Abstract

Osteoarthritis (OA) is a degenerative disease that often occurs, both in Indonesia and abroad. OA is characterized by progressive destruction of joint cartilage and causes structural changes in the joints. OA disease from grades 1 to 4 has symptoms that are usually difficult for the sufferer's life. One of these symptoms is joint pain, but reducing pain intensity, especially OA grades 1 to 3, it can be done with exercise. The use of this exercise is an isometric quadriceps exercise. The design of this study used a quasi-experimental design with a pre-post test control group design with a sample of 40 respondents who met the inclusion and exclusion criteria, namely patients with knee osteoarthritis, stage II, and III who underwent the initial rehabilitation program. Based on the results of data processing using the Wilcoxon test in the control group, the p-value = 0.01 < 0.05, while in the intervention group the p-value = 0.00 < 0.05. From these results, it can be concluded that isometric quadricep exercise is effective in reducing pain intensity in osteoarthritis genu grades 1 to 3. So isometric quadricep exercise can be recommended as an independent therapy in reducing knee osteoarthritis pain intensity.

Abstract

Penulisan ini bertujuan untuk mengetahui dan menjelaskan mengenai kegiatan Sustainable Development Goals (SDG’s) yang dilakukan oleh sepuluh mahasiswa pada saat melakukan “Community Service” di Kuala Terengganu, Malaysia. Sebagai upaya dalam melakukan perubahan yang lebih baik untuk kedepannya. Dalam melakukan penelitian ini, para penulis langsung terjun ke lapangan dan berinteraksi dengan target yang akan dicapai. Penelitian ini akan menjelaskan mengenai strategi dari SDG’s dalam upaya meningkatkan mutu pendidikan bagi para pengungsi Rohingnya dan para murid sekolah umum di Kuala Terengganu, mencegah serta mengurangi semua jenis pencemaran laut, dan melakukan campaign terkait dengan konflik yang terjadi antara Palestina-Israel guna mengurangi kekerasan yang terjadi dimanapun serta memberikan keadilan bagi para korban. Melihat dari permasalahan yang ada saat ini dan bagaimana pula peran dari pemerintah dalam menangani hambatan-hambatan yang ada guna meningkatkan kualitas dari segi pendidikan, keadilan, serta menjaga lingkungan di Kuala Terengganu, Malaysia. Dengan adanya kegiatan SDG’s ini, diharapkan bisa mengatasi permasalahan yang ada dan yang belum merata ini sehingga bisa menjadi lebih baik kedepannya.

Abstract

This study aims to identify what assessments can be used to screen cognitive function in children with chronic illnesses. The method used is a systematic review using the PRISMA flow diagram. Researchers used PIOs and searched for English language articles published from 2013 to October 2023. The databases used were PubMed, ProQuest, Science Direct, Taylor & Francis, Scopus, and EBSCO Medline. This systematic review showed that the initial total of articles obtained from all databases was 5889. After limiting the articles, 297 articles were obtained, which were then screened according to the inclusion criteria, with the final result being eight articles with a cross-sectional design. In conclusion, four instruments for assessing cognitive function in children with chronic diseases include Ouvrier modified MMSE, EpiTrack Junior (EpitrackJR), pedsFACIT-PCF, and Ped-ANAM. The Ouvrier-modified MMSE is widely used in several kinds of literature and is validated against standard cognitive function examinations by examination. Abstract

Evidence-based practice (EBP) is a nursing concept to improve the quality of health services. Many educational institutions have produced nursing research, but its application in nursing service practice has not been optimal because many nurses have not been exposed to that research. This research aims to find out the challenges of nurse managers in implementing evidence-based nursing practice (EBNP). The method used in this study is the qualitative method by conducting a Forum Group Discussion (FGD) with the five heads of rooms at RSJD Dr. Amino Gondohutomo Central Java Province regarding how to apply EBNP in providing health services. Based on the results of the FGD shows that although the application of EBNP is necessary to increase the knowledge, skills, and competencies of nurses so that innovations emerge in the provision of nursing care, there are still internal and external obstacles that hinder the implementation of EBNP. In the future, to be able to provide excellent patient care decisions, EBNP is needed for making appropriate clinical decisions based on existing cases.

Abstract

Prevalensi hipertensi terutama pada kelompok lansia di Indonesia semakin meningkat mencapai 63,2%. Hipertensi yang tidak terkontrol bisa mengakibatkan komplikasi seperti gagal jantung, stroke, ginjal, dan sindrom metabolik hingga kematian. Penatalaksanaan hipertensi dapat dilakukan dengan memberikan terapi non farmakologi berupa penerapan rebusan daun sirsak. Studi ini bertujuan untuk menerapkan rebusan daun sirsak agar menurunkan tekanan darah pada lansia. Metode penelitian yang digunakan adalah case report dengan multicase study melalui pendekatan asuhan keperawatan yang melibatkan 2 lansia yang mengalami hipertensi primer, berusia >45 tahun dan tidak sedang mengkonsumsi obat hipertensi. Daun sirsak direbus dengan 300ml air dan 10 gram daun sirsak selama 15 menit menggunakan api kecil hingga tersisa 150 ml. Kemudian air disaring dan disajikan selagi hangat. Intervensi diberikan dua kali sehari pagi dan sore selama tujuh hari. Pengukuran tekanan darah dilakukan menggunakan tensimeter digital dengan jeda waktu 5 menit sebelum dan 30 menit setelah intervensi. Hasil studi menunjukkan bahwa terdapat penurunan tekanan darah pada kedua subjek dengan nilai rata-rata penurunan tekanan darah sistolik sebesar 9,92 mmHg sedangkan diastolik sebesar 3,55 mmHg. Berdasarkan kesimpulan tersebut saran yang dapat diberikan yaitu meningkatkan partisipasi keluarga dalam penerapan rebusan daun sirsak untuk menurunkan tekanan darah lansia yang mengalami hipertensi.

Abstract

Pendahuluan: Persalinan prematur dikaitkan dengan beberapa faktor, antara lain faktor kehamilan, gaya hidup dan perilaku, genetik, dan riwayat obstetrik, termasuk riwayat keguguran berulang. Pada pasien dengan riwayat keguguran berulang investigasi faktor penyebab penting dilakukan sebelum merencanakan kehamilan lagi. Dalam sebuah tinjauan sistematis dan meta-analisis riwayat keguguran berulang secara keseluruhan meningkatkan risiko persalinan prematur sebesar 1,6 kali lipat. Laporan kasus ini akan membahas persalinan prematur pada pasien dengan riwayat keguguran berulang yang belum diketahui faktor penyebabnya.Presentasi kasus: Pasien berusia 32 tahun, G4P1A2 hamil 30 – 31 minggu, datang dengan gejala dan tanda persalinan. Kehamilan pertama cukup bulan dan bersalin normal. Pasien memiliki riwayat dua kali keguguran secara berturut-turut pada kehamilan kedua (usia gestasi 7 minggu) dan ketiga (usia gestasi 10 minggu), tanpa riwayat diabetes melitus ataupun penyakit kronik lain. Pasien tidak mengonsumsi alkohol/obat-obatan tertentu dan tidak merokok. Suami pasien juga bukan seorang perokok. Penyebab keguguran berulang belum dapat diidentifikasi karena pemeriksaan belum dilakukan secara terarah, sehingga tata laksana untuk mencegah risiko persalinan prematur pada kasus ini belum optimal. Kesimpulan: Pada kasus ini risiko persalinan prematur mungkin terkait dengan riwayat keguguran berulang, sehingga investigasi penyebab keguguran berulang harus dilakukan agar dapat diberikan tata laksana medis yang sesuai untuk mencegah keguguran berulang dan persalinan prematur.Preterm Birth In Patient With History Of Unexplained Recurrent Pregnancy Loss: A Case ReportAbstractIntroduction: Premature birth is associated with several factors, including pregnancy factors, lifestyle and behavior, genetics, and obstetric history, including a history of recurrent miscarriage. In patients with a history of recurrent miscarriage, investigation of risk factor is important before planning another pregnancy. In systematic review and meta-analysis a history of recurrent miscarriage increased the risk of preterm birth by 1.6-fold. This case report will discuss premature labor in a patient with a history of recurrent miscarriage whose cause is unknown.Case Presentation: 32 year old patient, G4P1A2 30 – 31 weeks pregnant, presented with symptoms and signs of labor. The first pregnancy was full term and normal. The patient had a history of two consecutive miscarriages in the second (7 weeks' gestation) and third (10 weeks' gestation) pregnancies, without a history of diabetes melitus or other chronic diseases. The patient does not consume alcohol/certain drugs and does not smoke. The patient's husband is also not a smoker. The cause of recurrent miscarriage cannot be identified because examinations have not been carried out in a targeted manner, management to prevent the risk of preterm labor in this case is not optimal.Conclusion: In this case risk of preterm labor may be related to history of recurrent miscarriage, an investigation into the cause of recurrent miscarriage must be carried out, and the appropriate medical treatment can be given to prevent recurrent miscarriage and premature labor.Key words: Recurrent miscarriage, premature labor, management

Abstract

Introduction: Surgical site infection (SSI) is the most likely complication following a Cesarean section (CS). SSI is considered a contributing factor to the occurrence of surgical wound dehiscence (SWD). This condition is often associated with various risk factors, such as type 2 diabetes mellitus (T2DM) and obesity. Understanding the risk factors and management of SWD in high-risk populations is essential for improving clinical outcomes.Case Report: A 27-year-old woman, G2P2A0, was referred due to SWD. The patient underwent a CS at a secondary-level hospital due to oxytocin drip failure 20 days prior to referral. She was diagnosed with SSI six days post-surgery, and had received antibiotics and undergone re-hecting 11 days after the CS. The surgical wound reopened two days after the re-hecting procedure, prompting referral. The patient had uncontrolled T2DM and morbid obesity as comorbidities. Physical examination revealed an open CS wound measuring 15x5x3 cm with exposed fascia and discharge of pus and blood. Wound care was performed using gauze coated with antibiotics changed every 12 hours. Antibiotic administration was based on culture, sensitivity, and resistance testing before another re-hecting procedure was performed.Conclusion: Obesity and diabetes mellitus were risk factors for SWD in this case. The use of antibiotics guided by culture sensitivity and resistance testing, effective SSI management, and early detection and management of comorbid conditions are necessary to prevent and treat SWD complications.Laporan Kasus: Recurrent Surgical Wound Dehiscence pada Pasien Infeksi Daerah Operasi yang Memiliki Komorbid Diabetes Tipe 2 dan ObesitasAbstrakLatar belakang: Infeksi Daerah Operasi (IDO) merupakan komplikasi yang paling mungkin terjadi setelah operasi Caesar (SC). IDO dianggap sebagai faktor terjadinya Surgical Wound Dehiscence (SWD). Kondisi ini sering dikaitkan dengan berbagai faktor risiko seperti diabetes melitus tipe 2 (DMT2) dan obesitas. Pemahaman mengenai faktor risiko dan penanganan SWD pada populasi berisiko tinggi menjadi penting untuk meningkatkan luaran klinis.Laporan Kasus: Wanita 27 tahun, P2A0, dirujuk karena SWD. Pasien menjalani SC di RS PPK tingkat II karena gagal drip oksitosin 20 hari sebelum dirujuk. Pasien didiagnosis IDO 6 hari setelah operasi dan telah mendapatkan antibiotik dan dilakukan re-hecting 11 hari setelah SC. Luka operasi terbuka kembali dua hari setelah re-hecting, sehingga pasien dirujuk. Pasien memiliki komorbid DMT2 tidak terkontrol dan obesitas morbid. Pemeriksaan fisik menunjukkan tampak luka bekas SC terbuka berukuran 15x5x3 cm dengan dasar fasia dengan nanah dan darah. Perawatan luka dilakukan dengan kasa dilapisi antibiotik dan diganti setiap 12 jam. Antibiotik diberikan berdasarkan hasil tes kultur, sensitivitas, dan resistensi, sebelum dilakukan re-hecting kembali. Kesimpulan: Obesitas dan diabetes melitus menjadi faktor risiko terjadinya SWD pada kasus ini. Penggunaan antibiotik sesuai hasil kultur sensitivitas dan resistensi, perawatan IDO, serta deteksi dan manajemen faktor komorbid diperlukan untuk mencegah dan mengobati komplikasi SWD.Kata kunci: Diabetes mellitus tipe 2, infeksi daerah operasi, luka operasi terbuka, rekurensi

Abstract

Objective: This study aims to translate, develop, and assess the validity and reliability of the Indonesian version of Professor Chapron’s screening method for endometriosis diagnosis based on the questionnaire as a tool for better endometriosis screening in Indonesia.Methods: Development of the questionnaire using 5 steps: translation, synthesis, back translation, expert committee review, and pretesting by involving language expert and subject expert. The analysis includes the validity and reliability of the questionnaire.Results: A sample of 30 subjects was enrolled. The questionnaire was consist of 3 sections, including characteristic patient questions, yes or no questions (8 questions), and pain scale questions (5 questions). Of the 8 yes or no questions, 7 were valid (r >0.300), and one question was not valid (r<0.300). The yes or no questions were reliable (Cronbach’s alpha 0.6968). All of the 6 pain scale questions were valid (r>0.300) and reliable (Cronbach’s alpha 0.790).Conclusion: This study represent the Professor Chapron’s questionnaire for endometriosis screening into another language (Bahasa) than the original. It showed that the questionnaire was reliable, and one question was not valid (removed from the questionnaire). The questionnaire can be used in Indonesia for screening method in Indonesia.Versi Indonesia dari Kuesioner Chapron untuk Metode Skrining dalam Diagnosis Endometriosis sebagai Alat Skrining Endometriosis di IndonesiaAbstrak Tujuan: Penelitian ini bertujuan untuk menerjemahkan, mengembangkan, dan menilai validitas dan reliabilitas versi bahasa Indonesia dari metode Skrining Professor Chapron berdasarkan kuesioner, sebagai alat untuk skrining endometriosis yang lebih baik di Indonesia.Metode: Pengembangan kuesioner menggunakan 5 langkah: penerjemahan, sintesis, penerjemahan balik, telaah komite ahli, dan pra-uji dengan melibatkan ahli bahasa dan ahli subjek. Analisis yang dilakukan yaitu uji validitas dan reliabilitas kuesioner.Hasil: Sampel penelitian sebanyak 30 orang. Kuesioner terdiri atas 3 bagian, yaitu pertanyaan karakteristik pasien, pertanyaan ya atau tidak (8 pertanyaan), dan pertanyaan skala nyeri (5 pertanyaan). Sebanyak 7 pertanyaan ya atau tidak, hasilnya valid (r>0,300) dan 1 pertanyaan tidak valid (r<0,300). Pertanyaan ya atau tidak hasilnya reliabel (Cronbach’s alpha 0,6968). Semua pertanyaan skala nyeri hasilnya valid (r>0,300) dan reliabel (Cronbach’s alpha 0,7900).Kesimpulan: Penelitian ini mengembangkan serta menilai validitas dan reliabilitas kuesioner Professor Chapron untuk skrining endometriosis dalam versi bahasa Indonesia. Kuesioner ini reliabel dengan 1 pertanyaan tidak valid (dihilangkan dari kuesioner). Kuesioner ini dapat digunakan di Indonesia untuk metode skrining endometriosis di Indonesia.

Abstract

Health education plays a critical role in enhancing individuals' knowledge, attitudes, and skills to address health issues such as HIV/AIDS. Given the rising prevalence of HIV/AIDS among adolescents, effective health education is essential for mitigating social stigma and boosting self-efficacy in this group. This study aims to evaluate the effectiveness of HIV/AIDS health education in improving self-efficacy and reducing stigma among adolescents. A scoping review methodology was employed, following the PRISMA-ScR guidelines, with literature searches conducted across academic databases such as PubMed, Google Scholar, and Scopus. The results indicate that while numerous educational interventions have been implemented, few directly address the psychological impacts of stigma and self-efficacy. This study concludes that well-designed educational programs can alter adolescents' perceptions of HIV/AIDS, enhance their self-confidence, and reduce societal stigma. These programs improve adolescents’ understanding of HIV/AIDS prevention while alleviating the anxiety and fear often linked to social stigma. The findings are expected to provide valuable insights for developing more effective HIV/AIDS health education initiatives, focusing on stigma reduction and the enhancement of self-efficacy among adolescents.

Abstract

There are changes of eutopic endometrium molecular fenotipe in endometriosis such as changes in gene expression, steroid hormone response, increase of inflammation marker and cellular adhesion molecule, decrease of apoptotic index and decidualization capacity, increase of oxidative stress marker, increase activity of angiogenesis and neurogenesis. This study was conducted to analyze the differences in the expression of angiogenic factors (VEGF A) and neurogenesis factors (PGP 9.5) eutopic endometrium of menstrual bleeding among patients with endometriosis and non endometriosis and then the correlation of the two factors. This study is a cross sectional that examines the relationship between the incidence of endometriosis with risk factors such as VEGF expression and PGP 9.5 in menstrual blood. There were significant differences in the expression of VEGF A eutopic endometrium menstrual blood between endometriosis and non endometriosis group (p=0.002). There are significant differences expression of VEGF A and PGP 9.5 eutopic endometrial of menstrual blood between endometriosis and non endometriosis groups. There is a positive correlation between the expression of VEGF A with PGP 9.5 eutopic endometrial of menstrual blood on endometriosis patients.Keywords: eutopic endometrium, menstrual blood, VEGF A expression, PGP 9.5 expression, immunocytochemistry
