Abstract

Maternal and fetal mortality is 31.15% due to undetected preeclampsia leading to eclampsia. Pre-occurrence of eclampsia is often characterized by severe preeclampsia symptoms such as increased blood pressure, and proteinuria, furthermore, preeclampsia is also divided into 2 types, based on the time of symptom onset, in early-onset type preeclampsia appears at <34 weeks of gestation while for late-onset appears at >34 weeks of gestation. Extreme age and multigravida parity are risk factors for severe preeclampsia. This study aims to determine the relationship between age and parity with late-onset severe preeclampsia. This study used a cross-sectional study design, sampling method used nonprobability sampling techniques with observational analysis on pregnant women diagnosed with preeclampsia at PKU Muhammadiyah Surabaya Hospital for January 2022 - November 2023. The data collection instrument used medical record data, which was analyzed using Chi-Square. In the analysis obtained for the relationship between age and preeclampsia, the large p-value is 0.16 <0.05. The relationship of parity with preeclampsia is 0.004 < 0.05, thus proving a relationship between age and parity with late-onset severe preeclampsia at PKU Muhammadiyah Surabaya Hospital. In conclusion, there is a relationship between age and parity with late-onset severe preeclampsia at PKU Muhammadiyah Surabaya Hospital.

Abstract

Diabetes Mellitus is a group of metabolic diseases characterized by hyperglycemia that occurs due to abnormalities in insulin secretion, insulin action, or both. Uncontrolled DM progresses to complications. A way to prevent complications is self-care management. To analyze the correlation between self-care management and the blood sugar levels in type 2 diabetes mellitus patients in the working area of Kedaton Health Center. An analytical observational study with a cross-sectional design. Data were collected by completing a questionnaire and measuring blood sugar levels. The sample consisted of 158 respondents, selected using a purposive sampling technique. The analysis test used the Spearman correlation test. There are 98 respondents (62.0%) with poor self-care management and 60 respondents (38.0%) with good self-care management. The majority of respondents have uncontrolled blood sugar levels, namely 132 respondents (83.5%). The results of the Spearman correlation test showed a significant correlation between self-care management and blood sugar levels in type 2 diabetes mellitus patients in the working area of Kedaton Health Center (p-value = 0.002), with a weak positive correlation (r = 0.242). There is a significant correlation between self-care management and blood sugar levels in type 2 diabetes mellitus patients in the working area of Kedaton Health Center. The worse self-care management of the respondents, the more uncontrolled the blood sugar levels will be (³180 mg/dL).

Abstract

Patients with diabetes may develop diabetic neuropathy, which damages peripheral nerves and impairs distal sensation. This study's aim was to determine the relationship between duration and fasting blood glucose with diabetic neuropathy. Patients with diabetes mellitus at Siti Khodijah Hospital in Sidoarjo, East Java, participated in this cross-sectional study. Fifty-one individuals were gathered using purposive sampling. The tools utilized were the Michigan Neuropathy Screening Instrument (MNSI) and a clinical chemistry laboratory examination—logistic regression for multivariate analysis and the contingency coefficient test for bivariate data analysis. Most type-2 DM patients who experienced diabetic neuropathy had a duration of DM >5 years (35.3%) and had fasting blood sugar levels ≥126 mg/dl (41.2%). Bivariate analysis of duration obtained a value (p = 0.003), and fasting blood sugar levels obtained a value (p = 0.000). Multivariate data analysis using logistic regression duration obtained value (Exp (B) = 22.386), and fasting blood sugar levels obtained value (Exp (B) = 35.305). There was a significant relationship between duration and fasting blood glucose levels on the incidence of diabetic neuropathy. The factor that had a higher risk of diabetic neuropathy was fasting blood sugar levels than the duration of DM.

Abstract

Academic stress is associated with mental disorders such as depression, anxiety, physical,and academic challenges. Medical students are particularly susceptible to high levels ofstress due to demanding academic workloads, intense competition, and prolonged studyperiods. Assessments like Objective Structured Clinical Examination (OSCE), whichevaluate students' clinical skills, also contribute to stress. One of the key factors insuccessfully facing this test is ego strength and emotional intelligence. This study aims toinvestigate the relationship between ego strength, emotional intelligence, and clinicalskills performance. A quantitative research design employing an analytical observationalapproach and a cross-sectional methodology was utilized. The instrument used tomeasure academic stress is the MMPI-1, while emotional intelligence is the TEIQue-SF.A total sampling method was employed for participant selection. The research findingsshow that the majority of respondents have low levels of ego strength (60%), a mediumlevel of emotional intelligence (77%), and OSCE scores was predominantly A grades.There was a weak association between emotional intelligence and OSCE performance (p= 0.784). Similarly, relationship between ego strength and OSCE performance (p =0.314). It is important that the identified relationships are correlative rather than causaland their impact is not statistically significant.

Abstract

Myopia is a common refractive disorder, and its prevalence continues to rise globally, including in Indonesia. In young adults over 21 years of age, the prevalence of myopia with a refractive error greater than -0.5 D reaches 48.1%. Myopia has been associated with various ocular complications, including elevated intraocular pressure (IOP), which can lead to further vision impairment. This study aims to analyze the relationship between the degree of myopia and intraocular pressure in myopic patients. A cross-sectional and consecutive sampling of myopia patients at Dr. Wahidin Sudirohusodo Hospital. Data from medical records and direct examination. Chi square correlation analysis revealed a significant relationship between the degree of myopia and intraocular pressure. In the right eye, the p-value was 0.03 (p < 0.05), while in the left eye, it was 0.01 (p < 0.05). Patient characteristics showed that the majority were female (69.6%), and most patients were young adults aged 21–40 years (79.7%). There is a significant relationship between the degree of myopia and intraocular pressure at Dr. Wahidin Sudirohusodo hospital.

Abstract

Thalasemia affects hemoglobin production. Transfusion-dependent thalassemia is a demanding clinical condition requiring lifelong care and follow-up. A 38-year-old woman is pregnant with her fourth child at 35 weeks gestation. This woman was diagnosed with Hb-E thalassemia-dependent transfusion (Hb electrophoresis showing Hb E 51.5%, Hb F 41.5%, and Hb A 3.5%). The Patient appeared pale with anemic conjunctiva, white sclera, and abdominal enlargement due to pregnancy. The Patient weighs 42 kg, and her height is 150 cm (BMI 18.67). Laboratory results were microcytic hypochromic anemia with hemoglobin 7.4 g/dL, erythrocytes 3.12 million/mL, mean cell volume (MCV) 74.0 fl, mean cell hemoglobin (MCH) 23.7 pg, mean cell hemoglobin (MCHC) 32 g/dL, and platelets with a platelet count of 133,000/µL. A cesarean method was chosen to terminate the pregnancy. There was no intrapartum and postpartum complication in this Patient. Her baby was 1.860 grams and 47 cm in length. There are no abnormalities found in the neonatal examination. Management of pregnant women with thalassemia requires special attention from preconception, during pregnancy, and toward delivery time. Multidisciplinary involvement such as hematologists and obstetricians is the primary key to the safety of mothers and children.

Abstract

Erectile Dysfunction (ED) is a condition that affects quality of life and is often associated with cardiovascular disease. By 2025, global ED cases are expected to reach 322 million, with strong links to depression and reduced well-being. This study aimed to analyze the relationships between age and body weight and ED among government and private-sector workers. This observational analytic study involved 64 participants selected via purposive random sampling in Makassar from June to July 2024. ). ED was measured using the International Index of Erectile Function-5 (IIEF-5), a validated questionnaire for assessing erectile function. Data were analyzed using univariate and bivariate methods (Chi-Square test). Most respondents were aged 31–40 years (34.38%), overweight (37.5%), and had normal erectile function (56.25%). A significant association was found between BMI and erectile function (p = 0.024), where normal and overweight individuals reported better erectile function. No significant associations were found between erectile function and age (p = 0.922), medical history (p = 0.304), medication use (p = 0.508), or frequency of sexual intercourse (p = 0.318). In conclusion, BMI was significantly related to erectile function, while age, disease, medication use, and sexual frequency were not.

Abstract

Low back pain (LBP) is a leading cause of global disability, significantly impacting work productivity. Known risk factors include age and obesity. However, while Body Mass Index (BMI) is a common indicator of obesity, BMI provides limited insight into true body composition. This study used Bioelectrical Impedance Analysis (BIA) to provide a more precise measurement of fat mass and to analyze its correlation with LBP, along with age and BMI, among employees at the University of Muhammadiyah Malang (UMM) General Hospital. The study used a cross-sectional design involving 112 hospital employees. Age was collected through a questionnaire. Fat mass was measured using the Tanita BC-418, and LBP was assessed using the Nordic Body Map (NBM) questionnaire, which distinguishes between complaints and disabilities. Statistical analysis used the Kruskal-Wallis test for correlations involving LBP complaints (age and fat mass) and the Spearman correlation test for correlations involving LBP disabilities (age and BMI). This study found no statistically significant correlations between the analyzed variables and LBP complaints or disabilities. Spearman's correlation test showed a weak, insignificant relationship between age and LBP disability (r=0.141; p=0.130) and between BMI and LBP disability (r=0.148; p=0.116). Similarly, the Kruskal-Wallis test did not show a significant correlation between age and LBP complaints (p=0.299) or between fat mass and LBP complaints (p=0.564). Conclusion: Age, fat mass, and BMI were not found to be significant predictors of LBP complaints or disability in this specific hospital employee population. These findings suggest that occupational or ergonomic factors may play a more dominant role in the etiology of LBP in hospital staff than anthropometric parameters alone.

Abstract

Low Back Pain (LBP) is one of the most common pain complaints in daily life, with significant impacts on patients' quality of life. This study aims to analyze the correlation between pain severity, as measured by the Numeric Rating Scale (NRS), and lumbosacral radiographic findings in LBP patients at Siti Khodijah Sepanjang Hospital. This observational-analytical study used a cross-sectional design. A total of 45 patients who met the inclusion and exclusion criteria and had undergone a lumbosacral X-ray (AP/lateral) within the last 3 months were included in the sample. Pain severity was measured using the NRS questionnaire, and lumbosacral radiology findings were evaluated for muscle spasms, spondylolisthesis, osteophytes, intervertebral disc (IVD) narrowing, and compression fractures. The analysis revealed that 84.4% of patients experienced moderate pain. Regarding lumbosacral radiology abnormalities, 62.2% were classified as moderate in severity. Osteophyte formation was the most prevalent radiology finding, present in 86.7% of cases. Spearman's correlation test showed a significant correlation between pain severity and lumbosacral radiology findings (p = 0.00 < 0.05). The analysis demonstrated a strong, statistically significant positive correlation between radiology findings and pain severity (r = 0.62). These findings suggest that pain severity may serve as an indicator of the severity of lumbosacral radiological conditions in patients with low back pain and could be used as an adjunct in clinical evaluation and management strategies.

Abstract

Efficiency is used as an indicator of hospital performance, including the overall performance of hospital health services. The effectiveness of services related to hospital bed management is assessed using the metrics Bed Occupancy Rate (BOR), Average Length of Stay (ALOS), Turnover Interval (TOI), and Bed Turnover (BTO) on the Barachman-Johnson graph. The construction of a new building resulted in an increase in the number of patients and beds from 2022 to 2023, according to medical record data from the Islamic Hospital (RSI) Surabaya inpatient ward. The availability of community services may be affected by changes in bed availability. This study aimed to evaluate the efficiency of inpatient services at RSI Surabaya in 2022–2023 using bed utilization indicators (BOR, ALOS, TOI, and BTO) and the Barber–Johnson graphical approach. This study used an observational analytical design with a quantitative approach. Descriptive and comparative analyses were conducted to evaluate inpatient bed management indicators (BOR, ALOS, TOI, and BTO) at RSI Surabaya in 2022–2023 using the Barber–Johnson method, based on aggregate secondary hospital data.  The RSI Surabaya inpatient ward's Barber Johnson graph for 2022 and 2023 shows that the BOR value has decreased, the ALOS is low, the TOI has reached an optimal value, and the BTO is high. The Barber Johnson efficiency area is not where the four parameter points are located. In conclusion, inpatient bed utilization at RSI Surabaya in 2022 and 2023 did not achieve optimal performance.
