Abstract

Non-adherence of hemodialysis patients to diet and restriction of fluid restriction has a negative impact on health status. Various factors are associated with non-adherence to dietary and fluid restriction in chronic kidney disease patients undergoing hemodialysis including patient characteristics such as age, gender, education, occupation, length of time undergoing hemodialysis, self-efficacy, motivation and social support. This study aims to analyze the most dominant factors associated with dietary compliance and fluid restriction in chronic kidney disease patients undergoing hemodialysis. This study used a descriptive correlational design with a cross-sectional approach. The sample in this study were patients with chronic kidney disease who underwent hemodialysis as many as 80 respondents. Collecting data using the respondent characteristic questionnaire, General Self-Efficacy Scale, Treatment of Motivation Questionnaire, Multidimensional Scale of Perceived Social Support dan Dialysis Diet and Fluid Nonadherence Questionnaire. Data were analyzed using multiple linear regression. The result of the correlation analysis showed that there was no relationship between gender, education and occupation with dietary compliance and fluid restriction (p > .005). Factors related to dietary compliance and fluid restriction were age, length of hemodialysis, self-efficacy, motivation and social support (p < .005). multivariate analysis using multiple linear regression tests showed that the most dominant factor associated with dietary compliance and fluid restriction was self-efficacy (R2 = .504, p = .000). Thus, it is important to periodically assess the patient's self-efficacy and plan actions to increase self-efficacy so that adherence to diet and fluid restriction can also be improved.

Abstract

Bullying is a social phenomenon that often occurs in society, especially in adolescents. There are many biological, psychological, and social changes that can cause conflict problems between themselves and the surrounding environment. This causes the emergence of victims of bullying which causes anxiety in the teenager. This study aimed to determine the effectiveness of expressive writing therapy in reducing anxiety levels in adolescent victims of bullying. The research method used quasi-experimental research with a pre-test and post-test control group design approach. The sampling technique with a total sampling of 44 respondents and divided into 2, namely the experimental group and the control group. The results of the study using the Paired Sample T-test showed that p = 0.000 (p <0.05) means that expressive writing therapy is effective in reducing anxiety levels in adolescent victims of bullying. This can be seen from the decrease in anxiety scores in adolescent victims of bullying from 50.0% to 41.0% severe anxiety, which is mild anxiety after expressive writing therapy intervention. Therefore, it is suggested that expressive writing therapy can be used as one of the ways to overcome the anxiety of teenagers who are victims of bullying both on campus and at home.

Abstract

One of the effects that occur in chronic kidney failure can cause metabolic waste not to be eliminated by the kidneys and accumulate on the skin, which causes itching or commonly known as pruritus. Treatments to reduce pruritus can use emollient oil or baby oil that can keep the skin moist. This implementation aims to determine the effectiveness of baby oil in chronic kidney disease (CKD) patients with uremic pruritus. This implementation is based on the results of previous research using experimental, pretest and posttest design. The population was CKD patients undergoing hemodialysis (HD) with uremic pruritus. This sample determination is based on the inclusion and exclusion criteria that have been built and the sample size was 22 HD patiens. Results showed that pruritus score was decreased up to 2,72. It shows the effectiveness of baby oil on uremic pruritus in CKD patients undergoing HD treatment.

Abstract

Appendectomy is done as soon as possible to reduce the risk of further perforation such as peritonitis or abscess. If not handled properly it can cause complications, slow down the recovery process, and increase the length of hospital stay. The purpose of the study is to analyze the effectiveness of the combination of nature sound and foot massage on pain in post-appendectomy patients. Method: True experiment with pre-post test control group design. The intervention group (n=21) was given nature sound therapy for 15 minutes, the combination group (n=21) was given a combination of nature sound therapy and foot massage for 15 minutes, and the control group (n=21) received routine nursing care. Data analysis used the Wilcoxon test. Results: The combination of nature sound and foot massage for 15 minutes/day is effective and significantly reduces pain on the average Numeric Rating Scale (NRS) score. There were differences in pain before and after the intervention in the three groups. Combination therapy of nature sound and foot massage has a greater value than the hospital SPO group and the nature sound group with a p-value of 0.004 and delta a mean of 0.70. In conclusion: The combination of nature sound and foot massage is effective in reducing pain in post-appendectomy patients.

Abstract

Stroke is damage to brain tissue due to disruption of blood flow to the brain. One impact in stroke patients is weakness on one side of the body or hemiparesis. The management of stroke includes physical rehabilitation. However, many post-stroke patients still do not comply with physical rehabilitation, so education is needed about the importance of complying with physical rehabilitation to accelerate recovery and restore movement function in post-stroke patients. The purpose of this study is to develop Stroke Care Education and analyze its effectiveness on adherence to physical rehabilitation in post-stroke patients. The design of this study used Research and Development (R&D) and a quasy experiment with a pretest-posttest control group design which was carried out for 21 days. The sampling technique used purposive sampling which consisted of 60 respondents who were divided into the intervention and control groups. The intervention group was given YouTube-based Stroke Care Education twice a day (20 minutes), while the control group was given a leaflet. The measurement method used a level of adherence questionnaire, namely the Morinsky Medication Adherence Scale (MMAS) with low, medium, and high measurement values. The data were analyzed using the Wilcoxon test and the Mann-Whitney test. The results of this study YouTube-based Stroke Care Education is suitable for use for post-stroke patients with a validity test result value of 87.5%. The statistical test results showed an increase in adherence to physical rehabilitation in the intervention group compared to the control group, from an average score of 4.40 to 7.57. Meanwhile, in the control group, it increased from 3.77 to 4.23 with a value of p = 0.000. In conclusion, YouTube-based Stroke Care Education is effective in increasing adherence to physical rehabilitation in post-stroke patients.

Abstract

Characteristic of heart failure patients is decreased activity tolerance which is defined as a decreased capacity to carry out activities accompanied by symptoms of dyspnea and/or significant fatigue. This study aimed to determine whether Inspiratory Muscle Training (IMT) can improve activity tolerance in heart failure patients. A systematic review design with the PICO (Population, Intervention, Comparation, and Outcome) approach has been arranged based on the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guideline. Articles were collected through the online database ClinicalKey Nursing, EBSCOhost, Oxford Journals, ProQuest, Sage Journals, ScienceDirect, Scopus, and SpringerLink. Articles with Randomized Controlled Trial (RCT) and quasi experimental design in English were included. Quality of the article was assessed using JBI Critical Appraisal Checklist. There were seven research articles that met the criteria in this study. Those studies show that IMT can increase activity tolerance in heart failure patients. IMT can be applied as a single intervention or in combination with other interventions. Interventions that can be combined with IMT include Cardiac Rehabilitation (CR), Peripheral Muscle Training (PMT), or Functional Electrical Stimulation (FES) programs.

Abstract

Evidence-based practice (EBP) is a nursing concept to improve the quality of health services. Many educational institutions have produced nursing research, but its application in nursing service practice has not been optimal because many nurses have not been exposed to that research. This research aims to find out the challenges of nurse managers in implementing evidence-based nursing practice (EBNP). The method used in this study is the qualitative method by conducting a Forum Group Discussion (FGD) with the five heads of rooms at RSJD Dr. Amino Gondohutomo Central Java Province regarding how to apply EBNP in providing health services. Based on the results of the FGD shows that although the application of EBNP is necessary to increase the knowledge, skills, and competencies of nurses so that innovations emerge in the provision of nursing care, there are still internal and external obstacles that hinder the implementation of EBNP. In the future, to be able to provide excellent patient care decisions, EBNP is needed for making appropriate clinical decisions based on existing cases.

Abstract

Non-adherence in hypertensive patients is a serious challenge faced by health workers. The motivational interviewing (MI) is a patient-centered intervention and has been shown to change a person's compliance. The aim of this review is to investigate the impact of MI on adherence in hypertensive patients. This review was conducted on four databases, PubMed, Cinahl, Web of Science (WOS), and Scopus. Only randomized controlled trial (RCT) articles were used in this review, the quality assessment of the articles used the JBI critical appraisal tools, the selection and extraction of articles referred to the PRISMA guidelines. There were 5 articles that met the inclusion criteria. MI is applied with 2 methods, conventional and non-conventional. 2 articles showed an increase in medication adherence, 1 article showed adherence to dietary and physical activity recommendations, 1 article showed a change in smoking consumption patterns, and 4 articles showed a decrease in blood pressure in post-intervention hypertensive patients. MI can be an effective option in terms of improving hypertension adherence in the future. Although the evidence is currently validated, more research is needed to explore the evidence for the effectiveness of the combined MI method.

Abstract

Osteoarthritis is the most common disease of arthritis which is defined as chronic inflammation of the joints due to damage to the cartilage. The global prevalence of knee osteoarthritis is 3.8% and 0.85% of hip osteoarthritis. Meanwhile in Indonesia, based on the results of the 2018 Riskades, it showed that the prevalence of joint disease was recorded at around 7.3%. The aim of this study was to determine the effect of stretching the hamstring muscle groups on muscle flexibility and quality of life in osteoarthritis patients at the Kaliwungu Health Center. This research uses the quasy experiment with pretest and posttest with control group design. Statistical test of the effect of stretching the hamstring muscle groups on muscle flexibility in the group obtained p = 0.00. Statistical test of hamstring muscle stretching on quality of life obtained a p value of 0.00. Stretching the hamstring muscle group is a therapy to lengthen muscles that experience shortening or decreased muscle elasticity and flexibility due to both pathological and physiological factors. Increased muscle tone can affect the patient's quality of life. There is an effect of stretching the hamstring muscles on muscle flexibility and quality of life in patients with osteoarthritis at the Kaliwungu Health Center.
