Abstract

Salmonella typhi is a persistent global health problem and is mostly reported in developing countries. Commercial antibiotics used to inhibit S. typhi are chloramphenicol, ampicillin, ciprofloxacin, cefixime, and ceftriaxone. The use of these antibiotics has been prolonged, resulting in an impact on antibiotic resistance. So it is necessary to investigate the activity of commercial antibiotics to see the latest sensitivity data against Salmonella typhi bacteria. The test was conducted using the Kirby-Bauer method (disc diffusion) with sample and standard concentrations of 3 mg/ml. Ciprofloxacin had the largest inhibition diameter (42.00 ± 0.00 mm), Activity Index (AI) 1. 38 and Relative Percentage Index (RPI) 138.46% followed by cefixime (inhibition diameter 35.00 ± 0.82 mm; AI 1.15; and RPI 115.38), ampicillin (inhibition diameter 34.67 ± 0.47 mm; AI 1.14; and RPI 114.29), and ceftriaxone (inhibition diameter 32.67 ± 0.47 mm; AI 1.08; and RPI 107.69) with chloramphenicol as standard. All commercial antibiotics proved to be still sensitive to the pathogenic bacteria S. typhi.

Abstract

Gelatin is widely used in the food, pharmaceutical and cosmetic industries. The demand for gelatin is increasing every year, specifically gelatin from pork and beef is the main source of gelatin on the market. Gelatin from fish has become a better prospect to be developed, yet the manufacture of gelatin using organic materials has not been widely used. One of the natural ingredients that can be used in the manufacture of gelatin is from pineapple skin. Pineapple is one of the fruits that contain acids including citric acid and bromelain enzymes which have a high ability to convert collagen protein into gelatin. In this study, a characterization of the gelatin quality of catfish (Pangasius hypopthalmus) bones was carried out which was soaked with pineapple waste liquid with a time difference of 6 hours (P1), 12 hours (P2), and 24 hours (P3). The quality characteristics of gelatin include proximate characteristics, physicochemical characteristics and identification of functional groups using Fourier Transform Infra Red (FTIR) spectrophotometry. The results of this study indicate the percent yield of each of the results of the immersion P1, P2, and P3 that are 0.36%; 0.42% and 0.71%, the results of the water content are 12.20%,; 15.91%, and 12.08%, the results of the ash content are 34.38%,; 30.03%; and 30.00%, protein content are 32.59%; 20.57%, and 23.99%, the pH results are 5.80; 4.71; and 4.77, the viscosity results are 0.959cP, 0.997 cP, and 0.972 cP, respectively. Based on the FTIR spectrum, the functional groups of catfish bone gelatin that can be identified include the O-H, N-H, C-N, C=O, C-H groups. From the results obtained, it can be concluded that the bone gelatin of catfish (Pangasius hypopthalmus) has not fully met the quality standard of gelatin.

Abstract

Chalcone is a secondary metabolite belonging to the minor group of flavonoids which has various biological activities. Several chalcone derivatives with amino groups attached to their rings have anti-inflammatory, antibacterial, antitumor and antimalarial activities. However, because the amount is limited, the percentage in plants is small, and the structural variations are relatively small, it needs to be synthesized in the laboratory. This study aims to compare the results of the synthesis of 4'-aminochalcone compounds using the stirrer method at room temperature and modified reflux at an increased temperature. Synthesis using the stirrer method was carried out for 4 hours at room temperature with NaOH 40% as catalyst, while synthesis using the modified reflux method was carried out using a monowave-50 for 11 minutes at a temperature of 80oC with the same catalyst. The molecular structures of the synthesized compound were confirmed by UV spectroscopy, FT-IR, and 1H-NMR spectral data. Based on this study results, it was found that the synthesis of 4'-aminochalcone compounds using the stirrer method had better results compared to the modified reflux method. The 4'-aminochalcone compound using the stirrer method had a pure compound yield of 47.36%, while the 4'-aminochalcone compound using the modified reflux method had an impure yield of 2.57%. Characterization using UV spectroscopy, FT-IR, and 1H-NMR shows that the synthesis results match the structure of the target molecule.

Abstract

Drug distribution is a crucial step in ensuring the quality, safety, and availability of drugs in the community. Good Drug Distribution Practices (CDOB) are established as a standard by the Food and Drug Monitoring Agency (BPOM) for all Pharmaceutical Wholesalers (PBF). This study aims to compare the implementation of CDOB in state-owned and non-state-owned PBFs in Riau Province. The study used a mixed methods study approach. The research design used Sequential Explanatory by combining quantitative and qualitative methods. The quantitative approach used a cross-sectional design with instruments in the form of a CDOB checklist consisting of 12 CDOB aspects and in-depth interviews with Responsible Pharmacists (APJ). Quantitative data analysis was carried out by calculating the percentage (%) of conformity based on the results of the BPOM CDOB checklist form and the Chi-Square test. Qualitative data analysis was conducted descriptively through interviews using content analysis methods to identify discrepancies in the results of the BPOM CDOB checklist and to identify factors influencing CDOB implementation in state-owned and non-state-owned enterprises (BUMN) PBFs. The results showed that the overall compliance level of CDOB implementation reached 99.6% of the maximum score, with an average score of 114.5 out of a total score of 115. BUMN and non-BUMN PBFs achieved the same average score of 114.5/115 (99.6%). Almost all aspects of CDOB were implemented well, but BUMN PBFs still faced deficiencies in organizational and personnel aspects related to personnel discipline. Meanwhile, non-BUMN PBFs faced major challenges in buildings and equipment, particularly limited storage space and temperature control in cool rooms. A chi-square test showed no significant difference between BUMN and non-BUMN PBFs (p > 0.05). Factors influencing CDOB implementation include: competence and human resources, quality management systems, adequate facilities and infrastructure, and compliance with SOPs and regulations. In conclusion, the implementation of CDOB in state-owned and non-state-owned enterprise (BUMN) PBFs in Riau Province has been very successful, with a compliance rate of >99%. Although strengthening of human resources and storage infrastructure is still needed to ensure the quality of sustainable drug distribution.

Abstract

This study aimed to formulate a microemulsion of celery ethanol extract (Apium graveolens L.) and evaluate its physical properties and effects on rabbit hair growth. Four formulations were prepared: three containing celery extract at concentrations of 5% (FI), 7.5% (FII), and 10% (FIII), and one blank formula (F0). The microemulsions were assessed for organoleptic properties, percent transmittance, pH, density, viscosity, rheology, stability (including heating–cooling cycles and centrifugation), skin irritation, and globule size. Hair growth activity was tested in rabbits, and data were analyzed using two-way ANOVA. All formulations demonstrated good physical stability and caused no skin irritation. The results showed that the microemulsions significantly promoted hair growth, with the 10% extract formulation (FIII) showing the most pronounced effect, enhancing hair length by 2.7 times compared to the blank. These findings suggest that celery extract microemulsions, particularly at higher concentrations, are effective in stimulating hair growth.

Abstract

The Chronic Disease Referral Program (Program Rujuk Balik/PRB) of the Indonesian National Health Insurance (BPJS Kesehatan) is designed to ensure continuity of care for patients with chronic diseases through affiliated community pharmacies. The success of this program is not only determined by drug availability but also by the implementation of pharmaceutical care standards that emphasize patient-centered services. This study aimed to analyze the characteristics of PRB patients, assess the implementation of pharmaceutical care standards in PRB pharmacies, evaluate patient satisfaction based on SERVQUAL dimensions, and examine the relationship between pharmaceutical service standards and patient satisfaction. This research employed a descriptive analytic design with a cross-sectional approach. A total of 140 PRB patients were recruited using proportional random sampling from affiliated pharmacies in Pekanbaru. Data were collected using patient characteristic questionnaires, observation checklists for pharmaceutical service standards, and the SERVQUAL instrument. Data analysis was performed using univariate, bivariate (Chi-Square test), and gap analysis. The results showed that most PRB patients were female (52.14%), middle-aged adults 41–60 years (62.86%), with secondary education (50.0%), occupations mainly entrepreneurs (38.57%), and monthly income above IDR 3,508,776 (57.9%). The overall implementation of pharmaceutical service standards was good (82.73%), with drug management (85%), clinical services (56.25%), human resources (90%), and facilities (100%). Patient satisfaction was categorized as very satisfied, with the highest score in empathy (95.93%) and the lowest in tangibles (90.35%). The Chi-Square test indicated a significant relationship between the implementation of pharmaceutical care standards and patient satisfaction (p = 0.002). In conclusion, PRB patient satisfaction in Pekanbaru was very high, yet clinical pharmacy services still require improvement. It is recommended that pharmacies strengthen counseling, therapy monitoring, and adverse drug reaction reporting to enhance both patient satisfaction and long-term clinical quality.

Abstract

Hypertension is a condition in which the systolic blood pressure is above 140 mmHg and the diastolic blood pressure is above 90 mmHg. Kidney failure is a progressive, irreversible disorder of renal function, in which kidney function declines in maintaining metabolism, fluid, and electrolyte balance, resulting in uremia. The purpose of this study was to determine the effectiveness of ethanol extract of papaya leaves (Carica papaya L) in reducing blood pressure and serum creatinine levels in male white rats. This study was experimental in nature, using 60 rats divided into 12 groups, namely negative control, positive control, comparison group (captopril), and dose groups (100, 200, 400 mg/kg body weight). The inducer used for hypertension was 8% NaCl and the inducer for kidney failure was CCl4. Blood pressure measurements were carried out using a Non-Invasive Blood Pressure (NIBP) device, and the measurement of serum creatinine levels was performed using a photometer. Data analysis was conducted using One Way ANOVA followed by Duncan's test. The results showed that the administration of papaya leaf ethanol extract significantly affected the reduction of blood pressure and serum creatinine levels (p<0.05). It can be concluded that the most effective dose for lowering blood pressure is 400 mg/kg body weight, and the most effective dose for reducing serum creatinine levels is 100 mg/kg body weight.

Abstract

Hypertension is a non-communicable disease that remains a major health problem worldwide, especially in Indonesia, as it can cause severe complications such as stroke, kidney failure, and heart disease. The success of hypertension control does not only depend on medication but also on patients’ adherence to taking the medicine regularly. Patients with a good level of adherence are usually able to maintain stable blood pressure, thereby preventing complications. This study aims to analyze the relationship between medication adherence and clinical outcomes among hypertensive patients at Puskesmas Teladan, Medan City. This research used a non-experimental method with a cross-sectional study design involving 135 hypertensive patients who met the inclusion criteria at Puskesmas Teladan Medan during May–June 2025. Adherence data were obtained using the MMAS-8 questionnaire, while clinical outcome data in the form of patients’ blood pressure were collected through direct examination. Data were analyzed using the Spearman Rho test with the SPSS program. The results showed an average adherence score of 6.64±1.437, with systolic and diastolic blood pressure averages of 135.11±16.385 and 72.31±11.303, respectively. There was a significant relationship between medication adherence and clinical outcomes (p=0.000; r=0.463) and between adherence and blood pressure control (p=0.000; r=0.387), indicating a moderate correlation (r=0.40–0.69). It can be concluded that higher medication adherence leads to better blood pressure control and improved clinical outcomes in hypertensive patients.
