Abstract

Background: The role of vitamin D toward cognitive function is as neuroprotective in which vitamin D will increase neurotrophic factor production which contributes to supporting neuron life sustainability, development, and function. The study aims to analyze the association between calcidiol serum level and cognitive functions of geriatric patients.Subjects and Method: A cross-sectional study was conducted at Geriatric Polyclinic of dr. Moewardi Hospital, Surakarta. A sample of 30 geriatric patients was selected randomly. The dependent variable was cognitive functions (MMSE score). The Independent variable was calcidiol serum level. The data were analyzed by Spearman test.Results: There was no difference of calcidiol serum level with MMSE score (OR= 1.00; p= 0.478).Conclusion: There was no difference in calcidiol serum level with MMSE score.Keywords: cognitive functions, calcidiolCorrespondence: Sandy Anandhita. Abstract

Background: Neck pain is the fourth leading cause of disability, with an annual prevalence rate exceeding 30%. Nearly 50% of individuals will continue to experience pain or it often occurs to some degree. This study aimed to analyze the magnitude of the effect of using a cervical collar on reducing the degree of pain in neck pain patients compared to manual therapy (exercises).Subjects and Method: This research is a systematic review and meta-analysis carried out by following the PRISM diagram flow. The process of searching for articles through a journal database that includes: PubMed, Google Scholar and Science Direct by selecting articles published in 2000-2020. The keywords used include: “cervical collar” OR “neck collar” OR “soft collar” OR “hard collar” AND “neck pain” AND “cervical pain” AND “cervical radiculopathy” AND “cervical syndrome” AND “randomized controlled trial”. The inclusion criteria were full paper articles with a randomized controlled trial study design, articles using English and Indo­nesian, and bivariate analysis with adjusted odds ratio. Articles that meet the require­ments are analyzed using the Revman 5.3 application.Results: A total of 6 articles were reviewed in this study with a randomized controlled trial study design. Meta-analysis of 6 randomized controlled trial (RCT) studies showed that there was no difference in effectiveness in using cervical collars compared to manual therapy (exercises) in reducing neck pain (ES = 0.04; 95% CI -0.31 to 0.39; p = 0.083).Conclusion: This meta-analysis concluded that there was no difference in effectiveness between the use of cervical collars and manual therapy (exercises) in reducing the degree of pain in neck pain.Keywords: neck pain, cervical collar, exercises,meta-analisisCorrespondence: Hisyam Syafi’ie. Abstract

Background: Physical activity has a positive role in breast cancer risk. Physical activity inten­sity is often expressed as metabolic equi­valents (METs) and the analytic guidelines for international physical activity question­naires define low activity as 4 METs and high activity as 8 METs. This study aims to estimate the effect of physical activity on the survival of breast cancer survivors based on a number of previous similar studies.Subjects and Method: This study is a syste­matic review and meta-analysis study. The arti­cles used in this study were obtained from seve­ral databases including PubMed, Science Direct, Google Scholar, and Springer Link. The articles used in this study were those published from 2000-2020. The population in the study were breast cancer survivors with an inter­vention in the form of high physical activity. Comparison, namely low physical activity, and an outcome in the form of survival. Keywords for searching articles are as follows: "physical activity" AND "breast cancer survivors" AND "lifesustainability" OR "survival" AND "breast cancer" AND "adjusted hazard ratio" AND "cohort". The articles included in this study are full text articles with a cohort study design. Articles were collected using PRISMA flow dia­grams. Articles were analyzed using the Review Manager 5.3 application.Results: There were 8 articles originating from Denmark, Norway, Canada, Germany and the United States. The results of the meta-ana­lysis showed that physical activity was statisti­cally significant in increasing the survival of breast cancer survivors (HR= 0.74 95% CI= 0.63 to 0.87; p= 0.0003) and I² = 0%.Conclusion: High physical activity increases the survival of breast cancer survivors.Keywords: High physical activity, survival, breast cancer survivorsCorrespondence: Enge Surabina Ketaren. Abstract

Background: Diabetes mellitus (DM) is a metabolic disease that is developing into a serious global problem. Diabetes mellitus is characterized by an increase in blood levels that are more than the normal reference. In type 2 DM generally occurs due to reduced insulin secretion and sensitivity over time. Control of blood sugar levels can improve the patient's quality of life. This study aimed to analyze the effect of curcuma longa rhizome extract on fast­ing blood sugar levels and HbA1C in patients with type 2 diabetes.Subjects and Method: This was a meta-analysis of a number of randomized con­trolled trials. The articles were obtained from PubMed, Google Scholar, Springerlink, BMJ, and Sci­encedirect data­bases, published from 2010-2020. The article search was carried out by con­sidering the eligibility criteria defined using the PICO model. P: Type 2 diabetes patients, I: Cur­cuma longa rhizome extract, C: Placebo, and O: Fasting blood sugar levels and HbA1C. The keywords to find articles are as follows: "Curcuma longa", OR "Curcu­min" OR "Tume­ric" OR "Curcuma" AND "Fast­ing blood glu­cose" AND "HbA1C" OR "Glice­mic" OR "Dia­betes Mellitus" AND "Rando­mized Controll Trials". Articles were collected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 14 articles were reviewed in this study. Meta-analysis of 12 articles showed that the curcuma longa rhizome extract reduced fasting blood sugar levels (Stan­dar­dized Mean Diffe­rence= -0.48; 95% CI= -0.61 to -0.34; p <0.001). The meta-analysis of 11 articles showed that admi­nis­tration of curcuma longa rhizome extract decreased HbA1C levels (Standardized Mean Difference= -0.40; 95% CI= -0.59 to -0.20; p <0.001). Abstract

Background:Knee osteoarthritis (OA), known as degenerative joint disease, is typically the result of wear and tear and progressive loss of articular cartilage, most common in elderly women and men. Knee OA is a leading cause of disability and increase with an aging and overweight population. Total Knee Arthroplasty (TKA) is a common surgical treatment of OA knee that highly cost-effective procedure. Stable placement of components is difficult and thus it presents technical challenges for the surgeon but various options are available for managing these defects.Case Report: A 58 years-old housewife came to our clinic due to chronic left knee pain with associated instability since two years ago and getting worse in last three months. She already had knee joint supplementation injections and physical therapy but no improvements. Based on our clinical and radiological findings, our assessment was secondary left knee osteo-arthritis with medial instability and tibial defect due to chronic abscess. Patient underwent TKA to correct this condition. A revised implant was used to close the defect in the medial part of the tibia and soft tissue reconstruction procedure was used to correct the instability.Result: On the first postoperative month, the patient underwent rehabilitation and physical therapy to strengthen her lower limb muscles. On the second month, she started to walk using walking assisted device. On the ninth months follow-up, the patient was able to walk normally without assisted device, the pain disappeared and the symptoms of knee instability were reduced.Conclusion:Bone defects are commonly encountered during TKA, which still has no standard treatment to cure. Abstract

Background: Accurate early detection of bacteriemia plays an important role in the treatment and prognosis of infected neonates, but is constrained by the limitations of spe­cific sepsis markers for detecting bacteriemia. NLCR or neutrophil to lymphocyte count ratio in adult population shows a more sen­sitive parameter in predicting infection and has the advantage of being economical and widely available. The role of NLCR in the neonatal population has not been widely studied. The purpose of this study was to analyze NLCR versus CRP as a predictor of neonatal sepsis.Subjects and Method: This study take ana­lytic observatinal with diagnostic test approach to subject with sepsis risk factor and neonatal sepsis at dr. Moewardi hospital from April until May 2017. Characteristics data as gestational age, gender, birth weight, IT ratio, leucocyte count, neutrophil and lym­phocyte count, NLCR, CRP and blood culture. Data presented descriptively and statistical analysis was performed.Results: No significant correlation between subject’s characteristics and neonatal sepsis. NLCR with cutoff point for 2.22 has sensi­ti­vity, specificity, positive and negative pre­dictive value for 87.5%, 75%, 70% and 90%, respec­ti­vely. CRP with cutoff point for 0.25 has sensi­tivity, specificity, positive and nega­tive predic­tive value for 83.3%, 55.6%, 55.6% and 83.3% respectively. While IT-rasio has sensitivity, spe­cificity, positive and negative predictive value for 29.2%, 91.7%, 70% and 66%, respectively.Conclusion: NLCR has a satisfactory pre­dictive value for neonatal sepsis that widely available and economic predictor alternative with cutoff point for 2.22.Keywords: NLCR, CRP, predictor, neonatal sepsisCorrespondence: Irfan Dzakir Nugroho. Abstract

Background: Pain is a common complaint that causes decreased work productivity and a cause of disability. Biomedical interventions failed to inhibit the increase in chronic low back pain (CLBP) treatment costs. The pain perception of CLBP patients is influenced by emotional psychological and cognitive factors that require biopsychosocial intervention. This study aimed to summarize the existing literature data regarding the effectiveness of biopsychosocial interventions with Cognitive Behavioral Therapy (CBT) and Exercise Therapy Program (ETP) in CLBP.Subjects and Method: This research was systematic review and meta-analysis of the Randomized Control Trial study on the electronic database Pubmed, Google scholar, Willey Online Library, Research Gate, Science Direct. Data analysis was performed using the Revman 5.3 program with Cohen's effect size d.Results: There are 7 articles included in this research. The results of the analysis of studies on the effectiveness of biopsychosocial interventions for CBT and ETP in reducing the intensity of CLBP pain were very large with the effect size (d= -1.31; 95% CI= -2.39 to -0.23; p <0.001), while the effect size of the disability index was very large (d= -1.62; 95% CI= -2.70 to -0.54; p<0.001).Conclusion: Biopsychosocial interventions for CBT and ETP are effective in reducing the pain and disability index of CLBP. The effectiveness of biopsychosocial interventions with CBT and ETP methods is due to the patient's efficacy in completing the rehabilitation program.Keywords: biopysichosocial, CBT, ETP, CLBPCorrespondence: Arif Fadli. Abstract

Background: Cholesterol imbalance occurs in Diabetes Mellitus, where high-density lipo­protein (HDL) levels are low and low-density lipoprotein (LDL) increases, where this im­balance results in the appearance of athero­sclerosis. HDL has antiarterosclerotic poten­tial so it needs to be targeted for therapy. One of the bioactive compounds that have this effect is found in chayote (Sechium edule (Jacq.) Swartz). This study aimed to examine the effects of ethanol extract, Chayote (Sechium Edule (Jacq.) Swartz) fraction and juice on the hdl level in male white miceSubjects and Method: A randomized controlled trial study was conducted to test HDL level in 54 male white rats of the Wistar strain (Rattus novergius sp.) after received ethanol extract, Chayote (Sechium Edule (Jacq.) Swartz) fraction and juice. The depen­dent variable was the HDL level. The indepen­dent variable was the variation in the dose of ethanol extract, fraction and Sechium edule (Jacq.) Swartz Juice. Abstract

Background: Complaints of pain in cervical cancer patients make the patient suffer more. Pain is a subjective matter so that hypnothe­rapy can reduce the pain suffered by patients. The use of hypnotherapy in cancer patients focuses on the emotional aspects and dynamics that occur in the client's subconscious mind. Hypnotherapy can change sensations, percep­tions, thoughts, feelings or behavior after being suggested. Positive suggestions given are able to influence the perception of someone who is in a hypnotic state so that they behave or per­ceive according to the suggestions given. The purpose of this study was to analyze the effect of hypnotherapy as a complementary therapy in reducing pain in cervical cancer patients.Subjects and Method: The experimental study used a random control trial which was conducted at the Dr. Moewardi Hospital from January to September 2019. A sample of 90 patients was selected by purposive sampling. 45 subjects from the treatment group received the hypnotherapy intervention and 45 subjects from the control group received standard care. The dependent variable is the level of pain. The independent variable is hypnotherapy. The level of pain is measured by a numerical pain scale of 0 to 10. Hypnotherapy is performed using hypnosis recording/ audio. Data analysis was performed using an independent t test.Results: The mean pain level after hypno­therapy (Mean= 0.8; SD= 1.96) was lower than the control group (Mean= 3.2; SD= 1.7), effect size= -1.31, and it was statistically significant (p <0.001).Conclusion: Hypnotherapy complementary therapy can help reduce pain levels in cervical cancer patients.Keywords: hypnotherapy, pain, cervical cancerCorrespondence:Hanung Prasetya. Abstract

Background: Light's criteria was reported 25% of misclassification transudates as exu­dates. This study aimed to analyse the accuracy of examining uric acid levels and pleural fluid uric acid levels, and pleural fluid cholesterol and cholesterol ratios to distinguish the exudates and transudates in pleural effusions.Subjects and Method: This was a cross-sectional study de­sign conducted at Dr. Moe­wardi Hospital, Surakarta, Central Java, from July to August 2019. The study subjects were 30 pleural effusion patients treated in the pul­monology ward. The dependent variables were pleural fluid exudates and transudates. The indepen­dent variables were (1) Uric acid levels in pleu­ral fluid; (2) The ratio of uric acid levels be­tween pleural fluid and serum; (3) Pleural fluid cholesterol levels; and (4) The ratio of cho­lesterol levels between pleural fluid and serum. The study instruments were Light's cri­teria and laboratory examination. The diag­nosis's accuracy was analysed using sensi­tivi­ty, specificity, and the area under the ROC (AUC) curve.Results: Pleural fluid cholesterol showed sen­­sitivity and specificity of 86% and 83%, with a cut-off of 32.00 for transudate results. AUC value = 0.82 with p = 0.012. Serum cholesterol showed sensitivity and specificity of 71% and 61%, with a cut-off of 175.50 for transudate re­sults. AUC value = 0.67 with p = 0.194. Pleu­ral fluid uric acid levels showed a sensitivity and specificity of 86% and 87%, with a cut-off of 7.25 for transudate results. AUC value = 0.83 with p = 0.009. Examination of serum uric acid levels showed a sensitivity and specificity of 86% and 70%, with a cut-off of 7.10 for trans­udate results. AUC value = 0.65 with p = 0.249.Conclusion: Examination of uric acid and pleural fluid cholesterol levels can be used in routine pleural effusion examinations to distin­guish exudates and transudates.Keywords: accuracy, uric acid, exudates, cho­les­te­rol, transudatesCorrespondence: Yusup Subagio Sutanto.
