Abstract

IHI Power Service Indonesia merupakan perusahaan yang bergerak di bidang fabrikasi baja yang mengutamakan Keselamatan dan Kesehatan Kerja (K3) dalam setiap aktivitas pekerjaannya. Berdasarkan data dan observasi lapangan, telah terjadi 8 (delapan) kasus kecelakaan kerja pada tahun 2023 di area welding PT IHI Power Service Indonesia. Maka dari itu penelitian dilakukan bertujuan untuk mengidentifikasi bahaya atau resiko yang ada di area welding PT. IHI Power Service Indonesia dan seberapa besar tingkat bahaya tersebut. Guna mendukung aspek Keselamatan dan Kesehatan Kerja (K3) maka penelitian dilakukan dengan menggunakan Metode Hazard Identification, Risk Assessment and Risk Contol (HIRARC) dan Job Safety Analysis (JSA) di area welding. Hasil penelitian menunjukkan bahwa pada proses welding, teridentifikasi 22 potensial hazard, sedangkan kategori risiko terbanyak yaitu low risk sebanyak 11 dan kategori risiko tertinggi yaitu extreme risk sebanyak 2 hazard. Dua kategori lainnya yitu medium risk sebanyak 7 dan high risk sebanyak 2 hazard. Adapun pengendalian yang disarankan untuk mengurangi potensi bahaya yang beresiko sangat tinggi yaitu melakukan pemeriksaan berkala terhadap alat angkat beserta seluruh komponennya dan pemeriksaan terhadap kesesuaian proses pengangkatan dengan prosedur. Serta pemasangan rambu peringatan keselamatan yang mudah dibaca dan mudah dipahami untuk membantu para pekerja agar mudah memahami maksud dan larangan atau peringatan dari simbol rambu terpasang.

Abstract

Koperasi Telekomunikasi Seluler perusahaan yang bergerak dibidang jasa penjualan dan distribusi, layanan umum serta layanan infrastruktur telekomunikasi (telco) sangat memperhatikan kualitas produknya. Namun terdapat reject produk yang dihadapi perusahaan sebesar 16% pada packaging simcard dalam 4 tahun terakhir sehingga mempengaruhi standar kualitas produk yang seharusnya dibawah 5%. Hal ini memerlukan pengendalian kualitas, pengendalian kualitas merupakan suatu kegiatan yang dilakukan agar produk memiliki kesesuaian dengan standar yang telah ditetapkan. Oleh sebab itu, untuk mengetahui penyebab reject produk dan memperbaiki kualitas produk, penelitian ini menggunakan metode New Seven Tools yang terdiri dari beberapa tahapan, antara lain Affinity Diagram, Relationship Diagram, Matrix Diagram, Matrix Data Analysis, dan Process Decision Program Chart. Berdasarkan hasil analisa menggunakan New Seven Tools diperoleh faktor penyebab kecacatan pada packaging simcard berasal dari faktor manusia, faktor inventory, faktor metode, dan faktor lingkungan. Sehingga saran perbaikan yang dapat dilakukan adalah melakukan pelatihan dan memperbaiki serta mensosialisasikan SOP di setiap divisi.

Abstract

 Micro and small businesses engaged in ice cube production are highly dependent on the reliability of their production machines. A decrease in machine performance can have a direct impact on product quality and quantity. This study aims to analyze the actual condition of the machine, determine the appropriate type of maintenance, and apply the Total Productive Maintenance (TPM) concept to improve machine effectiveness. The methods used include direct inspection, cycle time measurement, and Overall Equipment Effectiveness (OEE) calculation. The results showed that the OEE value was at 76.12%, with an availability value of 91.67%, a performance rate of 92.31%, and a quality rate of 90%. This value indicates that the machine is in fairly good condition but needs periodic preventive repairs. Maintenance recommendations include the implementation of Autonomous Maintenance, operator training, and a structured maintenance schedule. Abstract

MYI is one of the largest food companies in Indonesia. In the production process of this factory, there are various high-risk activities, especially in the Candy production section. The objective of this research is to implement the Hazard Identification, Risk Assessment and Determining Control (HIRADC) method as an effort to prevent work accidents among workers in the Production Department of the Candy Unit at PT. MYI. This research was conducted observationally. Observations were made on the sources of hazards and control efforts. Interviews were conducted with workers in the input, production, and output sections of the Candy Unit at PT. MYI. The research variables include work in the Candy production process, hazard identification, risk assessment, effectiveness of risk control, and residual risk. The results of the hazard identification that has been carried out show that there are 39 potential hazards that can lead to 39 risks. The risk assessment conducted revealed a risk level of Very High for 2 cases, High for 10 cases, Medium for 8 cases, and Low for 19 cases. The recommendations that can be given from this study are that PT. MYI needs to create and socialize work instructions, provide training, create a periodic maintenance checklist, install covers and limit switches, install conveyor covers, regularly check light and noise levels, provide fire extinguishers, and provide a disposal site for hazardous waste (B3).  Keywords: Production hazard risk of the candy unit, HIRARDC, PT. Abstract

PT APL is a company engaged in the distribution of medical equipment and medicines, with a finance division. Financial data is processed in the operational area. Computers are used in all activities to run business operations. The purpose of this study is to analyze the relationship between ergonomic positions and work posture using the Rapid Office Strain Assessment (ROSA) method in relation to musculoskeletal disorder (MSD) complaints among employees of PT APL. This study shows that at PT APL Semarang, there are four main factors contributing to inadequate workplace ergonomics, namely humans, machines, materials, and methods. The ROSA assessment results revealed that 76.92% of employees are at high risk of musculoskeletal disorders, mainly influenced by the use of non-ergonomic chairs. Employees often lack awareness regarding proper chair height adjustment, leading to poor sitting posture. In addition, improper monitor placement and incorrect positioning of mouse and keyboard use also contribute to the high risk. Awareness of workplace facility adjustments and education on ergonomics in the workplace are crucial to reducing health-related risks for employees.   Keywords: Ergonomics, Musculoskeletal Disorders (MSDs), Work Posture, Employees, Risk, Rapid Office Strain Assessment (ROSA), and PT APL.

Abstract

CV. Larissa Boga Utama is local businesses moving in the food industry, in recent years experienced an increase in product demand due to changes in its marketing system through social media and e-commerce. However, the inventory management process is still carried out manually without demand planning. This study aims to analyze demand planning using the Triple Exponential Smoothing (TES) method. The forecasting results using the TES method with two years of historical sales data show a demand of 1,990 units for 2025, with a MAPE accuracy of 3.08% (very good category) and an MSE of 56.05. Abstract

Sutrakabel Intimandiri is a company operating in the cable and copper industry. One of the products is copper busbar products. In copper busbar production, there is an obstacle in the form of a reject rate that exceeds the company's target ratio. Based on these problems, the methods used are Quality Control Circle (QCC) and Failure Mode Effect Analysis (FMEA) which aims to find factors that cause rejects in copper busbars. The results of the QCC and FMEA methods show the same high Pareto numbers, namely in the QCC method there are 7056 reject bubbles with a percentage of 43% and in the FMEA method there are 112 RPN reject bubble values with a percentage of 52.3%. In this analysis, looking for factors that cause reject bubbles is assisted by using fishbone with the 4M1E factor and improvements using the 5W1H method. The proposed improvements are to emphasize information related to SOPs strictly during daily briefings to reprimand workers if they still do not follow the SOPs, stricter supervision from the Unit Head by asking the relevant operator whether there are any problems currently occurring during the process, emphasizing the system. Autonomous Maintenance, implementing a checksheet for checking the quality of dies, revising the standard process by adding a water setting item to the manufacturing specification document, even though the manual provides the steps to be carried out, holding a machine and control checksheet every 30 minutes, the Section Head submitted a request to repair the roof which was already worn out.   Keywords: Quality, QCC, FMEA, fishbone, 5W1H

Abstract

The Sumbagut Gas Engine Power Plant (PLTMG) operating in Lhokseumawe City, Aceh Province, recorded 19 minor workplace accidents during 2024. In response to this condition, this study aims to identify the risk factors and main causes of workplace accidents in order to develop effective prevention recommendations. This study uses two integrated risk analysis methods. Failure Mode and Effect Analysis (FMEA) is applied to assess and prioritize the level of occupational safety and health (OHS) risks in operational activities. The results of the FMEA analysis indicate that fingers caught in a press machine are the highest risk with a Risk Priority Number (RPN) of 252. Despite having a lower frequency, electrocution incidents are also identified as a critical risk due to their very high potential for fatality. Furthermore, Fault Tree Analysis (FTA) is used to dissect these highest risks, successfully identifying seven (7) basic events that are the root causes of accidents. These causal factors include human error (lack of focus), technical failure (sensors or machine hydraulics), and administrative failure (ignored LOTO procedures). Based on the FMEA and FTA findings, this study presents strategic recommendations to the management of the Sumbagut Gas Processing Plant (GPP) to minimize and prevent future accidents.   Keywords: FMEA, FTA, Occupational Safety and Health

Abstract

Pengujian hydrotest merupakan metode penting dalam memastikan integritas dan ketahanan pipa terhadap tekanan internal sebelum digunakan dalam sistem perpipaan industri. Penelitian ini bertujuan untuk menganalisis kesesuaian antara hasil perhitungan ketebalan dinding (wall thickness) pipa heating coil berbahan Stainless Steel ASTM A312 TP304L dengan hasil pengujian hydrotest. Perhitungan dilakukan berdasarkan standar ASME B31.3 dengan mempertimbangkan tekanan desain, diameter luar pipa, dan faktor keamanan. Pipa yang digunakan memiliki spesifikasi Schedule 20S, yang dikenal memiliki ketebalan dinding relatif tipis. Pengujian hydrotest dilakukan dengan tekanan 1,5 kali dari tekanan kerja maksimum untuk mengevaluasi potensi kebocoran dan deformasi. Hasil penelitian menunjukkan bahwa perhitungan wall thickness yang sesuai standar mampu menjamin keberhasilan pengujian hydrotest tanpa terjadi kebocoran atau kerusakan struktural. Temuan ini memperkuat pentingnya validasi desain melalui simulasi perhitungan sebelum implementasi fisik, serta memberikan rekomendasi teknis untuk pemilihan material dan ketebalan pipa dalam aplikasi heating coil bertekanan.
