Abstract

Skin and soft tissue infections caused by S. aureus and S. pyogenes remain a significant public health concern, particularly in tropical regions. Pineapple peel (Ananas comosus [L.] Merr.), an abundant agricultural by-product, contains several bioactive compounds such as flavonoids, tannins, saponins, and bromelain, which are known for their antibacterial properties. This study aimed to evaluate the antibacterial activity of the ethanol extract of pineapple peel against S. aureus and S. pyogenes. This in vitro experimental study employed a post-test-only control group design. The extract was prepared by maceration using 96% ethanol and diluted with DMSO to obtain final concentrations of 5%, 10%, and 15%. Antibacterial activity was assessed using the well diffusion method on Mueller-Hinton Agar, with 70% ethanol as the positive control and DMSO as the negative control. Inhibition zones were measured after 24 hours of incubation at 37°C. The ethanol extract exhibited inhibitory effects against S. aureus with mean inhibition zone diameters of 4.72±0.88mm (5%), 5.53±0.75mm (10%), and 8.31±1.11mm (15%), and against S. pyogenes with 4.34±0.53mm (5%), 6.20±0.73mm (10%), and 9.29±0.36mm (15%). Statistical analysis using the Kruskal–Wallis test showed significant differences among treatment groups (p < 0.05), confirming that the antibacterial effect was concentration-dependent. In conclusion, the ethanol extract of pineapple peel demonstrated significant antibacterial activity against S. aureus and S. pyogenes, with increasing efficacy at higher concentrations, suggesting its potential as a natural antibacterial agent for future therapeutic or topical applications.

Abstract

Soil Transmitted Helminths (STH) infection is a worm whose life cycle requires suitable soil to reproduce into an infective form. One of the jobs related to land is cattle raising. The purpose of this study was to determine the presence of STH eggs in the nails and faeces of cattle breeders in Kedawung Village, Jumapolo District, Karanganyar Regency. This study used descriptive research using concentrated salt flotation method. Research data is presented in tabular form and expressed in percent. The conclusion showed that 2 (13.3%) out of 15 cattle breeder respondents in Pamotan Hamlet RT 12/06 Kedawung Village, Jumapolo District, Karanganyar Regency were infected with STH which was indicated by the finding of hookworm eggs in the stool samples and negative results in the samples nail.

Abstract

Dermatophytes are a classification of fungi that attack and damage keratinized tissue, including hair, skin, nails, and fur. The cause of this infection is keratinolytic filamentous fungi referred to as dermatophytes. Islamic boarding schools are often at risk of dermatophyte transmission, with warm and humid environments accelerating fungal growth in female students. Tinea capitis is a dermatophytosis that infects hair follicles and can cause hair loss and scaly lesions. The purpose of this study was to determine whether dermatophyte species cause Tinea capitis infections in female students. The method used was analytical observation with a cross-sectional design. The sampling technique used was non-probability sampling with a purposive sampling method. The results of the study with 20 respondents tested positive for dermatophyte fungi, 10 respondents and 10 respondents tested negative. In conclusion, the identification of dermatophyte fungi obtained Trichophyton mentagrophytes and Trichophyton rubrum species in positive results. In negative results, non-dermatophyte fungi were found.

Abstract

Diabetic ulcers are one of the serious complications in patients with diabetes mellitus, who are highly susceptible to infections caused by pathogenic bacteria, particularly gram-negative bacteria such as Pseudomonas aeruginosa and Klebsiella pneumoniae. Infection management requires identification of the causative bacteria and antibiotic sensitivity testing to determine effective therapy. This study aims to identify the presence of Pseudomonas aeruginosa and Klebsiella pneumoniae in ulcer samples from diabetes mellitus patients and to determine their sensitivity patterns to several types of antibiotics. The research employed a laboratory-based observational method with a cross-sectional approach to describe the presence of Pseudomonas aeruginosa and Klebsiella pneumoniae as well as their antibiotic sensitivity based on test results from ulcer samples of diabetic patients. The samples were collected from diabetic patients with ulcers. Bacterial isolation and identification were carried out through gram staining and standard biochemical tests. Antibiotic sensitivity testing was performed using the Kirby-Bauer (disk diffusion) method against imipenem, ciprofloxacin, gentamicin, and ampicillin. The results of the study showed that from a number of samples analyzed, positive isolates of Pseudomonas aeruginosa and Klebsiella pneumoniae were found, in the study respondents, around 70% were male and aged over 50 years. Sensitivity tests showed that Pseudomonas aeruginosa had high sensitivity to imipenem but resistance to ampicillin. Klebsiella pneumoniae showed sensitivity to ciprofloxacin, but some isolates showed resistance to ampicillin.

Abstract

Human Immunodeficiency Virus (HIV) remains a major health challenge because the infection progressively weakens the immune system by reducing CD4 cell counts and increasing the risk of opportunistic infections such as pulmonary tuberculosis. Antiretroviral therapy (ART) is essential to suppress viral replication and support immune recovery. This study aims to describe the changes in CD4 counts and Viral Load in HIV/AIDS patients before and after six months of ART at Banyumas Regional Hospital. A descriptive case study approach was used, involving two male patients diagnosed with stage-3 HIV/AIDS accompanied by pulmonary TB. Data were obtained from medical records, including CD4 counts and Viral Load measurements taken before and after treatment. The results show an increase in CD4 levels in both patients. In the first case, CD4 increased from 9 cells/µL to 88 cells/µL, while in the second case, CD4 rose from 156 cells/µL to 533 cells/µL. Both patients also achieved a Viral Load of <40 copies/mL after six months of ART, indicating successful viral suppression. Overall, the findings demonstrate that ART contributed to meaningful immunological improvement and a substantial reduction in Viral Load. Consistent treatment adherence and regular monitoring remain essential to ensure optimal therapeutic outcomes.

Abstract

Tinjauan ini membahas pengaruh vitamin D terhadap dinamika sitokin IFN-γ dan IL-10 pada pasien tuberkulosis. Vitamin D memiliki peran imunomodulator melalui peningkatan fungsi antimikroba makrofag serta pengaturan respons inflamasi. Literatur menunjukkan bahwa efek vitamin D terhadap IFN-γ bersifat kontekstual, sedangkan terhadap IL-10 lebih konsisten dalam meningkatkan regulasi anti-inflamasi. Perubahan kadar sitokin selama terapi OAT mencerminkan perbaikan respon imun. Secara keseluruhan, status vitamin D yang optimal berpotensi memperkuat mekanisme pertahanan terhadap Mycobacterium tuberculosis serta mendukung proses pemulihan inflamasi. Kajian ini menegaskan potensi vitamin D sebagai komponen adjuvan dalam tata laksana tuberkulosis.

Abstract

Gonorrhea is a sexually transmitted infection with clinical symptoms of mucosal inflammation and pus formation in the genital area due to infection with the bacteria Neisseria gonorrhoeae. Transmission of gonorrhea infection can occur when the bacteria are transmitted through bodily fluids, sexual contact with sufferers, and deviant social behavior (MSM or FSW). Examination of patients diagnosed with gonorrhea requires accurate validation of results to improve internal quality, so that internal quality assurance of examinations is necessary. This study aims to determine the microscopic appearance of Neisseria gonorrhoeae bacteria in gonorrhea patients and to strengthen its internal quality assurance at the Puskesmas (Primary Health Center) Gedongtengen, Yogyakarta. The study used a descriptive study using a cross-sectional method. Data samples were taken from the medical records of patients who underwent gonorrhea examinations from January to May 2025 with a total of 35 patients, and positive results were dominated by female patients, with 8 patients. Gonorrhea examinations were carried out using the Gram staining method and then examined microscopically. The positive specimens showed clear morphology of DGNI (Gram-Negative Intracellular Diplococci) and PMN (Polymorphonuclear Cells), coffee bean-shaped, non-spore-forming, and non-motile bacteria. The entire examination process, including result validation and quality assessment, followed standard operating procedures (SOPs). The internal quality assurance assessment for gonorrhea examinations at Puskesmas Gedongtengen laboratory have met the standards with good qualifications, as stated in Puskesmas Gedongtengen SOP policy 43/UKPP/LAB/2023 concerning Internal Quality Assurance for Gonorrhea Examination.

Abstract

Bananas are a natural source of carbohydrates and potassium that are commonly consumed as an energy supply before physical activity. Their nutritional content is thought to help maintain metabolic stability during exercise, yet their influence on cardiac biomarkers and immune responses remains unclear. This study aimed to determine the effect of banana consumption on Creatine Kinase-MB (CK-MB) levels and white blood cell counts in students before and after exercise. This research employed a pre- and post-test design involving 15 randomly selected students from the Medical Laboratory Technology Program at Universitas Al-Irsyad Cilacap. Blood samples were collected before and after exercise under two conditions: without banana consumption and after consuming bananas. CK-MB levels were measured using a photometer, while leukocyte counts were analyzed using a hematology analyzer. Paired t-test results showed no significant differences in CK-MB levels before and after exercise (p > 0.05), although a slight increase in mean values was observed within physiological limits. Similarly, there was no significant difference in leukocyte counts following exercise (p > 0.05), with the effect size categorized as very small for both variables. These findings indicate that consuming bananas prior to exercise does not significantly affect cardiac biomarkers or inflammatory responses in students performing light-to-moderate physical activity. Bananas remain safe to consume before exercise and can serve as an energy source without causing notable physiological changes.

Abstract

Abstract Infectious diseases are medical disorders caused by pathogenic bacteria that infect and multiply within the body in large numbers, leading to illness. Antibiotics are the choice for combating infections and diseases caused by pathogenic bacteria. Guava seeds can be used as a natural ingredient for medicinal plants because they contain secondary metabolite compounds such as flavonoids, phenols, and terpenoids, which are known to have activities as antioxidants, antibacterial, antifungal, antiviral, and anticancer agents. The purpose of this study is to determine the antibacterial activity of ethanol extract of guava seeds in inhibiting the growth of Bacillus subtilis bacteria. The method used in this study is the Kirby-Bauer method, which is a disk diffusion method. Based on the results of the research conducted with concentrations of 20%, 40%, 60%, and 80%, with varying results of 12.43 mm, 15.37 mm, 15.78 mm, and 16.67 mm. These results can be concluded that the inhibition zone produced by the guava seed extract against Bacillus subtilis bacteria falls into the category of strong inhibitory

Abstract

Extrapulmonary tuberculosis (EPTB) is tuberculosis that occurs outside the respiratory system and often experiences delays in diagnosis due to the characteristics of low-concentration specimens. The Xpert MTB/RIF-based Molecular Rapid Test (TCM) is recommended as an initial diagnostic method because it can detect Mycobacterium tuberculosis and rifampicin resistance quickly. This study aims to describe the results of TCM examination and rifampicin resistance status in EPTB cases at Cilacap Regional Hospital during January–October 2025. This study used a non-experimental observational design with a retrospective descriptive approach. Data were obtained from TCM examination results on extrapulmonary specimens of EPTB patients. The results showed that of all EPTB samples examined, a small proportion were detected positive for Mycobacterium tuberculosis. The majority of positive samples showed sensitivity to rifampicin, although cases with resistance and intermediate categories were still found. Variations in positive EPTB specimens included biopsy tissue, pleural fluid, feces, gastric lavage, abscess pus, and ultraascites fluid, reflecting the spectrum of extrapulmonary TB infection. The conclusion of this study shows that rifampicin sensitivity in EPTB cases in Cilacap Regency is still dominant, indicating that the effectiveness of the first-line anti-TB regimen is still maintained, but vigilance against the potential for drug-resistant TB is still needed through continuous TCM examination.
