Abstract

Populations with comorbidities, especially hypertension, are susceptible to COVID-19 infection and are known to be at risk of increasing disease severity, increasing the risk of poor clinical outcomes, and high mortality. There is no specific therapy to treat this disease, but several types of antivirals have been developed. The purpose of this study was to study the impact of comorbidities and antiviral therapy on clinical outcomes in the form of a length of stay and vital signs (pulse, respiratory rate, temperature, and oxygen saturation) of COVID-19 patients with or without comorbid hypertension. The study was conducted with a cross-sectional design. Data were collected retrospectively through the medical records of 80 patients who met the inclusion criteria and were treated during the period April 2020–March 2021 at Bukittinggi Hospital. Data were analyzed using the Mann-Whitney U Test, Kruskal Wallis Test, Friedman Test, and Repeated Measures Anova. The results showed that comorbidity had a significant impact on the patient's respiratory rate (p<0.05). The administration of different antivirals in patients with or without comorbid hypertension had a significant impact on the length of stay, body temperature, and oxygen saturation of patients (p<0.05). Patients receiving favipiravir therapy showed a shorter length of stay than the antivirals Oseltamivir, Ramdesivir and Hydroxychloroquine.

Abstract

Skin hyperpigmentation is a common dermatological condition characterized by excessive melanin production, primarily regulated by the enzyme tyrosinase. Natural tyrosinase inhibitors have gained increasing interest as safer alternatives to synthetic depigmenting agents. This study aimed to evaluate and compare the tyrosinase inhibitory activity of peel extracts and solvent fractions of three Citrus species: C. aurantifolia, C. microcarpa, and C. hystrix. Extraction was carried out using Soxhlet and fractionation with n-hexane, ethyl acetate, and methanol. Tyrosinase inhibition was assessed using L-DOPA as the substrate. The results showed significant differences among species and fractions. The n-hexane fraction of C. hystrix exhibited the strongest activity (IC50 34.24 ppm), followed by C. microcarpa (144.05 ppm), while C. aurantifolia showed the weakest inhibitory effect across all fractions. The high potency of C. hystrix is associated with its rich monoterpene content, while lower activity in C. aurantifolia may relate to degradation of thermo-labile flavonoids during Soxhlet extraction. These findings highlight the potential of C. hystrix peel, particularly its non-polar fraction, as a promising natural tyrosinase inhibitor for cosmetic applications.

Abstract

Hyperlipidemia is a major risk factor for cardiovascular diseases, highlighting the need for safe and effective cholesterol-lowering agents. This study aimed to evaluate the effectiveness of Calotropis gigantea (biduri) and Ziziphus spina-christi (bidara) leaf extracts in reducing cholesterol levels in male mice. The leaf powders were extracted with 96% ethanol using maceration for 3x24 hours, yielding 6.8% and 3.1% extract, respectively. A total of 36 male mice were divided into six groups: negative control, positive control (propylthiouracil), drug control (simvastatin), and three treatment groups receiving 14, 28, and 56 mg/20 g body weight (BW) of extract. Cholesterol levels were measured using a spectrophotometer. Results showed that cholesterol increased to 189.3 mg/dL in the positive control, while simvastatin reduced it to 117.7 mg/dL. Both biduri and bidara extracts decreased cholesterol in a dose-dependent manner, with the highest dose (56 mg/20 g BW) reducing levels to 150.5 mg/dL, or 20.4%. This effect is likely due to flavonoids, saponins, and alkaloids that inhibit cholesterol absorption and enhance bile excretion. The study concludes that biduri and bidara leaf extracts have potential as effective and safe natural cholesterol-lowering agents.

Abstract

Caesarean section (SC) is a surgical procedure to address complications during childbirth, a complication that is often experienced by post-SC patients is pain. Adequate pain management is important to ensure optimal recovery of post-SC patients. This study aimed to evaluate the decrease in the pain scale of post-SC patients after the use of Ketorolac injection. This study uses an observational study design with a retrospective analytical approach. The study sample is the medical records of post-SC patients who received Ketorolac injection as an analgesia in the February-April 2023 period. The sampling technique used accidental sampling according to the criteria totaling 89 people. The data collected included the demographic characteristics of the patient, the pain scale using NRS, FPS before and after the use of Ketorolac injection. The results of the study showed the age characteristics of pregnant women who had the most SC at the age of 26-30 years. The use of Ketorolac injection lowered the pain scale of post-SC patients with moderate and mild pain levels as evidenced by 85.3% of respondents experiencing a decrease in pain. Pain reduction after ketorolac injection averaged 1-2 pain scales with a duration of 2-4 hours after administration. The NRS,FPS pain assessment method can be used effectively in measuring the decrease in the pain scale of post-SC patients. Adequate post-SC pain management using Ketorolac injection can be an option to improve the quality of care for post-SC patients

Abstract

Background: Tuberculosis (TB) is a global health problem, and anti-tuberculosis drugs (ATD) often cause hepatotoxic side effects (Drug-Induced Liver Injury/DILI). Turmeric (temulawak) with its curcumin content has the potential to be a hepatoprotector. This study aims to analyze the use of Curcuma tablets and the factors influencing the success of therapy in TB patients with impaired liver function. Methods: This study uses a retrospective cohort design with a Cost-Effectiveness Analysis (CEA) approach. A sample of 70 TB patients with DILI at Dr. Soetomo General Hospital Surabaya from January to December 2024 was taken using consecutive sampling. Demographic, clinical, laboratory (SGOT, SGPT), and data were extracted from electronic medical records. Data analysis was performed using univariate, bivariate (Chi-Square), and multivariate (logistic regression) tests. Results: The research results showed that the majority of participants were male (75.7%), with the largest age groups being between 51-60 years and >60 years (32.9% each). Residing in Surabaya (52.9%). The majority of patients were normal weight (55.7%), had a history of smoking (52.9%), did not have a history of diabetes mellitus (67.1%), and had a history of hypertension (82.9%). The BTA examination result was positive (55.7%) and the TCM result was "MTB Detected Rifampicin Sensitive" (61.4%). Undergoing therapy with the OAT 4 FDC regimen or intensive phase (65.7%) in the intensive phase (90%). The most common length of stay was between 6-10 days (34.3%), and the most frequent therapeutic outcome was improvement (78.6%). Based on the bivariate data analysis, only one variable had a p-value < 0.05, which was age, indicating a significant relationship with the treatment outcome. Patients aged 51-60 and > 60 years old influenced the treatment outcome with a p-value of 0.016, which is less than α (p < 0.05). Conclusion: The research findings indicate that gender, smoking history, diabetes mellitus history, hypertension history, BTA test results, TCM (Molecular Rapid Test) results, length of hospital stay, and duration of curcuma tablet administration did not show a significant relationship with treatment outcome. Age, domicile, BMI (Body Mass Index), treatment phase, and OAT (Anti-TB Drug) therapy regimen showed a significant relationship with treatment outcome. Age is the most influential risk factor for treatment outcome. Further research with a larger and multi-center sample is recommended to strengthen the findings.Keywords: Curcuma, DILI, Pulmonary TB, Hepatoprotector.

Abstract

ABSTRACT The use of prophylactic antibiotics prior to surgery has been shown to reduce the incidence of post-operative infections. Prophylactic antibiotics are recommended only for procedures with a high incidence of infection and procedures with very serious consequences of infection. Open fractures are a very serious infection. This study aims to determine the effectiveness of cefazolin prophylaxis on bacterial colony growth 24 hours post-operatively. This study used a prospective observational cohort method conducted at Dr. Saiful Anwar General Hospital in Malang from Agustus to December 2024, and total sampling was used for sample collection. The effectiveness of cefazolin was assessed based on the growth of bacterial colonies and bacterial sensitivity to antibiotics. The results of wound culture examinations in 30 subjects with grade I and grade II wounds showed 43 positive bacterial colonies, namely coagulase-negative Staphylococcus, out of a total of 43 bacterial colonies that grew, reaching a sensitivity of 100% at 24 hours postoperatively. The use of cefazolin prophylaxis was effective in reducing bacterial colony growth 24 hours postoperatively. Keywords: Cefazolin, open fractures, prophylaxis antibiotic

Abstract

Magh or gastritis is a disease that arises due to inflammation of the gastric mucosa which can cause symptoms such as heartburn, flatulence, nausea, vomiting, and other digestive disorders. Magh often occurs among students who have high levels of stress and habits that tend to be irregular. Magh is a common disease that can be treated with self-medication or self-treatment with medications without a doctor's prescription. The level of knowledge and behavior in self-medication plays an important role in the success of treatment to obtain maximum therapeutic results. This study aims to determine the relationship between the level of knowledge and self-medication behavior in students at Abdurrab University. The research design used was observational analytics with a cross sectional research design. The sample collection technique used stratified random sampling with a sample of 315 respondents who met the inclusion criteria. The results of the study showed that the level of knowledge of students had good knowledge, namely 201 respondents (63.8%). The self-medication behavior of magh students had sufficient behavior, namely 129 respondents (49.5%). Based on the results of the Chi Square correlation test, it can be concluded that there is no relationship between the level of knowledge and self-medication behavior in students at Abdurrab University Pekanbaru with a p value of 0.102 (>0.05). Abstract

Xerosis is a skin condition characterized by decreased water content in the stratum corneum, leading to dryness, scaling, roughness, and pruritus, which may cause discomfort and increase the risk of infection. Purified used cooking oil has potential as an alternative emollient that is economical and environmentally friendly. This study aimed to evaluate the quality characteristics of purified used cooking oil, formulate a skin balm preparation, and assess its effectiveness in treating heel xerosis. This study employed a quantitative experimental method, including purification of used cooking oil, quality characterization based on national standards, toxicity testing using the Brine Shrimp Lethality Test(BSLT), formulation of skin balm with concentrations of 0%, 9%, 18%, and 27%, as well as physical stability and effectiveness evaluation using a skin analyzer. The results showed that the purified used cooking oil met quality standards. The skin balm preparation demonstrated good stability in terms of organoleptic properties, homogeneity, pH, spreadability, and adhesiveness during storage and cycling tests. Effectiveness testing indicated an increase in skin moisture corresponding to higher oil concentrations, with the highest improvement observed at 27% concentration (73.9% water content and 69.2% oil content). In conclusion, the skin balm formulation based on purified used cooking oil is effective in improving skin moisture and treating heel xerosis.

Abstract

Canavila rosea is a herbaceous plant in the Leguminosae family, commonly known as beach beans. C. rosea is naturally found along coastal areas, including the coast of Bengkulu. Beach beans leaves contain phenolic compounds, flavonoids, tannins, alkaloids, and saponins that have the potential to be developed as antioxidants. Antioxidant activity is the basis for the development of other treatments such as antidiabetic, antibacterial, anti-tyrosinase, and anticancer agents. Based on the potential, this study aims to test the antioxidant activity of beach beans leaf extract. Antioxidant activity is also related to flavonoid content, especially flavonoids bound to sugar group. The extraction method uses the Soxhlet method. Qualitative flavonoid content testing is carried out using flavonoid phytochemical screening reagents. Quantitative flavonoid content testing is carried out using the Chlorimetric method. Antioxidant testing uses the DPPH inhibition method. The yield percentage of 96% ethanol extract was 26%. The results showed that C. rosea extract contained flavonoid compounds. Antioxidant activity showed an IC50 value of 77,09±3,18 µg/mL for the ethanol extract. The total flavonoid content of the ethanol extract was 4.791 ± 0.93 %. The ethanol extract has strong antioxidant activity with an IC50 value in the range of 50-100 ppm.

Abstract

Background: Moringa leaves contain secondary metabolites with antioxidant activity that play a role in maintaining skin moisture. Body butter is a cosmetic preparation that is more effective in retaining skin moisture compared to other topical formulations. Objective: To determine the effectiveness of body butter containing Moringa oleifera L. leaf extract in improving skin moisture. Methods: This study was an experimental research. Moringa leaf extraction was carried out using the maceration method. The extract was formulated into body butter at concentrations of 5% (FI), 7% (FII), and 9% (FIII), while the base formulation without extract (F0) served as a negative control. The prepared formulations were evaluated for physical properties, including organoleptic characteristics, homogeneity, pH, viscosity, formulation stability, and skin irritation. The moisturizing effectiveness was measured using a skin analyzer. Results: The results showed that all body butter formulations containing moringa leaf extract were physically stable and did not cause skin irritation. The moisturizing test demonstrated a significant increase in skin moisture at all extract concentrations. The 9% extract formulation (FIII) showed the highest increase in skin moisture, from 18.33% to 57.33% after four weeks, approaching the effectiveness of the positive control (59%). These findings indicate that the moisturizing effectiveness increased with higher extract concentrations. Conclusion: Ethanolic extract of Moringa oleifera L. leaves can be formulated into a stable body butter preparation and is effective as a skin moisturizer.
