Abstract

The research was conducted to determine the effect of Sunkist orange (Citrus sinensis (L.) Osbeck) peel extract nanoparticles in granular form on lipid profile levels in alloxan-induced Wistar rats (Rattus norvegicus). The study employed an experimental method with a posttest-only control group design. The population for this research was Wistar rats. The study used five treatment groups, resulting in a total of 25 rats, with five rats in each treatment group, as determined using the Federer formula. After the administration of sunkist orange peel extract nanoparticles at different doses for 14 days, the results showed a decrease in total cholesterol, LDL, triglycerides, and HDL levels. The research findings demonstrated the influence of reducing lipid profile levels in diabetic rats. The effective dose for reducing the lipid profile in this study was found to be 100 mg for total cholesterol, 70 mg for triglyceride, and 70 mg for both HDL and LDL when using the ethanol extract of sunkist orange peel.

Abstract

Background: Pneumocystis jirovecii pneumonia (PJP) infections are common in immunocompromised patients and are rarely found in immunocompetent patients. Lung radiology in PJP patients could mimic the appearance of interstitial lung disease (ILD) and could be used to diagnose HIV-negative patients. Clinicians should rule out false negatives in patients with patterns suggestive of opportunistic infections and risk factors for HIV infection. Case illustration: A 34-year-old man presented with a chief complaint of shortness of breath, had history of 15 years of smoking, and daily chlorine exposure. The radiology pattern and initial HIV-negative test results suggested an ILD diagnosis. Owing to persistent symptoms despite initial management and the presence of risk factors, repeat HIV testing was initiated and was positive. The patient was treated with cotrimoxazole and showed rapid clinical improvement. Discussion: The diagnosis of PJP in our patient was based on radiology and an HIV-positive status.  In patients who are not immunocompromised, the diagnosis of PJP is unlikely, and other diagnoses, such as ILD, should be considered. However, in the HIV testing window period of infection, a poor advanced state of HIV could cause a false negative result. Therefore, clinical judgement is essential in suspecting such a result. The empirical treatment course of cotrimoxazole has been shown to provide better clinical outcomes in PJP. Conclusion: The possibility of PJP must be considered in patients with initially HIV-negative results, especially in patients with risk factors and clinical symptoms suggestive of immunocompromise. While some ILD showed similar PJP, the risk factors for ILD and PCP could be distinguishing factors. Retesting for HIV infection can confirm the diagnosis and rule out false-negative results.

Abstract

Efforts to prevent environmental pollution due to wastewater produced by car service station can be carried out using electrocoagulation technology. The purpose of this study was to analyze the effectiveness of electrocoagulation technology for treating car service station wastewater. This research is an experimental study conducted in a car workshop at PT Jaya Indah Motor Jambi City. The electrocoagulation machine used an Automatic PLC System WWTP 350 L with total dimensions of 1200 x 1200 x1700 mm, power 380Volt/20A (Stanby Steady State). Researchers compared the research data with the wastewater quality standards contained in the Ministry of Environment and Forestry Regulation No. P.68.MENLHK/Setjen/KUM/.I/8/2016. The results showed that the final value of the parameters of the treatment results using electrocoagulation had met the wastewater quality standards (TSS 19.75 mg/L; pH 6.92; BOD 8.46 mg/L; COD 25.07 mg/L; oil and fat 2.75 mg/L; and ammonia 7.12 mg/L).

Abstract

Frontal sinus mucoceles can present with a multitude of symptoms including ophthalmic disturbances. In benign conditions, they tend to expand by eroding the surrounding bony walls that displace and destroy structures through pressure and bony resorption. It becomes inflamed and filled with pus (pyocele) or a combination of both (mucopyocele). Due to its expansile nature, mucocele is important for diagnosis. Frontal mucoceles are benign and curable. Early recognition and management are important, as they can cause local, orbital, or intracranial complications. The purpose of this paper is to report a case of mucocele and emphasize the importance of radiological imaging, especially CT-Scan and MRI, in supporting the diagnosis of frontal sinus mucocele, detecting invasive processes to surrounding tissues and their extension to the intraorbita/intracranial, planning before surgery/biopsy, and evaluating the results of therapy.

Abstract

Tuberculosis is a public health problem caused by Mycobacterium tuberculosis. In 2021 there will be 10.6 million cases in the world, and Indonesia ranks 2nd with 700,000 cases. In 2022, there will be 17,303 cases in North Sumatra Province and 2,430 cases in Medan City. In current practice, evaluation and diagnosis of active tuberculosis relies on bacteriological examination and Chest Radiographs. However, Chest Radiographs have limited specificity and high intraobserver and interobserver variability. HRCT is also not widely used as a routine option for patients suspected of suffering from tuberculosis, even though HRCT has high accuracy in detecting tuberculosis. We present a case of a patient with active pulmonary tuberculosis and lung destruction e.c. advanced pulmonary tuberculosis (MDR-Tuberculosis) (declared cured in 2018 after 2 years of treatment). This patient was evaluated with chest radiograph and chest HRCT. In this case, the initial findings on the Chest Radiograph, showed the impression of inactive tuberculosis but on further examination with chest HRCT, there was a tree in bud image which indicated active tuberculosis. This shows that there are limitations in diagnosing tuberculosis activity using Chest Radiographs. Due to the significant role of HRCT in the diagnosis of tuberculosis activity, it is necessary to consider the use of HRCT in the evaluation of patients with tuberculosis.

Abstract

Obesity is a growing global health concern, including in Indonesia. This study aimed to determine the relationship between obesity and age, sex, and physical inactivity. This study used secondary data from Puskesmas Johar Baru in Central Jakarta in 2021. The total initial data were 102,647 patients, the data eligible for sampling was 72,680 patients, and 22, 297 (24 %) patients were obese. Data processing was performed using SPSS 25 software, bivariate analysis (chi-square test), and multivariate analysis (multiple logistic regression with the enter method), at a significance level of 0.05. The Chi-Square test showed that age, sex, and lack of physical activity had a significant relationship with ob with (p ≤ 0.05). In multivariate analysis, the gender variable had the highest OR value of 12.925, where the majority of respondents who suffered from obesity were female, so they had a greater risk of suffering from obesity. These findings underscore the importance of sex variables in the context of obesity in Puskesmas Johar Baru and highlight the differences in obesity prevalence by age and sex. Middle-aged adults, especially women, are more prone to obesity, which is supported by a lack of physical activity.

Abstract

This study aimed to analyze the nutritional status of children in these districts, taking into account factors such as age, gender, maternal education, family income, and children's eating patterns. The study used a cross-sectional design, with 279 children aged 1-10 years as subjects. Data were collected by distributing structured questionnaires to the participants’ parents. The data were analyzed using the chi-square test to determine the significance of the relationship between the predictors and nutritional status of the children (α = 0.05). The results showed that maternal education (0.012), parental income (0.001), and eating patterns (0.001) were significantly associated with the nutritional status of children who were stunted and normal. However, for overweight and obese children, maternal education (0.835), parental income (0.951), and eating patterns (0.309) were not predictors of nutritional status. Age and sex did not significantly influence the nutritional status of children in all categories. Family based interventions through health education are needed to improve nutritional literacy so that parents can plan and prepare nutritious meals for their children. The government must provide policies to support families, especially those with low incomes, to ensure the availability of adequate food and access to nutritious food.

Abstract

High blood sugar levels can be influenced by several factors, including high-carbohydrate food consumption and a lack of physical activity. To minimize the side effects of hyperglycemic treatment, efforts have been made to explore anti-hyperglycemic compounds from other sources, such as durian peel (Durio zibetthinus Murr.). This study aimed to determine the effectiveness of ethanol extract from durian peel in lowering alloxan-induced blood sugar levels in Wistar rats (Rattus novergicus). The study is an experimental research employing an alloxan-induced diabetic rat model. The rats were divided into five different groups: Group I (negative control), Group II (positive control), Group III (dose 25 × 10–30/gBW), Group IV (dose 50 × 10–30/gBW), and Group V (dose 100 × 10–30/gBW). The durian peel used in this study was extracted using the maceration method with ethanol as the solvent. The results of this study indicate that ethanol extract from durian peel significantly reduced blood sugar levels after 10 to 15 days of durian peel extract administration (p < 0.05).

Abstract

Coronavirus disease 2019 (COVID-19) is an infectious disease caused by a new coronavirus, Severe Acute Respiratory Syndrome Corona Virus 2 (SARS-CoV-2). Pro-inflammatory cytokines play a central role in many viral respiratory infections by coordinating and activating adaptive immune responses. Higher serum levels of pro-inflammatory cytokines (TNF-α, IL-1β, IL-2, IL-6 and IL-17) and chemokines (IL-8) have been observed in many patients with severe COVID-19 compared with individuals with a mild degree. This research is an observational study with cross sectional data collection method. This study took blood samples from mild and severe COVID-19 patients who were treated at the HAM Hospital as many as 34 patients. Samples were examined only once for IL-17 in mild and severe COVID-19 patients. The research was conducted after obtaining ethical approval and informed consent. This study was followed by 18 female subjects (52.9%). The mean age of the subjects was 47.52 years with the youngest age being 23 years old and the oldest being 81 years old. By using the Mann Whitney test, it showed that there was a significant difference in interleukin 17 levels between Covid-19 patients with severe and mild degrees (p = 0.049). IL-17 examination can be used as an alternative diagnostic marker and assess the severity in COVID 19 patients. Further studies are needed that involve a larger number of subjects, and are associated with other inflammatory markers such as CRP or with coagulation status such as D- dimer.

Abstract

Patients with renal failure undergoing hemodialysis often experience anxiety due to various individual, socioeconomic, and environmental factors. This study aimed to analyze the factors associated with anxiety in these patients. This was a quantitative, cross-sectional study. The risk factors for anxiety investigated in this study were education, duration of hemodialysis, knowledge, and family support. The study included 50 patients with renal failure undergoing hemodialysis (total sampling). Data were collected directly by distributing questionnaires to the patients. Data were analyzed using the chi-square test and logistic regression analysis (α=0.05). The results showed that education level was not significantly associated with anxiety in these patients (p= 0.816). However, the duration of hemodialysis (p= 0.023) and patient knowledge (p= 0.048) were significantly associated with the anxiety experienced by the patients. Family support was a protective factor against anxiety in these patients (P = 1.00). The dominant variable affecting patient anxiety is the duration of hemodialysis (p=0.041; PR = 4.1; 95%CI 1.061-16.107). This means that patients undergoing hemodialysis for more than 6 months are 4.1 times more likely to experience moderate anxiety compared to patients who have been undergoing hemodialysis for 6 months or less.
