Abstract

Background: Management of Gartland type III supracondylar humerus fractures is conduc­ted by open and closed repositioning. An ade­quate re­p­o­sition and a stable and accurate fixation are des­perately needed to prevent fixation failure, defor­mi­ty, and complication. The study aims to com­pa­re the clinical and radiological results bet­ween crossed and lateral fixation techniques.Subjects and Method: The study was a retro­s­pective study toward Gartland type III SCHF children in Dr. Soetomo Hospital, Surabaya, Indonesia from 2013–2016. The dependent vari­able is Supracondylar hu­me­rus fracture. Independent variables were the type of fixation option, clini­cal functional test, degrees of satisfactory, and radiology evalua­tion. The radiology para­me­ter used was Skaggs criteria. An observation was conducted for the occurrence of complications in the form of infection and peripheral nerve injury. All data were analyzed using Kolmogorov Smirnov and Fischer exact test.Results: The study discovered 28 patients consisted of 20 males and 8 females with an age range from 3 – 13 years old with an average age in crossed fixation group was 7.6 years and in lateral fi­x­ation was 4.7 years. The injury sides were 46.4% right elbow and 53.5 % left elbow. Among the cros­sed fixation group, there were 54.5 % left elbow and 45.5 % right elbow. Among lateral fix­a­tion group, there were 50% left side and 50% right side. There was no significant difference in cli­nical fun­ctions, radiology as well as com­pli­ca­tion in the form of infection and peripheral ner­ves injury.Conclusion: There is no difference of functional clinical, radiology result as well as post-surgery com­plication in the form of infection and peri­phe­ral nerves injury between crossed fixation tech­nique and lateral fixation technique.Correspondence: Komang Agung Irianto. Abstract

Background: Children under five experience an average of three episodes of diarrhea each year in developing countries. Based on World Health Organization (WHO) guidelines, therapy for acute diarrhea is oral rehydration fluid and zinc to treat fluid and electrolyte loss. But unfortunately, this therapy does not reduce the duration of diarrhea without the intestinal barrier function of pathogenic microorganisms. The ability to inhibit pathogens is one of the three main mechanisms of probiotics. This study aims to examine the efficacy of using probiotics compared to oral rehydration solution (ORS) in the treatment of acute diarrhea in children under five in developing countries.Subjects and Method: This study is a meta-analysis conducted using PRISMA systematic guidelines. The process of searching for articles was carried out between 2009 and 2019 using a database search engine consisting of PubMed, British Medical Journal (BMJ), CAB Direct, Oxford Academy, Clinical Key, ScienceDirect, and Scopus. Based on a database search, six articles that meet the criteria of the Randomized Controlled Trial (RCT) and research conducted in developing countries were found. The study involved 1234 children who were divided into two groups: 762 probiotic groups and 472 ORS groups. The analysis was performed with the Review Manager (RevMan) software 5.3. The results were assessed using Standardized Mean Difference (SMD).Results: There was heterogeneity between experiments (I2 = 91%; p <0.001) so Random Effects Model (REM) was used. Probiotics could reduce the duration of acute diarrhea in infants rather than just ORS, with a pooled estimate of 1.13 (SMD = -1.13; 95% CI = -1.54 to -0.72; p <0.001).Conclusion: Probiotics can reduce the duration of acute diarrhea in infants in developing countries rather than just the administration of ORS.Keywords: Diarrhea, probiotics, oral rehydration solution, meta-analysis.Correspondence:Nurul Aini Suria Saputri. Abstract

Background: Thalassemia is a hereditary disea­se in which the sufferer is unable to produce suf­fi­cient amounts of hemoglobin, causing impaired function of red blood cells and shortened red blood cell age. Thalassemia sufferers are spread all over the world, and thalassemia is one of the health problems in the world. Therapy in patients with thalassemia is with routine blood trans­fusions. One side effect of repeated transfusions is iron overload which causes deposits of iron in the endocrine glands. This study aimed to deter­mine the relationship between ferritin levels with blood calcium levels in patients with β major thalas­semia.Subjects and Method: This was a cross-sec­tional study conducted at Dr. Moewardi, Sura­karta, from June to November 2017. Samples of 49 patients with thalassemia β major aged 5-10 years were selected using consecutive sampling. The dependent variable is blood calcium levels. The independent variable is ferritin level. Data were analyzed with Chi Square test.Results: High ferritin levels reduce calcium level in pediatric thalassemia β major patients, but not statistically significant (OR = 0.27; 95% CI = 0.04 to 1.64; p = 0.134).Conclusion: High ferritin levels decrease with calcium levels in pediatric thalassemia β major patients, but not statistically significant.Keywords: ferritin, calcium, thalassemia β majorCorrespondence: Andhika Trisna Putra. Abstract

Background: Biloma is loculated collection of bile that may develop due to iatrogenic causes, trauma­tically or spontaneously with biliary tree disruption. Hepatic bilateral subcapsular bi­loma is a rare complication of laparoscopic cho­lecys­­tec­tomy and an even more scarce when it occurs spontaneously.Case Presentation: A 65 years old man came to our hospital with abdominal pain and en-larged abdomen. Six weeks earlier he under­went laparoscopic cholecys­tectomy in a private hospi­tal, because of stone in the gall bladder and cho­lecys­titis. The physical examina­tion obtained no icteric and distended abdomen with pain on palpation. Laboratory findings were within nor­mal limit with negative viral infection markers. Abdominal ultrasonography revealed chronic liver disease with giant liver cyst and ascites. Contrast abdominal multi-slice computed mono­graphy (MSCT) demonstrated bila­teral hepatic sub­capsular biloma. Laparatomy and drainage were then performed and he was discharged from the hospital several days later in good condition.Discussion: Biloma was first introduced by Gould and Patel in 1979. The incidence of past laparoscopic cholecystectomy biloma is very low, between 0.6% and 1.5%. Early accurate diagnosis is very important to determine the proper mana­gement. In our case, the biloma was found by using USG and MSCT.  Usually it presents with right upper quadrant or epi­gastric pain, abdo­minal distention, fever and leukocytosis, but our patient did not have either fever or leukocytosis. Actua­lly the first treatment choice is percuta­neous catheter drai­nage but in our case laparo­tomy drainage was performed because of sub­capsular, biloma in both right and left hepatic lobes.Conclusion: Bilateral hepatic subcapsular bilo­ma is a rare case. Abstract

Background: Plants are currently used as medicine, i.e. medicinal plants. Medicinal plants con­sist of many chemical compounds, especially bioactive substances. One of them is Sambang Darah (Excoecaria cochinchinensis). Sambang Darah have so many benefits which usually applied in vulnus combutio. This study aimed to determine value of haemostasis after applied Sambang Darah leaves in white rat skins (Rattus norvegicus).Subjects and Method: This was a true experimental study with post test only control group de­sign. This study conducted at Mata-ram University, West Nusa Tenggara, Indo-nesia, from April to October 2017. A total sam-ple of 16 whi­te rats was selected for this study. The dependent variable in this study was the value of haemostasis and the independent vari-able was filtrate of Sambang Darah leaves. The data were analyzed using Man Whitney test.Results: The average level of bleeding time was 3.76 minutes for the control group and 2.58 mi­nu­tes for test group. The average level of clotting time were 2.20 minutes for control group and 1.36 mi­nutes test group. The number of platelet was 277,000/uL for the control group and 221,000/uL for test group. APTT level was25.06 second for the control group and there was no clot in test gro­up. PT level was 13.95 second for the control group and there was no clot in test group. TT levels was 18.2 second for the control group and there was no clot in test group. And the last, D Dimer level was 0.1 mg/l for the control group and there was no clot in test group. Abstract

Background: Beta-glucan has been frequently used in laboratory trials as an immunomodulator in both in vivo and in vitro studies, but the ef­fective dose for measuring its performance has not been established. Like other immunomodula­tors, researchers must establish the right dose of beta-glucan in their laboratory experiments for the purpose of testing substances as immuno­modulators without achieving false-positive or –negative results. This study aimed to determine the optimum dose of beta-glucan to induce cyto­kine production by peripheral blood mononu­clear cells (PBMCs) in vitro.Subjects and Method: This was a laboratory experimental study. This study measur­ed the production of two cytokines, including in­terferon gamma (IFN-γ) and interleukin 12 (IL-12), from the isolated PBMCs of healthy subjects. The doses of beta-glucan used as immunomodu­lator in­clu­ded 1, 5, 10, 20, and 50 μg/ml. Beta-glucan was add­ed to the PBMC culture medium, and the PBMCs were cultured for 6 days. On the sixth day, the supernatant was harvested and the cytokine production was analyzed using sandwich enzyme-linked immunosorbent assay (ELISA). Cytokines were also analyzed using the human IFN-γ ELISA kit and the human IL-12 ELISA kit, and data analysis was performed by one-way ANOVA.Results: IFN-γ levels were found to be increased in the group treated with 5 μg/ml beta-glucan. The highest IFN-γ levels (70.0 pg/ml) were observed in the group treated with 10 μg/ml beta-glucan. The production of IL-12 increased sharply in the group treated with5 μg/ml beta-glucan but decreased in the group treated with 10 μg/ml beta-glucan. The mean cytokine levels of the beta-glucan group were found to be significantly different from those of the control group (p=0.001). One-way ANOVA revealed that the highest IL-12 production (77.2 pg/ml) occurred at a dose of 5 μg/ml beta-glucan. This average value was significantly different from the average production of IL-12 in the control group (p=0.001).Conclusion: The optimum dose of beta-glucan for stimulating PBMCs to produce IFN-γ in vitro was 10 μg/ml, while for the production of IL-12, the dose was 5 μg/ml. Abstract

Background: Anxiety in hemodialysis pati­ents often impaired quality of life. Complex interaction between Chronic Kidney Disease (CKD) and anxiety are dynamic and multifac­torial, including socio-economic, lifestyle and biological factors. Biological factors are close­ly related to anxiety in CKD including the im­mune system, inflammatory pathways of dis­orders of Hypothalamic-Pituitary-Adrenal Axis (HPA). Erythrocyte sedimentation rate (ESR) is one of the non-specific inflammatory marker that show the degree of inflammatory reaction, Neutrophil-Lymphocyte Ratio (NLR) and Platelet-Lymphocyte Ratio (PLR) also found as an easy and inexpensive mea­sureable marker to show the degree of inflam­mation. This study aimed to examine rela­ti­onship between anxiety and hematology in­flammation markers in hemodialysis patient.Subjects and Method: This was an obser­vational study with a cross-sectional design. The study was conducted at Dr. Moewardi general hospital, Surakarta, Central Java. A sample of 30 hemodialysis patients was selec­ted for this study. The dependent variables were ESR, NLR, and PLR. The independent variable was anxiety. Anxiety was measured by Taylor Manifest Anxiety Scale (TMAS). Data on ESR, NLR, and PLR, were measured using an assessment from RAND Corpora­tion. The data were analyzed by a multiple linear regression.Results:  Anxiety increased NLR and it was marginally significant (b= 0.37; 95% CI= -0.01 to 0.22; p= 0.080). Anxiety decreased ESR and it was statistically significant (b= -0.57; 95% CI= -0.02 to 0.01; p<0.001). An­xiety decreased PLR (b= -0.16; 95% CI= -0.02 to 0.01; p= 0.440) and duration of hemo­dia­lysis (b= -0.27; 95% CI= -0.03 to 0.01; p= 0.167), but they were statistically non-signi­ficant.Conclusion: Anxiety increases NLR. Abstract

Background: Decreased endometrial recep­ti­vity is suspected to be one of the factors causing in­fertility in women with Polycystic Ovary Syn­dro­me (PCOS). Endometrial abnormalities in PC­OS­ due to impaired balance include estrogen, pro­­­­gesterone, growth factors, cytokines and cell at­tach­­­ment modulators including leptin. This study aims to determine the expression of leptin in the endometrium which is suspected to be more common in PCOS patients and related factors.Subjects and Method: A retrospective-case control study was conducted at Dr. Moe­wardi general hospital, Surakarta. A total of 60 women were involved; 30 infertile women with PCOS based on Rotterdam criteria (if found two of the three symptoms, including hyperandrogenism, ovu­la­tion dysfunction and/or polycystic ovaries from ultrasonography) and 30 normal women as con­trol group. Demographic data such as age, family history of PCOS, menarche, BMI, occupation, and edu­cation were recorded. Leptin expression was acquired from endometrium biopsy with IHC examination on day 19-24 of the menstrual cycle; under the condition that inclusion and exclusion cri­teria are fulfilled. The statistical analysis was per­formed using the Mann-Whitney test.Results: Mean of leptin expression was found significantly higher in PCOS group (Mean= 59.16; SD= 49.34) compared to control group (Mean= 6.00; SD= 22.98, with p= 0.001. PCOS (b= 0.42; 95% CI= 0.10 to 0.73; p= 0.010) and obesity (b= 0.31; 95% CI= 0.41 to 0.58; p= 0.025) increased leptin expression.Conclusion: There is a difference in leptin ex­pres­­sion in the endometrium of PCOS compared to nor­mal.  Leptin expression is higher in the en­do­metrium of women with PCOS and obesity compared to nor­mal.Keywords: Leptin expressions, endometrium, polycystic ovarian syndrome.Correspondence: Dwi Sakti Berlian Besar. Abstract

Background: Breast milk is a natural food to fulfill the nutritional needs of infants during the first 6 months of life. Nutrition is one of the factors that influence milk production. Protein is one of the most important nutrient factors needed by nursing mothers to increase milk production. This study aimed to analyze the effectiveness of giving goat milk to increase breast milk production.Subject and Method: A randomized controlled trial (RCT) was conducted in Madiun, East Java, in October 2019. A sample of 100 lactating mothers who had infants aged 1-3 months was selected by purposive sampling and divided into 2 groups. The dependent variables were an increase in milk production and protein intake. The independent variable was the effectiveness of giving goat milk. The data were collected by observation and 24-hour food recall sheets. The data were analyzed by t test run on Stata 13 program.Results: After having goat milk, the average of breast milk production in the intervention group (Mean= 121.5; SD= 12.81) was higher than in the control group (Mean= 97.06; SD= 13.59), (ES= 0.81; p<0.001). Protein intake after having goat milk was higher in the intervention group (Mean= 92.60; SD= 4.54) than control group (Mean 83.36; SD= 4.26), (ES= 2.10; p < 0.001).Conclusion: Goat milk is effective for increasing milk production and protein intake in lactating mothers.Keywords: goat milk, breast milk, proteinCorrespondence: Prita Alvina Reviana, Masters Program in Public Health, Universitas Sebelas Maret, Jl. Abstract

Background: Massage was a tactile stimulation that has physiological and biochemical effects in the body. Baby massage was a fun way to relieve tension and anxiety, especially in infants. Gentle massage will help relax the muscles so that the baby becomes calm and sleeps soundly. Good qua­lity sleep can achieve optimal growth and development. The purpose of this study was to de­termine the effect of pe'e culture (baby mass­age) on the sleep needs of babies aged 5-12 months.Subjects and Method: This research was a quasi-experimental study with a pre and post test design in the same group. The number of samples in this study were 30 respondents using pur­po­sive sampling technique. The dependent vari­able of this study was sleep resting children aged 5-12 months. The independent variable was Bima traditional massage (pe'e). Data analysis was per­form­ed using the Wilcoxon test.Results: Average sleep quality of children aged 5-12 months after treatment (Mean= 2.73; SD= 6.­40) is higher than before treatment (Mean= 1.47; SD= 5.07), and statistically significant (p= <0.001).Conclusion: Culture Baby massage (pee) can imp­rove sleep rest patterns in infants aged 5-12 months.Keywords: baby massage, sleep patterns, sleep quality, age 5-12 monthsCorrespondence: Kurniadi.
