Abstract

Background: Guidelines recommend that external cephalic version (ECV) should be offer to all women with fetus in breech presentation at term. Many literature show external cephalic version can lowering c-section rate caused by breech presentation.Objective: To explore the determinants (barriers and facilitators) affecting obstetricians and gynaecologists to do external cephalic version at Yogyakarta.Method: Explanatory mixed methods design with quantitative-qualitative model. Survey with validated questionnaire and in-depth interview with semi-structured question was done January 2019 until August 2019.Results and Discussion: 72 respondents (83.7%) was responded to questionnaire and in-depth interview was done to 12 respondents. Adherence to ECV guideline was varied: counselling (20.8%), advising for ECV (15.3%), and arranged for ECV to for (almost) all their clients (16.6%). Although 76.4% of respondents considered ECV to be an effective treatment for preventing caesarean childbirth, only 18.1% respondents agreed that every client with breech presentation should undergo ECV. Self-efficacy was the most important determinant influencing adherence. In-depth interview shows several determinants to performed or did not performed ECV: skill of clinicians, guideline for ECV, facility to emergency c-section, ECV characteristic, cost, other methods for breech presentation, perception about ECV in lowering c-section rate, perceived ECV risk and patient preferences.Conclusion: Most respondents agreed that ECV was effective intervention to reduce caesarean childbirth, but adherence to counselling, advising and arranging ECV for clients still very low. Several determinants influenced obstetrician and gynaecologists to perform or did not perform ECV.Keywords: External cephalic version; breech presentation; determinants.

Abstract

Background: Peer teaching is a teaching method that widely applied in many medical institutions. This research was designed to evaluate group discussion effectiveness, intrinsic motivation, and student achievement differences between peer tutor−facilitated students and faculty tutor−facilitated students.Method: This research is an experimental research using post−test only control group design during the implementation of third and fourth module of the Cardiovascular Block (consist of five modules). Fifty-nine third year students were randomly assigned into two groups. Three experimental groups (n= 30) was facilitated by peer tutors (PT) and three control groups (n= 29) was facilitated by faculty tutors (FT). Two students were randomly elected from each PT group as peer tutor. A six hour training was given to the peer tutors. In the end of the third and fourth module, group discussion effectiveness, intrinsic motivation, and student achievement in those modules were evaluated using Tutorial Group Effectivenes Instrument (TGEI) questionnaire, Intrinsic Motivation Inventory (IMI) questionnaire, and 20 multiple choice questions, respectively. Mean differences between groups were measured by Cohen's d with the practical significance at Cohen's d > 1.Results: There was no significant differences of group discussion effectiveness (Cohen’s d= -0.41) and intrinsic motivation (Cohen’s d= -0.83) between two groups. Significant difference in student achievement is found between those groups, peer tutor−facilitated students score higher than faculty tutor−facilitated students (Cohen’s d= 2.96). Peer tutors showed higher achievement than discussants (Cohen’s d= 1.86).Conclusion: The results showed that peer tutor can facilitated group discussion as well as faculty tutor in group discussion effectiveness, intrinsic motivation and student achievement paramaters. There was advantage for students who given role as peer tutor.

Abstract

Banjar Tanjung merupakan salah satu Banjar di Desa Sanur Kauh, Kota Denpasar, Propinsi Bali. Salah satu kelompok masyarakat di Banjar Tanjung adalah kader posyandu. Salah satu kegiatan posyandu kader adalah pengukuran antropometri bayi dan anak untuk mendeteksi permasalahan pertumbuhan pada anak. Masalah pertumbuhan anak yang menjadi perhatian desa adalah stunting. Beberapa permasalahan dari diskusi yaitu masih bervariasinya pengetahuan kader terkait dengan stunting dan isu pengelolaan keuangan di keluarga Solusi dari masalah tersebut adalah kegiatan pelatihan deteksi dan pencegahan stunting serta pelatihan pengelolaan budgeting keuangan keluarga. Pelaksanaan pengabdian masyarakat secara umum berjalan lancar. Evaluasi pelaksanaan pada pelatihan deteksi dan pencegahan stunting didapatkan peningkatan rerata pretest-posttest (46,25 menjadi 73,75). Pada pelatihan pengelolaan budgeting keuangan keluarga. didapatkan peningkatan rerata pretest-posttest (60,00 menjadi 70,00).  Kesimpulan dari pelaksanaan kegiatan ini antara lain didapatkan peningkatan pengetahuan mitra terkait deteksi dan pencegahan stunting meningkat serta pengelolaan budgeting keluarga.

Abstract

Flipped classroom (FC) have advantages compared with traditional lectures. One of them is the occurrence of active learning.  This study aims to assess the flipped classroom learning method's effectiveness, feasibility and acceptability. Research method with mixed method. Quantitative method with pre-test and post-test control group design. One with traditional lecture (TL) and another with the flipped classroom method. Effectiveness was assessed by pre-test and post-test. The post test was carried out on day 0, 3, 7 and 14. FGD assessed the feasibility and acceptability of FC. TL are in pregnancy physiology courses, while FC are in labor physiology courses. There were differences in pretest scores in the FC group compared to TL, 4.89 (± 0.85) vs 4.29 (±1.01) (p=0.000), respectively. There were differences in posttest scores d-0, d-3, d-7, d-14 between the FC group and TL group, 7.89 (± 0.85) vs 7.29 (±1.01) (p = 0.000); 6.82 (± 0.78) vs 6.14 (±0.81) (p= 0.000); 5.85 (± 0.80) vs 5.20 (±0.88) (p= 0.000); 5.81 (± 0.77) vs 4.78 (±0.82) (p= 0.000), respectively. FC are difficult, burdened students, stimulate more active learning, are not suitable for certain learning, and provide an overview of real situations. Flipped classroom is more effective than traditional lectures. Flipped classroom can be applied (feasible) in teaching medicine, but the acceptability of this method still varies.

Abstract

Mahasiswa kedokteran perlu mengembangkan kompetensi kepemimpinan untuk dapat berkontribusi dalam meningkatkan kualitas pelayanan kesehatan. Dampak pelatihan kepemimpinan sulit untuk dievaluasi karena lebih fokus pada peningkatan kemampuan secara kognitif. Penilaian beberapa perspektif merupakan salah satu cara untuk mengetahui dampak pelatihan kepemimpinan pada mahasiswa kedokteran. Penelitian ini bertujuan untuk mengeksplorasi pengaruh pelatihan kepemimpinan dalam mengembangkan kualitas personal mahasiswa kedokteran berdasarkan penilaian fasilitator. Penelitian deskriptif kualitatif ini berdasarkan penilaian empat fasilitator dalam pelatihan kepemimpinan daring. Pelatihan kepemimpinan didesain menggunakan domain demonstrasi kualitas personal dari Medical Leadership Competency Framework (mawas diri, manajemen diri, pengembangan diri berkelanjutan, dan integritas). Pelatihan dilaksanakan selama 15 hari yang terdiri atas sesi workshop dan student project. Setiap fasilitator memfasilitasi 8-10 orang mahasiswa dan mengisi tiga penilaian selama observasi pembuatan student project. Sebanyak 12 dokumen dianalisis menggunakan analisis tematik. Hasil penelitian mendapatkan 19 sub- tema berdasarkan elemen demonstrasi kualitas personal. Setiap subtema menggambarkan perkembangan kualitas personal mahasiswa saat mengikuti pelatihan kepemimpinan. Institusi perlu memfasilitasi pengembangan kepemimpinan mahasiswa secara berkelanjutan melalui aktivitas intrakurikuler maupun ekstrakurikuler.

Abstract

Objectives: To understand the overview of women's reproductive health problems among foreign tourists in several health services in Bali in 2022-2023. Methods: The research design used is descriptive research with a cross-sectional method. The study sample consists of 103 samples selected through purposive sampling. Data from the research results will be analyzed univariately and processed using SPSS version 27. The research locations to be used are two private hospital in Bali Results: The research results on foreign tourists with women's reproductive health problems revealed that the age group was predominantly 20-35 years, with 31 (60.8%) in 2022 and 34 (65.4%) in 2023. Most foreign tourists who visited did not have insurance, amounting to 32 (62.7%) in 2022 and 36 (69.2%) in 2023. The most common diagnosis was prolonged labor, with 20 (39.2%) in 2022 and 11 (21.2%) in 2023. The most frequent complaint was abdominal pain, experienced by 30 (58.8%) in 2022 and 19 (36.5%) in 2023. For the treatment of foreign patients with reproductive health issues, non-pharmacological treatment was predominant, with 34 (66.7%) in 2022 and 31 (59.6%) in 2023. Conclusion: Based on the research conducted, the overview of women's reproductive health problems among foreign tourists in several health services in Bali in 2022-2023 is dominated by the age group 20-35 years, most of whom have health insurance. The most common diagnosis is prolonged labor, with the majority experiencing complaints of lower abdominal pain, and treatment predominantly involving non-pharmacological methods.

Abstract

Preeclampsia is an idiopathic multisystem disorder that affects 2-8% of pregnancies worldwide. It usually occurs after 20 weeks of pregnancy and affects both the mother and fetus. This study aims to determine the difference in the value of neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), and mean platelet volume (MPV) between women with preeclampsia and normotensive pregnant women. This is a cross-sectional comparation study using patient’s medical record conducted in Sanjiwani Gianyar Public Hospital. The sampling technique used in this study is purposive sampling, with 60 samples in each group. The data collected then analyzed using SPSS for Windows by independent sampled-t-test. The result shows that the NLR, PLR and MPV values in women with preeclampsia were significantly different from normotensive pregnant woman. The NLR, PLR, and MPV values were found higher in women with preeclampsia, with mean difference value of 0.93 for the NLR variable (p=0.011), 21.97 for the PLR variable (p=0.011) and 0.47 for the MPV variable (p=0.008). The predictive value is seen from the effect size (Cohen's delta) where the three variables are included in the moderate category.

Abstract

Premature rupture of membranes (PROM) is the rupture of the membranes before delivery. PROM occurs at 37 weeks gestation or more and PPROM occurs before 37 weeks gestation. One of the risk factors for PROM is infection that begins with the entry of bacteria into the intraamnion from the ascending urogenital tract or infection of systemic origin. Elevated leukocyte levels are a sign of infection. This study aims to determine the difference between maternal leukocyte levels of PROM and PPROM. The research design used was descriptive analytic method with a cross sectional approach. The sampling technique used is simple random sampling method. Data were analyzed using the Saphiro Wilk test for normality and the Independent Sample T Test to compare the difference in the mean of the two different groups. The results of the Saphiro Wilk test obtained values of 0.107 and 0.086, respectively (P>0.05), which means that the data is normally distributed. The results of the independent sample t-test showed a significance value of p=0.008. Therefore, it can be concluded that there is a difference in the number of leukocytes between PROM and PPROM at the Sanjiwani Regional General Hospital, Gianyar.

Abstract

[The Relationship between Sleep Quality and Student Achievement Level of the Faculty of Medicine and Health Sciences, Warmadewa University] Medical students have a higher prevalence rate of poor sleep quality than the rest of the student population. The existence of abnormal sleep times can be related to academic stress. Seeing the large influence of a person's sleep quality on academic achievement, it is therefore necessary to conduct a study that examines this condition in a more specific scope, such as at the Faculty of Medicine and Health Sciences, Warmadewa University. The purpose of this study was to determine the relationship between sleep quality and achievement levels of FKIK Warmadewa University students in 2019, 2020 and 2021. This research is an analytical study using a Cross Sectional Study design. Data collection was carried out using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. The data will then be analyzed using the SPSS application with the Spearman Rank correlation statistical test method. The results of the study illustrate that the quality of sleep in each class is dominated by poor sleep quality with details of 65.6% in the 2019 class, 75.4% in the 2020 class and 73.7% in the 2021 class. The student achievement level in the 2019 class is dominated by excellent indicators (56 .6%), class of 2020 with excellent indicators (60.8%), and class of 2021 with very good indicators (71.8%). The results of the correlation analysis stated that there was a significant positive correlation between the two variables in the 2019 class (r = 0.242; p<0.01), 2020 (r = 0.247; p<0.01), and 2021 (r = 0.216; p<0.01).
