Abstract

Stunting is a serious nutritional problem that adversely affects children's physical and cognitive development. This study aimed to compare the effectiveness of Art Therapy and Play Therapy in enhancing cognitive abilities of preschool-aged children with a history of stunting in the Lawe Sumur Public Health Center working area. A quasi-experimental design with a control group was employed involving 80 preschool children randomly assigned to either the Art Therapy group (n = 40) or the Play Therapy group (n = 40). The intervention consisted of 12 sessions over six weeks, and cognitive abilities were assessed using the Preschool Cognitive Ability Scale (SKKP-15) before and after the intervention. Results indicated that Art Therapy was more effective than Play Therapy in improving cognitive abilities. Children receiving Art Therapy were 3.4 times more likely to achieve high cognitive ability compared to those receiving Play Therapy (p = 0.02; OR = 3.444; 95% CI: 1.310–9.058). Family environment factors, including maternal education level, employment status, family size, frequency of home stimulation, and quality of sleep, also influenced cognitive development. Nonetheless, Art Therapy consistently produced positive effects, even for children from families with limited stimulation and lower parental education. In conclusion, Art Therapy is an effective, practical, and inclusive non-pharmacological intervention for supporting the cognitive development of preschool children with a history of stunting. These findings provide a basis for health professionals, educators, and policymakers to integrate arts-based approaches into child development intervention programs in community health settings.

Abstract

Anxiety is one of the psychological problems often experienced by patients in the Emergency Department (ED), where this condition can affect comfort, compliance with medical procedures, and hinder the treatment and recovery process. Managing anxiety in emergency patients cannot be done through a single intervention, but requires a comprehensive approach. Comprehensive interventions that combine elements of education, therapeutic communication, distraction therapy, and family support are considered more effective in helping patients manage anxiety while in the ED. Based on this, this study aims to evaluate the effectiveness of a comprehensive intervention model in reducing anxiety levels in emergency patients. The study used a quasi-experimental design with a pre-test and post-test approach and involved 100 emergency patients as samples. Anxiety levels were measured using the Self-Rating Anxiety Scale (SAS), and data analysis was performed using the Wilcoxon Signed Ranks Test. The results showed that before the intervention, most patients (63%) were in the moderate to severe anxiety category, and 15% experienced severe anxiety. After the comprehensive intervention, there was a shift in the distribution of anxiety, with 67% of patients in the mild to moderate anxiety category and 19% in a normal condition. Statistical test results showed a significant difference between anxiety scores before and after the intervention.

Abstract

A splint is one of the nursing interventions used to fixate or immobilize injured body parts, particularly in cases of fractures. The primary function of a splint is to stabilize bone fragments and prevent displacement that may worsen the patient’s condition. In fractures, pain arises not only from tissue, nerve, and vascular damage but also from the uncontrolled movement of bone fragments. This pain can affect the cardiovascular, endocrine, respiratory, and immune systems, and may trigger both physiological and psychological stress. Therefore, pain management is a crucial component of fracture care and can be addressed through pharmacological interventions such as analgesics, as well as non-pharmacological methods, including splinting. This study aimed to examine differences in pain scores among fracture patients before and after splint application. A quantitative design with a one-group pretest-posttest approach was employed. The sample consisted of 25 respondents selected using purposive sampling. Pain scale observations were conducted before and after the intervention, and data were analyzed using the Paired Sample T-Test with a significance level of 0.05. The results showed a probability value of p = 0.000 (p < 0.05), indicating a significant difference between pretest and posttest pain scores. It can be concluded that splint application has a significant effect in reducing pain levels among patients with closed fractures at Grandmed Hospital, Lubuk Pakam.

Abstract

Breast cancer remains one of the leading causes of mortality among women globally. Early detection through Breast Self-Examination (BSE) is an essential preventive measure to reduce the risk of late diagnosis. Nevertheless, the level of knowledge among adolescents regarding BSE is still relatively low, indicating the need for effective health promotion interventions to enhance understanding and foster positive attitudes toward early detection. Educational media-based health promotion is expected to improve adolescents’ awareness and encourage autonomous practices in performing BSE. This study aimed to evaluate the effectiveness of health promotion delivered through leaflet and animated video media in improving adolescents’ knowledge, attitudes, and practices regarding BSE in a junior high school setting. The research applied a pre-experimental design with a one-group pre-test–post-test approach. A total of 41 students from Trisakti Private Junior High School, Lubuk Pakam, participated as respondents. The intervention consisted of health education using leaflet and animated video media. Data were collected through questionnaires measuring knowledge, attitudes, and practices before and after the intervention. The Wilcoxon signed-rank test was employed to analyze the significance of differences between pre- and post-intervention scores. The findings revealed a statistically significant improvement across all measured variables, particularly in adolescents’ knowledge about BSE (p = 0.000). The mean knowledge score increased from 6.07 prior to the intervention to 7.44 afterward. Moreover, significant enhancements were also observed in attitudes and practices following the intervention. These results indicate that health promotion using leaflet and animated video media is effective in increasing adolescents’ knowledge, attitudes, and practices toward performing BSE. Continuous implementation of such educational programs is recommended to foster awareness and proactive health behavior among adolescents in early breast cancer detection.

Abstract

Patient satisfaction is a key indicator in assessing healthcare service quality and reflects the overall performance of a hospital. Low satisfaction levels may reduce patient visits and impact service sustainability. Several factors influence satisfaction, including healthcare workers’ attitudes, communication effectiveness, and service efficiency. In medical record services, outpatient registration often faces issues such as long queues, extended waiting times, and limited access to doctor schedules. To address these problems, BPJS Kesehatan introduced the Mobile JKN application, which allows online outpatient registration. However, its utilization at Grandmed Lubuk Pakam Hospital remains limited due to technical barriers, quota distribution issues, and insufficient socialization. This study employed an analytical quantitative approach with a cross-sectional design to evaluate the relationship between Mobile JKN utilization and patient satisfaction with waiting time. The study was conducted from December 2024 to May 2025 with a total population of 6,737 outpatients. A sample of 124 respondents was determined using the Stanley Lameshow formula and selected through purposive sampling. Data were collected using questionnaires and analyzed with the Chi-Square test at a 95% confidence level (α = 0.05). Findings revealed that 54.1% of respondents rated the online registration as good, and 52.4% reported satisfaction with waiting times. The Chi-Square test produced a p-value of 0.037, indicating a significant relationship between Mobile JKN usage and patient satisfaction. In conclusion, higher utilization of Mobile JKN is associated with greater patient satisfaction regarding outpatient waiting times.

Abstract

Background: National health development in Indonesia aims to improve public health through the active participation of various national components. Despite progress, challenges such as health disparities between regions and epidemiological transitions remain. Homecare physiotherapy, which provides care at the patient's home, has become an important solution, especially during the COVID-19 pandemic, which restricted social mobility. Research Objective: This study aims to analyze the effectiveness of homecare physiotherapy services in the Jakarta, Bogor, Tangerang, and Bekasi areas, as well as to explore the factors influencing productivity, quality, efficiency, flexibility, and patient satisfaction with these services. Research Method: This research uses a qualitative approach with a phenomenological design. Data was collected through in-depth interviews, direct observation, and documentation from physiotherapists, patients, and service managers. Data analysis was conducted using Grounded Theory and Analytical Hierarchy Process methods to identify priority policies. Research Findings: Homecare physiotherapy services have been proven effective in improving patients' physical conditions. Patients reported improvements after therapy, although challenges related to punctuality and patient readiness remain. Physiotherapists' productivity is high, despite external obstacles such as traffic congestion. Conclusion: Homecare physiotherapy services are effective in improving patients' conditions, but improvements are needed in time management, punctuality of visits, and schedule flexibility to enhance patient satisfaction.

Abstract

Background: The role of Posyandu cadres is crucial in improving the quality of health services for the elderly. The elderly are vulnerable to various health issues and require special attention, especially in areas such as Asahan Regency, where the role of cadres as companions, communicators, and motivators significantly impacts the quality of services. Elderly Posyandu aims to enhance the quality of life for the elderly through integrated health services, but challenges remain in increasing participation and service effectiveness. Research Objectives: This study aims to analyze the relationship between the role of Posyandu cadres and the quality of Posyandu services for the elderly in the working area of UPTD Puskesmas Bandar Pasir Mandoge, Asahan Regency. Research Methods: This research uses a quantitative design with a cross-sectional approach, involving 86 elderly participants selected through accidental sampling. Data were collected using questionnaires and analyzed using Chi-square tests to examine the relationship between the role of cadres and elderly satisfaction with Posyandu services. Results: The analysis shows a significant relationship between the role of Posyandu cadres and service quality. The p-values for the roles of cadres as companions, communicators, and motivators are all 0.000, indicating a significant positive impact on elderly satisfaction with Posyandu services. The better the role of the cadres in accompanying, connecting, and motivating, the higher the satisfaction of the elderly. Conclusion: The role of Posyandu cadres as companions, communicators, and motivators significantly affects the quality of Posyandu services for the elderly. Regular training to improve cadre capacity is essential to enhance the effectiveness of elderly health services in Asahan Regency.

Pneumococcal Conjugate Vaccine (PCV) immunization is a preventive measure, yet coverage in Serdang Bedagai Regency remains low. Factors such as knowledge, education, occupation, attitude, belief, health service accessibility, and family support are suspected to influence PCV utilization. Research Objectives: This study aimed to analyze the factors influencing PCV immunization utilization at Sei Rampah Health Center, Serdang Bedagai Regency. Research Methods: This research employed an analytic survey with a cross-sectional design. The population consisted of 900 mothers with children aged 11–12 months, and a sample of 262 mothers was selected using simple random sampling. Data were collected through questionnaires and secondary data from PWS-KIA. Univariable, bivariate (Chi-square), and multivariable analyses (multiple logistic regression) were performed to identify the dominant factors. Results: Bivariate analysis indicated that education (p=0.002; OR=6.882), occupation (p=0.000; OR=6.088), and knowledge (p=0.000; OR=15.400) were significantly associated with PCV utilization. Multiple logistic regression revealed knowledge as the dominant factor (p=0.000; Exp(B)=13.063), followed by occupation, attitude, belief, and family support. Age, health service accessibility, and education did not show a significant effect. Conclusion: Parental knowledge is the key factor influencing PCV immunization utilization. Efforts to increase immunization coverage should focus on continuous education by health workers and family involvement to improve parents’ awareness and compliance with child immunization schedules.
