a significant relationship between the intensity of pain and sleep quality in post-caesarean section patients. Health services are expected to involve the active role of the family to overcome the pain management of post-cesarean patients, so that patients have good quality sleep.Keywords: Cesarean section, pain, sleep quality
