Abstract

Tuberculosis is one of the biggest health problems in the world. This disease is also the highest cause of death and morbidity in developing countries, such as Indonesia. Most nurses do not carry out treatment plans for pulmonary tuberculosis patients because there is no standard structure even though there are guidelines for nursing management of pulmonary TB patients. The purpose of this study was to determine nursing diagnoses that are often enforced by nurses in pulmonary tuberculosis patients. The study was conducted in the inpatient hospital. This study used descriptive observational and descriptive explorative approaches. The samples were the documentation of the TB patients by using nonprobability sampling, namely purposive sampling saturation. Data analysis used univariate analysis with frequency distribution. The results of the study show that the formulation of nursing diagnosis is ineffective airway clearance(52%), the writing of 1 (one) diagnosis of ineffective airway clearance into 13 different diagnostic names. The nurse wrote the diagnosis of ineffective breathing patterns into 7 forms of diagnosis. Nurses also wrote diagnosis of acute pain into 5 forms of nursing diagnosis, and hyperthermy diagnosis into 2 forms of nursing diagnosis. The research recommendation is that hospital management is expected to improve nurses' skills in writing the latest nursing diagnoses, monitoring and evaluating both qualitatively and quantitatively  in the writing of nursing diagnoses, applying the latest standards of nursing diagnoses. Nurses are expected to apply the latest nursing diagnoses in accordance with the use of NANDA.

Abstract

Motivation is not always obtained from a leader, but a nurse is able to be a leader in him/herself and in terms of caring for patients. This is very different from what happens in the field. Based on the findings in the field, it was found that there were still many leadership styles that did not focus on motivation in nurses' performance which had an impact on health services. Data supporting based on research conducted by the Directorate of Nursing at the Ministry of Health of the Republic of Indonesia found that 70% of nurses during the last 3 years had never attended training, 39.8% were still doing cleaning tasks, 47.4% nurses and midwives did not have job descriptions and not developed monitoring and performance evaluation of nurses and midwives especially regarding skills, attitudes, discipline and work motivation. Based on the results of research from several journals, namely motivation that gives influence to the performance of nurses, especially in terms of employment, shows that there is similarity in the terms of leadership knowledge relations with low nurse motivation in terms of assignments and training. Abstract

Workload is a mirror of nursing actions that can be carried out in quantity and quality by a nurse to a person or group of patients who are his/her responsibility. Routine questions that often arise are which patients are treated by which nurse, how many patients can be treated, whether the nurse's burden is maximum or optimum. This study aims to analyze the correlation between nurses' workload and caring behavior at the surgical ward. The design in this study was descriptive correlative with a cross-sectional approach. The population in this study were all nurses of the surgical ward at Ungaran Hospital as many as 30 nurses. Data collection was done by using questionnaires and forms. The data were taken by using purposive sampling technique, the results were analyzed descriptively and statistical tests used  Chi-Square test. The results show nurses with light workload as many as 17 people (56.6%), while those with moderate workload as many as 8 people (26.6%) and those with a heavy workload as many as5 people (16.6%). Nurses with good caring behavior as many as 22 people (73.3%) and nurses with poor caring behavior as many as 8 people (26.7%). From the results of statistical tests with Chi-square test show there is  no correlation between nurse workload and caring behavior, it is known that x2 count = 0.191 <x2 table = 5.991 or based on the probability Asymp-Sig = 0.909> 0.05 (α) then H0 failed to be rejected, so it can be concluded that there is no correlation between nurse workload and caring behavior.

Abstract

Nurse retention is a process that refers to the ability of an organization to maintain nurses within a maximum period of time. This hospital has a high nurse retention rate with a turnover rate of 7% in the past year. This study aims to determine the description of nurse retention. This research was a quantitative descriptive study with a survey approach. The research sample consisted of 110 executing nurses of a hospital in Semarang. The research instrument used a questionnaire with 18 questions. The results show that nurse retention on the autonomy and freedom dimensions are considered good by 92.7% nurses, the dimensions of work compatibility factors is considered good by 84.5% nurses, the dimensions of work support and recognition are considered good by 76.4% nurses, compensation dimensions and career growth are considered good by 75.5% nurses, and organizational culture dimensions are considered good by 51.8% nurses. It is suggested that the hospital to be more open to receive suggestions for improving services in order to increase nurse retention.

Abstract

The implementation of supervising the head of a room in one hospital in Semarang has not been optimal due to the absence of a schedule, assessment instruments, guidance, documentation of supervision results, and standard operating procedures (SOP) supervision. Supervision activities are incidental in accordance with needs and have not been implemented in a structured and well-documented manner. The writing of this article aims to find out the description of the implementation of head supervision in a hospital in Semarang. This study uses descriptive research design. The subjects in this study were all heads of inpatient rooms. The object of this research is the implementation of the supervision of the head of the room according to the SOP and the results of documentation of the implementation of supervision. The instrument in this study used a draft sheet for evaluation of the superficial room leader evaluation. Shows that there is a change in the implementation of supervision based on the SOP before and after the dissemination of supervision is carried out to the head of the room. Documented supervision results can help the head of the room to see the extent of the ability of staff and can jointly improve capabilities, correct errors in improving the quality of nursing care services. The implementation of supervising the head of a room in one of the Semarang hospitals needs to be improved, especially in terms of post-supervision documentation, development of thematic supervision themes, and structured supervision scheduling.

Abstract

The patient's condition and the environment of the Emergency Installation (ER) that are unique can affect the psychological condition of the patient's family. This causes the emergence of psychological problems in the patient's family, such as anxiety, stress, and depression, these problems can affect the role of the family as a supporting system of patients in the process of handling actions while in the ER. These problems can be minimized by fulfilling family needs while in the ER to accompany the patient. The purpose of this study is to find out the description of the patient's family needs in the emergency room. This research is a descriptive quantitative study with a survey approach, with the research instrument in the form of a CCFNI-ED questionnaire. Respondents numbered 96 patient families obtained through convenience sampling techniques. The results of this study indicate that family needs which according to the family are important to fulfill are communication needs followed by understandable needs, support needs, comfort needs and needs close to the patient. Communication needs are considered the most important needs needed by the patient's family while in the emergency room. This research is expected to be a source of reference for the nursing profession in meeting family needs while in the emergency room, so it is expected to increase the family's role in healing patients.

Abstract

Sharia is the standard quality indicators in Islamic hospitals consisting of indicators of minimum service standard (SPM) Quality Indicators and the compulsory Islamic Sharia. Indicators of the quality of the Sharia has a purpose in an effort to standardize the measurement and documentation of application of Islamic values in the service of the patient. Indicators Quality Syariah consists of 8 quality indicators Sharia SPM which consist of reading basmalah in administering drugs and action, hijab mandatory training for patients, for patients, educational fqih Islami, ECG examination according to gender, the wearing of a hijab nursing mothers , the wearing of hijab in the operating room and scheduling operations outside of prayer time. Indicators of the quality of the sharia is another indicator of the quality of compulsory Shariah which consists of mentoring talqin patients breathe death, reminiscent of the time of prayers and the installation of appropriate gender DC. This research aims to know the description of the implementation of the Sharia quality indicators conducted by nurses in inpatient PDHI Yogyakarta Islamic hospital which reflects the implementation of spiritual services to the patient.  This research uses descriptive quantitative research design. The population in this study are all the nurses in inpatient care. The respondents of the research as much as 63 respondents obtained with the Total Sampling techniques by observing the exclusion criteria in research. Instrument research using questionnaires. Analysis of univariate data by using a percentage. The results showed of 63 respondents, a total of 6.3% of the respondents the implementation of Sharia in the category quality indicators are lacking, 38.1% of respondents in the category is good enough and as much as 55, 6% in the category either. The conclusion that can be drawn from this study is the implementation of a quality indicator of the Sharia Islamic in PDHI Yogyakarta Hospital in the category of good.

Abstract

A handover is an application form for improving effective communication which is part of the patient safety goal. Ineffective handover has been shown to increase the risk of preventable side effects, length of stay and rates of complications. High patient mobilization, a lot of time taken, and increased workload are reasons why nurses do not have time for handover. While the ER Nurses are also required to develop, improve their knowledge and skills to improve quality services in line with the technological development of the era industrial revolution 4.0. The objective of this review to identify efforts for improving quality of handover in Emergency Departement in accordance with the development of the industrial revolution era 4.0. This review source search through CINAHL Ebsco, Science Direct, and pubmed from 2010 to 2019, in English, and full text. Searching keywords are Improvemenr, Quality Handover, Technology, and Emergency Department. From this literature review, it was found that efforts to improve quality of handover in the emergency departement included the establishment of appropriate communication / protocol tools / frameworks, developing multi-disciplinary teams, recognizing the influence of culture and implementing training. These efforts cannot be separated, all are interrelated to realize quality handover. Facing the digital revolution 4.0 based on Cyber Physical Systems in the health sector, improving quality handover may use electronic communication framework. Materials handover is using patient data in the medical record (RM) or electronic medical record (ERM). Electronic Handover tool aswer the challenges for facing the digital revolution 4.0. Efforts to improve the quality of handover in the emergency department are established effective communication tools / frameworks, Multidicipline Team meetings, training / coaching and recognizing cultural influences.

Abstract

The workload in the room is not always a cause of stress on nurses, the workload will be a source of stress if the number of workloads is not proportional to the ability of both the physical or expertise and the time available to nurses. Besides, nurses are faced with patients who trigger an increasing stress on nurses. This research was conducted at Siaga Medika General Hospital in Pemalang. The research design used quantitative associative analytic through cross-sectional approach. The sample of this study was 45 nurses who served in the Intensive Care Unit and Emergency Departments because the nurses' stress levels were higher than the nurses’ stress levels in the other treatment rooms (ward) obtained through total sampling because the population was less than 100, so that all population used as research samples. The Work Stress research instrument used the standard Depression Anxiety Stress Scale questionnaire, while the workload questionnaire adopted the previous research and was modified. The results showed that there was a correlation between workload and stress level of Intensive Care Unit nurses and stress of Emergency Departments nurses. This showed that nurses who have a heavy workload that is not comparable to the duties of nurses with the average number of patients, the number of types of work that must be done. If this is not managed properly, it will cause increased stress.H This is what becomes the workload of Intensive Care Unit and Emergency Departments nurses in Siaga Medika General Hospital in Pemalang which can cause an increase of stress.

Abstract

The use of the Surgical Safety Checklist reduces mortality and surgical complication. Nurse obedience in documenting the surgical safety checklist is still low. It is showed that there is a lack of nurses awareness about the importance of surgical safety checklist in an operation. This research aims to know about the relationship between the years of service of the nurse and compliance with filling surgical safety checklist at Central Operating Theatre. Design of this study uses cross-sectional approach. Sampling technique uses total sampling technique, samples taken for study as many as 23 respondents who met the inclusion and exclusion criteria. The data are described in table form and a Fisher test was conducted with spss 21. The highest work period are they who have light of work ≥ 3 years as many as 15 respondent (65,2%) and respondents who have length of work < 3 years as many as 8 respondent (34,8%), for nurses obedience  18 respondents (78,3%) who did not complete the surgical safety checklist while 5 respondents (21,7%) complete the surgical safety checklist correctly. Fisher statistic test obtained p-value data is 0,586 > 0,05. There is no relationship between the years of service of the nurse and compliance with filling surgical safety checklist at Central Operating Theatre
