Abstract

Introduction: Situs inversus, a congenital anomaly, is defined by the transportation of thoracic and abdominal organs. A variety during pregnancy this condition has often a challenge for obstetricians, gynecologists, and midwives as there is concern about cardiovascular dysfunction, leading to recommendations for cesarean section. Case Report: A 28-year-old primigravida at 40 gestational weeks. She was admitted arrival at the emergency room. Dextrocardia was already present, as was sinus inversus totalis and antenatal assessment relevealed no abnormalities. Patient underwent a spontanous vaginal delivery of a healthy male infant with normal birth weight. Discussion: Situs inversus totalis with dextrocardia does not compromise organ function or the typical physiological changes of pregnancy including cardiovascular system. Therefore, routine cesarean section is not indicated for these patients unless other obstetric factor necessitate it, as this condition does not independently contribute to increased maternal or fetal complications. Conclusion: Delivery of a pregnancy, dextrocardia, and situs inversus totalis via vaginal delivery is feasible without any significant cardiovascular risks. Cesarean section was not necessary unless other obstetric indications.

Abstract

Chronic Pelvic Pain Syndrome (CPPS) is a complex medical condition that affects patients' physical, emotional, and social quality of life. Its management involves two main approaches: surgical and conservative. However, there are significant differences in the effectiveness, risks, and long-term impacts of each approach. This study aims to evaluate the effectiveness of surgical and conservative approaches in managing CPPS through a systematic review and bibliometric analysis, focusing on pain reduction, improvement in quality of life, and risk of complications. This research employs the Systematic Literature Review (SLR) method, following the PRISMA guidelines to search, screen, and evaluate relevant literature from databases such as PubMed, Scopus, and the Cochrane Library. The quality of the relevant studies is assessed using standardized appraisal tools. Quantitative data are analyzed through bibliometric analysis to identify differences in effectiveness between surgical and conservative approaches. The results indicate that the surgical approach is more effective in reducing severe and refractory pain compared to conservative therapy, but it is associated with a higher risk of complications. Conversely, the conservative approach provides benefits for mild to moderate symptoms with minimal risk and lower cost. In the context of Indonesia, the conservative approach is more suitable to be implemented as the first-line treatment for CPPS, with surgical options considered for cases that do not respond to initial therapy.

Abstract

Osteoporosis is a systemic bone disease characterized by a decrease in bone density and deterioration of bone microarchitecture so that bones become brittle and break easily. Osteoporosis due to long-term use of glucocorticoids causes osteoporosis. There is a lot of clinical evidence about the role of phytoestrogens in the treatment of osteoporosis. Kenitu (Chrysophyllum cainito) contains isoflavone phytoestrogen compounds. The aim of this research was to determine the anti-osteoporosis effect of ethanol extract of kenitu leaves (Chrysophyllum cainito L.), by looking at the increase in bone density as well as the number of osteoblast cells and the number of osteoclast cells in the femur trabecular bone of female mice induced by dexamethasone. In this study, 49 female mice were grouped into 7 groups, namely normal control, negative control, positive control with cabone naturindo, and groups treated using 70% ethanol extract of kenitu leaves with varying doses of 100 mg/kg BW, 200 mg/kg BW, 400 mg/kg BW and 800 mg/kg BW. An increase in femur trabecular bone density, the number of osteoblast cells and the number of osteoclast cells were observed using the Histomorphometry method. The results showed that the ethanol extract of kenitu leaves has anti-osteoporosis activity. The ethanol extract of kenitu leaves has a dose of 400 mg/kg BW which is the optimal dose as anti-osteoporosis which is indicated by an increase in bone density, an increase in the number of osteoblast cells and a decrease in the number of osteoclast cells.

Abstract

HIV/AIDS remains a major health challenge among adolescents. Cultural beliefs and religious practices are believed to influence adolescents’ knowledge and their willingness to undergo testing and treatment. This study aims to examine the influence of cultural and religious beliefs on adolescents’ knowledge of HIV/AIDS and their willingness to seek testing and treatment. A cross-sectional quantitative approach was used involving 30 adolescents with structured questionnaires. Data were analyzed using chi-square tests and binary logistic regression in SPSS. The results showed no significant association between HIV knowledge and willingness to test (p=0.753). However, negative attitudes toward PLWHA significantly reduced the willingness to undergo testing (p=0.045), and religious influence showed a near-significant tendency toward affecting testing behavior (p≈0.064). It is concluded that attitudes toward PLWHA and religious values exert a greater influence than knowledge alone. Therefore, culturally and religiously sensitive interventions are needed to improve HIV testing awareness among adolescents.
