Abstract

Diabetes mellitus (DM) is a metabolic disease characterized by increased blood sugar levels (hyperglycemia) and is associated with impaired function of β cells. The uncontrolled and prolonged hyperglycemia in DM can cause complications. One of the most common is diabetic non-healing wounds, the leading cause of disability in people with diabetes, therefore adjuvant therapy is needed. Compounds from natural ingredients are currently getting the spotlight in recent studies conducted to prove their effect on non-healing wounds, such as Resveratrol (RSV). This review aims to discuss the potential of RSV as a novel adjuvant therapy and its delivery system strategy in the case of diabetic non-healing wounds. We conducted an extensive search from three online scientific databases such as ScienceDirect, ResearchGate, and PubMed on February-March 2023. Keywords used are "resveratrol", "diabetic wound healing", and "diabetes mellitus". As a result, 86 studies were reviewed, and only 79 selected studies met all inclusion criteria. The formulation of RSV nanoparticles in oral drug delivery systems and topical administration has shown promise to overcome the limitations of RSV as adjuvant therapy in diabetic non-healing wounds. The best option that we found through this literature review is the nano-oral drug delivery system as it could minimize drug metabolism in the gastrointestinal tract, therefore the drug bioavailability and drug concentration on the target site could be maximized, even though the systemic side effects might occur.

Abstract

Inappropriate use of antibiotics due to lack of understanding and public knowledge is one of the causes of antibiotic resistance. Providing education via video to the public is a strategy that can be used to increase knowledge about the use of antibiotics. This study aims to assess the effect of providing local language educational videos about antibiotics and their use on increasing the knowledge and attitudes of women living in the Banjar Regency area, South Kalimantan. This research is Quasi-experimental research with a pretest posttest intervention with a control group research design using a purposive sampling technique and an intervention in the form of educational videos in the Banjar language. A valid and reliable questionnaire was used to assess knowledge and behavior in using antibiotics. Analysis of changes in knowledge and behavior scores between the control and intervention groups was carried out with the Mann-Whitney test. The number of respondents involved in the study was 80 women who were divided into a control group (40 people) and an intervention group (40 people). The results showed that the average pre-test and post-test knowledge scores in the control group (7.58 ± 1.81 and 7.85 ± 2.11) and the intervention group (7.33 ± 2.30 and 8.78 ± 2.93) with a significance value of p = 0.026. The mean scores of pre-test and post-test behavior in the control group (6.03 ± 2.60 and 6.15 ± 2.70) and the intervention group (6.33 ± 2.37 and 7.50 ± 2.05) with a significance value of p = 0.016. It can be concluded that providing educational videos in Banjar language can increase Females knowledge and behavior in using antibiotics in Banjar Regency.

Abstract

Providing information when dispensing medicine is a pharmaceutical service that must follow applicable regulations. The best service makes patients satisfied and loyal to the pharmacy. Many things influence patient satisfaction due to varying patient perceptions regarding the quality of pharmacy services. This study aims to analyze the relationship between providing drug information and patient satisfaction, as well as confounding variables. Cross-sectional research was carried out by distributing Likert scale questionnaires and a checklist of types of drug information provided by pharmaceutical staff at the Indobat Padangsambian Pharmacy. The questionnaire was developed from literature studies and has been tested for validity and reliability. A total of 118 respondents who met the criteria were taken using convenience sampling techniques. Data were analyzed using descriptive statistics, chi-square test, and logistic regression. The chi-square test shows that the type of drug information conveyed by pharmaceutical staff is not related to patient satisfaction at the Indobat Padangsambian Pharmacy (p-value 0.580, CI 95%). All variables that are thought to be confounding variables do not have an independent effect on the dependent variable. However, it is known that occupation (health workers and non-health workers) and the patient's proximity to pharmaceutical workers interact and influence patient satisfaction. Pharmacy personnel should provide drug information according to regulations. To make patients satisfied and loyal to the pharmacy, pharmaceutical staff must pay attention to the patient's work background and personal relationship with the patient when providing drug information.

Abstract

Hypertension is a "silent killer". It is still one of the bigest challenges in Indonesia, where many cases are still found in primary health facilities. Hypertensive patients who require long-term therapy and adherence are very important factors to support the success of therapy. Based on the data from the Ministry of Health of the Republic of Indonesia (2018), 32.3% of hypertensive patients did not take medication regularly. Many factors influence medication adherence, such as level of knowledge, age, gender, working status, and length of illness. This study aimed to determine the effect of the level of knowledge and other factors on the level of medication adherence of hypertensive patients at the Dr. Mintohardjo Naval Hospital (RSAL). This research is a observasional study with a cross-sectional method and uses a questionnaire for data collection. Adherence level questionnaire uses MMAS-8. Analysis of the relationship between factors that influence the level of adherence uses logistic regression. The inclusion criteria of this study included hypertention outpatient aged ≤ 65 years old and are currently receiving antihypertensive therapy. The results of this study showed that out of 284 respondents, 92% of respondents had good knowledge regarding hypertension, while the level of adherence to taking medication was low at 47%. Factors that influence the level of patient adherence to taking medication in this study are gender (p-value = <0.001), education (p-value = <0.001), duration of hypertension (p-value = 0.033), and working status (p-value = <0.001). In contrast, the level of knowledge (p-value = 0.979) and age (p = 0.87) did not significantly influence the level of medication adherence. The Odds Ratio (OR) for working status was 5.73, OR for final education was 2.97, OR for the duration of hypertension was 2.24, and OR for gender was 0.34.

Abstract

Competitive strategy analysis can be fundamental for responding to external conditions and increasing internal capabilities in generating a company’s competitive advantage. The purpose of this research is to determine the company's position in the industry as a basis for selecting alternative strategies so the company can strengthen its competitiveness and overcome its weaknesses in facing competitors. The methodology is descriptive qualitative research. Observation is done by data obtained from secondary sources. The analysis carried out includes: The Input Stage with Competitive Profile Matrix; The Matching Stage with Boston Consulting Group (BCG) Matrix and the Grand Strategy Matrix based on competitive analysis according to Porter's Five (Plus One) Forces; The Decision Stage with the Quantitative Strategic Planning Matrix (QSPM). Justification was carried out based on a Forum Group Discussion with several experts using the Delphi Method. The result shows that there is a change in competitive conditions, thus the market leader company, Kalbe Farma, needs to make strategic adjustments. In conclusion, the results of strategy alternatives’ formulation through BCG Matrix and Grand Strategy, as well as the selection of strategies using the QSPM show that the priority strategies for Kalbe Farma, respectively sorted from the most recommended are Market Penetration, Market Development, and Product Development.

Abstract

The COVID-19 pandemic cases affect health and the economy and have become a burden for the global community. Effective and specific antiviral for COVID-19 is still controversial and has yet to be found. Antiviral use is the main mitigation strategy that tends to be more cost-effective but varies in effectiveness and cost depending on various factors. This study aims to evaluate the effectiveness, safety, and cost of antiviral therapy in moderate COVID-19. This observational-analytic study used a retrospective cohort design. Subjects were hospitalized patients with moderate COVID-19 who received antivirals in July-October 2021 and January-March 2022. Therapy effectiveness was measured by lowered NLR (Neutrophil-to-Lymphocyte Ratio) to a normal level and length of stay (LOS). Drug safety includes adverse drug events. The cost of therapy was calculated based on the hospital’s perspective (provider), using direct medical costs. A total of 170 patients were divided into two groups (94 patients remdesivir and 76 patients favipiravir). In this arm, there were 45 patients with lowered NLR (21 patients reached the target and 24 patients did not). The effectiveness of favipiravir and remdesivir was 70.6% and 32.1%. Remdesivir may lower NLR (2.92) versus favipiravir (1.55). Favipiravir had a shorter LOS than remdesivir (8 vs 9 days). Adverse drug events observed in remdesivir were nausea (47.9%) and abdominal pain (22.3%), while in favipiravir nausea (53.9%) and vomiting (22.4%). The average total cost of remdesivir is higher than favipiravir (IDR 19,772,985 vs IDR 13,819,727). Favipiravir has higher efficacy and a shorter LOS with a lower average total cost than remdesivir.

Abstract

Tuberculosis (TB) patient compliance with ongoing therapy and knowledge regarding TB has an impact on the cure rate. Despite implementing various strategies, the prevalence of TB in Indonesia remains high, and the success rate of its treatment is still below the global average, potentially leading to continued transmission of TB within the community. This study aims to determine the improvement of knowledge and compliance among pulmonary TB patients following an intervention involving education and the use of a medication reminder application in two community health centers in the city of Bandung. The research design was experimental, where 29 subjects recruited were randomized into two groups: one group receiving additional education and introduction to the mobile application (Jadwal Minum Obat), and the other group receiving only counseling at the health centers. Primary data that was collected include baseline test scores and final test scores of subjects based on the KATUB-Q questionnaire. Patient compliance was assessed using the self-assessment method with the MARS-5 questionnaire and the pill count method. The subjects' experiences in the intervention group while using the mobile application were also documented. A total of 16 subjects completed the study. The education method and medication reminder application used in this study have not significantly improved the knowledge and compliance of pulmonary TB patients in taking their medication (p > 0.05). However, percentage of consumed medication compared to the prescribed medication in the intervention group higher than the control group (87.4 ± 4.8% vs 78.8 ± 8.3%). Subjects found the application useful, though there were still some shortcomings. Further research with a larger sample size using an improved medication reminder application is needed.

Abstract

Microvascular complications can occur due to type 2 diabetes mellitus (DM). The cost of therapy increases due to the use of various antidiabetic drugs over a long period. This research is a pharmacoeconomic study that uses the Markov Model to compare costs and utilities in type 2 DM patients who receive metformin+glimepiride and metformin+vildagliptin therapy. Primary data was collected through the EQ5D questionnaire at Cirebon Harbor Hospital and secondary data (transition probability) was collected through a literature study. Characteristics of type 2 DM patients show that the majority of people suffering from this condition are aged between 55 and 59 years. Primary data from the hospital shows that the cost of therapy for controlled type 2 DM patients who receive combination therapy of metformin + glimepiride, and metformin + vildagliptin is Rp. 1,699,400, Rp. 5,807,786, and Rp. 10,100,426, respectively. Secondary data from articles treating type 2 DM shows that the 55-59 year age group is the largest. With an ICER of 61,493,943/QALYS, microvascular prevention in T2DM patients receiving the metformin+vildagliptin combination in Indonesia is considered effective and able to reduce the number of microvascular cases. This is based on the ICER value compared to the Indonesian Government's GDP, namely Rp. 71,030,183.3. This study shows that replacing glimepiride with vildagliptin for the prevention of microvascular complications is cost-effective and has better utility and effectiveness in reducing the incidence of microvascular complications. The use of vildagliptin can be recommended to reduce costs and the risk of microvascular complications.
