Abstract

Background: Diabetic foot ulcer is a compli­cation of diabetes mellitus which costs high, takes long wound care, increases mortality, morbidity and causes non-traumatic amputa­tion. There are risk factors that are thought to be the cause of lower limb amputation in diabetic foot ulcers, namely peripheral artery disease, hypertension and gender. This study aims to estimate the influence of peripheral artery disease, hypertension and gender on the risk of lower limb amputation in diabetes mellitus patients with leg ulcers.Subjects and Method: Meta-analysis studies and systematic reviews were applied to this study using electronic databases of Pubmed, Scopus, Google Scholar, and Springer Link. The keywords to search for articles are as follows: "diabetic foot", "lower limb amputation", "low­er extremity amputation", "risk factor", "predic­tor", "cohort", "retrospective", "adjusted odds ratio". Articles were collected using PRISMA diagrams, and analyzed using the Review Manager 5.3 application.Results: Meta-analysis of 9 cohort articles of diabetic foot ulcer patients with peripheral artery disease (aOR= 2.46; 95% CI= 1.70 - 3.55; p<0.01); Meta-analysis of 8 cohort articles, diabetic foot ulcer patients with hypertension (aOR= 1.05; 95% CI= 0.66 - 1.68; p= 0.83); Meta-analysis of 9 cohort articles of male diabetic foot ulcer patients (aOR= 1.60; 95% CI= 1.32 - 1.94; p<0.01); Peripheral artery disease, hypertension and male gender are risk factors that can increase the incidence of lower limb amputation.Conclusion: Peripheral artery disease, hypertension and male gender are risk factors that can increase the incidence of lower limb amputation.Keywords: Peripheral artery disease, diabetic foot ulcer, lower limb amputationCorrespondence: Anissa Eka Septiani. Abstract

Background: Open tibiotalar joint dislocation associated with medial malleolus and distal fibular fracture without neurovascular damage are rarely reported.Case presentation: This study present a case of 55-year-old female with open tibiotalar joint dislocation associated with medial malleolus and distal fibular fracture without neuro­vas­cu­lar injury treated with emergent debridement, open reduction, and internal fixation.Results: Six months follow-up showed com­plete bone and soft tissue healing with excellent functional outcome.Conclusion: Emergent debridement, open reduction, and internal fixation is needed to treat tibiotalar joint dislocation associated with medial malleolus and distal fibular fracture.Keywords: high energy injury, ankle joint, fracture dislocationCorrespondence: Asep Santoso. Abstract

Background: Extended-spectrum β-lactamase (ESBL) is an enzyme produced by certain Escherichia coli and Klebsiella pneumoniae. ESBL can induce theantibiotic resistance for betalactam antibiotics such as penicillin, cephalosporin, etc. Frequent utilization of carbapenem may induce the risk for carbapenem resistant against ESBLs producing microorganisms. This stud aimed to communicate Aminoglycocides as an alternative antibiotic therapy against urinary tract infection in children.Case Presentation: We present a case of 4 years child who was admitted to the state hospital of Madiun with main complaint of 10 days high fever accompanied by phimosis appearance. Laboratory results based on urine culture show the ESBL positive Escherichia coli. Results: We have used aminoglycoside for treating the patient. Clinical presentation in this patient start showing an improved clinical response and negative bacteriuria in the second urine test after the aminoglycoside therapy.Conclusion: Aminoglycosides such as gentamicin can be considered to be one of the best options for treating an ESBL infection beside carbapenem in order to reduce the carbapenem trend In clinical setting of the urinary tract infection caused by ESBL Eschericia coli.Keywords: Aminoglycosides, Carbapenem, Extended-spectrum β-lactamase, Gentamicin, urinary tract infectionCorrespondence: Muhammad Pradhika Mapindra. Abstract

Background: Myeloperoxidase (MPO) levels have been shown to have prognostic values for death events in acute myocardial infarction (AMI) patients. Increased levels of high sensitive troponin I (hs Trop I) are associated with poorer prognosis in AMI patients. This study aimed to determine the association between levels of MPO and hs troponin I at admisson on myocardial contractility in AMI patients.Subjects and Method: This was a prospective observational analytic study, conducted at Dr. Moewardi General Hospital, Surakarta, from November1, 2018 to January 31, 2019. The study subjects were patients who were diagnosed with AMI. A total of 23 patients were included in this study; 15 patients with ST elevation myocardial infarction (STEMI) and 8 with non ST elevation myocardial infarction (NSTEMI). The dependent variable was myocardial contractility. The inde­pendent variables were MPO and hs Trop I. Blood tests for MPO and hs Trop I was measured at admission and the left ventricular ejection fraction (LVEF) was measured when predis­charge by the Simpson method to examine myo­cardial contractility. The data were analyzed by Pearson correlation run on SPSS 22.  Results: hs Trop I (r= -0.53; p= 0.009) and MPO (r= -0.15; p=0.487) decreased LVEF.Conclusion: Hs Trop I at admission has a correlation with myocardial contractility, and no correlation of MPO at admission with myocardial contractility in AMI patients.Keywords: Myeloperoxidase, High sensitive Troponin I, Acute myocardial infarctionCorrespondence: Trisulo Wasyanto. Abstract

Background: Obesity is now no longer consi­dered a health problem in developed countries, but has also been faced by developing countries such as Indonesia. According to WHO, obesity worldwide has increased by more than two times from 1980. In 2016, there were 1.9 billion adults who were overweight and more than 600 million of them were obese or overweight. This study aims to analyze the effect of ear acu­puncture therapy on weight loss.Subjects and Method: This study was a systematic review and meta-analysis using the Randomized Controlled Trial design. The arti­cles used in this study were obtained from seve­ral databases including PubMed, Google Scholar, Springerlink, Hindawi, and Science­direct. The articles used in this study were those published from 2008-2020. The article search was carried out by considering the eli­gibility criteria defined using the PICO model. P: obese patients (BMI ≥25), I: ear acu­puncture therapy, C: no ear acupuncture thera­py O: weight loss. The keywords for searching articles are as follows: "auricular acupuncture", AND "obesity", "auricular acupuncture for obe­sity" and "acupuncture obesity randomized con­trolled trial". The articles included in this study are full text articles with a Randomized Con­trolled Trial. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 7 articles were reviewed in this study. The meta-analysis showed the results that ear acupuncture therapy was effec­tive for weight loss in obese people (SMD= -0.74; 95% CI= -1.31 to-0.16;  p<0.001). Abstract

Background: Preeclampsia (PE) is a major complication in pregnancy and a major cause of maternal and fetal morbidity and mortality. Analysis of risk factors and prevention of pre­eclampsia are needed to reduce the adverse effects of preeclampsia. Important supplements during pregnancy to prevent the increase in blood pressure of pregnant women are Vitamin D and calcium. This study aimed to analyze the effect of vitamin D and calcium supplemen­tation on reducing the risk of preeclampsia.Subjects and Method: This study is a meta-analysis of a number of randomized controlled trials. The articles used in this study were obtained from several databases including PubMed, Google Scholar, Springerlink, and Sciencedirect. The articles used in this study were those published from 2002-2020. The article search was carried out by considering the eligibility criteria defined using the PICO model. P: Pregnant women, I: vitamin D and calcium, C: placebo, and O: preeclampsia. The keywords to find articles are as follows: "Vitamin D" OR "Calcium" OR "Vitamin D and Calcium" AND "preeclampsia" AND "Rando­mized Controll Trials". The articles included in this study are full text articles with Cross-sectional study design. Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3.Results: A total of 9 articles were reviewed in this study. The meta-analysis showed that vitamin D supplementation reduced the risk of preeclampsia in pregnant women (RR= 0.45, 95% CI 0.32-0.63, p<0.001). The meta-ana­lysis of 8 articles also showed that calcium supple­mentation reduced the risk of preeclampsia in pregnant women with calcium (RR= 0.42; 95% CI= 0.31 - 0.57; p<0.001). Abstract

Background: Polycystic Ovarian Syndrome (PCOS) is an endocrine disorder in women cha­rac­terized by increased levels of androgens, ovu­la­tory dysfunction, and Polycystic Ovarian Mor­p­ho­logy (PCOM), as well as a combination of clas­sic clinical features. Women with PCOS expe­ri­ence changes in the endometrium and endo­met­rial receptivity markers such as VEGF (Vascular En­dothelial Growth Factor). VEGF increases the per­meability and dilation of endometrial blood vessels which play an important role in imp­lan­ta­tion. This study aimed to analyze the difference of VEGF expression between women with PCOS and nor­mal women.Subjects and Method: This study was a case-con­trol study design conducted at the polyclinic of the Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, Central Java, in July 2020. There were 30 infertile women with PCOS (diagnosis based on Rotterdam criteria, con­sensus 2003) and 30 women without PCOS as the sample of this study. The dependent variable was VEGF expression. The independent variables were age, BMI, family history with PCOS, menar­che, occu­pa­tion, and education. VEGF expression was ob­tained from an endometrial biopsy on days 19-24 of periods. The differences in VEGF exp­res­sion (in %) were analyzed by the Mann-Whitney test. VEGF determinants were analyzed by a mul­ti­ple linear regression.Results: The mean of VEGF expression in wo­men with PCOS (Mean=64.33; SD=44.43) was high­er than in women without PCOS (Mean­= 7.90; SD=8.73) and the result was statistically sig­ni­ficant (p<0.001). PCOS (b=42.09; 95%­CI­=­64.­2­6 to -19.93; p<0.001) and obese (b=38.63; 95%­CI=-63.44 to -13.82; p=0.003) increased VEGF expression.Conclusion: VEGF expression in the endo­met­rium of women with PCOS is higher than normal wo­men.Keywords: VEGF, endometrium, Polycystic Ova­rian SyndromeCorrespondence: Ricky Bernadi Saputra. Abstract

Background: Gestational Diabetes Mellitus (GDM) is the most common metabolic disorder first diagnosed during pregnancy. According to the American Diabetes Association (ADA) in 2006, GDM occurred in 7% of pregnancies each year. Family history of diabetes is an important risk factor for GDM. The relationshipbetween family history of DM and GDM has been inves­tigated in several primary studies with several contradictions in the outcome. There­fore, this study aimed to determine the relationship between family history of diabetes and GDM using a meta-analysis method.Subjects and Method: The meta-analysis was conducted by systematically reviewing articles published from 2007 to 2020 from the Pubmed, Google Scholar, Science Direct, and ResearchGate databases. The keywords were "Risk factor" OR "family history" OR "family history DM") AND "GDM" AND "cross sectio­nal". The article search was carried out using PICO: the study population was pregnant women, the intervention/exposure was family history of DM, the comparison was no family history of DM, and the outcome was GDM. The article search was carried out for one month. The articles included in this study were full-text articles with a cross-sectional design. The arti­cles were reviewed using the PRISMA flow diagram guidelines. This study used Revman 5.3 Software to analyze the articles.Results: Family history of DM increased the risk of Gestational Diabetes Mellitus (aOR= 2.02; 95% CI= 1.54 - 2.67; p<0.001). This study used 11 articles from Ethiopia, Cameroon, Peru, Iran, Pakistan, Malaysia, Kuwait, Saudi Arabia, and India.Conclusion: Family history of diabetes melli­tus increases the risk of gestational diabetes mellitus.Keywords: Gestational diabetes mellitus, Family history of diabetesCorrespondence: Ummu fatihah Ammutammima. Abstract

Background: Scoliosis is a spinal deformity that often occurs in adolescents with a pre­va­lence of 0.47% -5.2% worldwide. The preva­lence of scoliosis in Asia was about 2.25% of the population. In Surabaya, Indonesia, 6.37% of students from 784 schools who con­ducted sco­liosis screening tests positively had scoliosis. In addition, 9 out of 300 students in Surakarta positively had scoliosis. This study aimed to estimate the effect of using scoliosis orthosis in reducingcobb angle in scoliosis patients by conducting a meta-analysis on theseveral similar articles.Subjects and Method: This study used a meta-analysis by systematically reviewing the several similar articles from PubMed, ScienceDirect, and Google Scholar. The key­words were "scoliosis" AND "scoliosis ortho­sis" OR "scoliosis brace" AND "cobb angle" OR "curve" AND "randomized control trial". The intervention was the use of a scoliosis orthosis with a comparison of non-scoliosis orthosis and the scoliosis patients as the study subjects. The outcome of the study was a reduction of cobb angle. The included arti­cles were full-text articles with a randomized controlled trial design that showed the effect size (mean and standard deviations). The articles were collect­ed using PRISMA flow diagrams and analyzed using the Review Manager 5.3 application.Results: Based on a meta-analysis result of 9 randomized controlled trial articles from the United States, Canada, Germany, Sweden, Denmark, Poland, and Hong Kong, scoliosis orthosis reduced cobb angle in scoliosis pati­ents (SMD= -0.67; 95% CI= -1.02 to -0.33; p <0.001). Abstract

Background: Diabetes mellitus is one of the second biggest health problems. The Inter­natio­nal Dia­betes Feder­a­­tion said that diabetes cur­ren­­­tly affects 382 million people world­­­­­­­­­­­wide, with type 2 dia­betes mellitus (DM) being the larg­est pre­valence of 85-95% of the diabetes population. This study aimed to estimate the effect of high-inten­sity interval training (hiit) on increasing levels of high-density lipoprotein in patients with type 2 diabetes mellitus based on the results of several previous studies.Subjects and Method: This study was a meta-analysis and systematic study, with the follow­ing PICO Population =type 2 diabetes mellitus pati­ents aged 35-65 years. Inter­­­vention=HIIT. Com­p­a­r­ison = No HIIT. Outcome = increased levels of high-density lipoprotein. The articles used in this study were obtained from several databases, inclu­d­­­­­­ing PubMed, Science­Direct, and Google Scho­lar. The key­­­­words for finding articles were: "HIIT" OR "High­­-­Intensity interval Training" OR "Dia­betes Mellitus" OR "High-­­Density Lipo­pro­tein" AND "Randomized Control­­­led Trial". The articles included in this study were full-text with a randomized controlled trial. Articles were analyz­ed by PRISMA flow chart and Re­vMan 5.3.Results: A total of 9 articles reviewed in this meta-­analysis study originated from New York, Canada, France, Thailand, Berlin, Denmark, Aus­tralia, and the United Kingdom.
