Abstract

Background: Integrated Management of Childhood Illness (IMCI) is a key strategy in improving the quality of healthcare for sick children, with an approach that includes diagnosis, treatment, and disease prevention. The success of IMCI implementation is highly influenced by various factors related to healthcare workers, such as the availability of resources, training, and organizational support. Research Objectives: This study aims to identify the factors influencing the implementation of IMCI by healthcare workers in primary healthcare facilities, particularly at community health centers (puskesmas). Research Methods: This study employs a quantitative approach with a cross-sectional design. Data were collected through interviews and observations of healthcare workers at several puskesmas implementing IMCI. The analysis was conducted to determine the relationship between factors affecting the implementation of IMCI. Results: The study found that the most influential factors in the implementation of IMCI are the availability of resources, including medicines and medical equipment. Additionally, adequate training for healthcare workers and sufficient organizational support are crucial to ensure effective IMCI implementation. Other factors influencing the implementation include communication among healthcare workers and the involvement of families in the care process of sick children. Conclusion: The factors influencing IMCI implementation should be taken seriously, including enhancing the quality of training, fulfilling resource needs, and strengthening organizational support and communication among healthcare

Abstract

Background: The abuse of Narcotics, Psychotropic Substances, and Other Addictive Substances (NAPZA) in North Sumatra Province has become a serious issue. To address this, Regional Regulation Number 1 of 2019 was enacted, aiming to improve the rehabilitation of NAPZA victims. However, the implementation of this policy still faces challenges, such as ineffective intersectoral coordination, limited funding, and insufficient training for rehabilitation staff. These issues hinder the effectiveness of rehabilitation programs and the social reintegration of former NAPZA users. Research Objectives: This study aims to analyze the implementation of Regional Regulation Number 1 of 2019, identify the barriers in rehabilitating NAPZA victims in North Sumatra, and provide policy alternatives to address these barriers and improve the effectiveness of rehabilitation programs. Research Methodology: This research uses Grounded Theory with a qualitative approach. Data were collected through interviews with government officials, rehabilitation staff, and relevant committees. To determine priority policies, the researcher also employed the Analytical Hierarchy Process (AHP). Research Findings: The findings show that weak intersectoral coordination, limited funding, and insufficient training for rehabilitation staff are the main barriers. Additionally, social stigma towards former NAPZA users exacerbates their social reintegration. Conclusion: Enhancing intersectoral coordination, increasing funding, and providing continuous training for rehabilitation staff are necessary to improve the implementation of the regulation. The post-rehabilitation program, based on economic empowerment, must be strengthened to support better social reintegration.

Abstract

Background: Health Center Management is a series of planning, organizing, implementing and controlling processes to achieve targets/objectives effectively and efficiently. Objective: This study aims to analyze the relationship between Health Center management and service quality at Pon Village Health Center, Serdang Bedagai Regency. Research Method: This type of research is quantitative research in the form of an analytical survey with a cross-sectional design. Results: The results of the study showed that bivariately all variables, namely planning (P1), implementation (P2) and supervision (P3) had a significant value <0.05. Multivariately based on candidate selection, it was obtained that the three variables were eligible to be included in the multivariate model because the significant value was P <0.25. The final result of the logistic regression obtained that only 2 variables were included in the final regression model, namely implementation (p 0.003) and supervision (p 0.003). And the most related variable is supervision with Exp (B) 55.594. Conclusion: there is a relationship between health center management consisting of planning, implementation and supervision of the quality of service at the Health Center. It is expected that the Head of the Health Center must really carry out a series of management processes, starting from planning, because from the results of the study there were still respondents who answered that the planning was not good, as well as the implementation, even from the results of the study it was found that supervision was not running well at the Pon Village Health Center. For this reason, the Head of the Health Center must increase supervision routinely. So that the management process is really implemented, to achieve maximum service.

Abstract

Background: The Maternal Mortality Ratio (MMR) in Indonesia is still relatively high, with one of the causes being unintended pregnancies. One way to reduce MMR is by increasing the use of long-acting non-hormonal contraceptives (LA/NGCs), such as IUDs and MOWs. Factors influencing the selection of non-hormonal LA/NGCs include attitude, trust, location/distance, spousal support, and healthcare facilities. Objective: This study aims to analyze the factors influencing the selection of non-hormonal long-acting contraceptives (LA/NGCs) in Tanjung Balai District, Asahan Regency, focusing on attitude, trust, location/distance, and the role of healthcare workers. Methods: This study used a quantitative design with a cross-sectional approach. Data were collected through questionnaires distributed to 92 respondents, who were couples of reproductive age (PUS). The variables analyzed include attitude, trust, location/distance, spousal support, and healthcare facility services. The statistical test used was chi-square to examine the relationship between independent and dependent variables (the selection of non-hormonal LA/NGCs). Results: The results showed a significant relationship between attitude, trust, location/distance, and the role of healthcare workers with the selection of non-hormonal LA/NGCs (p < 0.05). Respondents with a positive attitude, trust in healthcare providers, easy access to healthcare facilities, and quality services were more likely to choose non-hormonal LA/NGCs. Conclusion: This study concludes that factors such as attitude, trust, location/distance, and the role of healthcare workers significantly influence the selection of non-hormonal long-acting contraceptives. Therefore, increasing knowledge and improving access to healthcare services is crucial to encourage the selection of these contraceptive methods

Abstract

Background: The Intensive Care Unit (ICU) plays a crucial role in the care of post-surgical neurosurgery patients, as patients with critical conditions require close monitoring. The changes in the consciousness level of post-neurosurgery patients are influenced by various factors, including age, gender, Glasgow Coma Scale (GCS), family support, and teamwork among medical staff. Objective: This study aims to analyze the factors influencing the changes in the consciousness level of post-neurosurgery patients in the ICU of Grandmed Lubuk Pakam Hospital in 2024. Research Method: This study uses a cross-sectional design with primary data collection through observation and direct measurement. The sample consists of 40 post-surgical neurosurgery patients treated in the ICU of Grandmed Lubuk Pakam Hospital. Data were analyzed using univariate, bivariate, and multivariate analysis, employing chi-square tests and logistic regression. Results: The results show that factors such as age, gender, GCS, family support, and teamwork significantly influence the consciousness level of patients. The GCS factor (?8) has the most dominant effect, with an Odds Ratio (Exp(B)) = 5.523, indicating that patients with a low GCS have 5 times the risk of experiencing a decrease in consciousness compared to patients with a higher GCS. Conclusion: Factors such as age, gender, GCS, family support, and teamwork influence the changes in the consciousness level of post-neurosurgery patients. Monitoring GCS and strengthening teamwork, along with family support, can accelerate the recovery of patients' consciousness.

Abstract

Background: Nurses' caring behavior significantly affects the quality of service and patient satisfaction. However, in Indonesia, the quality of nurses' caring behavior is still relatively low. High workloads and low work motivation can affect the quality of nurses' caring behavior. This study aims to analyze the relationship between work motivation and workload on nurses' caring behavior at Puskesmas Tigalingga in 2025.Objective: This study aims to determine the relationship between work motivation and workload on nurses' caring behavior at Puskesmas Tigalingga in 2025. Research Method: This study uses a quantitative design with a non-experimental descriptive correlation approach. The study was conducted at Puskesmas Tigalingga with a population of 57 nurses and midwives. The sampling technique used was Total Sampling. Data were collected using questionnaires and analyzed using Chi-Square tests and multiple logistic regression. Results: The results of the study indicate a significant relationship between work motivation and caring behavior (p-value = 0.003), as well as between workload and caring behavior (p-value = 0.003). Logistic regression analysis showed that workload has the greatest impact on caring behavior, with an Odds Ratio (OR) = 12.49. Conclusion: Managing workload effectively and increasing work motivation are crucial for improving nurses' caring behavior. Both factors play an important role in improving the quality of nursing services and patient satisfaction at Puskesmas Tigalingga.

Abstract

Background: The medication service at the Outpatient Pharmacy of RSUD Drs. H. Amri Tambunan experiences long waiting times, especially for BPJS guarantee prescriptions, which contradicts the established service time standards, i.e., dispensing medications ? 30 minutes and preparing compounded medications ? 60 minutes, in accordance with health service regulations. Research Objective: This study aims to analyze the waiting times for medication services at the Outpatient Pharmacy, identify types of waste within the service process flow, and identify factors that hinder service efficiency. Additionally, the study aims to provide recommendations for improvement to reduce waiting times and enhance service quality. Research Method: This qualitative study uses Value Stream Mapping (VSM) to analyze the medication service process and identify waste in each service stage. Additionally, a fishbone diagram is used to identify root causes affecting waiting times. Data is collected through interviews with pharmacy staff, the head of the pharmacy installation, and direct field observations. Research Results: This study found various types of waste in each stage of the medication service, including waiting, motion, defects, and overproduction. Factors hindering service time include patient congestion during peak hours, medication unavailability, and prescription inconsistencies with the formularium. Suboptimal use of information systems and a shortage of pharmacists were also identified as major hindrances. Conclusion: Improvements in process flow, coordination between departments, routine training, and the implementation of an integrated hospital management information system are needed to minimize waste and expedite service times at the Outpatient Pharmacy of RSUD Drs. H. Amri Tambunan.

Abstract

Background: RSUD Sultan Sulaiman in Serdang Bedagai Regency faces challenges in implementing the Hospital Information System (SIRS), particularly in the medical record unit. Although several departments have started implementing SIRS, manual record-keeping is still practiced in the medical record unit, leading to delays in patient data flow and a lack of coordination between units. Research Objectives: This study aims to analyze the implementation of SIRS at RSUD Sultan Sulaiman, focusing on the medical record unit, and to identify the factors that hinder the system's implementation. This research also provides recommendations to improve the efficiency of medical data management. Research Methodology: This research uses a qualitative approach with descriptive analysis methods. Data was collected through interviews with pharmacy officers, head of the pharmacy department, head of the medical record unit, and direct field observations. Thematic analysis was used to identify the factors hindering and benefiting the implementation of SIRS. Research Results: The implementation of SIRS has expedited administrative processes and improved coordination between units. However, challenges related to human resources, training, and inadequate hardware continue to hinder optimal implementation. Conclusion: Intensive efforts in training, provision of adequate hardware, and regular system evaluation are needed to support the optimal implementation of SIRS, which is expected to improve the efficiency of medical services at RSUD Sultan Sulaiman.

Abstract

Stunting is a chronic nutritional problem characterised by a child’s height being below the standard growth for their age, which impacts cognitive development, immunity, and future productivity. This study aims to analyse the influence of family financial literacy and household food security on the prevention of stunting among children under five in Lubuk Pakam District, Deli Serdang Regency, and to determine the most dominant factor. The study employed a quantitative design with a cross-sectional method. The population consisted of 130,417 families with children aged 6–59 months, with a sample of 256 respondents selected using multistage cluster sampling. Data were obtained through anthropometric measurements of children under five and questionnaires on financial literacy and food security, both of which had been validated and reliability tested. Data analysis was conducted using univariate, bivariate (chi-square test), and multivariate (logistic regression) methods. The results revealed that low financial literacy increased the risk of stunting by 2.18 times (OR = 2.18; p = 0.004), while low food security increased the risk by 3.12 times (OR = 3.12; p < 0.001). Mother’s education (p = 0.082) and family income (p = 0.162) had no significant effect. Multivariate analysis identified food security as the most dominant factor in preventing stunting, followed by financial literacy. In conclusion, strengthening household food security and improving financial literacy through education and household financial planning should be prioritised as strategies to reduce stunting prevalence among children under five.

Abstract

Adolescent malnutrition remains a major public health challenge in Indonesia. Accurate dietary assessment plays a critical role in understanding adolescents’ eating patterns and their implications for nutritional status. This study aimed to identify trends in dietary assessment methods used in adolescent research in Indonesia, focusing on method variation, data collection approaches, and validation of instruments. A narrative review was conducted by screening relevant articles according to inclusion criteria, resulting in 22 eligible studies. The findings show that the most frequently used method was the Food Frequency Questionnaire (FFQ) or Semi-Quantitative FFQ (SQ-FFQ) applied in 12 studies, followed by 24-hour dietary recall, either used alone or in combination. Other methods included food records, the Dietary Quality Index for Adolescent (DQI-A), and the Dietary Diversity Score (DDS). FFQ was reported as practical and cost-effective, although most studies did not mention instrument validation. In contrast, the 24-hour dietary recall provided detailed dietary intake but required more resources and was limited by respondents’ recall ability. Combining methods, such as 24-hour recall with FFQ or food record, improved validity but increased complexity. In conclusion, dietary assessment methods in adolescent studies in Indonesia are predominantly based on FFQ and 24-hour recall, with limited evidence of validation, highlighting the need for greater methodological rigor to ensure accurate dietary assessment among adolescent.
