Abstract

Microbiological contamination in drinking water remains a significant public health problem associated with an increased risk of waterborne diseases, particularly infectious diarrhea. Refillable drinking water depots (Depot Air Minum Isi Ulang / DAMIU) are widely used by the community as a more economical alternative to bottled drinking water (Air Minum Dalam Kemasan / AMDK). However, the microbiological quality of DAMIU is still frequently questioned due to inadequate sanitation and suboptimal water treatment processes. This study aimed to analyze and compare the bacteriological quality of DAMIU and AMDK in Pekanbaru City based on total coliform and fecal coliform examination using the Most Probable Number (MPN) method. This analytical observational study employed a cross-sectional design. The study samples consisted of 10 drinking water samples, including 5 DAMIU samples and 5 AMDK samples selected using a random sampling technique. Microbiological examination was conducted through presumptive tests to detect total coliform and confirmed tests to identify fecal coliform. Data were analyzed descriptively and inferentially using Fisher’s exact test with a significance level of p<0.05. All DAMIU samples were contaminated with total coliform, whereas contamination was found in only two AMDK samples. The confirmed test demonstrated that two DAMIU samples were contaminated with fecal coliform, while all AMDK samples were negative for fecal coliform contamination. Fisher’s exact test showed no statistically significant difference between the bacteriological quality of DAMIU and AMDK based on presumptive test results (p=0.08) and confirmed test results (p=0.22). Conclusion the bacteriological quality of DAMIU in Pekanbaru City tended to be lower than that of AMDK, as indicated by the higher contamination of total coliform and the presence of fecal coliform in several DAMIU samples. Although no statistically significant difference was identified, DAMIU still demonstrated a higher potential risk of microbiological contamination, highlighting the need for improved sanitation monitoring and periodic water quality assessments.

Abstract

Pregnancy is a condition in which the ovum and sperm meet. Changes during pregnancy include changes in erythrocyte mass, plasma volume, and blood volume, resulting in low hemoglobin (Hb) levels due to a decrease in erythrocyte in the blood. Anemia is a global health problem, especially in developing countries, with an estimated 30% of the world’s population suffering from anemia. Anemia is common in the community, especially among pregnant women. Anemia in pregnant women that is not treated properly can increase the risk of dangerous complications, such as premature labor. In addition, anemia can also increase the risk of low birth weight in babies. On the mother’s side, anemia can increase the risk of postpartum depression and postpartum maternal mortality. The purpose of this study was to determine the comparison of hemoglobin and MCV levels as indicators of iron deficiency anemia in pregnant women in the first and third trimesters at the Majenang I Community Health Center. This was an observational analytical study with a cross-sectional approach, with samples taken at the Majenang I Community Health Center from September 2024 to April 2025 according to the inclusion and exclusion cricetria, totalling 33 samples. The results showed a significant difference in hemoglobin levels between at first trimester (average 11.97 g/dL) and the third trimester (11.00 g/dL). Additionally, there was a significant difference in MCV values between the two groups. The average MCV value increased from 82.75 in the first trimester to 84.12 in the third trimester. The increase in MCV values was still within the normocytic range and reflected the adaptation of erythropoiesis during pregnancy. The conclusion of this study is that there are differences in hemoglobin and MCV levels in pregnant women in the first and third trimesters at the Majenang I Community Health Center. Both hemoglobin and MCV levels undergo significant changes and can be used as early indicators in detecting iron deficiency anemia in pregnant women.

Abstract

Type 2 diabetes mellitus is a chronic metabolic disorder with a continuously increasing prevalence and is influenced by genetic factors, including mitochondrial DNA mutations. The A3243G mutation in the MT-TL1 gene is known to exhibit maternal inheritance and is associated with mitochondrial dysfunction that may affect insulin secretion. This study aimed to analyze the inheritance pattern of the A3243G mutation in the MT-TL1 gene among children of mothers with type 2 diabetes mellitus. This study employed an observational design with a cross-sectional approach. The study samples consisted of 9 mothers with type 2 diabetes mellitus and 9 of their children, selected using a quota sampling technique. Venous blood samples were collected for DNA isolation and subsequently analyzed at the molecular level using the Polymerase Chain Reaction–Allele Specific Amplification (PASA) method. The results showed that the A3243G mutation in the MT-TL1 gene was detected in all maternal samples and was also identified in all corresponding child samples, suggesting the presence of heteroplasmy and a possible maternal inheritance pattern. This study has several limitations, including a relatively small sample size and the use of a cross-sectional design, which does not allow for direct causal inference. Therefore, these findings suggest that the A3243G mutation in the MT-TL1 gene may be maternally inherited and can be identified through molecular analysis using the PASA method.

Abstract

Abstract Dengue Hemorrhagic Fever (DHF) is a viral infection caused by the dengue virus that remains a major public health problem in Indonesia, affecting various age groups with different clinical manifestations. Laboratory examinations such as platelet count, hematocrit level, and neutrophil-to-lymphocyte ratio (NLR) are important indicators to assess the severity of DHF. This study aimed to compare the values of platelet count, hematocrit, and NLR between pediatric and adult DHF patients at Mitra Ariva Ajibarang Surgical Hospital. The study used a quantitative method with a total sampling approach involving 40 medical record data of DHF patients from December 2024 to March 2025, consisting of 20 pediatric and 20 adult patients. Data were analyzed statistically using the Shapiro–Wilk test for normality and the independent t-test to examine intergroup differences. The results showed a significant difference (p < 0.05) in all three variables. The mean platelet count was higher in children (90.45×10³/µL) than in adults (62.20×10³/µL), while hematocrit and NLR values were higher in adults (47.05% and 3.56) compared to children (42.15% and 1.77). These findings indicate that adult patients experience more severe thrombocytopenia, hemoconcentration, and inflammatory responses than children. In conclusion, age significantly affects hematological parameters in DHF, with adult patients showing greater disease severity. Routine monitoring of platelet, hematocrit, and NLR values is essential for evaluating disease progression and preventing complications in DHF patients. Abstract

Background: Blood glucose testing can be performed using venous and capillary blood samples and can be measured rapidly using a glucometer as part of Point of Care Testing (POCT). Physiological differences between venous and capillary blood may affect blood glucose measurement results. Objective: This study aimed to compare blood glucose measurement results between venous and capillary blood samples using a glucometer. Methods: This analytical study used a cross-sectional approach and was conducted in April 2026 at the Laboratory of the Health Analyst Department, Sanz Magnatya University. A total of 30 samples were selected using a simple random sampling technique. Data were analyzed using a paired t-test with a significance level of α = 0.05. Results: The mean glucose level in venous blood was 99.70 mg/dL, while the mean glucose level in capillary blood was 91.77 mg/dL. Statistical analysis showed a p-value < 0.001, indicating a statistically significant difference between the two sample types. Conclusion: There was a significant difference in blood glucose measurement results between venous and capillary blood samples using a glucometer. Suggestion: Future studies are recommended to involve larger sample sizes and more specific populations.

Abstract

Tuberculosis (TB) remains a global health problem, particularly in developing countries such as Indonesia. TB diagnosis can be performed using Rapid Molecular Tests supported by radiological examinations such as the Cardiothoracic Ratio (CTR). Additionally, smoking is a risk factor that can increase susceptibility to TB infection. This study aims to determine the distribution of TCM results in patients suspected of having tuberculosis based on smoking duration and CTR values. This study used a descriptive observational design with a retrospective approach. The study sample consisted of 72 patients suspected of having tuberculosis at the UPT Puskesmas Kragilan who had undergone TCM and X-ray examinations. Data were analyzed descriptively in the form of frequency distributions and percentages. The results showed that 23.6% of patients had positive TCM results and 76.4% had negative results. Based on smoking duration, the passive smokers group had the highest positive percentage (9.7%), followed by >10 years (8.3%) and <10 years (5.6%). Based on CTR values, all positive TCM results were found in the CTR <0.5 group.The conclusion of this study indicates that there is variation in the distribution of TCM results based on smoking duration but does not show a consistent pattern of increase. CTR values do not show a direct association with TCM results.

Abstract

Malaria remains a public health issue in tropical and subtropical regions, including eastern Indonesia. Based on data from the Ministry of Health of the Republic of Indonesia in 2025, there were 418,546 reported positive malaria cases nationwide. In North Maluku Province, 317 positive cases were found, with South Halmahera Regency recording 36 cases. The Gane Timur District itself is classified as a low-endemic area with only one positive case. This study aims to monitor malaria cases at Puskesmas Maffa, Gane Timur District, South Halmahera Regency. The research used a descriptive observational design with a cross-sectional approach and accidental sampling technique, involving 55 respondents who visited the Puskesmas during the study period. The examination was conducted microscopically thru thick and thin blood smears, and the results showed that all samples were negative for Plasmodium. The characteristics of the respondents showed that the majority were aged 15–45 years (52.7%), followed by those over 45 years (45%), and under 15 years (1.8%). Based on gender, females dominate (70.9%) compared to males (29.1%). From a clinical perspective, almost all respondents did not experience typical malaria symptoms such as fever, chills, and sweating (94.5%). Preventive behavior shows that the majority do not use mosquito nets (90.9%), but still protect themselves with closed clothing at nite (96.4%). The use of mosquito repellent is only done occasionally (69.1%). Environmental factors are relatively supportive of prevention, with the majority not having standing water around their homes (89.1%). However, some respondents still occasionally go out at nite (61.8%), and almost all houses have inadequate ventilation (96.4%). The absence of positive cases is suspected to be related to the low incidence of malaria and the success of health surveillance. Routine laboratory examinations are still necessary as an effort for early detection and prevention of transmission.

Abstract

Diabetes Mellitus is a serious, long-term condition that occurs when a person's blood glucose levels rise because the body cannot produce enough insulin. According to the International Diabetes Federation (IDF), there were approximately 20.4 million adults aged 20-79 years old with diabetes in Indonesia in 2024, and this number is expected to increase to 28.6 million by 2050. Diabetic nephropathy is a complication of diabetes mellitus that begins with microalbuminuria, which can be detected before kidney function declines, indicated by increased blood urea and creatinine levels. This study aims to determine the relationship between microalbuminuria and serum urea and creatinine levels in patients with type 2 diabetes mellitus  Method: This research is descriptive with a cross-sectional research design using medical record data from patients participating in the Chronic Disease Management Program (Prolanis) at the  Central Cilacap 1 Puskesmas in the period of April 2025. Data were analyzed using the Kolmogorov-Smirnov normality test and continued using the Spearman non-parametric correlation test. Result: The results of the study showed that the average microalbumin level was 122.9491 mg/g creatinine, the average urea level was 28.60 mg/dL and the average creatinine level was 0.91 mg/dL. The results of the Spearman correlation test showed a significant value (ρ-value) between microalbumin and urea levels of 0.003 and between microalbumin and creatinine levels of 0.003. he ρ-value of each is <0.05 Conclusion: can be concluded that there is a relationship between microalbumin levels and urea and creatinine levels in type 2 Diabetes Mellitus patients at the Central Cilacap 1 Puskesmas

Abstract

Diabetes merupakan gangguan metabolik kronis yang terjadi akibat produksi insulin yang tidak mencukupi atau penurunan efektivitas kerja insulin, yang berujung pada peningkatan kadar glukosa darah (hiperglikemia). Pada tahun 2024, di Indonesia tercatat sebanyak 20.426.400 orang dewasa  menderita  diabetes,  dengan  prevalensi  sebesar  11,3%  dari  total populasi  dewasa  yang  berjumlah  185.217.400  jiwa. Salah - satu efek kronis dari diabetes melitus adalah ulkus kaki diabetik (UKD) yang  merupakan  penyebab  komplikasi  mikroangiopati  neuropati perifer. Tujuan dari  penelitian ini adalah untuk melihat gambaran resistensi Staphylococcus aureus dan Klebsiella pnuemoniae terhadap antibiotik seftriakson dan meropenem pada pasien ulkus kaki diabetik. Penelitian ini merupakan penelitian deskriptif dengan pendekatan retrospektif. Pendekatan retrospektif digunakan untuk mendapatkan data-data spesies bakteri dan hasil uji kepekaan antibiotik bakteri melalui data sekunder yang diambil secara total sampling periode bulan Januari-Desember Tahun 2023-2025. Sampel pada penelitian ini merupakan hasil pemeriksaan kultur pus dan resistensi pada pasien UKD positif bakteri Staphylococcus aureus dan Klebsiella pneumoniae. Hasil penelitian ini didapatkan bahwa terdapat resistensi Staphylococcus aureus terhadap antibiotik seftriakson pada tahun 2025 (20%). Sementara itu, Klebsiella pneumoniae menunjukkan kecenderungan resisten (100%) terhadap seftriakson selama periode penelitian. Seluruh isolat bakteri, baik Staphylococcus aureus maupun Klebsiella pneumoniae tidak menunjukkan resistensi terhadap antibiotik meropenem selama periode 2023–2025.

Abstract

Platelets are a hematological parameter with a high risk of reading error on hematology analyzer, necessitating stringent quality control to ensure the accuracy and precision of test results. Control evaluations are conducted using Levey-Jennings charts by applying Westgard rules and six sigma method to provide a comprehensive overview of the instrument’s accuracy and precision. This study aims to determine the correlation between Westgard rule violations and Six Sigma in the platelet quality control. The method used is a retrospective observational study using secondary data from internal quality control low, normal, and high levels results for the period January 1 – December 31, 2025, using hematology analyzer Sysmex XN-550. The result showed that the violations of the 10x rule were the most dominant (66,67%). Accumulative Spearman correlation test indicated a significant moderate positive correlation (p=0,391; p=0,018). At the high level, a strong correlation was obtained (r=0,649; p=0,022). Meanwhile, no significant correlations were found at the normal and low levels. Interpretation of six sigma values must be performed by considering the characteristics of each control level. Laboratories are recommended to implement sigma-based QC design by simplifying rules at high sigma to avoid false rejection and tightening rules (multirule)  as well as control frequency at low sigma levels.
