Abstract

The use of nanotechnology in pharmaceuticals, especially for cancer therapy, has shown significant development through the innovation of liposome-based drug delivery systems. Liposomes as delivery nanosystems improve stability, bioavailability, and drug specificity to target cells, while lowering systemic toxicity. Various strategic approaches such as conjugation with antibodies (trastuzumab), enzyme (hyaluronidase), RNA, and combination with siRNA and gold nanoparticles demonstrate remarkable ability to penetrate biological barriers and overcome drug resistance. In addition, multifunctional liposomes allow simultaneous delivery of chemotherapy and immunotherapy synergistically. Based on the results of our literature/review study, it shows that nanotechnology-based liposomes significantly increase the effectiveness of cancer therapy by increasing stability, bioavailability, and drug specificity, and addressing resistance through various strategies such as conjugation and delivery of combinations of therapies. As shown in pancreatic, lung, cervical, and HER2-positive breast cancer therapy. These studies have confirmed that nanotechnology-based liposome formulations are not just a delivery device, but a precision, effective, and potentially changing the paradigm of cancer treatment.

Abstract

Seizures are characterized by sudden, uncontrolled electrical disturbances in the brain that affect behavior, movement, and consciousness. Diazepam, a benzodiazepine, is widely used to manage seizures by enhancing GABA-mediated inhibition. Previous studies suggest that Myristica fragrans (nutmeg) essential oil also has anticonvulsant properties through a similar mechanism. This study aimed to investigate the synergistic anticonvulsant effect of combining diazepam and nutmeg essential oil using a strychnine-induced seizure model in mice. Twenty-four male Mus musculus were divided into four groups: a negative control group, a diazepam group (2 mg/kgBW), a nutmeg oil group (200 mg/kgBW), and a combination group receiving both agents. All groups received 3 mg/kgBW strychnine to induce seizures. The Kruskal-Wallis test showed significant differences in seizure onset and duration among the groups (p = 0.001 and p = 0.000). Further analysis with the Mann-Whitney test showed that the combination group (Group 3) had significantly better outcomes (p = 0.004 and p = 0.003) compared to diazepam alone, with a mean rank of 57.00 vs. 21.00. Mortality in the combination group was also lower (33%) than the other groups (100%). These results indicate a potential synergistic effect between diazepam and nutmeg oil.

Abstract

One of the most critical electrolyte imbalances associated with CKD is hyperkalemia. Insulin-dextrose therapy has been recognized for its efficacy in rapidly lowering serum potassium concentrations. One of the critical factors in the effectiveness of this treatment regimen is the dosage of the insulin. This study aims to describe the effect of insulin dose on potassium levels in patients with hyperkalemia. This study is observational with a retrospective method in chronic kidney disease patients who experience hypokalemia in January 2023 to December 2023. The inclusion criteria included all chronic renal failure patients, with or without comorbidities, who received insulin and dextrose therapy. The combination of (Novorapid® 10iu + D40% + Calcium Gluconas 1g)x3 iv and the combination of (Novorapid® 10iu + D40%)x3 iv + Calcium gluconas (3x1g) iv + Calcium Polystyrene Sulfonate (3x5g) po were the most commonly administered combinations of insulin and dextrose with other hyperkalemia drugs. The highest average reduction in potassium levels reached 1.55 mEq/L obtained from the combination (Novorapid 10 iu + dextrose 40%)x4. In general, the higher the number of insulin doses and/or the frequency of insulin will show a greater decrease in potassium levels.

Abstract

Red betel leaves contain a high amount of secondary metabolites such as flavonoids, alkaloids, and essential oils that has been used as an anticancer, antidiabetic, and antiseptic. Granule formulation is one of the strategies to enhance the practicality and interest of the public in consuming herbal medicines; however, the dissolution time of instant granule preparations is relatively long. This study aims to improve the solubility of red betel leaf extract by adding β-cyclodextrin, so that the granules produced can dissolve well. The methods used in this study include the extraction of simplicia through maceration using ethanol 70%. The preparation of the complex between the extract and β-cyclodextrin used the spray drying method with ratios of 1:1, 1:2, and 1:3, and tested the solubility of the total flavonoids in the extract. Granule production was carried out using the wet granulation method. The extraction results yielded a yield of 10.92%. The results of the complex formation showed an increase in the solubility of total flavonoids from the extract of 14.221% (F1), 26.646% (F2), and 29.041% (F3), indicating that the addition of the β-cyclodextrin can enhance the solubility of the red betel extract through complex formation. The results of the physical properties test of the granules showed good results after storage for 28 days. The granule dissolution time test results showed a good dissolution time of, less than 5 minutes.

Abstract

Lips are highly vulnerable to ultraviolet (UV) radiation due to the absence of natural protective structures such as hair follicles and sweat glands. Excessive UV exposure may lead to dryness, irritation, cracking, and premature aging. This study aimed to evaluate the Sun Protection Factor (SPF) of carrot (Daucus carota L.) root extract and lip balm formulations containing the extract. Carrot roots were extracted using the reflux method with 96% ethanol. Lip balm formulations were prepared using varying concentrations of carrot extract: 0%, 10%, 15%, and 20%. SPF values were determined in vitro using UV-Vis spectrophotometry at wavelengths of 290–320 nm. Results showed that SPF values increased with extract concentration, from 8.65 at 500 ppm to 18.78 at 2000 ppm, classified as maximum to ultra protection. Among the lip balm formulations, the highest SPF was obtained from formula F3 (20% extract) with an SPF of 5.35, categorized as moderate protection. The photoprotective effect is attributed to β-carotene and flavonoids in carrot extract, which act as antioxidants and UV absorbers. These findings suggest that carrot root extract has promising potential as a natural active ingredient in sunscreen formulations, particularly for lip care products.

Abstract

Gastritis, commonly known as stomach ulcers, is a widespread condition caused by poor eating habits, certain foods, infections, and NSAID use. In Indonesia, WHO reports a 40.8% prevalence, making it a major health issue. This study aimed to evaluate the effects of ethanol-extracted shallot (Allium sp.) skin and bulb on malondialdehyde (MDA) levels and gastric histopathology in rats with indomethacin-induced gastritis. Gastritis was induced in Rattus norvegicus, followed by treatment with shallot extract. MDA levels and tissue damage were then analyzed using Hematoxylin-Eosin staining. Results showed a significant reduction in MDA levels, especially with 200 mg/kg body weight of shallot skin extract, which achieved a 92.18% decrease. Histopathological analysis revealed notable improvements in gastric tissue, with reduced ulceration and inflammation. The findings support the potential of shallot extract in lowering oxidative stress and repairing gastric damage, suggesting its promise as an alternative therapy for gastritis.

Abstract

Acne vulgaris is a common skin problem often caused by genetic, hormonal, dietary, psychological factors, climate, P. acnes bacterial infection, and cosmetic use (Sampelan et al., 2013). Gel preparations are chosen for their cooling effect, ease of spreading, and easy washability. Turmeric (Curcuma domestica Val.), a tropical plant originating from Southeast Asia, has long been utilized as a preventive and curative agent for various diseases, including its potential antibacterial properties. This study aimed to determine the effect of varying concentrations of turmeric extract (5%, 7.5%, and 10%) on the physicochemical characteristics and stability of gel preparations using the freeze-thaw cycling method. Turmeric extract was prepared by maceration using 96% ethanol solvent. The gel formula contained CMC-Na as a base, citrate buffer, propylene glycol, glycerin, PEG 400, Na EDTA, TiO2, and aquadest. The gel preparations were evaluated for physical characteristics (organoleptic, homogeneity, viscosity, flow type, spreadability) and chemical characteristics (pH), as well as their stability. The evaluation results showed that variations in turmeric extract concentration significantly affected the organoleptic properties (color change), viscosity, spreadability, and pH of the preparations. Although all formulas were homogeneous, the freeze-thaw cycling stability test revealed that Formula I (5%) was physically unstable (phase separation) and experienced a significant decrease in pH (p<0.005), indicating the instability of the turmeric gel preparation due to temperature influence. However, all preparations remained homogeneous and their pH stayed within the safe range for skin. Thus, this study confirms the significant influence of turmeric extract concentration on the characteristics and stability of the gel, with higher concentrations (7.5% and 10%) showing better physical stability.

Abstract

Tuberculosis is a lung disease caused by Mycobacterium tuberculosis. Antibiotics have a therapeutic effect by attacking infectious organisms and also eliminating other bacteria that are not the cause of the disease, leading to changes in the ecosystem of normal flora, which results in natural disturbances in microbial ecology. An imbalance in the normal flora can lead to the colonization of respiratory fungi, which will attack the mucosa and proliferate. This study aims to identify the factors that influence the success of antifungal therapy in TB patients with fungal co-infection at Dr. Soetomo Regional General Hospital. The design of this study is retrospective, using electronic medical record data with a total sampling technique conducted during the data collection period from January 1 to December 31, 2024. The total sample is 30 patients. Based on the results, the majority of the gender was male with 20 patients (70.0%), and the largest age group was 51-60 years old with 16 patients (53.3%). The most common Body Mass Index was normal in 18 patients (60%). As many as 20 patients (66.7%) had diabetes mellitus. The most common domicile was Surabaya, with 16 patients (53.3%), and 27 patients (90.0%) had a history of smoking. The BTA examination results showed the highest number of BTA-negative cases in 18 patients (60.0%), the highest number of Geneexpert tests with MTB detected as rifampicin-sensitive in 15 patients (50.0%), the highest number of treatment phase in the intensive phase in 19 patients (63.3%), and the highest number of OAT regimens in 4 KDT in 24 patients (80.0%). As many as 27 patients (90.0%) received antibiotic therapy, either single or combination, during their hospitalization, with the longest duration of antibiotic use being 1-13 days for 20 patients (66.7%). The results of positive fungal cultures were found in 16 patients (53.3%), with the longest duration of antifungal administration being < 14 days in 28 patients (93.3%). Based on the univariate data results, there are 5 variables with a p-value < 0.05, namely gender, age, body mass index, disease history, and tuberculosis drug regimen, which means there is a significant relationship with therapy outcomes. Gender affects therapy outcomes with a p-value of 0.049, which is less than α (p < 0.05). Age with a p-value of 0.029, which is less than α (p < 0.05). Body mass index with a p-value of 0.048 is less than α (p < 0.05). History of type 2 diabetes mellitus with a p-value of 0.033 is less than α (p < 0.05). From the analysis of factors influencing the success of antifungal therapy in TB patients with fungal co-infection, the factors are gender, age, body mass index, medical history, and tuberculosis regimen.

Abstract

Glaucoma is one of the leading causes of sensory disability and irreversible blindness worldwide. This study aims to analyze the effectiveness of 0.5% Timolol in reducing intraocular pressure and its impact on the quality of life of glaucoma patients at Dr. Haryo Eye Clinic, Bojonegoro, in 2024. The research method used is the Retrospective-Prospective research method using primary and secondary data, involving 57 patients as samples. The study was conducted in March 2025. Statistical analysis of the effectiveness of 0.5% Timolol was carried out using paired-samples t-test and simple linear regression analysis. The results showed that of the 57 respondents, 47.4% were male and 52.6% were female, with the most common age group being 58–67 years (40.3%). The average percentage of intraocular pressure (IOP) reduction among glaucoma patients using 0.5% Timolol was 9.12%, and it showed an influence in improving quality of life by 17%. The ANOVA test revealed a significance value of 0.001, which is below the alpha significance level of <0.05.The conclusion of this research is 0.5% Timolol is effective in reducing intraocular pressure with an average reduction of 9.12% and has an influence in improving the quality of life of glaucoma patients by 17%.

Abstract

Akar Kaik-kaik (Uncaria cordata (Lour.) Merr) can be used as herbal medicine to decrease LDL level in the body. This research aims to determine the effect of ethyl acetate fraction the leaf of akar kaik-kaik (Uncaria cordata (Lour.) Merr) toward LDL level in a male white rat serum (Rattus norvegicus). The measurement method used is Direct Homogeneous Assay using reagents and photometer (Mindray® BA-88A) at a wave length of 578 nm. The results obtained in the form of LDL levels and analyzed using two-way ANOVA. The results showed that the ethyl acetate fraction in the leaves of akar kaik-kaik (Uncaria cordata (Lour.) Merr.) doses of 200 and 400 mg/KgBW kgBW could reduce LDL levels as seen from the significant difference (p<0.05) with the negative control group. and has a comparable effectiveness with atorvastatin seen from the absence of significant differences (p>0.05) with the positive control group.
