Abstract

A metabolic disorder known as diabetes mellitus (DM) causes hyperglycemia, or abnormally high blood sugar levels, as a result of insulin resistance or insufficiency. This quantitative research was designed to find out how acupuncture treatment affects blood sugar levels in individuals with kidney yin deficiency syndrome and type 2 diabetes (Xiāo KĔ). Bekasi, West Java, was the site of the research, which lasted from March to June of 2025. A purposive sample strategy was used to select a total of 20 individuals, all of whom were 20 to 30 years old. Women made up a larger proportion of the individuals (65%) than males (35%), and the average age was 27 (the median). Employers made up 25% of the sample, while housewives accounted for 20%. The majority of participants were self-employed at 55%. The subjects had daily acupuncture treatments for a duration of six days. Prior to and during the intervention, blood sugar levels were monitored. A blood sugar level of 136.25 mg/dl was typical before the intervention, with a range of 128 to 142 mg/dl. The average dropped to 115.80 mg/dl after the intervention, with a range of 110–121 mg/dl. The average drop in blood sugar levels was 17.94%, or 24.45 mg/dl. In one study, 15% of participants had a reduction of 23 mg/dl. With a result greater than 0.05, the Shapiro-Wilk normalcy test confirmed that the data followed a normal distribution. A statistically significant difference was seen between sugar levels before and during medication, as shown by the paired sample t-test findings (sig. (2-tailed) value of 0.000 < 0.05). Research in individuals with type 2 diabetes and kidney yin deficiency syndrome found that acupuncture treatment significantly reduced blood sugar levels.

Abstract

This study aimed to identify factors associated with fatigue among employees of PT. XYZ in Medan, a futures brokerage firm where preliminary observations indicated a high prevalence of exhaustion and decreased alertness among employees. This study aimed to identify factors associated with fatigue. A descriptive analytic study with a cross-sectional design was conducted using a total population sample of 45 employees. Data were collected on workload (measured as %CVL), work duration, work environment (assessed through a questionnaire), and occupational fatigue (measured using the IFRC questionnaire). Univariate, bivariate (Chi-square), and multivariate statistical analyses were performed. The results showed that 33.3% of participants experienced fatigue. Bivariate analysis demonstrated statistically significant associations between fatigue and workload (p=0.015), work duration (p=0.001), and work environment (p=0.006). Multivariate analysis further quantified these relationships, identifying the work environment as the most dominant risk factor (OR=3.18), followed by work duration (OR=2.4) and heavy workload (OR=2.4). In conclusion, an unsafe work environment, long work duration, and heavy workload are significant and quantifiable risk factors for occupational fatigue, with the work environment serving as the strongest predictor.

Abstract

The National Health Insurance (JKN) program managed by BPJS Kesehatan implements the Indonesia Case-Based Groups (INA-CBGs) prospective payment system. Although INA-CBGs rates are determined based on diagnosis, the treatment class factor has the potential to influence the amount of claims paid. This study aims to analyze the effect of treatment class on the value of INA-CBGs claims in patients with chronic obstructive pulmonary disease (COPD, code J44.1) at Dustira Class II Hospital. This study used an observational analytical design with a cross-sectional approach. The sample consisted of all COPD patients treated at the Kenanga Ward in January 2025 (n=37), with a distribution of Class 1 (n=2), Class 2 (n=13), and Class 3 (n=22). Data analysis was performed using the Kruskal–Wallis test, followed by the Dunn–Bonferroni post-hoc test. The results showed that the highest median claim value was found in Class 1 (Rp 5,264,600), followed by Class 2 (Rp 4,611,500), and Class 3 (Rp 3,958,400). The Kruskal–Wallis test showed a significant difference (H=33.720; p<0.001). Post-hoc analysis showed a significant difference between Class 1 and Class 3 and between Class 2 and Class 3, while the difference between Class 1 and Class 2 was not significant. It was concluded that the treatment class significantly influenced the BPJS claim value in COPD patients. These findings have implications for the evaluation of INA-CBGs tariffs and the preparation for the implementation of the Standard Inpatient Class Policy (KRIS) in 2025.

Abstract

The JKN mobile application offers features such as online registration and real-time notifications to address barriers such as manual registration, but long queue times can reduce service satisfaction and accessibility. This study employed a descriptive qualitative method, aiming to determine how long BPJS Kesehatan patients wait for outpatient services, the factors that cause delays, and whether the JKN mobile application can be optimized. This study used 15 patients as an incidental sample, with percentage analysis divided into categories of <60 minutes and >60 minutes. The results showed that 73.33% (11 patients) met the waiting time standard of less than 60 minutes, while 26.67% (4 patients) exceeded 60 minutes. This was primarily due to the new patient status and lack of space in the surgical polyclinic for 100% of cases. The neurology and orthopedics polyclinics achieved 100% compliance with the standard. This study suggests that the JKN mobile application can accelerate the registration process and reduce queues, supporting the BPJS system and national health policy, and increasing universal accessibility to services. Furthermore, the JKN mobile application, which includes online registration and instant notifications, is being encouraged to expedite the new patient process and optimize resource allocation, particularly in surgical polyclinics. The app aims to improve accessibility and patient satisfaction.

Abstract

This study aims to analyze the psychological and organizational factors that influence the implementation of Electronic Medical Records (EMR) by medical records staff at Welas Asih Regional Hospital. This research method uses a descriptive qualitative method in the Medical Records installation of Welas Asih Regional Hospital. A sample of 33 people was taken using a purposive sampling technique, namely officers who actively use the EMR system. Data were collected using questionnaires, observation, and documentation. Although the implementation of EMR aims to improve efficiency and quality of service, challenges that arise are changes in work culture and psychological factors such as initial anxiety. From the organizational side, the main challenges are the need for additional training and constraints on infrastructure/system smoothness. Quantitative results show that psychological factors in the form of internal motivation are at a very strong level (average score of 4.87), in line with the high awareness of officers regarding the benefits of EMR (average score of 4.67). However, aspects of initial anxiety were still identified (average score of 2.90). Organizationally, there was a strong indication of the need for additional training (average 4.41) and the persistence of technical operational barriers (average 3.31), indicating the need to optimize facility support. Optimizing RME adoption requires a holistic strategy, including the provision of psychological support programs to mitigate anxiety, as well as targeted organizational investments in improving the quality of practical training and maintaining a smoother and more stable system infrastructure.

Abstract

Digital transformation in Indonesia's healthcare sector is increasingly developing through the implementation of information system integration that supports accelerated service delivery, particularly in healthcare facilities with high visitor loads. This study analyzes the dynamics of interoperability between the Hospital Management Information System (SIMRS) and the JKN Mobile application in the patient registration process at Welas Asih Regional Hospital, a hospital with approximately 2,000 daily visits, of which approximately 1,500 use JKN Mobile. This study used a qualitative approach with a case study design, through in-depth interviews and direct observation of the registration process. The results indicate that the SIMRS–JKN Mobile integration is effective when the system is functioning normally, characterized by a fast barcode check-in process, automatic queue number synchronization without re-input, and consistent receipt number locking across all service stages up to claims. However, several obstacles still arise, particularly BPJS server disruptions, internet network instability, GPS reading issues, and limited patient digital literacy. Staff overcome these obstacles through adaptive strategies such as GPS calibration, switching to the hospital's Wi-Fi network, arrival time validation, and the use of the SIMRS approval feature. This study demonstrates that successful interoperability is determined not only by technological readiness but also by organizational readiness, staff operational competence, and patient capability in using digital applications. These findings provide a novel scientific contribution by providing comprehensive field evidence on the implementation of interoperability in high-volume healthcare facilities and providing recommendations for technical improvements and service governance for hospitals and policymakers.

Abstract

BPJS Kesehatan claims are a crucial component of the continuity of hospital services, yet pending claims are still common. This study aims to identify the factors causing pending inpatient claims at Welas Asih Regional Hospital and understand the obstacles in the claims submission process. The main problems include incomplete supporting documents, mismatched diagnoses, and verification challenges from BPJS. The study used descriptive qualitative methods through interviews, observations, document reviews, and analysis of 521 pending claim files. The results indicate that the largest causes of pending claims stem from BPJS verification (57.39%) and coding (28.60%), while administrative issues account for only a small portion. Good internal coordination between units allows most claims to be resolved without escalation to BPJS. The study concluded that improving the completeness of medical records, coding training, understanding regulations, and reviewing multiple documents are effective strategies to reduce pending claims and increase the efficiency of the BPJS claims process at Welas Asih Regional Hospital.

Abstract

This study aims to evaluate the effectiveness of the implementation of the Hospital Management Information System (SIMRS) in the Medical Records Unit of Welas Asih Regional Hospital using the HOT-Fit model, which includes the Human, Organization, Technology, and Net Benefit aspects. The research method is a descriptive quantitative survey with a total sampling technique, involving all 41 medical record officers as active SIMRS users. The data collection instrument was a Likert scale questionnaire that has been tested for validity and reliability. Analysis was carried out descriptively by calculating the average value of each component. The results show that the implementation of SIMRS is in the very good category with an overall average value of 4.68. The highest value is found in the Net Benefit aspect, namely 4.73, which confirms that SIMRS provides direct benefits in accelerating service flow and improving data accuracy. The Organization aspect obtained a value of 4.70, Human 4.68, and Technology 4.62, which overall indicate user readiness, organizational support, and system reliability. However, respondents still noted the need to improve network stability and feature development to optimize SIMRS utilization in supporting the medical records service process.

Abstract

Thyroglossal duct cyst is the most common congenital neck abnormality, resulting from failure of the thyroglossal duct to obliterate during the embryogenesis of the thyroid gland. A 13-year-old female patient complaining of a lump under her chin for approximately 5 years prior to admission. Initially, the lump was painless, but two weeks before admission, it enlarged and was accompanied by throbbing pain and signs of local inflammation. Physical examination revealed a mass in the submental region with tenderness and erythema, without cervical lymph node enlargement. FNAB Examination result showed a thyroglossal duct cyst without signs of malignancy. Based on the anamnesis, physical examination, and supporting examination results, the patient was diagnosed with a thyroglossal duct cyst. The patient underwent a Sistrunk procedure under general anesthesia. The operation approximately 40 minutes and involved excision of the cyst and duct tract, resection of the middle part of hyoid bone, and placement of a handscoon drain. Postoperative wound care proceeded well, improvement in pain, stable condition, and no complications of bleeding or surgical wound infection. On the fourth postoperative day, the patient's condition was stable and she was allowed to go home with oral therapy.

Abstract

Tuberculosis, a highly infectious ailment, is triggered by the bacterium Mycobacterium tuberculosis, primarily affecting the lung's functional tissue. Transmission occurs when a person inhales airborne saliva droplets containing the bacteria expelled by a tuberculosis patient through coughing, thus spreading the disease. This investigation sought to determine the attributes of tuberculosis patients receiving outpatient care, along with the pattern of anti-tuberculosis drug prescriptions at X Hospital Medan between July and December 2023. Metode; The study utilized a non-experimental approach because of its design and structure, which involved collecting survey data from the hospital's historical records. Ninety-six tuberculosis drug prescriptions issued during July–December 2023 were selected through random sampling. The study's findings revealed that, among 96 patients, the 46–55 age group comprised 22 individuals (22.9%), with females accounting for 55 patients (57.3%), and 94 patients (95.8%) utilized BPJS as their payment guarantor. The research indicated that Category I anti-tuberculosis drugs were prescribed more frequently than Category II anti-tuberculosis drugs for 92 patients (95.8%), and 59 patients (61.5%) at X Hospital received OAT-FDC, a commonly employed tuberculosis treatment. This study proposed that future research should outline the anti-tuberculosis medication prescription profile, using patients’ health information.
