Abstract

Introduction: Oral health and non-communicable diseases (NCDs) in older adults are interconnected through shared behavioral, biological, and social risk factors. Dental caries and periodontal disease affect oral function and are associated with diabetes, cardiovascular disease, malnutrition, and reduced quality of life. However, oral health services and NCD management remain largely separated, limiting prevention and care coordination. This study aimed to map and synthesize evidence on the role of integrated NCD programs in improving oral health among older adults. A scoping review was conducted using the Arksey and O’Malley framework and PRISMA-ScR guidelines. Searches were performed in ProQuest, Scopus, and ScienceDirect for English-language articles published from 2021 to 2026. Eligible studies were descriptively analyzed and narratively synthesized. Review: Thirteen original studies met the inclusion criteria: ten cross-sectional, two qualitative, and one descriptive study. Findings demonstrated associations between oral health and NCD-related conditions, particularly cardiovascular risk, malnutrition, and periodontal disease severity. Interprofessional collaboration, community-based screening, and social support were identified as key components of integrated care. Community education and home-visit models improved service utilization, although healthcare access barriers and primary care infrastructure gaps persisted. Conclusion: Integrated NCD programs may improve oral health among older adults through holistic and collaborative care, but policy support and sustainable implementation strategies are essential.

Abstract

Introduction: The online reservation system implemented through the Mangusada Mobile application aims to enhance efficiency and improve patient satisfaction in outpatient care; however, system quality and user-related challenges may influence patient experience. This study examined system quality using the End User Computing Satisfaction (EUCS) model, levels of outpatient satisfaction, user constraints, and the relationship between system quality and outpatient satisfaction. Materials and Methods: A quantitative correlational survey was conducted among users of the Mangusada Mobile application. Results and Discussion: Findings showed that all EUCS dimensions scored above 80% and were categorized as “very satisfactory,” with the highest score in content (86.02%) and the lowest in timeliness (80.25%). Most patients reported high satisfaction with outpatient services, particularly in tangibles, reliability, responsiveness, empathy, and assurance. The most frequent constraints were internet connectivity issues (48.27%), user-related difficulties (30.94%), and technical problems (20.79%), which affected perceptions of timeliness. A significant association was found between online reservation quality and outpatient satisfaction (p < 0.001), with patients who perceived good system quality being 7.277 times more likely to be satisfied. Conclusion: These findings highlight that online reservation system quality is a critical determinant of outpatient satisfaction.

Abstract

Introduction: Obesity is a growing global health concern, largely driven by high-fat diet (HFD) consumption, which promotes adipose tissue accumulation and increases metabolic disorder risk. Beyond systemic effects, obesity may impair salivary gland function, which is essential for maintaining oral homeostasis. However, evidence regarding the effects of HFDs on salivary glands remains fragmented and inconsistent. This scoping review aimed to map the literature on HFD-induced salivary gland alterations and elucidate the underlying biological mechanisms. Review: The review followed PRISMA-ScR guidelines. Relevant studies were identified through comprehensive searches of PubMed and Scopus and screened using predefined eligibility criteria. Twenty-two studies were included. HFDs affected four major aspects of salivary gland physiology: reduced antioxidant and digestive enzyme activity; altered salivary proteins, including mucin and IgA; increased inflammatory biomarkers and oxidative stress; and morphological and functional changes, including reduced salivary secretion, fibrosis, vacuolization, inflammatory infiltration, and lipid accumulation. However, some studies reported no significant changes, suggesting that responses may vary according to gland type, HFD exposure duration, and metabolic status. Conclusion: HFD consumption induces oxidative stress, inflammation, and structural and molecular alterations in salivary glands, potentially contributing to salivary dysfunction. Further longitudinal and translational studies are needed to clarify the underlying mechanisms and clinical implications.

Abstract

Introduction: The Kolok (deaf–mute) community in Bengkala Village has limited access to health information due to the use of a local sign language restricted to the community. Consequently, poor knowledge and oral hygiene, as well as many complaints of toothache. This study aimed to evaluate the effectiveness of oral health education on caries-preventive behaviors among the Kolok community. Materials and Methods: A pre-experimental one-group pretest–posttest design was conducted involving 34 Kolok individuals aged 20–60 years, selected randomly. The intervention comprised face-to-face oral health education assisted by a local interpreter and the use of a toothbrushing calendar. Changes in oral health knowledge and preventive behaviors were assessed over two months. Knowledge was measured using questionnaires, while behaviors were assessed using questionnaires and the Oral Hygiene Index Simplified (OHI-S). Data were analyzed using paired sample t-tests, Wilcoxon signed-rank tests, independent sample t-tests, and Mann–Whitney U tests. Results and Discussion: The results indicated significant differences in knowledge and caries preventive behaviors between the intervention and control groups after the intervention (p < 0.05). However, significant improvements were observed in the control group, whereas no significant changes were found in the intervention group. In contrast, oral hygiene status significantly improved in the intervention group. Conclusion: Face-to-face oral health education assisted by a local interpreter was not effective in improving oral health knowledge and caries-preventive behaviors among the Kolok community. Nevertheless, the toothbrushing calendar demonstrated effectiveness in improving oral hygiene to prevent caries.

Abstract

Introduction: Short clinical crowns may lead to esthetic concerns, particularly excessive gingival display (gummy smile). Case: A 25-year-old female presented with a chief complaint of short teeth and excessive gingival display during smiling. Clinical examination revealed a gingival display of more than 2 mm, with no signs of periodontal inflammation and adequate keratinized gingiva. The diagnosis was excessive gingival display due to altered passive eruption type IB in the region of teeth 15–25. Case Management: Crown lengthening with a full-thickness flap and ostectomy was performed. Follow-up evaluations up to one month demonstrated favorable healing, patient satisfaction, no spontaneous bleeding or periodontal pocket formation, and stable treatment outcomes. Discussion: Accurate identification of altered passive eruption and appropriate management of the supracrestal tissue attachment are critical for successful esthetic crown lengthening. In this case, osseous resection was necessary to establish adequate periodontal dimensions and achieve stable esthetic outcomes. Conclusion: Crown lengthening with ostectomy is an effective and predictable treatment modality for altered passive eruption type IB.

Abstract

Introduction: The high burden of childhood dental caries highlights the need for innovative approaches. Artificial Intelligence (AI) offers a breakthrough in enabling more efficient, measurable efforts to prevent and detect dental caries. Therefore, this scoping review aims to map the existing evidence on the use of AI interventions in the prevention of childhood dental caries based on Leavell and Clark’s prevention model. Review: This scoping review analyzed 49 studies from various countries that examined the application of AI in childhood caries prevention, using the Leavell and Clark framework. At the health promotion level, behavior-based chatbot applications were shown to significantly improve caregivers’ knowledge and toothbrushing habits. In the specific protection setting, predictive models based on tabular data and the salivary microbiome identified high-risk children before lesions formed, with AUC values ranging from 0.77 to 0.94. For early detection, deep learning algorithms trained on radiographic images and intraoral photographs demonstrated high accuracy in detecting caries lesions, including incipient lesions, with a sensitivity of up to 94.7%. For disability prevention, AI models successfully predicted the success of caries arrest therapy with silver diamine fluoride. The main limitations identified were a small sample size, a lack of cross-population external validation, and gaps in model interpretability at the clinical level. Conclusion: The use of AI interventions for childhood dental caries is unevenly distributed across Leavell and Clark's five preventive levels, especially for specific protection.

Abstract

Introduction: Menopause is a physiological process that all women experience and is often regarded as a sign of aging in women. The signs and symptoms of menopause are very diverse, and oral discomfort is one of the symptoms. Oral discomfort can interfere with oral cavity function, indirectly affecting nutrient intake. This study aims to evaluate factors associated with oral discomfort and oral manifestations of the menopausal stage. Review: This review is based on the PRISMA statement. A literature review incorporated observational studies from the last five years and across 5 databases.  An investigative question was formulated using the PEOS model, studies were selected, and a quality analysis was performed in accordance with the STROBE guidelines. The bibliometric analysis was performed, and the data were qualitatively analyzed. A total of 19 articles met the inclusion criteria, with the majority of oral manifestations at the menopausal stage being xerostomia, burning sensation, taste disturbances, and periodontitis. Duration of menopause, bad oral hygiene, lower salivary flow rate, higher calcium saliva concentration, and salivary estrogen level were found to be associated factors for oral discomfort at the menopausal stage. Conclusions: The menopausal stage may increase the risk of oral discomfort due to decreased estrogen levels in saliva. Since the estrogen receptor was found in the oral cavity, especially in the mucosa, salivary gland, and periodontium, it can directly affect the oral cavity.

Abstract

Introduction: Periodontal disease during pregnancy continues to be a significant worldwide health concern with wide-ranging systemic effects. The association between maternal periodontitis and unfavourable pregnancy outcomes (APOs) has been shown by numerous systematic reviews. Studies conducted worldwide reveal significant variations in periodontitis prevalence and associated risk factors. This literature review aims to determine the prevalence and risk factors of periodontal disease during pregnancy in Indonesia. Article searches were conducted in 7 databases: PubMed, ProQuest, ScienceDirect, Scopus, Garuda, EBSCOhost, and Google Scholar. Review: This study followed the predefined PEOS eligibility criteria, with a population consisting of pregnant women of all ages and trimesters in Indonesia, various risk factors associated with periodontal disease as the exposure, and periodontal disease as the outcome. Inclusion criteria were original research published between January 2015 and April 2026, written in English or Indonesian. A total of 312 studies were identified, and 18 studies were included. The reported prevalence of periodontal disease during pregnancy in Indonesia ranged from 4.4% to 100%. Several associated risk factors found are oral hygiene, particularly calculus accumulation, crowded teeth, frequency of toothbrushing, betel nut consumption, smoking status, level of education, living area, gestational age, stress, health motivation, and contraceptive use history. Conclusion: The prevalence of periodontal disease during pregnancy in Indonesia has remained quite high over the past decade. The majority of pregnant women in Indonesia need intervention in the form of Dental Health Education (DHE) and dental scaling.

Abstract

Introduction: Root canal treatment failure is frequently associated with persistent microbial infection, particularly by Enterococcus faecalis and Staphylococcus aureus. Sodium hypochlorite remains the gold standard irrigant; however, its cytotoxicity has encouraged the exploration of herbal alternatives. Cymbopogon citratus contains bioactive compounds with potential antibacterial properties. Material and Methods: This laboratory experimental study evaluated the antibacterial activity of Cymbopogon citratus extract at concentrations of 10%, 20%, and 30% against E. faecalis ATCC 29212 and S. aureus ATCC 29213 using the disc diffusion method. Sodium hypochlorite 2.5% was used as the positive control and 96% ethanol as the negative control. Data were analyzed using one-way ANOVA followed by Bonferroni post hoc test. Results and Discussion: The highest inhibition zone for E. faecalis was observed at 20% concentration (1.63 ± 0.48 mm), while for S. aureus, the highest value was found at 10% concentration (2.06 ± 1.15 mm). No statistically significant differences were found among extract concentrations (p>0.05), whereas sodium hypochlorite demonstrated significantly higher antibacterial activity (p<0.001). Conclusion: Cymbopogon citratus extract demonstrated weak antibacterial activity and was not comparable to sodium hypochlorite. Its potential as an alternative root canal irrigant requires further optimization and validation.
