Abstract

Seiring masuknya covid-19 ke tanah air yang kemudian menjadi bencana wabah sehingga infeksi meluas secara cepat dengan fatalitas yang terbatas, cukup membuat kewalahan pelayanan kesehatan. Terbitnya Surat Keputusan Konsil Kedokteran Indonesia Nomor 18/KKI/KEP/III/2020 tentang Kewenangan Dokter Penanggung Jawab Penanganan Pasien di Fasilitas Pelayanan Kesehatan Pada Masa Darurat Bencana Wabah Penyakit Akibat Corona Virus Disease 2019 (COVID-19) di Indonesia mengijinkan mobilisasi seluruh tenaga medis dan spesialis baik paru dan non paru di saat pandemi covid guna mencukupi layanan covid, menjadikan dasar bagi RSUP. Persahabatan yang ditunjuk sebagai rumah sakit khusus covid untuk menerapkan hal tersebut.

Abstract

Introduction: Emotional mental disorder (EMD) is a state of psychological distress that, if not handled properly, can lead to severe mental disorders. One potential psychosocial hazard that is thought to be related to mental health disorders is work-family conflict (WFC). The issue of family-work balance has become an important issue among Indonesian female workers, especially with the rapidly growing female worker society and the strong family culture. This study aimed to identify the association between work-family conflict and emotional mental disorder in female nurses. Methods: This cross-sectional study included 264 female nurses who work at national referral general hospital in Jakarta. Their emotional mental disorder was measured using Self-Reported Questionnaire 20 (SRQ-20) and the work-family conflict was measured using the Work-Family Conflict Scale (WCFS). Results: The prevalence of emotional mental disorder in female nurses was 23.5%. The most dominant factor associated with emotional mental disorder is work-family conflict (OR 2,40, CI 95% 1,32-4,35, p=0,004). Conclusion: There is a significant association between work-family conflicts and emotional mental disorders in female nurses in Indonesia. Nurses with work-family conflicts are more likely to have emotional mental disorders. Regular mental health checks and counseling should be performed along with periodic health checks.

Abstract

Introduction: Adolescent-onset schizophrenia patients cover a small fraction of schizophrenia population, with greater symptoms’ degree of severity, affecting patients’ quality of life. One of the instruments which measures schizophrenia symptom severity is Positive and Negative Syndrome Scale (PANSS). This study’s purpose is to know the demographic characteristic and compare the PANSS scores between adolescent-onset and adult-onset schizophrenia. Method: This study is a cross-sectional descriptive study done throughout November 2019–February 2020. This study uses secondarily-sourced data from the schizophrenia patients’ medical records admitted in Dr. Hasan Sadikin Central General Hospital’s in 2018, sampled through total sampling and calculated using Mann-Whitney U test. Result: Result of this study shows that most adolescent-onset schizophrenia patients are male, middle-level educated, unemployed, and unmarried. Statistically significant difference is not found in total score, positive scale, negative scale, and general psychopathology scale. It is also found that in general psychopathology scale, adolescent-onset patient scored higher than adult-onset patients on “unusual thought content” between the two onsets (p=0,01).Conclusion: Adolescent-onset in schizophrenia is not significantly different to adult-onset in PANSS score, though it is significant in unusual thought content in general psychopathology scale.

Abstract

Introduction: Type 2 diabetes mellitus is caused by decreased tissue sensitivity to insulin. The prevalence of diabetes in the world has almost doubled since 1980, from 4.7% to 8.5% in adult population. Early diagnosis and treatment aimed at normalizing glycemic control are very important. The objective of this study was to evaluate and compare glycemic control of metformin and glimepiride in monotherapy of type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya. Method: This was a retrospective observational study using secondary data (medical record), include glycemic control (RPG) before and two months after receiving therapy of outpatients’ type 2 diabetes mellitus with metformin or glimepiride therapy in 2018. 96 samples were found that fit the inclusion criteria. The data were analyzed by Mann-Whitney test. Result: Most patients were female, aged 50-69 years old, and dosage of metformin therapy 1500 mg/day or glimepiride therapy 2 mg/day. There was no significant difference (p greater than 0.05) of glycemic control (RPG) of metformin compared to glimepiride therapies in type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya in 2018. Conclusion: Metformin and glimepiride were not significantly different in glycemic control (RPG). There were patients with RPG greater than 200 mg/dl after two months of metformin or glimepiride therapy.

Abstract

Introduction: There are 40 million people predicted to suffer from heart failure (HF), in Indonesia it is around 13% of the population. The incidence of anaemia in heart failure patients is estimated to be between 17-50%. AnGagal emia is known to increase mortality risk and worsening prognosis in HF patients and a better understanding of the mechanism will lead to better treatment effectiveness.Methods: The study uses analytical observational method and a cross-sectional design. Data are taken from medical records admitted in 2016 to 2018 and grouped based on New York Heart Association Functional Class (NYHA-FC) and anemia severity (WHO anaemia cut-off point). Results: Eighty-one anemic HF patients analyzed and consists of 34,57% NYHA II, 24,69% NYHA III, and 40,7% NYHA IV. A total of 50,6% patients are mildy anemic and 49,4% are moderately anemic. No severe anemia was found. Multivariate analysis shows chronic HF functional class did not have a significant relationship with anaemia severity (p = 0,179). Conclusions: There is no significant relationship between Chronic HF NYHA-FC and anaemia severity. Severe anemia are rarely found in HF patients without severe kidney failure.

Abstract

Since COVID-19 first broke out in Jakarta in early March 2020, the infection rate has been slowly but surely increasing. Health workers who work directly to serve patients, including the resident, are vulnerable to infection. The following is a story of a Obstetrics and Gynecology resident who was infected with COVID-19 and managed to recover after going through a period of independent isolation. In March 2020, the resident who at that time served in the Obstetrics polyclinic at a COVID-19 referral hospital in Jakarta, experienced myalgia, fever, headache, sore throat, cough and runny nose. Due to the condition causes PPDS to be rested. Complaints improved within a few days, after taking medication and multivitamin supplementation. One week later, the resident with the other resident and staff conducted a nasopharyngeal swab examination at the hospital, then the samples were sent to Litbangkes for reverse transcription polymerase chain reaction (RT-PCR) examinations against SARS-CoV-2. The results were obtained 20 days later and the resident was declared positive. When the results came out, PPDS who already felt healthy were undergoing rotation in another hospital. Undergo a 2-week self-isolation protocol, nutritious intake, supplementation, exercise, positive thinking and continued prayer. Transmission of COVID-19 may occur while serving patients, social distancing and wearing less awake masks when with PPDS friends, midwives, nurses and staff. The long duration of the RT-PCR swab results makes it difficult for tracing, the risk of widespread transmission and delays in efforts to break the chain of transmission of COVID-19 besides disrupting education rotation.

Abstract

Introduction: Myocardial perfusion scan (MPS) is a non-invasive procedure to evaluate the perfusion, viability, and contractility of the heart muscles. MPS is one of the important modalities using radionuclide in the management of heart disease. Single Photon Emission Computed Tomography (SPECT) is a technology applied to the MPS examination and can be combined with a CT scan.. Dextrocardia is an abnormality in the location and position of the heart, where the heart is located in the right hemithorax and accompanied by impaired perfusion. Case: We reported a 51-year-old woman, came with chest pain since 2 years. Chest pain appeared mainly with activity, radiates to the back, and sometimes with shortness of breath. Previous echocardiography had shown the heart was located in the right. CT scan examination gave inconclusive results. Patient was referred for an SPM examination on the indication of suspected coronary artery disease (CAD) with dextrocardia.Discussion: Patients with dextrocardia are at the same risk for developing myocardial ischemia as in the general population. SPM examination can be used to detect CAD in dextrocardia patients.Conclusion: SPECT/CT MPS could provide functional information and determine the type of dextrocardia in a patient suspected of having CAD.

Abstract

Introduction: Diabetic ketoacidosis (DKA), an acute complication of type 1 (insulin dependent) diabetes mellitus (T1DM), can be precipitated by infection. Procalcitonin (PCT) is an accurate marker of bacteremia, sepsis, and inflammation, however white blood cells (WBC) are still often used by clinicians. We aimed to analyze PCT levels and WBC counts in children with DKA.Methods: A cross-sectional study was conducted in Dr. Soetomo General Hospital, Surabaya, Indonesia, between 2015 and 2019. T1DM and DKA diagnosis was based on the International Society for Pediatric and Adolescent Diabetes. PCT levels and WBC counts were measured in samples from patients with and without DKA, and were compared using the Mann-Whitney test.Results: A total of 41 samples were included, with 15 samples (36.6%) from children with DKA, and 26 (63.4%) from children without DKA. PCT levels and WBC counts were significantly higher in those with DKA (p less than 0.001). The receiver operating characteristic curve analysis of WBC was lower than PCT (0.849 vs. 0.982). PCT had a higher sensitivity and spesificity as an infection marker than WBC (93.3 vs. 86.7; 92.3 vs. 88.5, respectively).Conclusion: PCT is a better infection marker in children with DKA than WBC

Abstract

A global pandemic, COVID-19, has struck 216 countries with more than 9 million cases and 420.000 deaths. In Indonesia alone, 49.000 people have been infected with more than 2.000 deaths. To reduce the transmission rate and break the chain of transmission, various countries in the world require people to limit and mitigate activities outside the house, especially for high-risk groups: the elderly and groups with comorbidities. Although there is no age group protected from SARS-CoV-2 infection, the elderly group shows significantly higher mortality and severity of the disease when compared with young and middle age. Case fatality rate in the elderly ranges between 8% in patients aged ≥80 years, 8% in patients aged 70-79 years, and up to 49% in critical cases. Other studies also show that the average age of patients who need the intensive care unit (ICU) is older than patients who do not require ICU.

Abstract

Introduction: The aetiologies of childhood pneumonia were varies. Initial therapy of pneumonia based on clinical assessment, empirical antibiotic is unavoidable. Observation of the responses to empirical antibiotic therapy are needed, because not all children showed good response to those therapy. Objective to analyse the risk factors associated with empirical antibiotic treatment failure of childhood pneumonia.Methods: The study was conducted as observational analytic study with a retrospective research approach from 1 January 2014 - 31 December 2016 in the inpatient ward of Dr. Soetomo Hospital. Subjects were children aged 2-59 months with initial therapy of ampicillin and gentamicin, then assessed at 2-3 days of treatment.Results: There were 297 children with pneumonia aged 2-59 months, 42 (14%) subjects had empirical antibiotic treatment failure. The nine of variables studied, multivariate logistic regression analysis obtained pleural effusion (PR 7.25; p less than 0.001), atelectasis (PR 4.56; p 0.008), and thrombocytosis (PR 3.32; p 0.008) statistically significantly affected responses failure to empirical antibiotic.Conclusion: Pleural effusion, atelectasis and thrombocytosis are associated with the empirical antibiotic treatment failure in childhood pneumonia in Dr. Soetomo Hospital.
