Abstract

Penelitian ini dilakukan untuk mengetahui efektivitas jamu tetes Soman 1® terhadap HbA1C dan mengetahui adverse events pada pasien diabetes mellitus tipe 2 di rumah Sakit UGM.Rancangan penelitian adalah experimental double blind, dengan teknik consecutive sampling pada pasien rawat jalan, dengan diagnosa diabetes mellitus tipe 2 di Rumah Sakit UGM. Sejumlah 30 pasien masuk dalam kriteria inklusi, kemudian dilakukan random allocation untuk menentukan pasien menerima perlakuan uji jamu tetes Soman 1® kombinasi metformin atau plasebo kombinasi metformin selama 12 minggu. Pasien yang di drop out berjumlah 3 orang. Uji independent t-test digunakan untuk membandingkan rata-rata penurunan HbA1C pada kedua kelompok, dengan nilai signifikansi p < 0,05. Sedangkan evaluasi adverse events yang dialami pasien selama penelitian dideskripsikan persentase kejadiannya.Hasil penelitian menunjukkan terdapat tendensi penurunan rata-rata HbA1C yang lebih tinggi pada kelompok uji (0,79%) dibandingkan dengan plasebo (0,38%), walaupun, secara statistik tidak ada perbedaan yang signifikan, dengan nilai p = 0,345 (p>0,05). Begitu pula secara klinis, penurunan rata-rata HbA1C pada kelompok uji cenderung lebih tinggi daripada kelompok plasebo, dengan nilai selisih perbedaan rerata penurunan HbA1C diantara kedua kelompok adalah 0,41%, walaupun secara klinis tidak berbeda signifikan (< 0,6%). Jumlah kejadian adverse events yang terjadi pada kelompok uji adalah 27 adverse events, antara lain mual (33,33%), diare (20%), nyeri perut (20%), nyeri badan, lemas dan pusing masing-masing 13,33%, alergi/gatal-gatal, nafsu makan meningkat, tenggorokan sakit, konstipasi, mulut terasa pahit, sakit pinggang, susah tidur, kaki kaku/pegal dan mengantuk masing-masing 6,67%, sedangkan pada kelompok plasebo, adverse events yang terjadi yaitu diare (13,33%) dan mual (6,67%).Kata Kunci: diabetes mellitus tipe 2, jamu tetes Soman 1®, HbA1c, adverse events

Abstract

Hypertension is a chronic disease, so patients with hypertension during his life always takes medication to control blood pressure. If blood pressure is not controlled it will result in a blood vessel complications, coronary heart disease, cardiac infarction, stroke and renal failure. The research has been conducted to determine the effect of a pharmacist counseling target blood pressure < 140/90 mmHg in hypertension patients non-diabetes mellitus and < 130/80 mmHg in hypertension patients with diabetes mellitus who was treated at the outpatients of Saras Husada Purworejo General Public Hospital. Retrieved 94 subjects of research, grouped randomly into intervention group (received counseling pharmacist) are 48 subjects and the control group (no counseling pharmacist) as many as 46 subjects. Data analysis using the Statistics Program for Social Science (SPSS). The results showed  that a pharmacist counseling had a significant effect toward the achievement target of < 140/90 mmHg blood pressure on the non-diabetes mellitus hypertension patients (p = 0.000) and < 130/80 mm Hg on the hypertension patients with diabetes mellitus (p = 0.001). Key words : pharmacist counseling, non-diabetes mellitus hypertension, hypertension with diabetes mellitus, decreasing of blood pressure, and the achievement blood pressure target.

Abstract

Kasus keracunan memberikan kontribusi sebesar 3,8% kunjungan di rumah sakit. Penelitian ini bertujuan untuk mengetahui karakteristik pasien keracunan dan hubungannya dengan jenis agen toksik, serta hubungan antara ketepatan terapi keracunan dengan biaya medis langsung dan lama tinggal di rumah sakit (Length of Stay/LOS). Penelitian ini menggunakan rancangan cross sectional dengan pengambilan data secara retrospektif. Subyek uji merupakan pasien rawat inap yang memiliki diagnosa utama keracunan, dengan atau tanpa penyakit penyerta dan komplikasi, serta pulang setelah dinyatakan sembuh. Pengolahan data berupa analisis univariat, evaluasi terapi, dan analisis bivariat. Sebanyak 103 pasien keracunan memenuhi kriteria sebagai subyek penelitian. Sebagian besar pasien keracunan yang dirawat inap di RSUP dr. Soeradji Tirtonegoro berdomisili di Kabupaten Klaten (80,0%), sedangkan yang dirawat inap di RSUP dr. Sardjito berdomisili di Kabupaten Sleman (32,1%). Mayoritas pasien keracunan merupakan remaja dan orang dewasa usia 15-64 tahun (62,2%), berjenis kelamin laki-laki (54,4%), berpendidikan rendah (32,0%), bekerja sebagai pelajar/mahasiswa (16,5%), dan merupakan peserta Jamkesmas (45,6%). Jenis agen toksik penyebab keracunan meliputi bisa ular (37,9%), insektisida (17,5%), obat (16,5%), makanan (14,5%), hidrokarbon (5,8%), alkohol (4,9%), serta pewangi dan pemutih pakaian (2,9%). Hasil uji Chi-Square menunjukkan bahwa karakteristik pasien yang mempengaruhi jenis agen toksik adalah umur (p=0,027), pendidikan (p=0,029) dan pekerjaan pasien (p=0,002), serta ketepatan terapi keracunan pada pasien Jamkesmas tidak mempengaruhi efisiensi biaya medis langsung (p=0,292) dan lama tinggal di rumah sakit/LOS (p=0,188). Kata kunci: keracunan, karakteristik pasien, agen toksik, terapi, biaya, LOS

Abstract

Anemia is mostly associated with chronic kidney disease (CKD) patients. Anemia in CKD patients occurs due to the lack of erythropoietin. Low hemoglobin concentration relates to bad clinic outcome toward CKD patients. Epoetin therapy for CKD patients has been clinically confirmed to improve the quality of life and lower the morbidity and mortality rate. This research was intended to investigate the anemia profile therapy, adjuvant per-oral anemia therapy adherence profile and the effect of adjuvant per-oral anemia therapy adherence toward achievement target of anemia therapy using epoetin in CKD Askes patient receiving regular haemodialysis therapy at PKU Muhammadiyah Hospital Yogyakarta. The research was conducted using observational study design with prospective data collection, then the data were analyzed in order to know the success of anemia primary therapy via epoetin and adjuvant per-oral anemia therapy. The success of therapy were measured based on National Kidney Foundation-Kidney Disease Outcomes Quality Initiative (NKF-K/DOQI), 2006, while adherence was measured via Modified Morisky Scale (MMS) questionnaire. Based on the research, epoetin used in this research was epoetin alpha and beta, each 69.05% and 30.95%. Adjuvant per-oral anemia therapy used in this research was combined folic acid, iron and vitamin B complex (82.05%); folic acid (10.26%); combined folic acid and vitamin B-complex (7.69%). Groups of epoetin sub dose with unachieved target and epoetin complete dose with unachieved target were 64.29% and 21.41%; Groups of epoetin sub dose with achieved target and epoetin complete dose with achieved target showed similar results 7.14%. The adherence profile of adjuvant per-oral anemia therapy were consisted of obedient patients and disobedient patients, each 69.23% and 30.77%. Askespatient adherence in using adjuvant per-oral anemia therapy, in this research, was able to assist the achievement of epoetin therapy target. Keywords : Anemia, adherence, chronic kidney disease, regular haemodialysis, epoetin, adjuvant per-oral anemia

Abstract

Hospitals which are engaged in services, not only serve the sick people but must also consider aspects of customerssatisfaction. This research aims to determine whether it is the characteristic of patient that causing significant differenceon their satisfaction, to determine service attribute which require attention in order to service improvement towardscustomers, and to determine the gap between service quality with patient satisfaction in Pharmacy Department PKU Muhammadiyah Bantul hospital from the dimensions of SERVQUAL: reliability, responsiveness, assurance, empathy, and tangible.The research was designed by non experimental methods. Samples of this research are customers or families ofpatiens or outpatients taking drug at Pharmacy Department PKU Muhammadiyah Bantul hospital. Conveniencesampling is used as sampling method with number of samples 100 persons. Questioners are used as data collectionmethod taken on August 2009.The results showed that there is no significant difference between patient characteristic on patient satisfactionin Pharmacy Department PKU Muhammadiyah Bantul hospital. Attributes that become top priority to improvingservice quality are the friendliness and courtesy of employee, quick to follow up issues, and availability of thenumber and types of drugs. There are gaps in most of the service quality attributes in Pharmacy Department PKUMuhammadiyah Bantul hospital. Abstract

Optimalisasi dan pengembangan layanan kesehatan di rumah sakit diwujudkan dengan mempertahankan layanan bermutu tinggi oleh staf rumah sakit. Kunci pokok untuk menjaga layanan yang diberikan staf adalah menjaga kepuasan kerjanya agar tetap dalam taraf yang maksimal. Tujuan dalam penelitian ini adalah menganalisis pengaruh perilaku kerja, lingkungan kerja, interaksi sosial terhadap kepuasan kerja staf di RSUD Pandan Arang Boyolali dalam kondisi sebelum dan sesudah menggunakan motivasi sebagai variabel mediasi, kemudian membandingkannya. Penelitian dilakukan dengan rancangan penelitian deskriptif dengan survei langsung ke lapangan menggunakan kuesioner. Teknik pengambilan sampel menggunakan pendekatan Tabachic dan Fidell. Data dianalisis dengan menggunakan analisa jalur, dengan bantuan Lisrel 9.1. Hasil penelitian menunjukkan bahwa perilaku kerja, lingkungan kerja, dan interaksi sosial karyawan masing-masing mempunyai pengaruh yang signifikan dan positif terhadap kepuasan dan motivasi kerja staf di RSUD Pandan Arang Boyolali. Sedangkan  motivasi sebagai variabel mediasi dapat meningkatkanpengaruh perilaku kerja, lingkungan kerja, dan interaksi sosial terhadap kepuasan kerja.Kata kunci: perilaku kerja, lingkungan kerja, interaksi sosial, motivasi,kepuasan kerja

Abstract

Forecasting of pharmaceuticals and medical devices are an effort to estimate the needs of pharmaceuticals and medical devices in hospital. Pharmaceuticals and medical devices of class A based on ABC analysis represents a small amount in stock but has the largest inventory value. This study aimed to determine the exact forecasting that used to estimate the needs of pharmaceuticals and medical devices in class A in the Pharmacy Department Immanuel Hospital Bandung. This research was a descriptive study with retrospective data collection. Forecasting analysis of pharmaceuticals and medical devices needed used the average of last period, seasonal variation, and seasonal index with trend adjustment in 2011. Forecasting analysis was calculated based on consumtion of pharmaceuticals and medical devices in 2008-2010. Accuracy of forecasting methods were determined by the smallest value of mean absolute deviation (MAD) and mean squared error (MSE).  The results from 24 samples of pharmaceuticals and medical devices in class A showed that there were 12 samples (50%) with the smallest MAD for seasonal index with trend adjustment method, 8 samples (66.67%) with the smallest MAD values for average of last period method, and 4 samples (16.67%) with the smallest MAD for seasonal variation method. According to MSE, there were 14 samples (58.33%) with the smallest MSE value for the seasonal index with trend adjustment method, 8 samples (66.67%) with the smallest MSE value for the average of last period method, and 2 samples (8, 33%) with the smallest MSE for seasonal variation method. It can be concluded that seasonal index with trend adjustment analysis was better compared the method of seasonal variation and the average of last period to forecasting needs of pharmaceuticals and medical devices in A category.Keywords:seasonal index with trend adjustment, ABC Analysis, Immanuel Hospital Bandung

Abstract

Peranan farmasi klinik antara lain mengidentifikasi, mencegah, dan mengatasi Drug Related Problems (DRPs). Penelitian - penelitian sebelumnya menunjukkan faktor risiko terjadinya DRPs antara lain: usia, jenis kelamin, polifarmasi, jumlah obat yang diterima, diagnosa serta penyakit penyerta. Prevalensi penyakit dalam menurut Riskesdas pada tahun 2013 cenderung mengalami peningkatan. Oleh karena itu perlu adanya penelitian yang bertujuan untuk mengkaji hubungan faktor risiko terhadap kejadian DRPs pada pasien penyakit dalam. Penyakit kronis yang diteliti adalah diabetes melitus, hipertensi, hipertiroid, hipotiroid, sirosis hepatik dan PPOK.Penelitian ini merupakan penelitian case control dengan pengambilan data menggunakan metode purposive sampling. Data diambil dari resep rawat jalan poliklinik penyakit dalam pada bulan Mei 2015. Analisis data dilakukan dengan statistik deskriptif untuk menunjukkan kejadian DRPs. Untuk melihat hubungan faktor risiko dengan DRPs digunakan Metode Chi square dengan tingkat kepercayaan 95%. Untuk mengetahui hubungan antar faktor-faktor risiko dianalisis dengan metode regresi linear logistik berpasangan (multivariat).Hasil peneltian menunjukkan sebanyak 185 kejadian DRPs pasien rawat jalan dengan penyakit kronis Jenis kejadian DRP berturut- turut dari yang paling banyak adalah indikasi tanpa terapi 46 kejadian (24,73%), dosis obat berlebih 41 kejadian (22,04%), ROTD 41 kejadian (22,04%), obat tidak efektif 38 kejadian (20,43%), terapi tanpa indikasi 11 kejadian (5,92%) dan dosis obat kurang sebanyak 9 kejadian (4,48%). Usia lanjut, multiple comorbid, dan polifarmasi, berpengaruh terhadap kejadian DRPs (p < 0,05).Kata kunci: faktor risiko, DRPs, peresepan rawat jalan, penyakit kronis

Abstract

Although the number of pharmacists in primary health care is limited, counseling is very important. The Government obligates primary health care to use poster as a health promotion method. In this study, counseling and poster were used to inform patients about hypertension and its therapy. The objective of this study was to compare the influence of counseling and poster towards adherence and therapeutic outcome on hypertensive patients in Bakti Jaya Primary Care, Depok. Research was conducted by quasi experimental using non-equivalent control group design. Samples were hypertensive patients at Bakti Jaya Public Health Care Depok collected from March to May 2012. Samples were divided into two groups i.e. counseling and poster groups. In the first group, pharmacist gave counseling to the patients and in the other group, the patients were only given poster. Patient’s adherence and blood pressure as therapeutic outcome were measured before (pre test) and after intervention (post test). Patient’s adherence was measured using Morisky Medication Adherence Scale (MMAS-8) questionnaire. This study showed that counseling enhanced adherence (P=0.000) and decreased systolic and diastolic blood pressure (P=0.010 and P=0.018), whereas poster only increased adherence (P=0.028). The differences of MMAS-8 score between counseling and poster group were significantly different (P=0.017), and the decreasing of systolic and diastolic blood pressure between counseling and poster group were not significantly different (P=0.170 and P=0.410).Keywords : Adherence, Hypertension, Counseling, Poster

Abstract

Pharmacist is a profession whose existence is needed by community. However, within the community of pharmacy itself, the presence of pharmacist is quite alarming. One of the causes of low pharmacist who desire to practice in the community of pharmacy is the low level of professional fee offered. This study aimed to describe professional fee received by pharmacist and the workload of pharmacist in Sleman. This study was a descriptive non-experimental that was conducted in Sleman in August 2017. Population of this study was a pharmacist who practiced in community of pharmacy in Sleman. Total of 170 respondents from 128 pharmacies were involved in this research. However, 6 questionnaires cannot be analyzed because it is not completely filled. The highest amount of professional fee is between Rp 2.000.000-Rp 3.000.000. Almost all respondents (78%) stated the amount of professional fee that was received was not as expected. The workload of pharmacists is not so heavy and most of the patient are self medication. Pharmacists at the community of pharmacy should be able to show a better work performace, which is shown by increasing pharmacy revenues to get a better rewards.
