Abstract

Crossmatching is a critical pre-transfusion test to ensure compatibility between donor and recipient blood. This case report describes crossmatch findings in a 47-year-old male with O Rh-positive blood type and aplastic anemia who required packed red blood cells (PRBC). Using the gel test method, mixed-field reactions were observed in major tests 1 and 2, while the minor test was negative; the autocontrol was positive and the direct Coombs test was negative, suggesting partial incompatibility. Repeat ABO/Rh verification, replacement of the donor unit, and red-cell washing produced consistent results. After clinical assessment and discussion with the attending physician, transfusion proceeded with leukodepleted PRBC to reduce the risk of transfusion reactions. This case highlights the need for rigorous interpretation of crossmatch results and close collaboration between laboratory staff and clinicians in transfusion decision-making.

Abstract

Melena akibat perdarahan saluran pencernaan bagian atas dapat menyebabkan anemia dengan berbagai tingkat keparahan. Studi ini bertujuan untuk mendeskripsikan kondisi anemia pada pasien dengan gastritis erosif berdasarkan pemeriksaan laboratorium, endoskopi, dan histopatologi. Studi ini menggunakan desain studi kasus deskriptif yang melibatkan satu pasien yang dirawat di Rumah Sakit Daerah Dr. Soedirman, Kebumen. Data diperoleh dari hasil pemeriksaan hemoglobin, hematokrit, feses, endoskopi, dan histopatologi. Hasil menunjukkan bahwa kadar hemoglobin pasien berkisar antara 3,2 hingga 9,8 g/dL, menunjukkan anemia sedang hingga berat. Pemeriksaan endoskopi mengungkapkan esofagitis dengan erosi pada garis Z dan gastritis erosif pada mukosa antrum, sedangkan daerah lambung lainnya dan duodenum tampak normal. Pemeriksaan histopatologi menunjukkan gastritis kronis ringan dengan infeksi Helicobacter pylori positif tanpa bukti keganasan. Studi ini menyimpulkan bahwa gastritis erosif yang disertai infeksi Helicobacter pylori berperan sebagai sumber perdarahan saluran pencernaan bagian atas, yang berkontribusi pada perkembangan anemia. Pemeriksaan komprehensif diperlukan untuk mendukung diagnosis yang akurat dan penanganan yang tepat.

Abstract

Surgical Site Infection (SSI) is a post-surgical complication that can increase patient morbidity, mortality, and length of hospital stay. One of the causative bacteria of SSI is Methicillin-resistant Staphylococcus aureus (MRSA), which exhibits resistance to β-lactam antibiotics. This study aimed to detect MRSA bacteria in SSI among post-orthopedic surgery patients at RSUD Kraton Pekalongan. This exploratory descriptive study employed a convenience sampling technique. The research samples consisted of three bacterial isolates obtained from post-orthopedic surgery patients with SSI. MRSA identification was performed through bacterial culture, Gram staining, biochemical testing, and antibiotic sensitivity testing utilizing the BD Phoenix automated system based on the Minimum Inhibitory Concentration (MIC) values, interpreted according to the 2024 CLSI guidelines. The results showed that all examined isolates (3/3; 100%) were identified as Methicillin-resistant Staphylococcus aureus (MRSA). In conclusion, all bacterial isolates obtained from orthopedic postoperative SSI patients included in this study were identified as MRSA.

Abstract

Klebsiella sp and Shigella sp are rod-shaped gram-negative bacteria that are often found in the context of public health. Klebsiella sp is a pathogenic bacterium and can cause diarrhea, pneumonia infections, urinary tract infections, wound infections, meningitis to bacteremia. while Shigella sp is a pathogenic bacteria that causes dysentery diarrhea characterized by acute diarrhea containing blood and mucus, accompanied by abdominal pain and tenesmus, which is spread through contaminated food and drinks. Prevention of diarrheal diseases and respiratory tract infections caused by bacteria can be prevented by using antibiotics, but excessive use of antibiotics can lead to side effects. So natural treatment is used, namely papaya flowers because papaya flowers contain saponins, tannins and flavanoid compounds. The purpose of this study was to determine the antibacterial inhibition of papaya flower extract at a concentration of 35%, 50%, 65% against Klebsiella sp and Shigella sp. This study is an experimental research using a quantitative descriptive test method. The sample of this study is papaya flower extract on the growth of Klebsiella sp and Shigella sp. bacteria. The test method used is sub-diffusion. The results showed that the concentration of 65% was the most effective concentration to inhibit the growth of Klebsiella sp and Shigella sp bacteria with a yield of 27.6 mm which was in the medium category. Papaya flower extract has antibacterial compounds (flavonoids, tannins and saponins) that are able to inhibit the growth of Klebsiella sp and Shigella sp bacteria.

Abstract

Mosques and prayer rooms are frequently visited places of worship for Muslim communities. Prayer activities commonly use prayer mats or carpets that directly cover the floor and may serve as a medium for microbial contamination, particularly fungi. Open ventilation systems and frequent access from outside environments allow various airborne pollutants, including fungal spores, to enter the worship area. This study aimed to determine the presence and types of fungal contaminants on carpets in places of worship and their relation to carpet maintenance practices such as sun-drying, washing, and vacuuming. This study used a descriptive analytic method with a population and sample of 20 worship places consisting of mosques and prayer rooms in Tapos Village. Fungal examination was conducted using Sabouraud Dextrose Agar (SDA) media, and the results were analyzed descriptively using percentage analysis. The results showed that 100% of worship carpet samples were contaminated with fungi. The identified fungal species included Candida spp (70%), Monilia sitophila (50%), Penicillium spp (45%), Mycelia sterilia (40%), Aspergillus niger (30%), Aspergillus flavus (30%), Fusarium spp (30%), Rhodotorula rubra (30%), Absidia spp (20%), Aspergillus fumigatus (15%), Aspergillus versicolor (5%), and Curvularia spp (5%). Carpet maintenance practices such as sun-drying, washing, and vacuuming were found to help reduce the spread of fungal contamination in places of worship

Abstract

Chronic Obstructive Pulmonary Disease (COPD) is the third leading cause of global mortality, characterised by persistent lung inflammation partly associated with reduced levels of histone deacetylase 2 (HDAC2). Since vitamin D may theoretically modulate HDAC2, this preliminary study aimed to evaluate the effect of vitamin D supplementation on HDAC2 levels in stable COPD patients at Jemursari Islamic Hospital, Surabaya. A one-group pre–post test experimental design without a control group was used. Five stable COPD patients meeting the inclusion criteria received 5000 IU of cholecalciferol daily for three months, with serum 25(OH)D₃ and HDAC2 measured before and after the intervention by ELISA and analysed using a paired t-test (p<0.05 considered significant). Supplementation significantly increased 25(OH)D₃ (7.63 ± 2.73 to 14.26 ± 2.43 ng/mL; p=0.023) and HDAC2 (1.50 ± 1.02 to 3.90 ± 1.39 ng/mL; p=0.006), whereas all pulmonary function parameters showed only an increasing trend without reaching statistical significance (all p>0.05). Vitamin D supplementation was associated with increased HDAC2, suggesting a potential molecular anti-inflammatory effect not yet accompanied by significant functional improvement. Given the very small sample (n=5) and the absence of a control group, these findings are hypothesis-generating and require confirmation in larger controlled trials before vitamin D can be considered as adjunctive COPD therapy.

Abstract

Dental and oral health problems in Indonesia have a prevalence of 57.6%. This condition is related to the activity of microorganisms in the oral cavity, which can develop due to poor oral hygiene. The use of orthodontic appliances such as braces can increase the risk of plaque buildup and changes in the oral microbiota because the appliance components are difficult to clean. One of the bacteria potentially found in these conditions is Pseudomonas aeruginosa, an opportunistic gram-negative bacterium capable of forming biofilms. The purpose of this study was to identify the presence of Pseudomonas aeruginosa in individuals using fixed orthodontic appliances. This study used a descriptive observational design with a cross-sectional approach. Sampling was carried out by swabbing the orthodontic users' braces using a purposive sampling technique. Inclusion criteria were wearing braces for at least 4 weeks, brushing their teeth at least twice a day, and having no cavities. The samples were then cultured on BHI media, followed by cultivation on MacConkey media, Gram staining, and identification using biochemical tests. The results of a study of 12 samples showed the presence of Pseudomonas aeruginosa bacteria in 25% of the samples. Klebsiella pneumoniae was also found in 50% of the samples, while the remaining samples contained bacteria from the Enterobacteriaceae group. These findings indicate that various types of Gram-negative bacteria can colonize the surface of orthodontic appliances in braces users. Based on the results of the study, it can be concluded that Pseudomonas aeruginosa bacteria were found in 25% of braces users.

Abstract

The use of fixed orthodontic appliances (braces) contributes to increased plaque and debris accumulation on the tooth surface, which may disrupt the oral microbial balance and promote the proliferation of cariogenic bacteria, including Streptococcus mutans. This study was conducted to identify Streptococcus mutans in individuals wearing braces. The research employed a descriptive observational design with a cross-sectional approach by means of purposive sampling. Samples consisted of swabs from the braces of 14 participants, including 12 users, 1 positive control, and 1 negative control. Bacterial identification was carried out through cultivation in Kaldu Peptone Darah (KPD), isolation on Blood Agar Plates (BAP), gram staining, and biochemical testing. Data were analyzed descriptively. The results showed growth in all samples on KPD, with Streptococcus species dominating at 66.7%. Colony morphology, gram-positive cocci staining, negative catalase test, and carbohydrate fermentation profiles indicated that the isolates belonged to the Streptococcus viridans group. Therefore, it can be concluded that individuals wearing braces harbor Streptococcus species classified within the viridans group.

Abstract

Tuberculosis (TB) is one of the infectious diseases with a high mortality rate worldwide, including in Indonesia. Acid-Fast Bacilli (AFB) microscopic examination using the Ziehl-Neelsen method remains a widely used diagnostic method for TB. One of the crucial stages in Ziehl-Neelsen staining is the decolorization process using an acid-alcohol solution. Variations in the pH of the decolorizing solution are suspected to affect the quality of AFB staining results. This study aimed to determine the effect of acid-alcohol pH variations on the quality of AFB staining using the Ziehl-Neelsen method. The study utilized a laboratory experimental analytical design with acid-alcohol pH variations of 0.5, 1.0, 2.0, and 3.0. The research sample consisted of 24 smears, which were observed based on the microscopic quality of the AFB staining results. Data were analyzed using the Kruskal-Wallis test, followed by a post hoc pairwise comparison test using the Bonferroni correction. The Kruskal-Wallis test results showed a significant effect of acid-alcohol pH variation on the quality of AFB staining (p < 0.05). Post hoc test results indicated significant differences between pH 0.5 and pH 2.0 (p = 0.038), pH 0.5 and pH 1.0 (p < 0.001), as well as pH 3.0 and pH 1.0 (p = 0.013). The pH 1.0 group had the highest mean rank of 21.50, indicating the best staining quality in accordance with the AFB examination quality standards set by the Ministry of Health of the Republic of Indonesia. Therefore, variations in acid-alcohol pH affect the quality of Ziehl-Neelsen AFB staining, and pH 1.0 is recommended as the optimal pH for the decolorizing solution

Abstract

Staphylococcus aureus is a pathogenic bacterium that can cause various infections in humans and has the potential to develop antibiotic resistance. The utilization of medicinal plants as natural antibacterial agents has gained increasing attention, including nutmeg (Myristica fragrans Houtt.), which is known to contain various bioactive compounds. This study aimed to compare the antibacterial activity of nutmeg leaf, seed, and mace infusions against Staphylococcus aureus. The study was conducted using an experimental laboratory design with the agar well diffusion method. Infusions were prepared using a 1:10 (w/v) ratio of simplicia to distilled water and heated at 90°C for 15 minutes. A suspension of S. aureus was inoculated onto Mueller Hinton Agar (MHA), followed by antibacterial activity testing using leaf, seed, and mace infusions. The inhibition zone diameters were measured after incubation at 37°C for 24 hours. The results showed that nutmeg leaf infusion produced an average inhibition zone diameter of 13.67 ± 1.53 mm, whereas seed and mace infusions did not produce any inhibition zones. These findings indicate that only nutmeg leaf infusion exhibited antibacterial activity against S. aureus under the conditions of this study. Therefore, nutmeg leaves have the potential to be developed as a natural antibacterial agent that can be easily applied through a simple infusion method. Further studies are recommended to evaluate the phytochemical profile, Minimum Inhibitory Concentration (MIC), and Minimum Bactericidal Concentration (MBC) to support its development as an antibacterial agent.
