Abstract

Introduction : With the development of science and technology, the mobility in Indonesian are also higher and resulting in increased of motor vehicle accident so can increases the risk of of bone fractures. Mostly indonesian society still believe in the handling of fractures by bone setter, thus increasing the incidence of neglected fractures, which is often found in Haji Adam Malik General Hospital. This research purposes to know the characteristics and prevalence of neglected fractures in Haji Adam Malik General Hospital in January 2009 - December 2010.Methods : This is a retrospective descriptive study, in which data is extracted from the medical records of patients with neglected fractures were treated in Haji Adam Malik General Hospital in 2009-2010. Data are presented by frequency distribution tables and charts, and analysis in the analysis of meaning.Results : Neglected fractures patients were treated in the Haji Adam Malik General Hospital in 2009-2010 is more common in men (72.73%) and in the productive age (92.73%), with a low education level (37.73%) and previously have received treatment by bone setter (89.09%). The cause of the occurrence of fractures was traffic accident (98.18%), with the most location of fractures is femur (52.73%).Conclusion : The characteristics of patients neglected fractures in Haji Adam Malik General Hospital are common in men, with productive age, low education level, and has been handled by bone setter before came to Haji Adam Malik General Hospital. The most common cause of fracture due to traffic accidents and the most common location of fracture is femur. Abstract

Objective : To determine the correlation between characteristics and the level of knowledge, attitudes and behavior among midwife in primary health care in Medan about pap smear and cervical cancer.Method : This study was a descriptive analytical with cross sectional design conducted using a 3-sections questionnaire containing 15 knowledge questions, 12 attitude questions, and 8 behavior questions at 100 midwives in 39 primary health care in Medan from October until November 2013. Data was collected by.Data analysis was performed using two way Chi Square with a significance level of 95% (p<0.05).Results : Majority of respondents were good in knowledge (84%), attitude in adequate level (52%), and behavior in good level (52%). In Chi Square analysis, no significant relationship found between educations and knowledge, attitudes, behavior (p=0.184; p=0.417; p=0.185); no significant relationship found between age yet length of working with knowledge, attitudes (p=0.191; p=0.478 and p=0.22; p=0.649) while significant correlation found between age yet length of working with behavior (p=0.003 and p=0.0037).Conclusion : Based on these results, it is shown that better behavior is found in midwife who has an older age and long working age.Keywords : midwife; cervical cancer; pap smear

Abstract

Introduction : Reticulocyte Hemoglobin Equivalent (RET He) is a new parameter for evaluating iron status. RET He is not influenced by chronic inflammatory conditions thus could provide better information of iron status in patients with chronic kidney disease (CKD) with regular hemodialysis. The aim of this study was to investigate the changes in RET He after iron supplementation to determine the usefulness of this marker as iron status parameter in CKD patients with regular hemodialysis.Methods : Uncontrolled clinical study was conducted at Hemodialysis Center in Adam Malik Hospital Medan during April-May 2011. A total of 15 regular hemodialysis patients with iron deficiency anemia received intravenous iron supplementation (100mg Fe sucrose) every hemodialysis session (2x/weeks) for 4 weeks. The changes in hemoglobin, hematocrit and RET He before and after the intervention were analyzed using paired t test.Results : Mean hemoglobin, hematocrit and RET He before intervention were 8.6 (SD 1.09) g/dl, 28.0 (SD 4.25)% and 29.9 (SD 3.85) pg, respectively. After intervention they were 9.9 (SD 1.77) g/dl, 31.7 (SD 5.31)% and 32.6 (SD 3.24) pg, respectively (P=0.023, P=0.049 and P=0.019).Conclusions : RET He were significantly higher after administration of intravenous iron supplement in iron deficient anemia in CKD patients with regular hemodialysis. Abstract

Background : Stroke constitutes a national health problem with many risk factor. Leptin, produced by adipocyte was shown to be important risk factor for stroke. The circulating lipids and lipoproteins plays an important etiologic role in stroke. Waist to hip ratio (WHR) has been shown to be predictor of stroke. The purpose of this study is to investigate the correlation between plasma leptin, plasma lipid and WHR with stroke.Methods : A cross-sectional study performed in stroke and non stroke matched patients in Adam Malik General Hospital from March until June 2012. Concentration of plasma leptin, plasma lipid and WHR was examined and then performed a statistical analyzed.Results : From 62 subjects with stroke and non stroke have a significant correlation between plasma leptin with stroke in female group (p=0.036), but not in male (p=0.454), a significant correlation between HDL (p=0.000) and triglyceride (p=0.000) with stroke, no significant correlation between WHR and stroke in male (p=0.776), significant correlation between waist circumferences (p=0.005) and hip circumferences (p=0.002) with stroke in male. There was moderate correlation between plasma leptin and total cholesterol in stroke (r=0.413), weak correlation between plasma leptin and LDL in stroke (r=0.392), moderate correlation between plasma leptin and waist circumferences in stroke (r=0.505), strong correlation between plasma leptin and hip circumferences in stroke (r=0.624). No significant correlation between plasma leptin (p= 0.937), plasma lipid and WHR in stroke Conclusion : There was significant correlation between plasma leptin and stroke in female group, significant correlation between HDL and triglyceride with stroke, significant correlation between total cholesterol and LDL with plasma leptin in stroke. Abstract

Background : Malnutrition in hemodialysis patients is common and it’s a regular independent factors affecting morbidity and mortality. This relates to the level of uremic toxins clearance during the process of hemodialysis is less than optimal. Clinical Application of hemoperfusion combination with hemodialysis (HD/HP) demonstrated the optimal clearance in molecular uremic toxins clearance. Kidney Disease Outcomes Quality Initiative (KDOQI) recommends assessment nutritional status of regular hemodialysis patients with a combination of valid assessment tools. such as 7-point SGA (Subjective Global Assessment) and albumin. This aim of the study to determine correlation between the combination of HD/HP with the nutritional status of regular hemodialysis patients.Method : This prospective cohort study from December 2013 to March 2014 on 20 regular hemodialysis patient and do history. measurements of height. weight. body mass index. nutritional status assessment by 7-point SGA and examination of serum albumin.Results : At the end of observation. 20 subjects were observed one subject out of the study. There is a significant difference between the mean of 7-point SGA score before and after combination hemodialysis/hemoperfusion (p<0.05). There is a significant difference between the mean albumin before and after first combination Albumin HD/HP (p<0.05). There is a strong relationship between the 7-point SGA with BMI after combination (r=0.636.p<0.01).Conclusion : The combination of HD / HP is good clinical application for the treatment of malnutrition in regular hemodialysis patients. Regular assessment of nutritional status of regular hemodialysis patients is necessary. with a combination of a valid assessment tool (7-point SGA. albumin) is expected to help decrease morbidity and mortality.Key word : Hemodialysis; haemoperfusion; 7-point SGA; albumin

Acute kidney injury is characterized by a reversibe increase in the blood concentration of creatinine an nitrogenous waste products and by the inability of the kidney to regulate fluid and electrolyte homeostasis approprietly. Acute kidney injury is increasingly recognized as a cause of increased morbidity in critically ill children and adults, and affects patient survival. Abstract

Septic shock remains a major cause of morbidity and mortality in children admitted to pediatric intensive care unit. The incidence of septic shock increased in 30-40 years, with estimate 7% of total deaths in children due to severe sepsis and septic shock. The cardiovascular system is one of the most commonly affected organ in a state of severe sepsis and septic shock. Troponin I is a good marker for assessing the extent of heart muscle damage and elevated levels of troponin I in sepsis patients reflects the severity of the disease, myocardial dysfunction, and poor prognosis. Septic shock patients with impaired cardiac function have an increased risk of mortality 70% to 90% when compared to patients with or without cardiac dysfunction.Keywords : septic shock; troponin I; mortality

Abstract

Background : Urinary tract infection (UTI) is the most frequent infectious diseases occur and took second place after respiratory tract infections, where his first-line therapy is an antimicrobial. Many sufferers are not improved after treatment due to the onset of bacterial resistance to antimicrobials specific types and also the discovery of bacteria producing Extended-Spectrum BetaLactamase (ESBL). The aim of study is to determine the differences in patterns of antimicrobial resistance urinary tract infections caused by ESBL-producing bacteria and Non-ESBL.Methods : This study was an observational study with cross-sectional method included 50 outpatients in Departments of Internal Medicine Haji Adam Malik Hospital. Urinalisa examination found nitrites(+) or hematuria(+) or leukocytes> 10/lpb and continued urine cultures, Double Disk Synergy Test (DDST) and sensitivity test.Results : Antimicrobial resistance in ESBL groups were ampicillin (94.7%), and trimethoprim/sulfamethoxazole, ceftriaxon (84.2%), followed by cefotaxime, cefpodoxime and tetracyclin (78.9%) while in Non-ESBL group were ampicillin (88%), and trimethoprim/sulfamethoxazole (84%), and tetracyclin (76%). Differences patterns of antimicrobial resistance in E.coli ESBL producing group and Non-ESBL found cefotaxime significant (P = 0.021) and aztreonam significant (P = 0.031). Conclusion : ESBL-producing bacteria have an increased resistance to several antimicrobial mainly cephalosporin class although not statistically significantly different.Keywords : antimicrobial resistance; urinary tract infection; extended spectrum beta lactamase

Abstract

Introduction : Hypertension in pregnancy is one of the causes of maternal morbidity and mortality is high. It cause of 30-40% of perinatal deaths. This state is related to blood viscosity and hematocrit is an important determinant of the blood viscosity. Careful observation of the several indicators, such as hematocrit levels can prevent from unwanted circumstances. Methods : This research is to design pre test - post interventions to determine the relationship between hematocrit levels in preeclampsia before and after childbirth. Pre and post test data is performed using the t test (t-test) and to find the relationship of hematocrit levels with the degree of preeclampsia with Chi-square test.Results : Of the total 55 cases, most cases of severe preeclampsia occurs in the age group 21-35 years were 43 cases (78.2%) cases, have never given birth were 22 cases (40%), history of antenatal care is often enough (4 - 8x during pregnancy) that is found in 30 cases (54.5%), gestational age at term 30 cases (54.5%), and delivery baby with a normal weight in 29 cases (52.7%). Hematocrit levels showed a significant reduction in the time before and after 24 hours of delivery of 34.018 + 0.690 percent to 30.140 ± 0.656 percent (p<0.001). There were no significant differences in hematocrit levels based on the severity of severe preeclampsia both before birth and after 24 hours of labor (p>0.05).Discussion : Found a significant difference in hematocrit levels at the time before and after 24 hours of delivery. Hematocrit levels not influenced by the severity of severe preeclampsia.Keywords : Severe preeclampsia; hematocrit levels
