Abstract

Background: Penetrating facial injury can cause injury to the bone and soft tissue surrounding. Penetrating facial injury caused by organic foreign bodies or metal material increases the risk of infection in the wound. Identification of the location of the entry of foreign bodies, whether the vital organs, the presence of injury to the nerves, vascular, visual, and other muscle damage is very important to do. For that immediate and optimal treatment can be applied to reduce morbidity and mortality. Case presentation: A 13 years old boy arrived in ER admission with penetrating facial injuries. A corpus alienum penetrated the patient’s face at the medial meatus of the right eye along with bleeding from the nose and around the wound. Evaluation and management of the penetrating facial injury were performed in accordance with the ATLS protocol. The aim of management in penetrating facial injuries is to remove the foreign body with minimal trauma to surrounding structures and to maintain normal function and optimal appearance. The patient underwent emergency surgery the remove corpus alienum and wound exploration under general anesthesia. Postoperative evaluation shows no bleeding or signs of infection, no visual impairment, and tear flow were found. Conclusion: Penetrating facial injury due to motorcycle gear shifter lever is rare. Good Management of penetrating facial injury will give satisfactory results.

Abstract

Background. Impaired fetal growth is still a major problem in obstetrics. Birth weight is usually used as an indicator of the well-being of the newborn and as an indirect measure of intrauterine nutrition during pregnancy and the mother's nutritional status during pregnancy. Magnesium contribute as a cofactor for various enzymes in the human body for metabolic processes. Fetal magnesium is obtained from maternal by transport through the placenta. In conditions of low magnesium levels will affect the epigenetic process, HPA-axis and GH-IGF axis of the fetus which in turn will result in fetal growth disorders. This study aims to determine the differences postdelivery serum magnesium levels of women at term pregnancy who give birth with low vs normal birth weight. Methods. This study used an observational analytic method with a cross sectional approach. Conducted from April 2020-December 2020 at Dr. M. Djamil Padang Central General Hospital and Network Hospital Faculty of Medicine, Andalas University. Total of 44 samples were divided into 2 groups, women who gave birth with low and normal birth weight babies. Samples was carried out at the Pramitha Padang Private Clinical Laboratory. Results: The mean maternal serum magnesium level in low birth weight group was 1.97 ± 0.16 mg/dL lower than normal birth weight group was 2.06 ± 0.14 mg/dL. Statistic test using the independent sample T test showed no differences in both groups (p = 0.064). Conclusion. The mean maternal serum magnesium level in low birth weight 1.97±0.16 mg/dL. The mean maternal serum magnesium level in normal birth weight was 2.06 ± 0.14 mg/dL. There was no significant difference on both groups.

Abstract

Background. Head injury is still a major health problem that is a common cause of disability and death. Moderate head injuries are quite common with outcomes varying patient examination of neutrophils and lymphocytes is quite routinely carried out in health services and the combination of these two values has a stronger predictive value for assessing outcomes in patients because it describes an inflammatory reaction. The GOS is a commonly used scale for classifying patient outcomes. This study aimed to determine the relationship between the ratio of neutrophil-lymphocytes (RNL) to the Glascow outcome scale (GOS) in moderate head injury patients with intracerebral hemorrhage. Methods. This study has a cross-sectional with samples from moderate head injury patients with CT scan images of intracerebral hemorrhage in Dr. M. Djamil Padang in 2021 and 2022. The data comes from medical records and observations of samples that meet the inclusion and exclusion criteria, 36 subjects participated in this study. Results. Most of the subjects (61.1%) were male. A total of 55.6% of subjects had RNL ≥ 7.35 with 44.4% of subjects having poor GOS. In this study, a statistically significant relationship was found between the ratio of neutrophil- lymphocytes in patients with moderate head injury with intracerebral hemorrhage and the Glasgow outcome scale (p-value = 0.005). Conclusion. RNL can predict outcomes in moderate head injury patients with intracerebral hemorrhage.

Abstract

Background: Root canal treatment is performed to eliminate the infection in the pulp tissue. One of the critical steps in root canal treatment is irrigation, which aims to clean debris and smear layer. This study aims to compare the effectiveness of star fruit extract (Averrhoa bilimbi Linn) at concentrations of 5%, 10%, 15%, and 20% with EDTA 17% in cleaning the smear layer on the one-third of apical root canals. Methods: This type of research is an experimental in vitro laboratory with a post-test only control group design. The sample of this study consisted of 25 premolars which were randomly divided into five groups. The teeth were instrumented with a K-file and irrigated with the following materials: groups I, II, III, and IV using bilimbi leaves extract, each with concentrations of 5%, 10%, 15%, and 20% and group V (positive control). ) using 17% EDTA. SEM (scanning electron microscope) was used to measure the cleanliness score of the smear layer on the apical third of the root canal. This study showed that bilimbi leaves extract was able to clean the smear layer on the one-third of the apical root canal. Results: The results of the Kruskal-Wallis statistical test showed a significant difference (p<0.05) between all treatment groups. The highest average smear layer cleanliness score in the apical third of the root canals was EDTA 17%, while the lowest average smear layer cleanliness score was 5% bilimbi leaves extract. Conclusion: Bilimbi leaves extract with a concentration of 20% was considered more effective among the extract groups, and 17% EDTA was the most effective in cleaning the smear layer on one-third of the root canals.

Abstract

Backgrounds. A skin graft is one of the routine surgical procedures performed. This procedure gives excellent results when done as early as possible after the trauma. Caffeine has a mechanism as an adenosine-receptor A2 antagonist which can induce wound healing through increased angiogenesis. This study aimed to determine the efficacy of coffee caffeine in the initiation of full thickness skin graft autologous wound healing by assessing the number of macrophages and lymphocytes in Sprague-Dawley rats. Methods. The research design is an experimental study with a post-test-only approach with a control group design. Twenty male Sprague Dawley rats aged 15 weeks (140 – 150 grams) were randomly divided into four groups. One group became the control group (decaffeinated) while the other group received various doses of caffeine (3 mg/kgBW, 6 mg/kgBW, and 9 mg/kgBW). Autologous skin grafts were performed in all groups. HE examination was performed to confirm the number of macrophage cells in the tissue. Data analysis was carried out with the help of SPSS 25 software. Results. There were significant differences in the number of tissue macrophages in the four groups. The group that received 9 mg/kgBW of caffeine showed the highest number of macrophages compared to the other groups. Based on the number of lymphocytes in the peripheral blood, the group that received a dose of 6 mg/kg BW showed the highest number of lymphocytes compared to other groups. Conclusion. Caffeine showed the ability to initiate full thickness skin graft autologous wound healing by increasing the number of macrophages and lymphocytes in Sprague Dawley rats.

Abstract

Background: Metal ceramic restoration (MCR) is a gold standard due to its ability to withstand masticatory forces and aesthetics. However, it frequently appears to experience a fracture in the oral cavity. The ceramics in MCR consist of an opaque dentin and enamel layer, which contain silica to give the ceramic strength. Silica is an abundant material, mainly synthesized from rice husks. The study was aimed to investigate the effect of silica added to commercially opaque porcelain powder in increasing the flexural strength of MCR. Methods: The research sample was rectangular Co-Cr metal [(25±1) mm × (3±0.1) mm × (0.5 ± 0.05) mm], and opaque porcelain powder and dentin [(8±0.1) mm× (3±0.1) mm ×(1.1±0.1) mm]. White silica powder was synthesized from rice husk by sonication for 270 minutes, characterized by x-ray fluorescence (XRF). Silica was mixed with commercial opaque porcelain powder with various concentrations of 0, 0.5, and 1%, porcelain coating procedure on the centre of the surface metal, followed by sintering at 9500C. Results: Characterization with XRF showed that the elemental content of pure silica was 83.9%. The average flexural strength value with three-point bending in each sample is 109.67±12.163; 131.26±3.817; and 108.35±4.26. The results of statistical tests using One Way ANOVA stated a significant increase in the flexural strength of MCR (p<0.05) post added the rice husk silica to the commercial opaque porcelain powder. Conclusion: Adding 0.5% silica resulted in the optimal flexural strength of MCR.

Abstract

Background. Vesicouterine fistula (VUF) is a rare pathological communication between the uterus or cervix and the bladder. Youssef's syndrome is an atypical presentation of a patient with a vesicouterine fistula, characterized by a triad of cyclic hematuria, amenorrhea, and absence of urinary incontinence. Because of this atypical presentation, the patient may go undiagnosed and patient management will be delayed. Case presentation. A 39-year-old woman complained of hematuria. The patient underwent a second caesarean section in 2008 and a few weeks after the procedure the patient complained of leakage of urine from her vagina. Six months later the patient complained hematuria on her menstrual cycle and amenorrhea, with absence of urinary incontinence. Ultrasound examination found adhesions between the uterus and vagina, and irregular bladder mucosa. Patient underwent cystoscopy with endometriosis were suspected on vesicovaginal repair scars and a bladder biopsy was also performed with the results of no endometrial stroma and glands then followed by hysteroscopy. Hysteroscopy result showed a 20 mm diameter hole with irregular margins. Subsequently, the patient was treated with hysterorrhaphy laparotomy and fistula repair. Conclusion. One type of vesicouterine fistula is Youssef's syndrome, which is a complication following a lower segment caesarean section with undiagnosed bladder injury. The diagnosis can be confirmed by cystoscopy, intravenous pyelography, hysterosalpingogram, sonography, and other types of imaging tests. Management of patients with VUF can be conservative, medical, or surgical.

Abstract

Background. The mortality rate due to cardiovascular disease increases; one of the effects of hypercholesterolemia conditions, dyslipidemia, hypertriglyceridemia, and metabolic syndrome, not handled properly and thoroughly. This phenomenon becomes the factor that the researchers took this research topic. This study aims to determine okra (Abelmoschus esculentus L.) immersion water on an increase in high density lipoprotein (HDL) levels and decrease in LDL (low density lipoprotein) levels in high fat diet model Wistar rats. Methods. This experimental study used 30 Wistar male rats (Rattus norvegicus) divided into six groups, namely normal rats (P1), positive control (P2), standard (P3), dose 1 treatment groups (P4), dose 2 treatment groups (P5), and dose 2 treatment groups (P6). The data were collected by measuring HDL and LDL levels before and after receiving okra (Abelmoschus esculentus L.) immersion water. The data were analyzed by paired sample t-test, Wilcoxon test, ANOVA test and Kruskal-Wallis test. Results. This research showed a significant difference in the average level of HDL and LDL in all treatment groups (P4, P5, and P6) after induction of Abelmoschus esculentus L. in a high-fat diet rat model (p <0.05). Conclusion. It can be concluded that the administration of okra (Abelmoschus esculentus L.) immersion water could be used to increase HDL levels and decrease LDL levels in high fat diet model Wistar rats (p <0.05).

Abstract

Background. Syphilis management is based on the syphilis stage classification. The main treatment for syphilis is currently benzathine penicillin. However, patients allergic to penicillin antibiotics or who are not willing to be injected can be given doxycycline or erythromycin antibiotics. Case presentation. We present a case of a female with latent syphilis. Patient was previously treated with oral antibiotics, such as doxycycline for 30 days and followed by azithromycin for 30 consecutive days. Clinical laboratory examinations show increase of TPHA and VDRL, and the case was considered as unresponsive case. Later, the patients received benzathine penicillin G for three weeks and showed clinical improvement. Conclusion. For patient who refuse to be treated with penicillin injection, the alternative treatments are oral antibiotics. However, there is a reduced number of successful treatments for patients treated with oral antibiotics.

Abstract

Background. One of the bacteria that cause oral cavity infection is Staphylococcus aureus. This study aims to explore the antibacterial potential of red betel extract and siwak extract against Staphylococcus aureus bacteria, in vitro. Methods. This research is an in vitro experimental study to assess the diameter of the inhibition zone of red betel leaf extract and siwak against Staphylococcus aureus. The extract was made from red betel leaf simplicia and siwak wood with a dose of 50% each. The bacterial suspension was made by taking one pure colony from a test tube. Data analysis was performed using SPSS version 25 software. Results. Red betel leaf extract 50% showed the largest diameter of the inhibition zone, namely 9.1 ± 0.68 mm. Meanwhile, the siwak extract showed a smaller inhibition zone diameter of 7.9 ± 0.68 mm. One-way ANOVA analysis showed that there was a significant difference between the diameters of the inhibition zones of each test group. Conclusion. Red betel leaf extracts are 50% greater in inhibiting the growth of Staphylococcus aureus than 50% siwak extract.
