Abstract

Introduction: DDX41 gene mutations can occur in hematopoietic malignancies, especially acute myeloid leukemia (AML) and myelodysplastic syndrome. The average incidence of myelodysplastic syndrome (MDS) in the general population is 4.5 out of 100,000 people per year. Myelodysplastic syndrome (MDS) can occur with different genetic mutations and will provide different prognoses and therapy results. Hypoplastic myelodysplastic syndrome (MDS) is often found in 10-15% of MDS patients. There have been no case reports or research studies in Indonesia that have reported a case of hypoplastic myelodysplastic syndrome associated with a mutation in the Dead Box Helicase 41 (DDX41) gene.Case Report: A 66-year-old man presented with complaints of weakness. The patient had a history of myelodysplastic syndrome diagnosed two years earlier. Bone marrow aspiration revealed cellularity with decreased granulopoiesis and thrombopoiesis, and myeloblasts accounted for 2.0%. Genetic testing using a Next Generation Sequencing (NGS) panel identified DDX41 p.(Pro258Leu) at 47.9%, DDX41 p.(Arg525His) at 1.7%, and ASXL1 p.(Gln977Ter) at 1.6%. The patient was treated with a combination therapy of venetoclax and azacitidine. Follow-up bone marrow aspiration showed improved cellularity, increased erythropoiesis activity, and a reduction in the number of dysplastic cells. Case Discussion: Mutations in the DDX41 gene can mostly be found in cases of hypoplastic myelodysplastic syndrome, while in normocellular or hypercellular myelodysplastic syndrome, genetic mutations such as SF3B1, TET2, STAG2, ASXL1, and BCOR are mostly found. The DDX41 gene is related to cellular molecules and innate immunity. The DDX41 gene mutation is a determining factor in the administration of chemotherapy. The DDX41 gene mutation is a determining factor in the selection of chemotherapy drugs.Conclusion: DDX41 genetic testing with NGS method is essential for determining the prognosis and appropriate therapy in the management of hypoplastic myelodysplastic syndrome cases.

Abstract

Introduction: Hajj and Umrah are Islamic pilgrimages performed in Saudi Arabia, often taking place during the summer season when temperatures may exceed 40°C. Such extreme conditions increase the risk of dehydration, especially among the elderly. Dehydration in older adults can lead to severe complications, even death. Preventive measures can be achieved through adequate knowledge and positive attitudes toward fluid intake.Methods: This was an observational analytic study with a cross-sectional design. Respondents were selected using purposive sampling, consisting of 50 elderly Hajj and Umrah pilgrims. The study was conducted from February to December 2023 at the NRA Hajj and Umrah Bureau, South Jakarta. Data were analyzed using the chi-square test to assess the relationship between knowledge and attitudes toward dehydration prevention.Results: The majority of respondents had good knowledge (62%) and moderate attitudes (56%), with most being female aged 60–65 years. Bivariate analysis showed a significant relationship between knowledge and attitudes of elderly Hajj and Umrah pilgrims toward dehydration prevention (p = 0.035).Conclusion: There is a significant relationship between knowledge and attitudes of elderly Hajj and Umrah pilgrims toward dehydration prevention.

Abstract

Introduction: Drug-resistant tuberculosis (DR-TB) is a major health concern in Indonesia, where treatment success rates remain low. Comorbidities such as diabetes mellitus (DM) and human immunodeficiency virus (HIV) may worsen patient outcomes by complicating disease management and increasing the risk of treatment failure. This study aimed to assess the impact of comorbidities on treatment success among DR-TB patients at dr. Soebandi Hospital Jember.Method: An analytic observational study was conducted on 228 DR-TB patients at RS dr. Soebandi in 2018-2022 who had completed treatment, selected using total sampling. Data were analysed using bivariate analysis to examine associations between comorbidities and treatment outcomes.Results: The overall treatment success rate was 36.8%. Comorbidities were present in 37.3% of patients and were associated with a significantly lower success rate compared to those without comorbidities (21.2% vs. 46.2%; p=0.000; OR=3.19). DM (p=0.018; OR=2.17) and HIV (p=0.026; OR=9.31) each significantly reduced treatment success. A combination of DM and hypertension also showed a significant association (p=0.041; OR=9.44).Conclusion: Hypertension alone, chronic kidney disease, and chronic hepatitis were not statistically related to treatment success. Comorbidities, particularly DM, HIV, and the combination of DM with hypertension, significantly affect DR-TB treatment success. Addressing these conditions could improve patient outcomes and enhance treatment programs.

Abstract

Human behavior is shaped by the complex interaction between the nervous and endocrine systems. The pituitary gland, as a major component of the endocrine system, plays a crucial role in regulating both physiological and psychological functions through the secretion of various hormones. This study aims to systematically review the role of the pituitary gland and the influence of the neuroendocrine system on human behavior. The research employed a Systematic Literature Review (SLR) method, analyzing eight selected scientific articles published between 2021 and 2025. Articles were sourced from Google Scholar, ScienceDirect, and ResearchGate using relevant keywords, and filtered following PRISMA guidelines. The review revealed that pituitary-secreted hormones such as ACTH, GH, prolactin, LH, FSH, and oxytocin have significant impacts on stress regulation, emotions, social bonding, and cognitive functions. The hypothalamic–pituitary–adrenal (HPA) axis plays a central role in the stress response and is strongly linked to psychological symptoms including anxiety and depression. Additionally, non-pharmacological interventions such as music therapy have been shown to stabilize neuroendocrine activity and effectively reduce anxiety. The pituitary gland functions as a hormonal control center that influences various aspects of human behavior. This review underscores the importance of neuroendocrine understanding within psychological contexts and supports the development of physiological-based interventions for mental well-being.

Abstract

Hypertension is one of the major risk factors for cardiovascular disease worldwide. Evidence has demonstrated that effective blood pressure control can reduce cardiovascular morbidity and mortality while slowing the progression toward end-stage renal disease. However, blood pressure control rates in the global population remain far from optimal despite the availability of various lifestyle-based and pharmacological treatment options. Recent data indicate that among approximately 120 million adults with hypertension in the United States, only about 23% have successfully achieved the target blood pressure of kess then 130/80 mmHg, whereas nearly 45% continue to have blood pressure levels ≥140/90 mmHg.

Abstract

Introduction: Anemia in pregnant women is a serious public health problem that increases the risk of maternal and neonatal morbidity and mortality. Prevention requires the active role of community health cadres; however, limited knowledge and skills remain major challenges. In the Bandarharjo Primary Health Care area, mostvolunteers are aged 46–55 years, have long service periods, and are predominantly unemployed, indicating that conventional education methods may be less effective. Animated video media that combine visual and audio elements are considered more communicative and easier to understand. This study evaluated the effectiveness of an animated video in improving health cadres’ knowledge of anemia in pregnant women.Methods: This quantitative pre-experimental study used a one-group pretest–posttest design involving 49 active health volunteers selected through total sampling. The intervention consisted of an animated video developed using Canva with a duration of 4 minutes and 41 seconds. Data were analyzed using univariate analysis andthe Wilcoxon test.Results: The mean knowledge score increased significantly from 56.87 ± 13.008 to 78.30 ± 15.680 (p less than 0.05).Conclusion: Animated video media are effective in improving health volunteers’ knowledge of anemia in pregnant women.

Abstract

Introduction: The incidence of LBW babies, namely less than 2500 grams, is one of the main factors that can affect neonatal death, postneonatal death and morbidity in infants. Several factors affect LBW including anemia. Anemia is a condition of pregnant women who have hemoglobin levels less than 11 gr/dl which can be a risk for giving birth to low birth weight babies due to lack of blood supply to the placenta which will affect the function of the placenta to the fetus. Mothers with anemia are also at risk of experiencing bleeding before and during delivery, it can even cause the death of the mother and her baby if the mother is anemic. The aim of this research is to analyze the relationship between anemia and the incidence of LBW babies in pregnant women at Waled Hospital in 2018–2021.Method: This research is an observational study using the cross sectional method with a sample of 247 respondents. Researchers used secondary data with univariate analysis, and bivariate analysis using the Spearman correlation test.Result: The researchers obtained a percentage of 50.2% of pregnant women with a history of anemia who had LBW babies and 49.8% of pregnant women who did not have LBW. The relationship between anemia and LBW has a significant relationship (p=0.000, rs=0.321).Conclusion: There is a relationship between Anemia and the incidence of LBW babies in pregnant women at Waled Hospital in 2018–2021.

Abstract

Introduction: Generalized pustular psoriasis (GPP) is a rare, acute inflammatory and potentially life-threatening variant of psoriasis, characterized by widespread sterile pustular eruptions on an erythematous base, often accompanied by systemic symptoms such as fever, malaise, and leukocytosis. GPP differs from chronic plaque psoriasis due to dysregulation of the IL-36 pathway and IL36RN mutations, classifying it as an autoinflammatory keratinization disease, which poses significant therapeutic challenges, especially in settings with limited healthcare resources and restricted access to biologic agents.Case Report: A 43-year-old man presented with high fever, severe skin pain, and widespread pustules coalescing into “lakes of pus.” Histopathological examination revealed orthokeratosis, parakeratosis, regular acanthosis, thinning of the suprapapillary plate, Munro microabscesses, and Kogoj’s spongiform pustules. Case Discussion: A diagnosis of Von Zumbusch-type GPP was established based on clinical and histopathological findings. Although biologic agents are the first- line treatment according to current guidelines, short-term systemic corticosteroids remain an important option in resource-limited settings. The patient responded well to intravenous methylprednisolone and topical corticosteroids, with significant reduction in the Psoriasis Area and Severity Index (PASI) and no relapse during eight weeks of follow-up.Conclusion: This case underscores the feasibility of achieving remission in generalized pustular psoriasis using conventional systemic corticosteroids under close monitoring, particularly in resource-limited settings where access to biologic agents may be restricted.

Abstract

Introduction: Irrational antibiotic use in pediatric populations remains a significant challenge in Indonesia, contributing to antibiotic resistance. Despite interventions and public awareness campaigns, inappropriate antibiotic prescribing and self-medication persist among patients, healthcare providers, and pharmacies. This study aims to identify trends, challenges, and potential solutions for promoting rational antibiotic use in Indonesia.Methods: A retrospective review was conducted on 11 unpublished survey studies by YOP related to antibiotic use in pediatric populations and healthcare practices in Indonesia from 2004 to 2023. Data sources included prescription surveys (2004-2012), community perception surveys (2014-2023), and healthcare provider assessments (2010-2013). Prescription patterns, antibiotic perceptions, and dispensing practices were analyzed.Results: The public surveys over two decades show a decline in the behavior of buying antibiotics without a prescription (from 91% to 23%), while the prescribing of antibiotics for colds and coughs remains high and unchanged (around 68-79%). Pharmacy assessments found that 85% of pharmacies dispensed antibiotics without a prescription and 43% advised antibiotics for URTI.Conclusion: Misconceptions about antibiotics, lack of stringent regulations on antibiotic sales, and continued overprescription by healthcare providers are the key factors of irrational antibiotic usage in pediatric population in Indonesia.

Abstract

Introduction: Mechanical Low Back Pain (mLBP) is a common issue linked to obesity. This study evaluates the effectiveness of Proper Body Mechanics Program in managing pain and enhancing functional ability in obese patients with mLBP.Method: Single-blind, randomized controlled trial conducted three times a week for six weeks. Thirty eligible participants at Cipto Mangunkusumo Hospital were divided into two groups: the intervention group (14 subjects) received the Proper Body Mechanics Program in addition to a standard treatment plan including diet education, aerobic exercise, transcutaneous electrical nerve stimulation (TENS) and William flexion exercise, while the control group (16 subjects) only received a standard treatment plan.Result: Results showed a significant reduction in pain levels as measured by the Visual Analog Scale (VAS) (p=0.011) after six weeks; however, no significant difference was found in functional ability scores (p=0.976).Conclusion: While pain and functional ability improved in each group, the study indicates that the Proper Body Mechanics Program significantly reduces pain when added to standard Williams Flexion Exercises. Although the improvement in functional ability was not statistically significant, the intervention group showed higher overall functional improvement after six weeks.
