Abstract

Background: Cardiac rehabilitation (CR) is an important post-acute management for Cardiovascular Heart Disease (CHD) patients. CR has benefits, such as preventing recurrence and improving patients physically. However, the CR participation rate is low. This condition can produce negative effects, such as recurrence and depression. There are inhibiting factors contribute to the CR participation. The purpose of this study is to identify the distinction of inhibiting factors in patients who actively and inactively participate in phase II CR in Bandung. The result will be as advices to improve policy and nursing intervention to improve the participation number of phase II CR in Bandung. Method: This study used comparative design with cross-sectional approach on 72 respondents who recruited by using purposive sampling at a hospital in Bandung, Indonesia. Data were collected by using Cardiac Rehabilitation Barrier Scale (validity r = 0.46-0.55 and reliability 0.887). Data were analyzed by using descriptive frequency and comparative analysis by using Mann-Whitney. Result: It was showed that there were two significant differences between both of the group in terms of health services aspect (p-value = 0.002) and time (p-value = 0.001). However there were no significant differences between both of the group in logistics aspect (p-value = 0.134), and functional status aspect (p-value = 0.057). It indicates that there were distinctions in inhibiting factors on health services and time aspects. Conclusion: There were differences in the health services, which was related to the lack of CR information and also time aspects, which was related to the lack of CR facilities in remote areas. Therefore, providing information about CR and community or home-based CR are needed.

Abstract

Due to its location in the ‘Pacific Ring of Fire”, Indonesia is frequently prone to natural disasters. Therefore, Indonesian nurses need to have the ability to assist in disaster situations as they are considered an important element of the workforce in disaster preparedness and response. However, the current nursing curricula in Indonesia does not adequately prepare nurses to respond in these situations. The primary aim of this study is to understand the experiences of Master of Nursing students’ roles and experiences in disaster settings in a nursing school in East Java, Indonesia. A single-case study design has been used. Data colection occurred via semi-structured interviews. The participants were enrolled in the Master of Nursing in a School of Nursing at a University in East Java. The data for the study were thematically analysed. Four main themes were identified: ‘personal feelings’, ‘working outside their scope of practice’, ‘lack of disaster preparedness’, and ‘a lack of mental health knowledge and care’. This study found that the Master of Nursing students in a nursing school in East Java, Indonesia were not adequately prepared for responding to disaster situations. This study found that a lack of disaster preparedness, working outside scope of practice, and a lack of mental health knowledge were the prominent issues for these Master of Nursing students. As well as commencing disaster training earlier in their education, providing training in the psychological context of disaster preparedness is recommended.

Abstract

Mastectomy is a surgical treatment of breast cancer can result in permanent changes in the appearance of women. this change can cause a negative change in a woman's self-concept. If the self-concept impairment continues it will lead to emotional weakness. Individuals will be depressed and feel constantly anxious, emotional frustrations are getting worse, making the individual create self-destruction. For that needed therapy that can overcome the negative problems of this self-concept of Spiritual Therapy Emotional Freedom Technique (SEFT).The purpose of this study was to identify the effects of SEFT therapy on self-concept in breast cancer women with mastectomy. This study uses quasi experimental design with one group pretest-posttest design approach with inclusion criteria willing to be the subject of research, conscious, no hearing loss or self-concept cognitive impairment, has never done SEFT therapy, interacted with family and community at least 1 week post mastectomy . This therapy is done for 3 days on 33 subjects. Data were analyzed by chi square statistic test with 95% CI with significance p <0,05.The result of this research is prior to being given SEFT treatment from 33 respondents, 4 respondents (12,1%) have positive self concept, and the majority of 29 respondents (87,9%) have negative self concept. However, after SEFT treatment there was an increase in self-concept of respondents with positive self concept as many as 10 people (30.3%) and still have negative self concept as many as 23 people (69,7%). Once tested with chi-square statistical test, there are differences in self-concept before and after SEFT. It is advisable to practice nursing to use this therapy as a therapy to prevent self-conceptual disorder in breast cancer patients who have mastectomy

Abstract

Thalassemia is a major genetic blood disorder that has negative impacts on thalassemic children and their family. The needs of parents while caring for thalassemia children are important matters because the failure of meeting needs of parents may affect their mental and physical health. Yet, litle is known about factors that meeting the needs of thalasemic child parents. The study aim was to identify factors that correlate with the needs of parents having a school age thalassemic child in west jave province. This research used correlational analytic with cross sectional approach. The population in this study were parents of school-age children with thalassemia major totaling 136 people using the total sampling method. Data were analyzed with bivariate using chi square and biserial points. Then a multivariate analysis used logistic regression. The results showed the age of parents related to information (p = 0.005) and professional (p = 0.004), parent gender related to information (p = 0,000), professional (p = 0.017), social (p = 0.035), financial (p = 0.023) and spiritual (p = 0.009). Parental education is related to information (p = 0,000), social (p = 0,005), emotional (p = 0,000), financial (p = 0,016) and spiritual (p = 0,001). Parent income is related to information (0,000), social (p = 0,006), emotional (p = 0,000), financial (p = 0,003) and spiritual (p = 0,004). The number of thalassemia major children is related to emotional (p = 0.015) and financial (p = 0.013). Parental needs are related to several factors. The importance of increasing the role of nurses as educators and consultants and helping parents find sources of spiritual, emotional and financial support for parents of school-age children with thalassemia.

Abstract

Adolescent reproductive health (KRR) in Indonesia is still poor. The KRR known as Triad KRR consisted of sexuality, HIV / AIDS, and drugs become a national problem. Teenagers were more comfortable talking about their KRR with their friends than their parents or teachers. The purpose of this research was to know the relation of age, gender, and peer group with behavior of adolescent reproductive health. The research method was  cross sectional study design. Data were analyzed using descriptive statistic, Chi-square and binary logistic. Research conducted in 2017 in junior and high school district Bandung. The population of this study was all students of SMP and SMA amounted to 12,000. The samples amounted 668 students. The results of the research showed that there was a significant relationship between gender with behavior (p = 0,006), there was a significant relationship between age and behavior (p = 0,031), and there was a significant relationship between peer group and behavior (p = 0,042). Multivariate analysis found that age had opportunity 0,527 times, peer group had opportunity 0,211 times, and gender 2,208 times on the behavior support of Triad KRR. The results concluded that KRR behavior is influenced by age, sex, and peer group. Based on the results of the study suggested for the provision of education using a method that involves peer group by considering the age and gender in improving knowledge, adequate and sustainable behavior especially about adolescent reproductive health.

Abstract

Patients with head and neck cancer usually experience physical and psychological changes and adjustments related to the disease and management of therapy. The patients will experience symptom cluster and will use effective symptom alleviation self-care to relieve the symptoms. The proper identification of symptom cluster and the effectiveness of using symptom alleviation self-care will be the basis for the success of disease management. This study aimed to investigate the symptom cluster and symptom alleviation self-care in patients with head and neck cancer, and which has an impact on the quality of life. This research was a pilot study using a qualitative design and involved five patients at the public hospital in Semarang, Indonesia. The qualitative design has been chosen to explore the varied of symptom experienced by the patients about the nature, number, location, duration and intensity of experiences, which may different experiences of symptom cluster and symptom alleviation self-care for each patient with Head and Neck Cancer. Data were collected through semi-structured interviews and analyzed used the qualitative content analysis process. Three themes were identified in this study, including: the patients’ experience of symptom cluster, the patients’ experience of symptom alleviation self-care, and the impaired quality of life domain. The results of this study showed that the patients’ experience sickness and gastrointestinal symptom cluster during illness and undergoing therapy, as well as variations in the symptom alleviation self-care, including: diet/ nutrition/ lifestyle changes, mind/ body/ spiritual control, biological treatment, herbal treatment, and prescribed medicine. The symptom cluster and symptom alleviation self-care has an impact on the patients’ outcome that is the quality of life. This study showed that the experience of symptom cluster and symptom alleviation self-care varied and highly individualized, which has an impact on the quality of life. The importance of proper identification about symptom cluster and the effectiveness of using symptom alleviation self-care by the nurses will be the basis for the success of disease management to improve the patients’ quality of life. Therefore, optimizing the nurses’ role is needed as the basis for the development of symptom management nursing programs.

Abstract

Low back pain is a pain syndrome that is often felt in the lower back region accompanied by spreading to the legs. It is often associated with work-related musculoskeletal disorders. This condition is often experienced by the Intensive Care Unit (ICU) nurses, from mild to severe LBP, which may affect the productivity of work and physic of the nurses. Aim to analyze and explore the factors that affected the number of low back pain incidents in ICU nurses in hospitals in the Banten Province. This study used the correlational analytic method with a crosssectional approach. The study sample was 82 ICU nurses, obtained by total sampling. Data collection used the LKQ questionnaire (Low back pain Knowledge Questions) and observation sheets. Data analysis used the Chisquare and multiple logistic regression analysis with a prediction model.   Knowledge factor p (0.001), height p (0.021), night shift frequency p (0.003), and weight with a p-value (0.021) had a significant relationship with low back pain, whereas the ICU space environment factor p (0.668), work period p (0.462), and age p (0.079) did not have a significant relationship with low back pain. From related factors, knowledge had the most significant relationship with low back pain incident (OR = 38.62). This study has significantly proven that knowledge, height, weight, and frequency of night shifts affected the low back pain in ICU nurses. The nurse’s knowledge factor is the most influential factor of low back pain incident in ICU nurses. Increasing the ICU nurse’s knowledge about body biomechanics by following training  and developing  standard operating procedures. It is suggested to reduce the amount of excessive night shift burden ICU nurses, select and setting criteria for nurses working in the ICU.

Abstract

Post-stroke patients experience in various disturbances including physical, psychological and spiritual aspect. Post-stroke patients in the rehabilitation phase focus more on physical needs while the unfulfilled spiritual needs can reduce the patient's health condition. Therefore, it takes action to meet the spiritual needs of patients. This study was conducted to identify the spiritual needs among post-stroke patients.The descriptive quantitative was used among post-stroke patients in Neurological Polyclinic and stroke center in one of the Hospital in Bandung in 2017. The sampling technique used was consecutive sampling with 83 samples included post-stroke patients who have full awareness and didnot experience of aphasia. The instrument of this study used Spiritual Needs Questionnaire (SpNQ) which consists of 4 dimensions including religious, peace, self-existence, and dimension of the giving needs. SpNQ had a validity coefficient of 0.73 and r 0.75. Data were analyzed using frequency distribution. The results showed that the most of respondents feel the spiritual need in all dimensions. In the religious dimension, the majority of respondents desired to pray with others, to move closer to God and participate in various religious activities (98.8%). In the dimension of peace showed the most of respondents desired to be more cherished by others (96.4%). The most perceived item of respondents were forgiving someone from the past in the dimension of self-existence (98.8%) and a solace for others in the dimension of the giving needs (98.8%). The conclusion from this study was most respondents feel the spiritual needs in all dimensions. Based on these results, nurses are expected to provide advice to people around the patients to invite them in various religious activities. Nurses supposed more expressing affection to them and involving the patients in some family activities.

Abstract

For year research on quantifying how well individual’s function has been reported. Assessing function is particularly important in the older adults, as the prevalence of functional disability increases with age. In Indonesia, there is a lack of studies that measure the functional status of the older adults. There is even less research on evaluating the psychometric properties of an instrument. Therefore, this study aimed to compare the psychometric properties of the evidence supported functional status instrument consisting of the Short Physical Performance Battery (SPPB), the Functional Status Questionnaire (FSQ) and the Physical Performance Test (PPT). This study using the validation design with descriptive approach. And 401 subjects aged more than 60 years old were recruited purposively from five districts in Aceh. Every instrument showed good validity and reliability and has been used either for research purposes or in clinical setting. All subjects completed the FSQ, SPBB, and FSQ assessment. Correlation between SPPB and PPT were higher than FSQ when assessed for convergent validity the FSQ had comparable correlations with the reported health status. However, relationship between SPPB, PPT and FSQ were inconsistent. The findings of this study is expected to support the psychometric properties of all three instrument for functional status assessment in Indonesian Older adults, and SPPB appear to be the best among the other instruments to use in the nursing practice.

Abstract

Medication adherence is behavior that refers to client obeys in following a medication, and makes lifestyle changes in accordance of recommendations from health care providers. Antiretroviral adherence is paramount for HIV/AIDS patients. The effects were often a problem in antiretroviral treatment and toxicity and often be the reason for replacing or stopping antiretroviral treatment. This study aimed to determine the “E-Patuh” Applications effect on antiretroviral adherence in patients Of HIV/AIDS In West Java. The research design was a quasi-experimental with nonequivalent control group design. The location of this research was in RSUD Kota Bandung and in RSUD Kota Banjar. The respondents was selected without randomization and used purposive sampling technique. Respondents in this study were 30 respondents. Data were obtained using self-report questionnaires. The intervention group was monitored a 30-day android-based E-Patuh application and monitored on an E-Patuh website and then measured adherence value with self-report. Data were analyzed using SPSS 22 with chi-square test. The results showed a significant difference between adherence value before and after application of E-Patuh in the intervention group with (p <0.05) with obtained p value = 0,006. The results of this study prove a positive effect on the using of E-Patuh applications against ARV medication adherence in the intervention group with the support system of the E-Patuh application. The used of E-Patuh is helpful in improving ARV adherence in HIV/AIDS patients. The features contained in E-Patuh were directly reminiscent of the timing of taking medication for PWLH. E-Patuh should be consideration for PLHIV and health care providers in hospitals to improve ARV adherence to reduce mortality rates in people living with HIV.
