Abstract

Background : Psychological changes that occur during pregnancy can alter behavior during and after childbirth. Throughout pregnancy, especially near term, raised concerns about child care and lifestyle changes. The conflict between the desire of procreation, social norms and cultural problems in the pregnancy itself can be an originator of a variety of psychological reactions, ranging from mild emotional reaction to the rate of mental disorders. Pregnancy and childbirth have safety risks that will affect the level of maternal anxiety during pregnancy and before delivery. So which is the formulation of the problem in this research is how the characteristics of pregnant women at term factor affecting the level of anxiety.Methods : The subjects were patients at the outpatient clinic of Obstetrics and gynecology Dr Haji Adam Malik, and hospital networks. the way of sampling through consecutive sampling with criteria for inclusion and exclusion to a minimum sample size of each group 55 samples. using bivariate data analysis with comparative analytic using Chi Square test for categorical variables and independent t-test for numerical variables.Results : Pregnant women who give birth either spontaneous vaginal delivery (87.3%) as well as a Cesarean section (87.3%) did not feel anxious. mild anxiety pregnant women who will give birth to spontaneous delivery (12.7%) and cesarean section (12.7%). Statistically with Chi-square test showed no significant difference.Conclusion : Conclusion pregnant women who planned spontaneous vaginal delivery or cesarean section does not feel any anxiety in the face of labor .Keywords : anxiety; pregnancy; spontaneous vaginal delivery; cesarian section

Abstract

Background: Endotracheal intubation is routinely performed during general anaesthesia in patients undergoing intraocular surgery. However, intubation is associated with an increase in intraocular pressure (IOP).Aim: To compare the effect of clonidine and lidocain to prevent the increase of IOP during endotracheal intubation. Method: Forty patients with no pre-existing eye diseaseundergoing elective non-ophthalmic surgeriesundergeneral anesthesiawererandomly allocated to two groups of 20 each.In Group A, patients received clonidine 2μg/kg/iv, and in group B lidocain2% 1.5mg/kg/iv for premedication.The hemodynamic profile (systolic and diastolic blood pressure, mean arterial pressure, heart rate) and IOP (using Schioetz tonometer) were measured before premedication (pre anesthesia), before intubation, immediately after intubation, and every 1 minute for 2 minutes.Result:There was a significant decrease in IOP and hemodynamic profile within both groups before and after endotracheal intubation compare to pre anesthesia periode (P <0.05). But the decreasewas not different between two groups (P >0.05). Conclusion: We conclude that both drugs have the same ability in blunting the hemodynamic response and attenuating the IOP, and preventing the increase in IOP caused by endotracheal intubation.Key word : clonidine, endotracheal intubation, intra-ocular pressure, lidocain

Abstract

Background : Chikungunya outbreak was reported in Indonesia on 1973 at Samarinda and DKI Jakarta. On 2000-2007, chikungunya outbreak occurred in all provinces with 18.169 cases without mortality in Indonesia. Now, almost all provinces in Indonesia are potential for chikungunya outbreak. Chikungunya is a disease caused by chikungunya virus, which is transmitted by mosquitoes Ae. aegypti. Some studies have been conducted to find out the chikungunya viral in some endemics area, however still few studies conducted in Sumatera Utara. The aim of this study was to find out chikungunya viral in mosquito of Ae. aegypti in four villages in District of Sergai. Minimum Infection Rate (MIR) is a measure of proportion of the mosquitoe population which is carrying a particular virus. When infection rate increases the risk of virus transmission to humans also increases.Method : This was a descriptive study for 50 samples of Ae. aegypti which were caught randomized from four villages of two endemis chikungunya subdistricts in district of Sergai then examined by using Reverse-Transcriptase Polymerase Chain Reaction (RT-PCR) technique to find out chikungunya virus. Chikungunya virus was examined by using TitanTM One Tube Reverse Transcriptase-Polymerace Chain Reaction RT-PCR) kit and CHIKnsP1-S sense primer (5’-TAG-GAC-AGG-AAA-TTGATC-CC-3’), CHIKnsP1-S anti-sense primer (5’-CTT-TAA-TCG-CCT-GGT-GGT-AT-3’). RT was running on temperature of 37C for an hour to gain cDNA, then amplifying by 35 recycling on temperature of 94oC for 3’, 94oC for 1’, 54oC for 90”, 54oC for 2’ and extension on temperature of 72oC for 2’.Result : 13 samples were positive for chikungunya virus from 50 samples which were examined. MIR was 130%.Conclusion : 13 samples which were found positive for chikungunya virus. MIR 130%, this was appropriate with high infection of chikungunya on patients where the mosquitoes were caught.Key words : Ae. aegypti, chikungunya virus, RT-PCR

Abstract

Background : Fibrinogen is a coagulation factor that plays a role in thrombus formation. Many studies have reported that relatively high levels of fibrinogen plays a role in the occurrence of thrombosis in the coronary arteries (myocardial infarction). Atherosclerosis is known by angiographic examination. The purpose of this study to determine whether the levels of fibrinogen in CHD patients with stenosis >70% higher than the ≤70% stenosis.Methods : This analytical cross-sectional study was conducted in H.Adam Malik hospital, during August -November 2013. Subjects were patients with suspected CHD and angiography performed. Criteria for CHD when angiography showed coronary artery stenosis >70% as cases and group ≤70% stenosis as a control. Blood samples were taken 24 hours before angiography to checked fibrinogen levels.Results : In this study, we found significant differences in fibrinogen level between non-CHD 266.85 (47,644) and CHD patients 315.17 (80.184) with p<0.05.Conclusion :. Abstract

Introduction : Health-related quality of life’s (HRQOL) assessment has become a vital tool. In this study, we analyze two generic instruments ECOG Scale and SF-36 in NHL patients receiving CHOP chemotherapy. This study intends to find out the the correlation between SF-36 and ECOG scale in NHL patients receiving CHOP chemotherapy.Method : Cross sectional research was conducted from October 2012 up to December 2013. Samples were selected consecutively to NHL patients. SF-36 and ECOG scale was assessed after CHOP chemotherapy cycle 6.Results : There were 21 patients of NHL, 12 male, 9 female, age 20 – 64 years old, mean 47.95 (SD 10.85). The range score of SF-36: 23.9 – 95.9, mean 75,3 (SD 17.93). The range scale of ECOG 0 – 4, mean 0.81 (SD 1.12). By using Spearman correlation, we found a significant correlation between SF-36 and ECOG scale (r = - 0.744, p = 0.0001). SF-36 showed good diagnostic performance to predict good ECOG scale with cut-off •'3d 54.66, with sensitivity 94.7%, specificity 100%, PPV 100%, NPV 66.67%, LR (+) ~, LR (-) 0.05, accuracy 95.23%, and AUC 98.7% (95% CI: 0% - 100%; p = 0.027).Conclusion : SF-36 correlates with ECOG scale and has a very good diagnostic in predicting ECOG scale.Key word : SF-36; ECOG scale; Quality of life; Non Hodgkin Lymphoma; CHOP chemotherapy

Abstract

Introduction : In COPD patient, need to lung function examination and measurement ventilatory muscle strength. MVV recommended as a test more spesific to measure ventilatory muscle fatigue, lung and chest mechanically capacity. MVV is a maximal air breathing exhaled in a period during hardly and fastly breathing. The aim of this study is to assess effect of pulmonary rehabilitation program in copd patient to MVV and FEV1 value before and after pulmonary rehabilitation.Method : This study as clinical trial with pretest and post test design during 12 weeks from october 2013 until december 2013 as many as 14 persons. Spirometry examination doing in these patients with MVV and FVC manouver and also pulmonary rehabilitation.Results : There is no difference in FEV1 value but increasing MVV value although not significant based on statistic. Mean of MVV value before rehabilitation is 37.379 (SD 17.1296) and after rehabilitation is 38.229 (SD 16.5461) with p= 0.849. mean of FEV1 value before rehabilitation is 45.93 (SD 21.334) and after rehabilitation is 45.21 (SD 19.510) with p= 0.768. Conclusion : Statistically there is no significant changed in FEV1 value before and after rehabilitation (p=0.768) Keywords : copd; mvv; fev128

Abstract

Objectives : To know the charachteristic of femoral fracture patients in RSUP Haji Adam Malik Medan period of Januari 2009- Desember 2010.Background : Femoral fracture has a high incidence among other kinds of fracture that can caused the serious physical suffering, mentally affected, and loss time of daily working of patients. Thus femoral fracture have low mortality rate but with high morbidity rate. The method of prevention can be done by identifying the related variables through the epidemiology studies comprehensively.Method : Collecting the secunder data that recorded on medical record of patients that diagnosed femoral fracture in RSUP. Haji Adam Malik Medan (RSUP HAM) period of Januari 2009 – Desember 2010.Results : Found that femoral fracture in RSUP HAM most cases is male with age 21-30 years old that cause 93.92 % by traffic accident. Most region of fracture is mid-diaphysis about 54.7%. Also found about 179 samples (72.46%) from outside Medan with most of education level is elementary school about 178 subjects (72.06 %). And they were brought to the RSUP HAM below than 72 hours is about 163 subjects (66.0%). For the subjects with age 0-50 years, most often region of femoral fracture is mid diaphysis, meanwhile for age above 50 years old is neck femur. For the fracture classification, found that closed fracture is more often than open fracture femoral fracture.Conclusion : Most of femoral fracture patient in RSUP HAM is male that classified as productive age caused by road traffic accident. And most of them is in mid diaphysis region of femoral. The preventive method by focusing to decrease incidence of road traffic accident is the main priority followed by improving the tools and skills for treating the femoral fracture cases. Abstract

Beta-thalassemias major are a group of hereditary blood disorders disease. Growth disorders are afrequent complication of patient with Beta-thalassemias major. The pathogenesis of disorders is multifactorial and is mainly due to chronic anemia and hypoxia, chronic liver disease, zinc and folic acid deficiency, iron overload and intensive use of chelating agents. Proper management of each patient with beta-thalassemias major include early recognition of growth disorders and appropriate of iron chelation therapy is very important to improve the quality of life.Keywords : beta-thalassemias major; growth disorders
