Abstract

Antepartum depression frequently precedes postpartum depression. Postpartum depression entails a direct impact on the laboring mother and is a long-term risk to the mother's mental health, therefore it is essential to screen and detect the risk of postpartum depression since pregnancy. This study aimed to analyze the factors influencing pregnancy depression descriptively in the Pekalongan Regency Health Office. This research is a quantitative descriptive study using a cross-sectional approach. The population in this study were pregnant women in the Community Health Center in the working area of the Pekalongan Regency Health Office. The sampling technique used is proportional random sampling. The collection of data on the characteristics of pregnant women used a demographic questionnaire, while depression experienced by pregnant women used the EPDS (Edinburg Prenatal Depression Scale) questionnaire. The results of the study showed that most respondents (75.9%) did not experience pregnancy depression, however, 24.1% experienced pregnancy depression, and those who experienced pregnancy depression occurred among pregnant women of reproductive age, with secondary education or above, unemployed respondents, multigravida pregnancies, have never experienced an abortion, and some respondents were depressed by unwanted and unplanned pregnancies, as well as were not supported by their spouses. Thus, special services for pregnancy as an effort to prevent and take action for screening/early detection of signs of pregnancy depression are required and necessary to be procured.

Abstract

Endometriosis is a gynecological condition characterized by the development of endometrial tissue outside the uterus, often leading to pain and infertility. We explore the relationship between endometriosis and the effects of DNA methylation on the Epidermal Growth Factor Receptor (EGFR) gene. DNA methylation, an epigenetic mechanism, involves adding a methyl group to cytosine bases followed by guanine in CpG islands, thereby influencing gene expression through hypermethylation or hypomethylation. In endometriosis, methylation patterns on specific genes can lead to transcriptional changes, impacting inflammatory processes and hormonal functions, such as estrogen, that support the growth of ectopic tissue. Variations in the EGFR gene's DNA methylation are linked to elevated cellular activity and expression, which aids in the pathophysiology of endometriosis. These findings highlight the potential of DNA methylation as a therapeutic target in treating endometriosis, offering hope for improved patient outcomes.

Abstract

Immunity plays an important role in many areas, including dental health. Dental caries is the main and most common problem in dental and oral health fields. Dental caries was influenced by 4 important factors, among them was a bacterial infection. The bacterial infection can cause inflammation of the pulp. Chronic inflammation pulpitis can stimulate pulp polyps or hyperplastic pulpitis. This study aimed to identify the role of innate immune response to hyperplastic pulpitis process. Bacteria Streptococcus mutans was the most prevalent bacteria in dental caries. Its lipopolysaccharide could trigger an immune response by activating the cellular and humoral components in the pulp cavity. The cellular components include polymorphonuclear cells, monocyte and macrophage cells, dendritic cells, basophil cells, and mast cells. The humoral component was histamine and pro-inflammatory cytokines, such as IL-1β, IL-4, IL-6, and IL-12. The presence of bacterial infections or pathogens causes the body to stimulate defense mechanisms through innate immune cells and humoral components.

Abstract

A wound is a condition where body tissue is damaged as a result of sharp objects, chemicals, animal bites, electric shocks, and so on. Good and correct wound management can prevent wounds from becoming infected. In general, for infected wounds, aminoglycoside antibiotic ointments are used which are effective against gram-negative and gram-positive bacteria, such as gentamicin. Where the use of gentamicin ointment is still the "Drug of Choice" for infected wounds. However, due to several side effects that can be caused such as allergic reactions that will cause discomfort in patients with hypersensitivity to antibiotics in this group and the possibility of resistance to the use of this type of antibiotic, other alternative therapies have emerged for treating wounds with ingredients that have minimal side effects, one of which is by using traditional medicine from nature. One of the herbal medicines that is believed to be able to treat skin wounds is the Gotu Kola plant or Centella asiatica. The gotu kola plant or Centella asiatica contains several active substances that are useful in accelerating wound healing, namely Asiatic acid, Madecassocide, and Asiaticoside. This study aims to determine and assess the activity of these substances in Centella asiatica which play a role in the healing process of infected wounds in experimental animals. This research is a review article, by searching data from Google Scholar, PubMed, and NCBI databases with keywords used such as, "Effectiveness of Centella, Active Ingredients of Centella, Centella on Wounds". Many articles appear, but those used in this article that fit the inclusion criteria are 6 articles. The results of the 6 articles stated that Centella asiatica has the potential to heal infected wounds because it contains triterpenoid compounds which have antibacterial activity against Staphylococcus aureus and Escherichia colli. This review article concludes that the active substances found in pegagan leaves, namely Asiatic acid, Madecassocide, and Asiaticoside, have benefits in healing infected wounds in experimental animals.

Abstract

Post-stroke dysphagia (PSD) affects up to two-thirds of stroke patients, potentially leading to complications such as malnutrition, pneumonia, and increased mortality. This condition results from disrupted neural coordination in the swallowing process, with patients often experiencing severe symptoms. Current treatments combine compensatory strategies and rehabilitative therapies, including swallowing exercises. However, the effectiveness of these exercises remains inconclusive due to small sample sizes and short follow-up periods. This systematic review followed the PRISMA-P 2020 checklist, with literature sourced from PubMed, ScienceDirect, Sage Journals, Cochrane Library, and Proquest using the keywords "Swallowing Exercise" AND "Stroke" AND "Dysphagia.". Eight randomized controlled trials with a total of 278 participants focused on exercises such as head lift exercise (HLE), jaw opening exercise (JOE), oral neuromuscular training, kinesiology taping (KT), Chin Tuck Against Resistance (CTAR), respiratory muscle training (RMT), and community-based rehabilitation. All studies demonstrated improvements in swallowing function, but limitations included small sample sizes, short follow-up periods, and insufficient prior research. This review concludes that swallowing exercises are effective for PSD; however, further research is needed to validate their long-term effectiveness.

Abstract

Candidiasis, caused by Candida species, remains a significant global health concern, with oral and vulvovaginal forms prevalent among immunocompromised populations and women, respectively. Increasing antifungal resistance, particularly among non-albicans species, underscores the need for novel therapeutic options. This systematic literature review assesses the antifungal efficacy of Morinda citrifolia (noni fruit) against Candida albicans. Utilizing the PRISMA protocol, we analyzed studies from Scopus, ScienceDirect, PubMed, and Google Scholar databases published between 2014 and 2024. Seven in vitro studies were included, examining the inhibitory effects of M. citrifolia extracts on C. albicans. Findings indicate that M. citrifolia exhibits significant antifungal properties, attributed to its bioactive compounds such as scopoletin and flavonoids, which disrupt fungal cell membranes. The effectiveness of M. citrifolia is concentration-dependent, with higher concentrations yielding greater antifungal activity. Given the increasing resistance to conventional antifungals, in vitro studies suggest that M. citrifolia may be a promising alternative, warranting further in vivo and clinical investigations to substantiate its therapeutic potential.

Abstract

Salbutamol is a bronchodilator commonly used in the management of asthma, both in acute and chronic cases. As part of the Short-Acting Beta-2 Agonist (SABA) class, salbutamol is effective in relieving bronchospasms. The purpose of this study was to determine the rationality of drug use at the Merdeka Community Health Center Palembang. A drug utilization study was conducted at Merdeka Public Health Center Palembang in 2024. 96 patients meeting inclusion criteria. Drug use patterns were evaluated for dose rationalization, duration, frequency, contraindications, and interactions. Data analysis was performed using univariate methods. Sociodemographic characteristics showed that women were more prevalent than men (58.3%), with the most common age groups being pre-elderly and elderly (each 36.5%). All salbutamol doses (2–4 mg per administration) were appropriate (100%), with a duration of ≤3 days for all patients. The frequency of salbutamol administration of 3–4 times daily was found in 51% of cases, in line with recommendations. A total of 74% of patients received salbutamol with appropriate consideration of contraindications, while 26% received it despite contraindications, such as hypertension or hypertensive heart disease. Drug interactions were absent in 67.7% of cases, while antagonistic interactions, primarily with amlodipine, occurred in 17.3% of cases. The use of salbutamol at Merdeka Public Health Center Palembang mostly rational to drug use guidelines. However, attention is needed for contraindications and antagonistic interactions, especially with antihypertensive drugs. These findings can serve as a reference for improving asthma management and patient safety.

Abstract

Periodontitis is a complex disease that attacks periodontal tissue by progressive damage caused by various factors lead to inflammatory process. The progression of periodontitis is associated with bacterial activity that induces an inflammatory response in the host. Changes in the composition of bacteria within dental plaque leading to increased inflammation of the periodontal tissue which triggers the activation of inflammatory mediators from the host. Relative studies were identified and collected through case report and review article in Medline/Pubmed, Sciencedirect, Wiley Online Library and Google Scholar websites from 2011 until 2024. In addition to producing a chronic inflammatory process, periodontitis induces immune system dysregulation, which leads to microbial dysbiosis in the subgingival and submarginal locations. Both the humoral and cellular components of the innate and adaptive immune systems are plays crucial role for periodontitis. Periodontitis is a chronic disease and an advanced stage of gingivitis, when the immune system fails to counter bacterial invasion, the inflammatory process persists, and leading to extensive damage to periodontal tissues.
