Abstract

This study aims to analyze the influence of positive attitude (X1), social support (X2), and ease of form completion (X3) on Patient Safety Incident (PSI) reporting behavior (Y), with reporting intention (Z) as a mediating variable. Using a causal associative approach with a cross-sectional design, data were collected through questionnaires from 150 inpatient nurses selected via purposive sampling. Structural Equation Modeling (SEM) was employed to examine variable relationships. Key findings reveal: (1) positive attitude, social support, and ease of form completion significantly affect both reporting intention and PSI reporting behavior; (2) reporting intention partially mediates the relationship between the three independent variables and reporting behavior; (3) social support and ease of form completion exhibit the strongest direct effects on reporting behavior. The study highlights that strengthening patient safety culture requires integrating psychological (attitude), social (support), and technical (system usability) factors. Practical implications suggest hospital management should develop policies fostering positive attitudes through training, enhance social support via team collaboration, and streamline technology-based PSI reporting systems. These measures are expected to improve reporting transparency and accuracy, ultimately advancing patient safety service quality.

Abstract

Dementia is one of the major health problems in the elderly, characterized by progressive cognitive decline that affects daily functioning and quality of life. Physical activity plays a protective role against dementia by increasing cerebral blood flow, improving neuronal function, and reducing cognitive degeneration. The Elderly Social Services Unit (UPTD Lanjut Usia) in Binjai accommodates elderly people with various characteristics, making it a representative setting to examine the relationship between physical activity and dementia. This study aimed to determine the association between physical activity and dementia among elderly residents of UPTD Lanjut Usia Binjai, North Sumatra Province, in 2024. This was an analytical quantitative study with a cross-sectional design. A total of 133 elderly respondents were included using total sampling. Physical activity levels were measured using the Physical Activity Scale for the Elderly (PASE), while cognitive status was assessed with the Mini-Mental State Examination (MMSE). Data analysis was performed using univariate methods for frequency distribution and bivariate analysis with the Spearman Rank test, with a significance level of p < 0.05. The findings showed that the majority of respondents had low physical activity levels (70.7%), while 21.1% had moderate activity and only 8.3% had high activity levels. Regarding cognitive status, 50.4% of respondents experienced definite dementia, 31.6% probable dementia, and only 18.0% remained cognitively normal. The Spearman Rank test revealed a significant association between physical activity and dementia (p = 0.000). There is a significant relationship between physical activity and dementia among the elderly at UPTD Lanjut Usia Binjai. Elderly individuals with lower levels of physical activity were more likely to experience dementia compared to those with moderate or high physical activity.

Abstract

Pneumonia is as an acute respiratory infection that affects lung tisuue and stands as one of the primary causes of mortality among children worldwide.. According to the World Health Organization (WHO, 2022), pneumonia is caused by infectious agents such as Streptococcus pneumoniae, Haemophilus influenzae type b (Hib), and Respiratory Syncytial Virus (RSV). UNICEF (2024) reports that more than 700,000 children under the age of five die annually due to pneumonia. Radiological chest X-ray examination plays an essential role in detecting pulmonary abnormalities such as infiltrates, consolidation, and bronchopneumonia (Az et al., 2021). It also assists in distinguishing between bacterial and viral pneumonia (Aprilia et al., 2024). Objective to determine the radiological characteristics of chest X-ray findings in pediatric patients with pneumonia at Royal Prima Ayahanda Hospital Medan. Methods this research employed a descriptive retrospective study design using a cross sectional approach. Secondary data were collected from the medical records of pediatric pneumonia patients at Royal Prima Ayahanda Hospital, Medan, during 2022–2023. A total of 57 pediatric patients who met the inclusion and exclusion criteria were included. Data were analyzed univariately and presented in frequency and percentage distributions based on variables such as age, gender, infiltrate, and consolidation findings. The results showed that the most common age group was 0–1 years (42.1%), with females predominating (61.4%). Chest X-ray examination revealed that 75.4% of patients had abnormal radiological findings. The most frequent feature was pulmonary infiltrate (77.2%), while consolidation was found in 21.1% of patients. Conclusion most pediatric pneumonia patients demonstrated abnormal radiological chest X-ray findings, predominantly alveolar infiltrates. Chest X-ray examination remains an important diagnostic tool in assessing lung involvement and determining the severity of pneumonia in children.

Abstract

The phenomenon of quiet quitting has become a global concern, representing a decline in work engagement, particularly among Generation Z. This literature review aims to analyze the factors influencing the emergence of quiet quitting and to explore strategies for addressing it, drawing on findings from various international studies. This study employs a narrative review method, utilizing PubMed, Elsevier, and Google Scholar as databases to identify relevant articles. The keywords used are "quiet quitting," "Gen-Z Worker," and "Gen-Z in Workplace." The literature used is limited to the years 2015-2025. The results of the review indicate that quiet quitting is influenced by a combination of psychological factors (such as burnout and emotional well-being), organizational factors (low organizational support, unclear career paths, and lack of empathetic leadership), technological factors (digital fatigue due to excessive online workload), and generational values that emphasize work-life balance and the search for meaning. To address this phenomenon, the literature recommends multidimensional policies that include burnout prevention, increasing perceived organizational support, implementing healthy, flexible work systems, managing digital workload, and strengthening leadership capacity based on human-centric management.

Abstract

Maxillofacial fractures are among the most serious injuries involving facial bones and are often caused by trauma, particularly traffic accidents. This condition not only affects the aesthetic and physiological functions but also has a significant impact on the patient’s quality of life. The high incidence of maxillofacial fractures has become an important concern in the fields of dentistry and oral and maxillofacial surgery. This study aimed to describe the characteristics of patients with maxillofacial fractures at Haji General Hospital Medan in 2022–2024, including the number of cases, age distribution, gender, causes of fractures, and types of fractures. This research employed a descriptive design with a retrospective approach. Data were obtained from the medical records of patients diagnosed with maxillofacial fractures who were treated at Haji General Hospital Medan during the 2022–2024 period. A total of 62 patients who met the inclusion criteria were included as research samples. Data were analyzed univariately in the form of frequency and percentage distributions, and presented in tables and bar charts. The results showed that the number of maxillofacial fracture cases increased each year, with 16 cases (25.8%) in 2022, 20 cases (32.3%) in 2023, and 26 cases (41.9%) in 2024. Based on age, the highest number of cases occurred in the 21–30 years age group (33.9%), followed by the 11–20 years group (30.6%). Based on gender, males experienced more maxillofacial fractures (46 cases, 74.2%) compared to females (16 cases, 25.8%). The main cause of fractures was traffic accidents (58.1%), followed by falls (17.7%). The most common types of fractures were maxillary fractures (41.2%) and zygomatic fractures (40.5%), followed by mandibular (6.9%), frontal and nasal (3.8% each), naso-orbito-ethmoidal (NOE) fractures (2.3%), and orbital fractures (1.5%). In conclusion, maxillofacial fractures at Haji General Hospital Medan in 2022–2024 were predominantly found in male patients of productive age (21–30 years), mainly caused by traffic accidents, with the most frequent fracture types being maxillary and zygomatic fractures. It is recommended that the public increase awareness and safety measures, especially while driving, including the use of protective equipment. Health institutions should strengthen educational programs on facial trauma prevention. Patients are advised to seek immediate medical attention to prevent complications. Further research is suggested to analyze more specific risk factors and evaluate post-treatment clinical outcomes.
