Abstract

Cardiomyopathy in pregnancy, particularly peripartum cardiomyopathy (PPCM), is a rare but potentially life-threatening condition that poses serious risks to both maternal and fetal health. Its clinical manifestations, such as dyspnea, fatigue, and edema often resemble normal pregnancy symptoms, contributing to delayed diagnosis and treatment. This narrative review aims to summarize the etiology, diagnostic approach, and multidisciplinary management strategies for cardiomyopathy during pregnancy, focusing on PPCM, dilated cardiomyopathy (DCM), and other less common subtypes. Literature was obtained through a non-systematic search of PubMed, ScienceDirect, and Google Scholar for studies published between 2010 and 2025. Diagnosis is based on clinical evaluation, echocardiography, NT-proBNP, troponins, and exclusion of other causes. Management includes symptom control, prevention of arrhythmias and thromboembolism, and careful adaptation of standard heart failure therapy to the physiological changes of pregnancy. Medications such as beta-blockers, diuretics, and low molecular weight heparin are considered safe, while ACE inhibitors are contraindicated during gestation. The use of bromocriptine shows potential benefit in improving cardiac function but remains controversial due to the risk of thrombosis, requiring concurrent anticoagulation. Device therapy and mechanical support may be indicated in severe or refractory cases. Prognosis depends on cardiomyopathy subtype, timing of diagnosis, and left ventricular function recovery. Early recognition, close monitoring, and individualized care are essential to improve maternal and fetal outcomes. Further research is needed to guide safe and effective management strategies in pregnant patients with cardiomyopathy.

Abstract

Hypertension is a serious condition that increases the risk of cardiovascular diseases such as coronary heart disease, heart failure, stroke, kidney failure, and other complications. This study aimed to identify risk factors associated with hypertension in the working area of the Ohoijang Watdek Public Health Center. A quantitative analytic method with a cross-sectional design was used. A total of 127 respondents aged 19–64 years, selected through convenience sampling, participated in the study. Univariate analysis described respondents' characteristics, and chi-square tests examined associations between variables. Most respondents had stage 1 hypertension (32.3%), were aged 36–45 (29.9%), female (59.1%), non-smokers (62.2%), non-alcohol consumers (73.2%), had type 1 obesity (26%), and engaged in moderate physical activity (35.4%). Significant associations were found between hypertension and age (p=0.001; r=0.402), family history (p=0.028; r=0.195), smoking (p=0.014; r=0.217), alcohol consumption (p=0.012; r=0.224), and physical activity (p=0.002; r=0.320). Gender (p=0.256) and nutritional status (p=0.440) showed no significant relationship. The study concludes that age, family history, smoking, alcohol consumption, and physical activity are significantly associated with hypertension, while gender and nutritional status are not.

Abstract

Cleft lip and palate and intrauterine growth restriction (IUGR) are common congenital conditions and may suggest subtle genetic abnormalities. When no chromosomal defects are detected through standard testing, this combination presents a diagnostic challenge in prenatal care. . This case report presents a 33-year-old multigravida woman at 34 weeks of gestation, referred for suspected fetal growth restriction and cleft palate. Serial ultrasound examinations confirmed asymmetric IUGR along with a complete cleft involving the lip, alveolus, and palate. Elective cesarean delivery of pregnancy was performed at 36 weeks of gestation via cesarean section, with the infant diagnosed with unilateral cleft lip and palate. This case underlines the importance of considering advanced genetic testing in non-syndromic cases of CLP with IUGR. This case highlighting a gap in standard prenatal diagnostics and emphasizing the need for a broader genetic approach in such cases. Clinicians should remain alert and pursue early, comprehensive evaluation and multidisciplinary planning to ensure accurate diagnosis, timely intervention, and better neonatal outcomes.

Abstract

East Java records the highest percentage of child marriage in Indonesia, at 10.44%, with Malang Regency ranking first with 1,455 marriage dispensations. Early marriage commonly occurs among adolescents who still need parental support. This study focuses on parental characteristics, including age, education, socioeconomic status, and sociocultural background. This study aimed to examine the relationship between parental characteristics and adolescents’ knowledge and attitudes regarding the dangers of early marriage. The study uses an analytical design with a cross-sectional approach, involving 43 students and 43 of their parents. Data are collected through questionnaires and analyzed using the Chi-Square test with a significance level of 0.05. There was no significant relationship between parents’ education, socioeconomic status, or sociocultural background and adolescents’ knowledge and attitudes. However, parents’ age showed a significant relationship with adolescents’ knowledge (p = 0.015) and attitudes (p = 0.012). The age of parents significantly affects adolescents’ knowledge and attitudes toward early marriage at SMPN 2 Singosari, Malang Regency.

Abstract

The presence of a malignant pleural effusion (MPE) indicates advanced stage of malignancy, which severely compromises in a poor prognosis. LENT score has been developed as a prognostic tool for MPE, yet its association with survival has not been thoroughly evaluated in Indonesia. This study aims to examine the association between LENT score and 3-month survival in patients with MPE at Dr. Mohammad Hoesin General Hospital, Palembang. This prospective cohort study included 35 patients with MPE. Survival analysis using Kaplan-Meier curves. In this study cohort, the most frequent primary malignancy observed was lung cancer (62,8%). Kaplan-Meier analysis showed a median survival of 22 days for the high-risk LENT scores and 85 days for the moderate-risk group (p < 0,001). LENT score is significantly associated with three-month mortality in EPG patients. LENT score remains a robust tool for risk stratification in MPE management.

Abstract

Moringa oleifera Lam. is recognized for its versatile nutritional and medicinal benefits, largely attributed to its rich polyphenolic content, which includes bioactive compounds with significant antioxidant, anticancer, antimicrobial, and antidiabetic properties. This study addresses the gap in knowledge regarding the enhancement of phenolic compounds in Moringa leaves through innovative biotechnological methods. The primary objective is to analyze the extraction and quantification of polyphenols from Moringa leaves subjected to boiling and fermentation processes to produce kombucha. Using the Folin-Ciocalteu method, quantified the total polyphenol content, revealing that boiled Moringa leaves contained 67.66 ± 2.79 mg/g of polyphenols, while the resulting kombucha tea exhibited a higher concentration at 70.66 ± 2.79 mg/g. These findings demonstrate the potential of Moringa leaves as not only a dietary supplement but also as a functional ingredient in health-promoting beverages, such as kombucha. The implications extend to enhancing the nutritional value of plant-based diets and establishing Moringa as a sustainable antioxidant source. This research underscores the importance of advanced biotechnological approaches in optimizing extraction and cultivation methods, thus potentially increasing phenolic yields. By providing crucial insights into the functional applications of Moringa leaves, this study contributes to the broader fields of plant biotechnology and food science, encouraging further exploration into their health benefits and utility in promoting sustainable nutrition.

Abstract

Systemic lupus erythematosus (SLE) is a persistent autoimmune disorder marked by immunological dysregulation and widespread inflammation.  Tumor necrosis factor-α (TNF-α), a pivotal pro-inflammatory cytokine, has been associated with the pathophysiology of systemic lupus erythematosus (SLE), although its clinical relevance is still debated.  This study aims to assess the connection between serum TNF-α levels and disease activity, as quantified by the Mexican Systemic Lupus Erythematosus Disease Activity Index (MEX-SLEDAI), in Indonesian patients with systemic lupus erythematosus (SLE).  A cross-sectional study was performed including 34 patients exhibiting mild-to-moderate systemic lupus erythematosus activity (MEX-SLEDAI ≤12) at Dr. Mohammad Hoesin General Hospital in Palembang, Indonesia.  Individuals with significant active disease were excluded.  Serum TNF-α concentrations were quantified via a commercial ELISA kit, while disease activity was evaluated through MEX-SLEDAI.  Correlation analysis was conducted using Spearman’s rank test, with p < 0.05 being statistically significant.  The average age of participants was 33.6 ± 11.0 years, with 94.1% identifying as female.  The median MEX-SLEDAI score was 3 (range 2–7), and the median TNF-α level was 1.834 pg/mL (range 0.99–8.62).  No substantial connection was detected between serum TNF-α levels and disease activity (r = 0.111, p = 0.533).  This suggests that serum TNF-α levels did not correlate with or forecast clinical disease activity in this investigation.  The results indicate that the efficacy of TNF-α as a biomarker in SLE may be contingent upon contextual factors, especially illness severity.  Additional multicenter and longitudinal studies involving individuals with a broader spectrum of disease activity are necessary to elucidate its clinical significance.

Abstract

Systemic lupus erythematosus (SLE) is characterized by persistent inflammation and oxidative stress, which contribute to the progression of the disease.   Lipid peroxidation is indicated by malondialdehyde (MDA).   The objective of this investigation was to evaluate the correlation between serum MDA levels and disease activity in patients with mild to moderate systemic lupus erythematosus (SLE).   This investigation was a cross-sectional correlation analysis that employed baseline data from adult patients with systemic lupus erythematosus who were receiving conventional treatment.   Spearman's correlation was employed to determine the relationship between serum MDA levels and disease activity, which was evaluated using the MEX-SLEDAI score.   The study encompassed 38 patients with mild to moderate SLE.   The MEX-SLEDAI score was 3 (range 2–7), and the median serum MDA concentration was 1.42 µmol/L (range 0.90–1.99).   Spearman's correlation analysis demonstrated a moderately positive correlation between serum MDA levels and MEX-SLEDAI scores (r = 0.415, p = 0.010), indicating that elevated oxidative stress is associated with increased disease activity in SLE patients.   A considerable positive correlation between serum malondialdehyde levels and disease activity is statistically significant in patients with mild to moderate systemic lupus erythematosus (SLE).   These results support the existence of oxidative stress in the pathophysiology of lupus and suggest that MDA may serve as a potential biomarker for disease monitoring.

Abstract

Imminent preterm labor (IPL) is defined by irregular uterine contractions, back pain, and vaginal discharge, with approximately 25-30% of cases progressing to preterm delivery. Preterm birth remains a major global issue, contributing to 35% of neonatal deaths, and often results in long-term health complications for the infant. This case report discusses a 35-year-old woman (G4P2A1) at 30 weeks of gestation, with a history of type 2 diabetes mellitus (T2DM), bad obstetric history (BOH), and a right maxillary abscess. She presented with swelling and pus discharge from the maxillary region, along with abdominal cramps, but no signs of imminent labor. The patient’s diabetes was managed with insulin, and her clinical condition was complicated by high blood glucose and an ongoing infection. The cervical length was measured via ultrasound, which indicated a risk for preterm delivery, and the patient was diagnosed with IPL. Initial management included fluid resuscitation, tocolytics to prevent labor, and antibiotics for the maxillary abscess. The patient was closely monitored, and insulin therapy was adjusted to stabilize glucose levels. Despite the complexity of managing IPL in diabetic patients, the patient's condition improved after debridement of the abscess and management of blood glucose levels. This case emphasizes the importance of multidisciplinary care in managing complicated pregnancies with comorbidities like T2DM.

Abstract

Gynecologic cancers, particularly cervical, ovarian, and endometrial cancers, remain among the leading causes of mortality in women. Surgery, the cornerstone of treatment, is often associated with significant perioperative risks, necessitating careful evaluation and management to optimize outcomes. This review analyses perioperative and postoperative factors influencing morbidity and mortality in gynecologic oncology and evaluates the role of Enhanced Recovery After Surgery (ERAS) protocols in improving outcomes. A literature search by Google Scholar, PubMed, and ScienceDirect identified eight relevant evidence-based studies published within the last decade. Findings demonstrate that ERAS accelerates recovery, reduces complications, and lowers healthcare costs. Key components—such as patient education, multimodal analgesia, bowel function optimization, and early mobilizations significantly enhance postoperative recovery. Patients with comorbid endocrine, cardiovascular, renal, and electrolyte disorders require intensive monitoring and tailored management to prevent complications. Overall, the application of ERAS in gynecologic oncology improves recovery, reduces morbidity, and enhances quality of life, particularly when combined with vigilant monitoring in high-risk patients.
