Abstract

Adverse events can be minimized the level of madness in the hospital by proclaiming behaviors that encourage the formation of patient safety known as the patient safety culture. Various health services, especially hospitals, data on Adverse events are still found annually. This study aims to determine the description of patient safety culture at nurses at the Intensive Care Installation Arifin Achmad Hospital of Riau. The population are all nurses in the Intensive Care Installation Arifin Achmad Hospital of Riau with size are 64 nurses with total sampling techniques in accordance with the inclusion and exclusion criteria. Data collection used a questionnaire adopted from Hospital Survey on Patient Safety (HSPSC) questionnaire published by The Agency For Healthcare Research and Quality (AHRQ). The results of the study prove that 56.3% of the patient's safety culture is negative category, and 43.8% of the patient's safety culture is positive category, importance to make programs that are related to patient safety culture are was need to improve the patient safety culture in Arifin Achmad Hospital of Riau

Abstract

Counseling is one of the effective ways in health education to teach patients to limit their fluid intake. Through counseling the relationship between nurses and patients can be more open, where patients can express all complaints and limitations and / or their abilities and weaknesses, nurses can also explore patient feelings and understand the diversity of each different patient, so that communication between nurses and patients can be adjusted with the diversity of characteristics that patients have. In patients Renal failure is often found as a complication of long-standing hypertension and can interfere with the process of hemodialysis. Therefore sufferers need to be assisted in the form of counseling. This study aims to determine the effect of counseling on hemodialysis patients with recurrence rates of hypertension in Medan City Hospital. This type of research is quantitative with a quasi-experimental design. The sample used in this study amounted to 54 people and the sampling technique with purposive sampling was that sample taking by researchers came to do research and a portion of the sample was taken from the population. Data collection uses questionnaires and data analysis using the T-Test. From the results of the study, the effect of giving counseling with the recurrence rate of hypertension was obtained that the p value was 0.001. It is expected that nurses will provide education and counseling to hemodialysis patients to be able to control diet patterns.

Abstract

The success of the malaria elimination program in Kabuupaten Aceh Singkil, the Annual Parasite Incident (API) per 1000 population will reach 0%. Nowadays, the frequency of implementing the program has decreased so that it is suspected that it can cause people to suffer from malaria. The purpose of the study was to analyze the evaluation of the implementation of the malaria elimination program. This type of research is qualitative. The informants consisted of 5 key informants namely the person in charge of P2P, malaria management officers, village midwives and 4 main informants namely the head of the puskesmas and village head and 6 additional informants namely the community, pregnant women, cadres. Data were analyzed by interactive methods through the stages of data reduction, presentation and drawing conclusions. The results showed that blood tests were not carried out unless the community had a fever. Distribution of delivery nets, screening for pregnant women is reduced, and biological vector control is not monitored. Epidemiological surveillance and epidemic prevention are not carried out because there are no outbreaks so monitors are made in the form of monthly reports. Educational activities in the form of outreach continue to the present and responsibilities are more focused on village midwives/cadres. Training and guidance activities for officers have never been done. The percentage of API reaches 0% in 2018. It is recommended that health workers remain vigilant and prioritize programs supporting malaria elimination programs along with other health programs through community empowerment today.

Abstract

Hemodialysis is the process of removing metabolic waste substances, other toxic substances through a dialiser that functions as an artificial kidney. Hemodialysis or dialysis is the transfer of the patient's blood from his body which occurs in disfusion and ultrafiltration. This study aims to determine the relationship between the level of anxiety and sleep quality in patients undergoing hemodialysis at Rasyida Kidney Special Hospital Medan. This research design used analytical survey with cross sectional approach. The sampling technique was accidental sampling, a sample of 75 respondents. The data used are primary data by distributing questionnaires, secondary data, and tertiary data while the data analysis used is univariate analysis and bivariate analysis with Chi-Square test. The results of this study with stastic test showed that the p-value result was 0.033 (<α 0.05), so there was a relationship between the level of anxiety and the quality of sleep of patients undergoing hemodialysis in the Rasyida Kidney Special Hospital Medan. The conclusions in this study indicate that there is a significant correlation between the level of anxiety and sleep quality in patients undergoing hemodialysis in Medan Renalida Kidney Special Hospital. It is recommended for patients to control anxiety so that sleep quality will be better and recommended to health workers especially hemodialysis nurses to further enhance the knowledge of patients who increase hemodialysis by providing education/health education related to the effects and complications of hemodialysis.

Abstract

Weaning food for breastfeeding babies should always be given at the appropriate time, meaning that all babies should start get weaning food from age six months old and counting. The research was done in Puskesmas Stabat (Stabat Community Health Center), Stabat District, Langkat Regency, from December 2016 to February 2017. Theobjective of this research is to assess the correlation of early weaning food giving and diarrhea incidence in 0 – 6 months babies. This research used a descriptive correlation study design. The population of the research is all mothers with 0 – 6 months old babies in Puskesmas Stabat numbering 389 people. The sample of the research is 40 people, or 10 % of the population, using accidental sampling technique. The independent variable is early weaning food giving to 0 – 6 months old babies, and the dependent variable being diarrhea incidence. The data was collected using questionnaires and interviews, then analyzed using chi squaretest (CI 95% anda= 0,05). The statistical test showed the correlation of early weaning food giving and diarrhea incidence in 0-6 months old babies (p-value= 0,001) as p < 0,05 which means there is a significant correlation between early weaning food with diarrhea incidence. The conclusion of the research is that early weaning food giving influences diarrhea incidence in 0 – 6 months old babies at Puskesmas Stabat.

Abstract

Exclusive breastfeeding is breast milk that given to babies from birth for six months without and / or replace with other food or drinks. Exclusive breastfeeding can provide the immunity needed for the growth and development of infants. The purpose of this study was to examine the relationship between knowledge and exclusive breastfeeding at Perdagangan Health Center in Simalungun Regency. The population is in this study were all mothers who have infants aged 7-12 months who live at ​​Perdagangan Health Center in Simalungun Regency. The sample in this study was 115 respondents. The study was conducted from September 2 to October 1, 2019. The instrument in this study used a questionnaire created by the researcher. Statistical test is using Chi-square. The results of the study obtained at Perdagangan Health Center in Simalungun Regency that the majority of mothers' knowledge about exclusive breastfeeding is in the poor category of 78 respondents (67.8%), the majority of mothers do not provide exclusive breastfeeding, namely 92 respondents (80%). Statistical test results show that P-value is 0.01 <0.05, so that the null hypothesis (HO) is rejected, which means there is a significant relationship between maternal knowledge and exclusive breastfeeding. It is recommended that health workers who work in the area of ​​the mother and child area should improve education programs for pregnant and breastfeeding mothers to increase the knowledge and interest of mothers to breastfeed exclusively in the first six months of a baby's life.

Abstract

Smoking during pregnancy can endanger pregnancy and the fetus, especially the health of pregnant women and fetal development in the womb. One of the complications of pregnancy that causes fetal death is due to oxygenation disorders. In Indonesia, more than half of households have at least one smoker, and almost all smokers’ smokes at home. The cause of neonatal death is fetal death in utero, asphyxia or respiratory problems due to smoking and premature. This study aims to determine the knowledge of pregnant women about the dangers of smoking to pregnancy and the fetus in Gasaribu Village, Laguboti, Toba Samosir Regency. This research was conducted in September-October 2019 using adescriptive research design. The number of samples was in this study were 40 pregnant women. Sampling is done by using saturated sampling. The results showed that good knowledge of 16 people (40%) was enough 22 people (55%) and less 2 people (5%). The results of this study are expected to be used as additional material in adding knowledge and information to increase real health education (real) to the public about the dangers of smoking to pregnancy and the fetus.

Abstract

Stunting is a disruption of physical development that has passed with decreasing in the growth rate of children. Puskesmas Pancur Batu conducts stunting prevention through health promotion to increase knowledge and attitudes of pregnant women about exclusive breastfeeding. The number of mothers giving exclusive breastfeeding only reached 44.9% (target 80%). The purpose of the study was to analyze the effect of health promotions with leaflet media about exclusive breastfeeding on the knowledge and attitude of pregnant women in stunting prevention. The study design was quasi-experimental with a sample of 40 pregnant women. The study was conducted in October 2019 in Puskesmas Pancur Batu. The sampling technique is done purposively. Data were analyzed using independent t test. The results showed that before the pre-test, the average score of knowledge variables from the intervention group 6.60 and the comparison group 6.22, after the post-tes increased to 10.30 and 8.34. The average score of attitude variables from the intervention group was 4.04 and the comparison group was 4.44, after the post-test increased to 5.80 and 4.84. The results of the statistic test showed that health promotion influences the knowledge and attitude of pregnant women in preventing stunting before and after the intervention with p = 0.005 and p 0.028 <0.05. It is recommended that Puskesmas staff conduct health promotion through the distribution of leaflets to facilitate pregnant women in understanding the benefits of exclusive breastfeeding in an effort to prevent stunting.

Abstract

Weight gain is based on dry weight. When the body has to bear excess fluid between two dialysis times, this is what is called interdialitic body weight / Inter Dialitic Weight Gain (IDWG). IRR in 2015 noted the most frequent complication in hemodialysis durante was hypertension (38%), followed by hypotension (15%), where the etiology of the two complications above is very closely related to the number of ultrafiltration. The purpose of this study was to determine the relationship between inter dialitic weight gains (IDWG) with the occurrence of complications of durante hemodialysis. By using a research design analytic method that uses a cross sectional approach. The sampling technique uses total sampling, in which 60 respondents measured body weight and blood pressure and observed whether experiencing complications durante hemodialysis. Furthermore, the results of observations are written in the observation sheet and tested by statistical data with the Chi-Square Test. The results showed the majority of respondents had a moderate IDWG of 33 people (55%), of which 27 people (45%) had complications consisting of 7 people (11.7%) hypotension and 20 people (33.3%) hypertension. Statistical test results with the chi-square test showed that the value of p = 0,000, which means that there is a significant relationship between IDWG and the occurrence of complications of hemodialysis durante. Future researchers are expected to be able to further develop research on other complications that may occur related to IDWG.

Abstract

Diabetes mellitus is the result of the interaction of genetic factors and environmental exposure. Genetic factors will determine the individuals susceptible to DM.Uncontrolled diabetes and over time cause serious damage to many systems of the body, especially the nerves and blood vessels. By 2014, 8.5% of adults 18 years and over have diabetes. By 2015, diabetes is a direct cause of 1.6 million deaths and by 2012 high blood glucose is the cause of 2.2 million other deaths. This study aims to determine the relationship of stress levels to elevated blood sugar levels in patients with type II DM in Royal Prima Hospytal Medan. The research design used is correlation (Correlation Study) with Cross Sectional approach. The sample of this research is patient of DM type II disease with sample number 67 respondent using chi-square statistic test. Based on chi-square statistic test can be obtained result of 0,04 that is <0,05 means smaller than value α = 0,05. Thus drawn conclusion there is a relationship of stress level to increase blood sugar levels in DM tipe II patients in Royal Prima Hospytal Medan. From the results of the study concluded that there is a relationship level of stress to elevated blood sugar levels in patients DM tipe II. Suggested to Royal Prima Hospytal Medan recommends that patients keep control of stress on natural problems and diseases in order not to lead to other forms of disease.
