Abstract

Abstract A preliminary study showed that most health workers in primary health care (PHC) claimed that they need a refreshing course because of their lack of updated knowledge and skill. This study enrolled 27 primary healthcare workers recruited from the PHC. The intervention used were classic lectures and workshops. The knowledge was evaluated using a paper-based test and practice, while the skill was evaluated using a practice test. Multiple questions (pre-test and post-test) based on current emergency management for pediatric were used for paper-based evaluation. Semi-structured interviews were conducted to confirm the subject's perspective on the intervention. A paired t-test was used for evaluating the pre- and post-test results, which was confirmed by a triangulation approach. There was a significant difference between the pre- and post-test results (p<0.001), and 8 of 10 subjects can demonstrate the procedure learned correctly after the intervention. A total of 14 interviewed subjects stated great effectiveness of the intervention, with several limitations on applicability in daily clinical practice. Classic lecture and workshop as an intervention in health education effectively increase health workers' knowledge and skill in PHC. Abstract

ABSTRACT  Hoarseness due to paralysis of vocal cord, as in Cardio-vocal syndrome, is caused by mechanical affection of left recurrent laryngeal nerve from enlarged cardiovascular structures. Mitral valve prolapse is rarely found to cause this syndrome. Case report presenting a 47 years old male visited the outpatient department with a clinical history of dyspnea and hoarseness since a year ago. Physical examination revealed late systolic murmur in apex and low-grade diastolic murmur in right second intercostal space. Echocardiography confirmed severe mitral regurgitation due to flail anterior mitral valve leaflet with severe left atrium dilatation (9.0 cm) and moderate aortic regurgitation due to mal-coaptation of aortic valves. Laryngoscopy revealed an immobile left vocal cord. He underwent successful double valve replacement after three months follow up the patient showed improvement of hoarseness. The incidence of Cardio-vocal syndrome in mitral valve disease varies from 0.6% to 5%. In cases diagnosed with thoracic disease, paralysis of the left vocal cord was reported 1.75 times more frequent than the right side. The aim of this case report is we have to aware that Cardio-vocal syndrome is a rare cause of vocal cord paralysis and should be considered as a differential diagnosis of hoarseness, particularly if the patient has a cardiac history. Comprehensive evaluation and prompt treatment may allow reversal of the damage to left recurrent laryngeal nerve. Abstract

ABSTRACT Cervical cancer is known as the highest cause of death after breast cancer. It is gynecological cancer contributing to the top cause of death in women. Sexual and reproductive issues can reduce the quality of life of women with cervical cancer. WHO defines the quality of life as an individual's perception in the cultural context and value system in which the individual experiences and is related to their goals, expectations, standards, and affairs. It gives the concept of an individual's physical health, psychological conditions, one's beliefs, social relationships, and one's involvement with something meaningful in their environment. Abstract

Abstract Sequestration of Plasmodium berghei ANKA-infected erythrocytes occurs in BALB/c mice as characteristic of  Plasmodium falciparum infection in humans. Animals’ bile has been widely used for centuries in Traditional Chinese Medicine. Goat bile has been used in healing infectious and non-infectious diseases; however, no report on the use of goat bile against malaria infection and sequestration. The purpose of this study was to analyze the correlation between parasitemia and sequestration in the liver of P.berghei ANKA-infected BALB/c mice treated with goat bile. This research was an in vivo experimental study using the post-test control group design. The male BALB/c mice aged ± 6 weeks, body weight 20-25 g were used. The mice were divided into five groups where Group 1-3 were mice treated with goat bile 25%, 50%, and 100%, respectively. Group 4-5 were negative (sterile water) and positive controls (DHP). Parasitemia was observed daily from each mouse and the number of sequestered infected erythrocytes on the endothelium of sinusoids. The data were analyzed using t independent test. Antimalarial activity of goat bile was shown by the lower parasitemia in goat bile-treated mice compared with the negative control. The average number of sequestration was goat bile concentration-dependent manner. The higher the concentration, the lower the number of sequestration. Sequestration was correlated with parasitemia (p=0,0001). Abstract

ABSTRACTABSTRACTMicrobiota contributes a crucial part in the human hosts' health and actively provides to the emergence of various diseases. The optimal composition of healthy intestinal microbiota varies from person to person. The more various and abundant of the microbiota, the greater their resistance to outside hazards. Colonization of the microbiota in the human body starts after delivery and develops continuously from infant to adult. The largest microbial colony is constructed in the lower part of the adult human digestive tract. The composition of the human intestinal microbiota alters promptly during the beginning of life and is steady. It has been described the close relationship among dysbiosis of the intestinal microbiota with intestinal and non-intestinal diseases. Nevertheless, it is uncertain whether dysbiosis is the culprit of the disease or only as a result of the disease. Human microbiota's role must be investigated more deeply so that later it can be developed for the prevention, diagnosis of disease, and more effective treatment strategies in the future. In this minireview, we will describe the development of the gut microbiota, its interaction with our bodily systems and defense, the multiple causes of dysbiosis, and its impact on several metabolic in inflammatory diseases in humans. Abstract

Backgrounds: Cervical degenerative disc disease is a pathological change in the cervical associated with the degenerative process. Surgery is one of the therapeutic modalities in cases of cervical degenerative disc disease. Post-surgical clinical outcomes are needed to evaluate the results of the actions that have been taken. Methods: This study is an observational retrospective study to evaluate clinical and functional outcomes using Neck Disability Index & SF36 on cervical degenerative disc disease before and after surgery at Dr. Soetomo Hospital Surabaya in 2013-2018. Data is processed using SPSS and compared using the T-test. Results: A total of 12 patients with details of 8 male patients and four female patients who met the inclusion and exclusion criteria in this study. There was a significant decrease in the average NDI score (p = 0.05) before surgery (47.33) compared with after surgery (15.58). There was a significant increase in the average SF36 score (p< 0.05) before surgery (50.75) compared to after surgery (88.16). Conclusions: There is an increase in clinical and functional outcomes in patients with cervical degenerative disc disease who undergo operative treatment.

Abstract

ABSTRACT Extranodal Non-Hodgkin Lymphoma (NHL) remains an odd issue and something to discuss. Clinical evidence and guidelines on the treatment have not yet been broadly published and accessible. Here, we present a case of a thirty-eight years old woman complaining of mass on both lower lids of the eyes. The CT Scan examination showed enhancing solid masses on right-sided and left-sided palpebra inferior that press bulb oculi, suspected as bilateral malignant masses of bilateral palpebra inferior. Laboratory data revealed the surge of liver function marker. The patient was eventually diagnosed with Non-Hodgkin Lymphoma (NHL) of palpebra inferior B cell type high-grade stadium II E. Although there are still ongoing research and development of definitive treatments, R-CHOP a regiment given to this patient has shown complete favorable result with dosage adjustment due to the surge of basal liver function. Abstract

ABSTRACTCovid-19 patients with Diabetes mellitus (DM) ranked third after hypertension and cardiovascular disease with an estimated 36% of all Covid-19 cases. These patients have a risk of experiencing a higher complication possibility since their metabolic disorder can cause hyperglycemia to the patient. It is showed that the number of deaths reached 7,3%, which is higher than non-diabetes. The increase of DM patients caused by stress factors may trigger the onset of glucose in blood sugar, and the glucose variability became abnormal. This circumstance may cause a glycemic increase that causes the predisposition intensification of susceptible affected by Covid-19. Good management is truly needed for DM patients affected by Covid-19. It is hoped to reduce the risk factor, such as preventing complications and increasing life quality by regular medical check to have a good prognosis. Some research showed that Covid-19 patients with DM are essential in ICU and need more treatment attention as they may experience Acute Respiratory Distress Syndrome (ARDS). Diabetes mellitus patients' treatment strategy is to manage the blood glucose level, especially in post-prandial glucose. Abstract

Abstract  Muscle wasting in critical illness is seen from inadequate nutrition intake and stress catabolism. Modified Nutrition Risk in the Critically Ill (NUTRIC) Score is a screening tool that measures starvation, inflammation, disease severity, and organ dysfunction. Relationship between muscle strength and muscle thickness causes the evaluation of muscle thickness is quite relevant. This study aimed to analyze the relationship between modified NUTRIC score and nutrition with muscle wasting in critically ill patients. The study was conducted on 30 patients in the ICU Dr Soetomo Hospital Surabaya. Nutritional intake and thickness of the quadriceps femoris muscle were monitored for one week. There was a relationship between mNUTRIC score with quadriceps femoris muscle thickness on days 3rd, 5th, and 7th. In the comparison between the low risk group and the high-risk group, it was found that there was a significant difference in the change in the percentage of muscle thickness difference on the 3rd, 5th and 7th day observation. There is a relationship between protein debt with quadriceps femoris muscle thickness (p = 0.008) with a positive correlation with moderate correlation strength (r = 0.477) on day 7. In conclusion, the modified NUTRIC Score had a moderate relationship with quadriceps femoris muscle thickness in critical patients from day 3rd until day 7th. The calorie intake not had a relationship with quadriceps femoris muscle thickness. Abstract

 Developmental problems concerned by parents are a further diagnostic step for children with Autism Spectrum Disorder (ASD). Research is needed to find a picture of the child's behavior complained of by parents. Investigations about first concerns among parents of children with ASD and comparisons of the same with children diagnosed with other developmental disorders are rare. We want to examine the type and age of parents ‘concern in children with ASD and other developmental disorders. A cross-sectional study was conducted in Child Developmental Centre in Surabaya Indonesia from August 1 to December 30, 2019. T-test and chi-square tests were used to analyze differences in subject variables and types of concern. Most parents of both children diagnosed with ASD and non-ASD indicated first concern were in communication, social skill, and behavior problem. The average age of parents’ first concern of ASD children is 30,14 months. Significant differences were found between ASD and non-ASD groups in communications, behavior, and sensory problem concerns. There is no specific difference between parental concerns of children with ASD and non-ASD children.
